        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

      • 국내 주요 호수의 육수학적 조사(4): 주암호

        In this study limnological characteristics of Lake Juam was surveyed from June 1993 to May 1994 in order to provides important information regarding water resources. Seschi disc transparency, epilimnetic chlorophyll a(chl-a), tatal nitrogen(TN), total phosphorus(TP) concentration and primary productivity were in the range of 2.0~4.5 m, 0.9~13.6 mgChl/㎥, 0.78~2.32N/l, 11~56 mgP/㎥, 270~2,160mgCm^(-2)day^(-1), respectively. On the basis of TP, Chl-a and Secchi disc depth, the trophic state of Lake Juam can be classified as mesotrophic lake. The phosphorus inputs from non-point sources are concentrated in heavy rain episodes during the monsoon season. As a result, phosphorus concentration are higher in summer than in winter. TP loading from the watershed were estimated to be 0.9 gPm^(-2)yr(-1), which correspond to a boundary of the critical loading (1.0 gPm^(-2)yr(-1), which correspond to a boundary of the critical loading (1.0 gPm^(-2)yr(-1)) for eutrophication. From the results of the algal assay, both phosphous and nitrogen act as limiting nutrients in algal growth. The seasonal succession of phytoplankton community structure in Lake Juam was similar to that observed in other temperate lakes. Diatoms(Asterionella formosa and Aulacoseira granulata var. angustissima) were dominant in spring and winter, cyanobacteria (Microcystis aeruginosa, M. sp. and M. viridis) were dominant in warm season. The organic carbon, nitrogen and phosphorus content of lake sediment were 9.5~14.0 mgC/g, 1.01~1.82 mgN/g, and 0.51~0.65 mgP/g, respectively. The allochthonous organic carbon loading from the watershed and autochthonous organic carbon loading by primary production of phytoplankton were determined to be 1,122 tC/yr and 6,718 tC/yr, respectively. To prevent eutrophication of Lake Juam, nutrient management of watershed should be focus on reduction of fertilizer application, proper treatment of manure, and conservation of topsoil as well as point source.

        The limnological survey of Lake Okjong was conducted for one year from June 1993 to May 1994 on the monthly basis. The loading of phosphorus, nitrogen and organic carbon from the watershed into the lake were monitored at the main in flowing sites. Secchi disc transparency , epilimnetic chlorophyll a, total nitrogen, total phosphorus concentration and primary production were in the range of 1.3~4.H m, 2.4~ 18.7 mg Chl/m³. 1.25~2.87 mg N/l, 7~65 mg P/m³, 325~2,113 mg C/m²/day, respectively. TN/Tl atomic ratio varled from 129 to 443. N/P ratio decreased in summer because phosphorus concentration was higher than in winter, while nitrogen did not vary much. The seasonal succession of phytoplankton was distinct. In winter and spring, diatoms, Aulacoseira granulata var. angustissima and Aulacoseira italica were dominant while cyanobacteria, Microcystis sp.,M. ichthyovlabe, Phormidium sp. and P.valderianum var. tenuis were dominant in warm seasons. The dominant zooplankton species were Thertmocyclops taihokuensis in warm seasons while Boosmina longirostris were dominant in cold seasons. The organic carbon, nirtogen and phosphorus content of lake sediment were 8.0~14.8, 0.59~0.71, 1.14~1.87 ng N/g, respectively. The sediment of Lake Okjong can be classified as oligohumic based on C/N ratio. The total phosphorus loading from the watershed and fishfarm were estimated to be 2.7g P/m²/yr, which far exceeded the critical loading for eutrophication. The organic carbon loading from the watershed and primary production were determined to be 998t C/yr, 6,348t C/yr, respectively.       • Furfural 어류급독성 및 조직병리에 관한 연구

        Furfural, an organic solvent, is widely used as synthetic component material in producing chemical products. However, furfural has been reported that it shows strong toxicities to human being showing intense stimulus to skin, eyes, mucous membrane and nerve system. It is also known to cause anemia, liver cirrhosis, kidney failure and genetic toxicity in the human being working in the exposed area. LD50 of furfural for peritoneal injected mouse has been known around 20mg/kg, but the acute toxicity on aquatic organisms such as fish, daphnid or algae are not well known, compared to those on rodents. In this experiment, we studied on the fish toxicity of furfural using Japanese Meclaka (Oryzias latipes) and Common Carp (Cyprinus carpio). We also observed histological changes in the fish organs. The LC50s were 12.8 mg/L in Japanese Medaka and 21.8 mg/L in Common Carp, respectively. When Common Carps were exposed to 120 mg/L of furfural concentration for 30 minutes, blood congestion in gills and lysis of secondary lamella were shown. Though the muscle of caudal fin was not completely eroded, its epidermic cells were shown to be necrotic in various parts. Tissue atrophy and cell necrosis were also shown in the liver of Common Carps exposed to furfural. From these results, furfural seems to cause histological damages on liver, an internal organ as well as on external organs such as gills and fins eventhough the fish were exposed for a short-term.

      • 식물플라크톤의 세포외배출유기물을 고려한 소양호의 1차생산과 유기물 부하

        The autochthonous and allochthonous organic carbon loading were measured in Lake Soyang, to estimate the amount of carbon loading into the lake and the contribution of their sources to the lake's carbon loading. Autochthonous carbon loading was estimated from phytoplankton primary production with the extracellular organic carbon(EOC). Allochthonous loding was determined by measuring dissolved organic carbon(DOC) and particulate organic carbon(POC) concentration in the main inflowing Soyang River. Both autochthonous and allochthonous organic carbon loading were high during the summer, from July to September, and accounted for 43.2% and 71.7% of the annual loading, respectively. Primary productivity was elevated up to 1,000 mgCm^(-2)d^(-1)during summer and lowest in winter. EOC production from phytoplankton was also large in summer, resulting in a high DOC concentration in the lake water. Primary production of phytoplankton and allochthonous organic matter loading from the watershed contributed to 53.6% and 46.4% of total loading, respectively. The EOC production accounted for 4.4~21.2% of POC primary production, implying that EOC production of phytolankton must be considered in estimation of primary production.

      • 자동영상추적장치를 이용한 카드뮴 처리 Oryzias latipes의 행위독성연구

        Changes in certain fish behaviors are known to be very sensitive indicators of sublethal exposure to environmental contaminants. Therefore, behavioral toxicity tests, if properly designed, can be very useful to assess the influence of hazardous chemicals on fish. However, quantitative analysis of xenobiotic -mediated changes in locomotor behavior in fishes are rare, due mainly to the methodological difficulties. In general, fish movement has been known to be hypertrophic or hypotrophic according to the chemicals. As a study of fish behavioral toxicities, we quatified the swimming movement of Orydas latipes using computer-automated video tracking system. Oryzias latipes was exposed to cadmium of 128 mg/L for 1 hour in a limited aquaria, then the total swimming distance, the average swimming velocity, the histogram of turning angles, and the turning frequency were analyzed. Fish treated with cadmium showed decreased swimming activities, decreased velocity, and decreased turning frequency, which means hypotrophic activity. From these results, the computer-automated video tracking system of this study seems to be a good tool for the evaluation of the potential ecotoxicological studies.

        Cadmium, a major aquatic pollutant in many parts of the world, is toxic and readily accumulated in aquatic organisms. It mainly exists in water as complexes with organic ligands such as dissolved organic carbon and this complexation is known to have effects on the aquatic toxicities. In this study, acute toxicity, histology, and bioaccumulation were studied to evaluate effects of humic acid on cadmium toxicity and bioavailability to fish using Oryzias latipes and Cyprinus carpio. 96h-LC50 of cadmium was 6.38 ㎎/L using Oryzias latipes. However, the mortality showed the dose-dependent decrease when humic acid was added to the test solution. When fish were exposed to 5㎎/L of cadmium, histological changes in the exposed organs(caudal fins, gills, kidney, liver, and gut) of Cyprinus carpio were decreased by humic acid, especially in kidney and liver. Bioaccumulation of cadmium also decreased by treatment of humic acid. It seems that the formation of complexes between cadmium and humic acid may decrease bioavailability of cadmium to fish, and thus reduce the toxic effects of cadmium.

        불명열 환자에서 진단된 중증 만성 활동성 Epstein-Barr virus 감염증

        최근 장기적이고 심각한 임상 증상을 유발하는 만성 활동성 EBV 감염증의 개념이 정립되면서 그 증례들이 보고되고 있다. 만성 활동성 EBV 감염증은 소아나 젊은 성인들에게서 주로 발생되며, EBV-VCA IgG, EBV-EA IgG의 수치가 높게 관찰되고, 지속적인 발열, 전신쇠약감, 광범위한 임파선비대증, 간비장비대, 범혈구감소증, 다크론성감마병증을 나타낸다. 이 질환은 환자의 임상소견, 혈액학적 소견, EBV 항체 역가 및 침범 장기의 EBV genome 검출로 확진 되며 치료로 항 바이러스 체제, 항암 치료, 골수이식술 등이 시도되고 있으나 아직 검증되어 있지 않다. 저자들은 불명열을 주소로 내원하여 중증 만성 활동성 EBV 감염증으로 진단된 증례 4례를 경험하였다. 한 례에서는 항 바이러스 제제를 추여 후 임상적, 혈액학적 호전을 보였으나 치료 20일째 발생한 세균성 복막염과 패혈증으로 사망하였고, 두 례에서 총 6회의 CHOP 항암치료와 자가조혈모세포이식을 시행 받은 후 현재까지 증상 없이 추적관찰 중이다. 지속되는 발열을 주소로 내원한 불명열 환자들에게서 EBV에 대한 검사를 시행할 경우 불명열의 원인을 규명하는데 하나의 단서를 제공할 수 있을 것으로 사료된다. Epstein-Barr virus (EBV) is one of eight human herpesvirus. Primary infection with EBV in childhood is generally asymptomatic or mild, however, often causes overt diseases such as infectious mononucleosis (IM) and lymphoproliferative disorder (LPD), the latter occursing in immunologically compromised individuals. Historically, EBV has been considered to be etiologically linked to human malignancies such as EBV genome positive Burkitt's lymphoma and nasopharyngeal carcinoma. Recently, however, another category of EBV-related disease, "chronic active EBV infection", has been made to describe persons without a clearly defined underlying disease. We report 4 cases of patients, presented with episodic fever of unknown origin (FUO), who were diagnosed as severe chronic active EBV infection. A combined application of serology for EBV and in-situ hybridization established the diagnosis of the EBV infection.

      • Compact-size and High-Q Meanderline Resonator for ITS Application

        A A compact and high quality factor (QL) meanderline resonator for intelligent transport system (ITS) application on low dielectric substrate is presented. In order to get high QL and frequency responses, the resonator is connected directly to feeding line which is designed to operate at 5.8 GHz. The simulation result of the designed bandstop type resonator shows the QL value of 201. The dimension of the resonator is 1.85 x 1.59 mm2.
