      • 비지시적 지도방법에 의한 유아의 읽기와 쓰기 행동의 발달

        Many studies(Clay, 1972; Rossman, 1980; Hildebreth, 1936; Schickedanz, 1978) showed that young children's reading-writing behaviors develop through certain fixed stages. To investigate whether Korean childern also follow similar developmental stages of reading, the authors of this study revised Clay and Rossman's reading development stages according to the results of a pilot study and proposed six hierachical stages of reading development. They are: (1) pre-reading stage, (2) making-up stage, (3) paraphrasing stage, (4) the stage of paraphrasing plus repeating actual words by memory, (5) the stage of paraphrasing plus reading actual words of the text, and (6) the stage of reading actual words of the text. The authors also proposed that there are two transitional stages between stages 1 and 2 and stages 5 and 6. The authors also revised the Hildebreth and Schickedanz' writing development stages as follow: (1) scribbling stage, (2) the stage of appearance of one or two distinctive features of print, (3) the stage of appearance of intentional markings that look like actual alphabet letters, (4) the stage of creating actual letters, (5) the stage creating actual words, and (6) the stage of creating sentences. The main purpose of this study were to see whether the development of reading and writing behavior followed the hypothesized stages in order and to examine the effects of undirective teaching method of reading. Besides of these purposes, this study had two additional purposes of seeing whether reading readiness had effects on young childen's development of reading and writing behaviors and of developing a good reading and writing readiness scale. For these purposes, 32 preschoolers of age 3 to 5, were selected from the kindergarten located in middle class neighbors on the basis of reading-writing readiness scores and classified into high and low readiness groups. Those who were able to read were excluded from the sample. A half of each group(while the other half served as controls) were observed individually during undirective reading sessions in which the children were read 4 storybooks and asked to read them back to the experimentet once everyweek over a three month period. The children's responses during these sessions were audiorecorede and transcribed on coding sheets afterwards. The data were analyzed with the stastical method of correlations and ANOVA. The results showed that preschooler's reading and writing behaviors progressed through the hypothesized stages and the undirective teaching method in reading facilitated their development. The results also showed that the reading and writing readiness scale consisting of subtests of visual discrimination, phonological awareness, and abilities of drawing basic lines, had a moderate predictability in reading and writing development in the future. Furthermore, this study demonstrated significant differences in reading ability between low and high readiness groups. The results of this study suggest that we, educators in early childhood education, should understand young children's reading-writing developmental stages and encourage them to read and write actively on their own level and to retain their initial interest in reading and writing. The results also imply that the undirective method of reading and writing could be a good example of such encouragement.

      • B형 만성 활동성 간염 환자에서 Prednisolone 단기이탈 및 Alpha Interferon 병합요법의 효과

        To evaluate the safety and efficacy of Pd withdrawal followed by recombinant alpha- Interferon 2b therapy. Nine patients were given a single daily subcutaneous injection of ^α-Interferon at a dose of 3 x 10 exp(6) units/day for first weeks and thrice weekly for next 15 weeks (maintenance) after pd tappered over 6 weeks. To evaluate the efficacy of therapy, we measured serum aminotransferase activities monthly during and after therapy, and also measured serm HBsAg, HBeAg and DNA probe before and after therapy. To evaluate the safety of therapy, all patients checked peripheral blood WBC, Hemoglobin, Platelet, BUN and Creatinine. The results were as follows : 1. In control group, the normalization of s-AST & ALT level, negative seroconversion of DNA probe & HBeAg were observed in 1/12(8%) & 7/10(17%), 0/10(0%) & 0/12(0%) at 6 months later, and maintained response in 1/11(9%) & 1/11(9%), 2/9(22%) & 2/11(18%) until 1 years later. 2. In treatment group, Serum AST and ALT returned to normal in 8/9 & 8/9 cases (89%) after 1 montns Tx. (mean serum AST : 33±8, ALT : 34±12 IU/ml) and maintained to normal response in 5/9(56%) & 6/9 cases(67%)after 6 months Tx. (mean s-AST : 55±43, ALT : 66±84 IU/ml), 3/9(33%) & 4/9 cases(44%) after 1 years Tx. It was statistically significant reponse except s-AST of after 1 years Tx. 3. In treatment group, s- HBeAg returned to normal in 3/9 cases(33%) after 1 months Tx., and maintained to normal response in 1/7 & 1/4 cases among follow up patients in after 6 & 12 months Tx. Serm DNA probe level were observed significantly negative seroconversion in 8/9 cases(89%) after 1 months Tx. (p<0.003), and maintained to negative response in 2/6 & 1/3 cases after 6 & 12 months Tx. 4. Serum AST, ALT, HBeAg and DNA probe were not significantly correlated with therapeutic response in relation to histologic diagnosis. 5. Fever and myalgia were noted in 9(100%) & 6(67%) cases. Headache, mild hair loss, anorexia, nausea, facial flushing & diarrhea were noted in few cases. Peripheral WBC(3cases) & platelet counts(3cases) were decreased transiently. This study shows that Pd withdrawal followed by recombinant alpha Interferon 2b therapy may be safe and effective in decrease the level of s-DNA probe, transaminase activity, loss of HBeAg and less adverse effect of clinical and hematologic examinations in patients with CAH type B. But it was necessary for study with large number of patients, more prolonged follow up duration and more strict control study.

        성인간호학회지 논문(1989-2000)의 내용과 경향 분석

        "Journal of Korean Academy of Adult Nursing" has been published since 1989 and more than 330 articles have been published up 2000. We reviewed all the articles published in this journal to analyze the contents and trend of published articles and to find out the way of improvement both in quantity and quality of studies in this area. The results are as follows : 1) Ninety percent of articles were quantitative studies. Experimental design was used in 16.1% and quasi-experimental design was adapted in 73.6% of articles. Qualitative research were 10.3% of total articles reviewed. 2) Only 5.1% of articles described theoretical framework and 73.6% of experimentally designed articles included research hypotheses. 3) Among experimental studies, only one was true experimental studie design. Eighty nine point three persent had descriptive survey method among non-experimental studies. Amog the thirty four qualitative studies, phenomenologic(73.5%), grounded theory(14.7%), and ethnographic (11.8%) design were used. 4) Most of research subject were patients and few healthy people, healthcare worker, and patient family. 5) Psychosocial data collection method which used questionnaire were 53.7% and interview. physical measurement, observation were used in 22.8%. 8.2% 5.4% of articles respectively. 6) Total number of keywords were 692, and average number of key words per article was 2.2. Health-related keyword was the most frequently used according to four metaparadigm of nursing (human, health, nursing, and environment). Environment related keyword was rarely appeared. The number and quality of nursing research in "Journal of Korean Academy of Adult Nursing" have been improved continuously. It is difficult to find out special trend or characteristics which is unique in this journal because of relatively short history of adult nursing studies. This review study was performed to analyze adult nursing studies only in the view of quantitative aspect, but it is needed to analyse qualitatively to find philosophical or theoretical trend in nursing.

        인터넷 중독 청소년들의 정신병리 및 대처 방식

        This study was conducted to investigate the differences in psychopathology and coping strategies among internet addictors, over users and non addictors in high school students. Subjects were 273 high school students in Seoul city. Self rating questionnaires included demographic data, Young’s Internet Addiction Scale, SCL-90-R, Beck Depression Inventory, Spielberger State-Trait Anxiety Inventory, and The Way of Coping Strategy. Subjects were classified into 3 groups (addiction, over use, non addiction) by Internet Addiction Scale score. Prevalence of internet addiction was 4.3% and male adolescents were more addictive than female. Students who were mainly involved with internet chatting or game showed higher internet addiction score. The addiction group and over use group showed higher level of somatization, obsession, interpersonal sensitivity, depression, anxiety, hostility, and paranoia than non-addicted group. Addiction group and over use group used passive coping strategies more frequently in the stressful situation than nonaddicted group. Also, the regression analysis for finding out causal factors that predicts internet addiction showed that emotion relieving strategy were important variable which predict internet addiction. Interpersonal sensitivity and emotion relieving coping strategy was important to predict addiction. This study showed that problematic internet use was closely associated with psychopathology and psychiatric comorbidity. And high interpersonal sensitivity and emotion relieving coping strategy were related with internet addiction in adolescence.

      • 내시경으로 확인된 상부소화관 용종의 임상적 고찰

        From January 1987 to October 1991, we performed 14,333 cases of upper gastrointestinal endoscopy and diagnosed 527 cases with U.G.I polyp. We assesed these 527 cases and obtained following results: 1) The overall incidence of U.G.I. polyps was 3.6%, and there was no sexual difference. The peak incidence was in 6th decades(32.4%). 2) The U.G.I. polyps were located chiefly at stomach(415 cases, 78.7%), among which antrum occupied antrum 43.5%, body 24.0% and funds 5.1%. And the others were esophagus(6.5%) and duodenum(12.3%). 3) The size of U.G.I. polyp was below 1cm in 72.3%, from 1cm to 1.9cm in 20.7% and above 2cm in 7.0%. According to Yamada' classification, type Ⅰ, Ⅱ, Ⅲ and Ⅳ was 31.9%, 51.3%, 13.7% and 3.0%. 4) Histological nature of U.I.G. polyp were hyperplastic polyp(85.8%), adenomatous polyps(9.3%), carcinomatous polyps(3.3%) and etc. The size of neoplastic polyp such as adenomatous polyps(mean 1.0±0.7cm) and carcinomatous polyps(niean 2.0±1.8) were larger man hyperplastic polyp(mean 0.6±0.4) (p<0.05) 5) The gastric polyps were associated with peptic ulcer(7.5%), gastric cancer(3.2%), other malignancy(3.5%), hepatobiliary disease(5.5%), post subtotal gastrectomy(5.5%). And others(75%) werenot associated with specific disease.

        간호실무에서 신봉이론(Espoused theories)에 대한 탐색적 연구

        As a nursing practice involves nurses' actions in a specific context of health care, this study has focused on exploring the espoused theories in nursing practice within the action science perspectives, Espoused theories are the belief, principles, and rationale expressed by the practitioner as guiding her/his actions in a situation of practice. The data were analysed qualitatively and 25 elements of espoused theories of nursing action were identified and clustered into 6 categories. The 25 elements of espoused theories are as follows: The clinical nurse worked in wholistic and individual nursing, focussed on the patient's needed, comfort and supportive nursing (5 theories of nursing goal) ; excellent skills, knowledge based, assessment and data collection, explaining, educating or a scientific basis(6 theories of nursing intervention): advocacy, value oriented, treatment, account- ability and commitment(4 theories of nursing ethics) ; human respect. partnership, trust(3 theories of patient-nurse relationship) : knowledgable, accumulated clinical experiences and personally lived experiences. positive perspectives(4 theories of nurse), role of intervention. rewarding peer relationship(3 theories of situations) The above mentioned espoused theories are similar to that of nursing textbooks which students learned through basic nursing education and almost the same as the Acts ofa Nurse in Korean. However, we are doubtful whether nurses actually do as they think. Therefore. it is recommended to review the theories-in-use in order to and any discrepancies between the espoused theories and the reality of nursing actions
