        A 57-year-old woman who was suffered from chronic back pain and radiating pain to left leg visited the local pain clinic. An accidental dural puncture happened at the L4-5 intervertebral space during the procedure with the loss of resistance technique using air. The epidural needle was removed, and a new insertion was done at the L3-4 intervertebral space with administration of the 0.5% lidocaine 8 ml mixed triamcinolone 40 mg. On the next day, she visited again the local pain clinic with complaining of headache. During an epidural autologuous blood patch procedure, she complained of a severe headache, nausea, vomiting, and developed generalized tremor of abrupt onset. There was no evidence of a dural puncture during the procedure. She was referred to our hospital at once. A brain CT scan showed air-filled cavities in basal cistern. She was given conservative treatment during 14 days. After 10 days, follow up brain CT scan showed no air-filled cavity in basal cistern. She was discharged from our hospital without complications.

        Critical Temperatures for Initiating and Arresting Delayed Hydride Cracking in a Zr-2.5Nb Pressure Tube

        <P>The hydrogen concentration limit and critical temperatures for a delayed hydride cracking (DHC) in zirconium alloys have been reanalyzed using Kim’s DHC model that a driving force for DHC is not the stress gradient but the supersaturated hydrogen concentration or ∆C arising from a hysteresis of the terminal solid solubility on a heating and on a cooling. The DHC initiation occurs generally at the temperatures corresponding to the terminal solid solubility for precipititation (TSSP), demonstrating that the supercooling from the terminal solid solubility for dissolution (TSSD) is required to initiate the DHC. The DHC arrest temperatures correspond to the temperatures where the ∆C is reduced to zero. Therefore, we conclude that the ∆C is the driving force for the DHC and that the Kim’s DHC model is feasible.</P>

        노치 형상 및 냉각속도에 따른 Zr-2.5Nb 압력관의 DHC거동

        The objective of this study is to investigate the delayed hydride cracking (DHC) velocity and the incubation time for the water-quenched and furnace-cooled Zr-2.5Nb tube with the different radius of a notch tip. DHC tests were carried out at constant K_1 of 20 MPa√m and 250℃ on the cantilever beam (CB) specimens subjected to furnace cooling or water quenching after electrolytic charging with 57 or 72 ppm hydrogen, respectively. An acoustic emission sensor was attached to the CB specimens to detect the incubation time before the start of DHC. The shape of the notch tip changed from the fatigue crack to the dull crack with its radius ranging from 0.1 to 0.15 mm. The DHC incubation time increased remarkably with the increasing radius of the notch tip, which appeared more strikingly on the furnace-cooled CB specimens than on the water-quenched ones. However, both furnace-cooled and water-quenched CB specimens indicated little change in the DHC velocity with the radius of the notch tip unless their notch tip exceeded 0.125mm. These results demonstrate that the uncleation rate of hydrides at the notch tip determines the incubation time and the DHC velocity becomes constant after the concentration of hydrogen at the notch tip reaches the terminal solid solubility for dissolution (TSSD). This observation agrees well with Kim's DHC model. A difference in the incubation time and the DHC velocity between the furnace-cooled and water-quenched specimens is discussed in terms of the uncleation rate of hydrides at the notch tip and the hysteresis of hydrogen solubility.

        Amitriptyline 과 Desipramine, Paroxetine 은 백서의 Schwann 세포 일차배양에서 산화질소의 생성을 억제한다

        Background : Antidepressants are being used as supplemental therapy in neuropathic and inflammatroy pain. The mechanism of their inhibitory effect on experimental animal inflammation is not clear. Studies during the past few years clearly indicate an important role for nitric oxide (NO) in the inflammation and pain-processing system. We evaluated the effects of amitriptyline, desipramine and paroxetine on NO production in primary Schwann cell cultures. Methods : Primary cultures of the Schwann cell were prepared from dorsal root ganglia of 1-to 3-day old Spraque-Dawley rats. Schwann cells were cultured in the presence or absence of interferon-?? (500 ng/㎖) plus tumor necrosis factor-α (500 ng/㎖), amitriptyline, desipramine or paroxetine. Production of NO was determined in the supernatant of the culture media. Results : Amitriptyline (10 ㎍/㎖), desipramine (10 ㎍/㎖) and paroxetine (10 ㎍/㎖) inhibited NO release by 29.8%, 51.4%, and 66.8%, respectively. No drug had a toxic effect on cultured cells, which was determined by an LDH assay. Conclusions : Inhibition of NO production by Schwann cells may be a mechanism by which some antidepressant medications affect inflammatory and neuropathic pain.         The hydrogen concentration limit and critical temperatures for a delayed hydride cracking (DHC) in zirconium alloys have been described on the assumption that a tensile stress gradient is a driving force for the DHC. By correlating the DHC initiation and arrest temperatures with the critical supersaturated hydrogen concentration or △C for the DHC initiation and arrest, we conclude that the △C arising from the hysteresis of the terminal solid solubility of hydrogen on a heat-up and on a cool-down is the driving force for the DHC and that the Kim's DHC model is feasible.

        Delayed Hydride Crack Velocity of Zirconium Alloys with the Direction of an Approach to Temperature

        <P>The delayed hydride cracking (DHC) tests were conducted on Zr-2.5Nb compact tension specimens with the test temperatures reached by a heating and a cooling. The Zr-2.5Nb specimens were either furnace-cooled or water-quenched after a hydrogen charging treatment to contain 10 to 100 ppm H. On an approach to the test temperatures by a cooling, both the Zr-2.5Nb specimens showed the DHC velocity increasing with an increasing temperature over a temperature range of 100–300°C, irrespective of the cooling rate. However, on an approach to the test temperatures by heating, the furnace-cooled Zr-2.5Nb showed a DHC arrest at temperatures over 180°C and no DHC at 250°C, and the water-quenched ones did have a DHC growth, even at 250°C. Using Kim’s DHC model we elucidate the DHC arrest in the furnace-cooled Zr-2.5Nb at temperatures over 180°C and the DHC growth in the water-quenched specimen, even at 250°C, upon an approach by a heating.</P>

        In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

        초음파공명분광법에 의한 Zr-2.5Nb 압력관 재료의 고온 이방성 탄성계수 측정

        중수로 압력관으로 사용되는 Zr-2.5Nb 재료의 고온 이방성 탄성계수를 고온 초음파공명분광법(RUS)로 측정하였다. 소형 가열로 내에 알루미나 waveguide와 광대역 초음파 센서를 사용하여 초음파에너지를 시편에 가하고 시편의 공명주파수를 측정하였다. 압력관의 축 방향, 반경 방향, 원주 방향에 일치하도록 장방형 시편을 가공하였으며 각 방향에 대한 탄성계수 텐서 9개의 값을 상온 ∼ 500℃ 온도 구간에서 측정하였다. 탄성계수 텐서, C_ij는 모두 온도가 증가함에 따라 점진적으로 감소하며 원주 방향의 탄성계수가 축 방향 및 반경 방향의 탄성계수보다 높았다. 이것은 Young's modulus 나 shear modulus 의 경우에도 일치하였으며 축 방향과 반경 방향의 경우 큰 차이를 나타내지 않았다. 축 방향 및 반경 방향의 비틀림 탄성계수가 150℃ 부근에서 서로 교차하였으며 이는 단결정 지르코늄의 C_44 및 C_66의 교차 현상과 일치하였다. Anisotropic elastic constants of Zr-2.5Nb pressure tube materials were determined by a high temperature resonant ultrasound spectroscopy (RUS). The resonant frequencies were measured using alumina wave guides and wide band ultrasonic transducers in a small furnace. The rectangular parallelepiped specimens were fabricated along with the axial, radial and circumferetial direction of the pressure tube. A nine elastic stiffness tensor for orthotropic symmetry was determined in the range of room temperature ∼500℃. As the temperature increase, the elastic constant tensor, C_ij gradually decreases. Higher elastic constants along the transverse direction compared to those along the axial or radial direction are similar to the case of Young's modulus or shear modulus. A crossing of shear elastic constants along axial direction and radial direction was observed near 150℃. This fact corresponds to the crossing of c44 and c66 single crystal zirconium.

      • Pristinamycin(Pyostacine®)의 methicillin-resistant Staphylococcus aureus,Penicillin-resistant Streptococcus pneumoniae, Haemophilus influenzae 임상균주에 대한 시험관내 항균력

        목적 : 국내 및 전세계적으로 methicillin-resistant Staphylococcus aureus (MRSA), penicillin-resistant Streptococcus pneumoniae(PRSP), β-lactamase 생성 H. influenzae 등의 항균제 내성 감염증이 증가되고, 이에 대한 효과적인 항생제 요법에 대한 필요성이 증가되고 있다. Streptogramin 계열의 경구 항생제인 pristinamycin의 임상검체에서 분리된 균주에 대한 시험관내 항균력을 검사함으로서 이러한 감염증에 대한 임상적 사용의 유용성여부를 알아보고자 하였다. 방법 : 1999년 8월1일부터 2000년 1월 1일까지 호흡기 감염, 피부 및 연조직 감염환자로부터 분리된 MRSA 60주, PRSP 50주, H. influenzae 40주를 대상으로 pristinamycin을 비롯한 8-9종 항생제의 시험관내 항균제 감수성을 측정하였다. 결과 : Pristinamycin은 MRSA에 대한 MIC 범위가 0.125-1㎎/L로서 100%의 감수성을, PRSP에 대한 MIC 범위가 0.125-1㎎/L로서 100%의 감수성을, β-lactamase를 생성하는 H. influenzae에 대해서도 MIC 범위가 0.25-2㎎/L, 96% 감수성으로서 다른 항생제들에 비하여 우수한 시험관내 항균력을 나타내었다. 결론 : 국내에서 문제가 되고 있는 MRSA, PRSP, β-lactamase 생성 H. influenzae에 대하여 pristinamycin은 우수한 시험관내 항균력을 나타내어, 이들 항생제 내성균들에 의한 감염증의 치료에 효과적으로 사용될 수 있을 것으로 생각된다. Background: Pristinamycin(Pyostacine®) is a natural antibacterial combination of compounds Produced by Streptomyces pristinaespiralis. It has been marketed in European countries over 30 years as an oral anti-stphyhcoocal agent. Because of the increasing prevalence microorganisms that are resistant to multiple antimicrobial agents in Korea. there are demand of noble antibiotics like pristinamycin against these resistant organissms. Methods The one-hundred and fifity bacterial strains which were collected from Korea University Guro hospital included sixty methicillin-resistant S. aurous (MRSA) , fifty penicillin-resistant S. pneumoniae (PRSP) and forty H. infuenzae clinical isolates. Using an microbroth dilution or agar dilution technique, we compared the antimicrobial activities of pristinamycin and other usual antibiotics against the studmicroorganisms. Results: All sixty MRSA strains were multiply resistant to other antimicrobial agents, but 100% of MRSA strains were susceptible to pristinamycin (MIC range: 0.125-1㎎/L). All of fifty PRSP strains were multiple resistant to other antimicrobial agents but 100% PRSP strains were susceptible to pristinamycin (MIC range: 0.125-1㎎/L). Twenty-seven strains of forty H. influenzae strains were β-lactamase positive, and 95% (38/40) of H. influenzae strains were susceptible to prisitinamycin (MICrange : 0.015-4㎎/L) . Conclusion : Pristinamycin demonstrated the highly active. in vitro antimicrobial activities against MRSA. PRSP. and H.influenzae isolated so it might be an effective oral therapeutic agent for treating patients with mild infection due to those organisms or on the outpatient setting.
