        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

        A curriculum for teaching materials in Science Education was studied as a model for modified xperiment on fundamental courses. Coscious effort can be made to desingn experiments that require minimum apparatus, that require low-cost equipment that can be made to from cheap locally available materials. Computer-aided instruction programs at high school, freshman course level have been developed. The software package was consist of five programs: The program contains explanation and problems for the calculation of resonance energy, molecular structure, mole concept, Rutherford's experiment, thermodynamic processes. Special course equipment package explanation mole concept, Rutheford's experiment, thermo dynamic processes. Special course were designed in Science Education with the understanding that to a certain extent science values would be covered in all of the modified experiments and program models.

      • Southern Blot Hybridization법을 이용한 악성 림프종의 면역유전형에 관한 연구

        Immunogenotyping using gene rearrangement analysis has emerged as a precise laboratory aid in the diagnosis and classification of malignant Iymphoid neoplasms. The lineage and clonality of the malignant Iymphoid neoplasms can be identified by the demonstration of rearrangements of antigen receptor genes of the immunoglobulin and T-cell receptor genes. The analysis of the gene rearrangements on the malignant Iymphoid neoplasms are also useful as a sensitive unique clonal markers to detect early recurrence in patients with malignant Iymphoid neoplasms after treatment. To analyze the sensitivity and specificity of gene rearrangements in the diagnosis of malignant Iymphoid neoplasms. 24 cases of malignat Iymphoma were examined by Southern blot hybridization using CTβ-T cell receptor β chain gene-DNA probe and JH-immunoglobulin heavy chain gene-DNA probe. The results of the immunogenotypings using Southern blot hybridization disclosed high correlation between the immunophenotyping using immunohistochemical stain with monoclonal antibodies (B-cell Iymphoma 84.2%. T-cell Iymphoma 75% ). The analysis of the gene rearrangement of the angioimmunoblastic lymphadenopathy(AILD) and unclassifiable Iymphoma using immunohistochemical stain could resolve the monoclonality and lineage . Rearranged bands to the CTβDNA probe were observed in one case out of 2 cases of AILD. One case of unclassifiable Iymphoma showed rearranged bands to the CTβ DNA probe. There were no rearrangements in reactive follicular Iymphoid hyperplasia and paracortical Iymphoid hyperplasia. In conclusion, DNA gene rearrangement study should be applied to differentiate the clonality and cell lineage in the malignant Iymphoma with indistinctive immunophenotype.

      • 연속형 센서와 웨이브 전파를 이용한 판 구조물의 손상감지

        Abstract : The response characteristics of long continuous sensors are studied for damage detection in a plate structure based on strain waves that are generated by damage. That is, the response characteristics of the long continuous sensors, which are long ribbon-like sensors and can be fabricatedby piezoceramic ribbon fibers, etc. are studied by simulation of wavepropagation in a panel to improve sensing capability of the waves that arepropagating by damage. The advantage of the long continuous sensor is toimprove damage detection by having a large coverage of sensors on thestructure using a small number of channels of data acquisition. Theresponses from the long continuous sensor are similar if the damages arelocated at the same normal distance to the sensor, and these similar patternscan make it easy to estimate the damage. Keywords : long continuous sensor, damage detection, wave propagation 요지 : 이 연구에서는 손상에 의해 발생하는 변형률 웨이브 검출을 통한 판 구조물의 손상위치추정을 위하여 연속형 센서의 응답특성에 대하여 연구하였다. 즉, 균열발생 및 균열진전등과 같은 손상에 의하여 발생되는 웨이브를 보다 효과적으로 감지하기 위하여, 피에조 세라믹 섬유 등을 이용하여 리본 형으로 길게 제작할 수 있는 연속형 센서의 응답특성을 웨이브 전파 시뮬레이션을 통하여 연구하였다. 이러한 센서를 적용하는 경우, 적은 수의 데이터 취득 채널을 이용하여 구조물의 넓은 영역을 감지할 수 있는 장점이 있다. 또한 연속형 센서와 여러 손상위치들의 직각거리가 같은 경우 센서에서 감지되는 각각의 응답들은손상위치들에 대해 거의 동일한 양상을 보이며, 이러한 응답패턴을 이용하면 판 구조물의손상위치를 더욱 효과적으로 추정할 수 있을 것으로 판단된다. 핵심용어: 연속형 센서, 손상추정, 웨이브 전파

        The studies was conducted to establish the indoor overwintering methods for productive honeybee colonies in the wintering chamber which provides satisfactory inside temperature ranging 2∼9。C and ventilation for the wintering honeybee colonies in Korea. Mortality of the indoor wintered honeybee colonies was 6.3∼7.1% during the winter season of 1997-1998 and 5∼10% during the winter season 1998∼1999. The decreased rates of colony weights during the wintering season were 10.6∼10.7% in the strong colonies and 10.2∼11.7% in the weak colonies. The increased rates of colony population overwintered in the chamber were 136.1∼142.3% in the strong colonies and 128∼136.5% in the weak colonies.

        코크스 제조 및 사용 공정에서의 코크스오븐 배출물질 연구

        This study was performed to evaluate the coke oven emissions (COE) and polynuclear aromatic hydrocarbon levels in coke manu- facturing industry, secondary lead smelting industry and glass bottle manufacturing industry. 1. There were no significant difference between the means of personal samples and area samples by the types of industry(p>0.05). The levels of airborne total particulates of the secondary lead smelting industry was the highest(2.30㎎/㎥), and those of the coke manu- facturing industry and glass bottle manufacturing industry were 1.95 ㎎/㎥ and 1.37 ㎎/㎥. The concentration of COE was the highest in the glass bottle manufacturing industry(0.79 ㎎/㎥), and in order of 0.19㎎/㎥ in the coke manufacturing industry and 0.05 ㎎/㎥ in the secondary lead smelting industry. COE/total particulates(%) was highest in the glass bottle manufacturing industry(58.1%) and in order of 10.3% in the coke manufacturing industry and 3.1% in secondary lead smelting industry. There were significant differences in the total particle concentration and Coe by the types of industry(p<0.05) 2. The levels of airborne total particulates was the highest at the smelting process of secondary lead smelting industry(2.30±0.72 ㎎/㎥), and the lowest at the smelting process of glass bottle manufacturing industry (0.99±1.22㎎/㎥) Concentration of COE was the highest at the casting process of glass bottle manufacturing industry (1.09±1.15㎎/㎥), the lowest at the smelting process of secondary lead smelting industry(0.06±0,03 ㎎/㎥). The COE/total particulates(%) was the highest at the casting process of glass bottle manufacturing industry(65.9±20.5%), and the lowest at the smelting process of secondary lead smelting industry(3.1±2.7%). 3. There were positive correlations between level of The airborne total particulates and concentration of COE in COKE manufacturing industry and glass bottle manufacturing industry(p<0.05), but negative correlation in secondary lead smelting industry. 4. The numbers of case and rates that over the Threshold Limit Values(TLVs) were 24 (77.4%)cases in glass bottle manufacture, 14(23.7%) cases in the coke manufacturing industry and no one case in secondary lead smelting industry. Total numbers of case and rates that over TLVs were 38(35.5%) cases. 5. The limit of detection(LOD) for PAH was 10 ㎍/㎖) in standard sample. All PAH levels of the cokes manufacturing industry and the secondary lead smelting industry and the glass bottle manufacturing industry were trace or not to detect.

      • 大學敎育課程 構成에 관한 硏究 : 大學의 特殊性을 살린 敎育課程의 模型開發 Development of a Model for a Curriculum Capitalizing on the Speciality

        The purpose of this study was to develop a curriculum model for the small, private and women's university in Seoul area. The curriculum was deeply interrelated with the missions of the university stated with the resources, the tradition and tile adopted view oil education. For the example university, the speciality web counted as follows : undergraduate education rather than research as a small sine university, integrated identity of the harmonious women as a women's university and preserving diversity as a university in Seoul area was considered. The principles for a curriculum model drawn from these consideration was as follows : 1) The university curriculum should be relevant to the ideal of the university, i. e. the whole person. 2) The university curriculum should be composed of general education, concentration, and electives. These should be distributed harmoniously. 3) The general education ie the breadth component of the curriculum, and the essential area should be prescribed by the university authority. 4) The concentration is the depth component of the curriculum. The students should be given the depth experience relevant to undergraduate level at least in one area. 5) The electives can be used fer the diverse academic and individual interest and need. 6) The curriculum should be operated with the help of the university curriculum committee. The models of the undergraduate curriculum was composed under the above principles and suggested as a conclusion.

      • 外國 大學敎育의 理念과 敎育課程의 變遷課程에서 본 韓國 大學의 敎育理念과 敎育過程

        The purpose of this study was to examine the idea and curriculum of higher education in Korea through the comparative study with other nations - Europe, America, Japan's cases. In the periods of 1960s and the first half of 1970s, popularization phenomenon of higher education according to changing political, econimical condition and technological development was occured internationally. Because of this, traditional idea and method of higher education was changed. In Korea, this phenomenon appeared in the end of 1970s and the beginning of 1980s. In the reforming of higher education. each nation got a little different position, But, ultimately it was derived from the struggle between the traditional idea of the higher education as an elite education system and the idea of equalitarianism with the need of the society. This gave some viewpoints to the direction of Korean higher education. The higher education systems should find their identities as a universities, i. e, the ideal of the educated person should be the essential part of the higher education rather than the technological training or of her services. The educated person through the university education should have comprehensive thought ratter than trained skill. Because, only the educated man with comprehensive thought can use technology as an instrument for the progress of peace and the welfare of mankind.

      • 탄소섬유와 석면을 보강한 시멘트 복합재료의 압축, 인장 및 휨 강도에 관한연구

        This study was carried out to examine the reducing ability of asbestos quantity added in asbestos reinforced cement composite(ARC). The using amount of asbestos is subject to restriction from 10-20 vol% to 5-10 vol%,because it is very harmful cancerogenic substance. Asbestos was replaced by pitch based isotropic carbon fiber, and then the mechanical properties, compressive, tensile and flexural strengths were tested as a function of mixing proportions. The results are as follows 1) The compressive strength of carbon fiber and asbestos reinforced cement composite(CFARC) was greater than ARC by superplasticizer, reducingW/C ratio to 20 percent. 2) The tensile strength of CFARC was showed increasing trend up to 52% compared to ARC at 5% carbon fiber volume fraction. 3) The flexural strength of CFARC was showed increasing trend up to 17% cowpared to ARC at 5% carbon fiber volume fraction.
