        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

      • 공침법에 의한 BiDy₂Fe_(5)O_(12) 결정형 나노입자 분말의 제조 및 응용 연구

        금속산화물들은 각종 산업재료로서 유용하며 더욱이 그것들의 결정형 나노입자를 경제적으로 제조하는 기술은 21세기 첨단산업에 매우 중요하다. 저자들은 前報에서 화학합성법에 의한 각종 금속산화물의 나노입자를 제조하는 표준공정법을 연구하여 보고하였다. 본 연구에서는 그 표준공정법을 이용하여, 가넷의 일종인 BiDy_(2)Fe_(5)O_(12) 결정 나노입자 분말을 제조하고 그것의 자성체로의 응용을 위해 제품특성을 조사하었다. 가네트 제조과정에 필요한 침전물은 제품설계에 의거 몰비로 계산된 Bi, Dy, Fe의 질산염들의 혼합수용액에 침전제로서 pH NH_(3)-NH_(4)CI 완충용액 일정량을 가하여 얻어졌다. 이렇게 제조된 침전은 열처리과정을 거처 BiDy_(2)Fe_(5)O_(12) 결정형 나노입자 분말로 만들어졌다. 얻어진 제품의 입자크기, 조성, 결정구조 및 광자기성 등 특성시험이 SEM, ICP, XRD, 그리고 VSM 등을 사용하여 실시되었다. 각종 분석법을 사용하여 제품의 특성을 조사한 결과, 최초에 설계한 대로의 자성체가 나노 크기의 분말로 얻어졌으며 이것을 통하여 전보에서 확립한 표준공정법의 유효성도 입증되었다. Metallic oxides are useful as various industrial materials. Futhermore, the cost-effective techniques of fabricating the crystalline nanopartides of metallic oxides are very important for the high-tech industries of the 21 century, In the previous study, the authors reported the standard process for the preparation of crystalline nanoparticles of metallic oxides by chemical synthesis, In this study, BiDyzFesOiz nanoparticles were prepared by using the reported standard process and the characteristics of the product were investigated for applicability as magnetic substance. The precipitation needed for manufacturing process of garnet was made by adding the NH_(3)-NH_(4)CI buffer solution of pH 10 to the mixed solution of bismuth (III) nitrate, dysprosium(III) nitrate and iron(III) nitrate. The amount of each nitrate was calculated by the designed mole ratios. The nano-size crystalline powders of BiDy_(2)Fe_(5)O_(12) were earned by heating the precipitation at 800℃ for 4 hours in air. The characterizations, such as particle size, composition, crystal structure and magneto-optical property of garnet were tested by SEM, ICP, XRD, and VSM analyses. The results of the analyses showed the following two facts. First, a good magnetic substance of BiDy_(2)Fe_(5)O_(12) was earned as crystalline nano-size powders according to the original design. Next, the established standard process for the fabrication of METALLIC oxides nanonartieles was approved as a very effective method.

      • CO₂아크 용접 근로자의 뇌 MRI 고신호강도 3례

        We experienced three cases of high signal intensity observed by Brain Magnetic Resonance Imaging in CO₂ are welders of steel-frame manufacturing industry. Case 1 was a 35 years old man who has been an CO₂ arc welder for 10 years, admitted a sudden onset of tonic clonic seizure. He complained fever, chilling and myalgia since 3 days before admission. On admission, in the test of manganese exposure indices, manganese concentrations of blood, urine and scalp hair were 5.17 ㎍/dl, 22.00 ㎍/l and 31.25 ppm respectively. Case 2 was a 35 years old man who has been an CO₂ arc welder for 20 years. On admission, He complained fatigue, numbness and weakness of extremities, and decrease of libido. In the test of manganese exposure indices, manganese concentrations of blood, urine and scalp hair were 6.34 ㎍/㎗, 14.62 ㎍/l and 57.87 ppm respectively. In neurologic examination, Palmo-mentle reflex and Myerson sign were observed. Case 3 was a 33 years old man who has been an CO₂ arc welder for 16 years. On admission, He complained loss of appetite, numbness of extremities, fatigue and decrease of attention. In the test of manganese exposure indices, manganese concentrations of blood, urine and scalp hair were 5.14 ㎍/㎗, 13.79 ㎍/l and 50.08 ppm respectively. In neurologic examination, Myerson sign was observed. In brain magnetic resonance imaging, T1WI showed symmetrical high signal intensity in basal ganglia and midbrain of three cases. Authors argued that they were developed by manganese exposure, and we considered that follow up study would be necessary.

        외상성 비장 손상 5 2예의 임상적 고찰

        Backgrounds : A total splenectomy was considered as the standard method to management of splenic injury. However, recently, splenic preservation has been emphasized because of the spleen ’s important immunologic function, especially in the young. This study was conducted to evaluate the safety and the indications of non-operative management of traumatic splenic injury. Methods : The medical records of 52 patients who were managed for traumatic splenic injury between Jan. 1, 1990, and Jun. 30, 1999, were evaluated. The clinical manifestations, the mecha-nism of injury, the diagnostic method, the grade of injury, the initial management, the initial treatment mode, treatment course, and complications were investigated. Results : Thirty(30) patients were initially treated medically with transfusions and intensive monitoring. A delayed operation was required in 9 patients (30%) who suffered a severe splenic injury more than grade III. Twenty-one(21) of the 30 patients (70%) who suffered a splenic injury less than grade III were successfully managed non-operatively. Nineteen(19) of the 22 patients (86%) who required an initial operation suffered a severe splenic injury more than grade III. There were differences between the non-operative and the operative groups in transfusion amount (1.1 vs 6.5 pints) and in hospital stay (14.8 vs 30.9 days). Conclusions : Non-opeartive management of traumatic splenic injury will be successful in patients who are hemodynamically stable with adequate resuscitation and a small amount of transfusion and who have suffered injuries below grade III.

      • 편성조직과 게이지에 따른 면 위편성물의 구조적 특성변화

        The scoured and undyed 100% cotton yarn (2/30's) were weft-knitted from 13 different stitches with 7 and 12 machine gauge using a V-bed knitting machine. Structure of knit fabrics was characterized and analyzed: fabric length and width, fabric count, fabric weight per area, and fabric thickness were measured. The thickness of the full cardigan 2✕2 and 4✕4 rib presented the highest values due to usage of tuck stitch. The fabric width of 4✕4, 1✕1, and 2✕2 rib stitches were the narrowest group resulted in relaxation shrinkage after knitting. Also 4✕4rib stitch gave the highest value in fabric density and weight. Fabric stitch and structure were well co-related each other, which give the changes of fabric appearance and properties.

        This investigation was on the prediction of the drop size and drop formation mechanism with pulse in the liquid solvent extraction column. The experiment was carried out by encapsulation technique(hexamethylene diamine) on the variations of pulse velocity, pulse frequency and pulse amplitude for the kerosene-sodium hydroxide system. As a result, the drop size formed on the distributor tended to decrease with the increase of pulse velocity. And the drop formation mechanism and drop size in the pulsing system was predicted by means of corrected Chazal's Momentum Balance Model.

      • 액화 LPG 다중분사 시스템을 가진 차량의 개발

        The two aspects of design are that we have developed a liquid-phase LPG port injection system and it includes a fuel rail pressure control system designed to solve the hot start difficulties associated with liquid-LPG injection. The injection and the spark timing are controlled by programmed electronic management unit. To achieve the goal of ULEV emission, the engine was operating in condition of stoichiometric fueling for all conditions, air/fuel ratio control around a slightly rich set point with closed-loop feedback from a heated oxygen sensor, ceramic-coated exhaust manifolds plus a close-coupled LPG specific catalyst for rapid catalyst light-off and advantage of port injection system compare to upstream LPG addition. In this paper, we report the result that we can have more torque, fuel economy and drive ability than those of the production gasoline vehicle while having much lower emissions.

      • 정전탐침과 레이저 단층 촬영에 의한 예혼합 난류화염의 구조

        The structure of premixed turbulent flames in a constant-volume vessel was investigated by using the electrostatic-probe method and the laser tomography. The eletrostatic-probe method, which perceives a flamelet by detecting its flame potential signal was used to measure the flame propagation speed, the thickness of flame zone and the number of existing flamelets. And the laser tomography was used to visualize the deeper structure of flames, where the laminar burning velocity and the turbulence intensity where varied independently. A laser sheet, 200 micrometers and 2.2cm wide, was passed through the turbulent flame, then 2-D cross section of the flame front was obtained by the intensity variation of the scattered light corresponding to the change in the gas density. As a result, at least on the 2-D cross section, reactant islands were found in a strong turbulence field.
