Background: Sugammadex is a novel neuromuscular reversal agent, but its associated hypersensitivity reaction and high cost have been obstacles to its widespread use. In the interest of reducing the necessary dosage of sugammadex, the reversal time of the combined use of sugammadex and neostigmine from moderate neuromuscular blockade were investigated. Methods: The patients enrolled ranged in age from 18 to 65 years old with American Society of Anesthesiologists class 1 or 2. The subjects were randomly assigned into one of the four groups (Group S2, S1, SN, and N; n = 30 per group). The reversal agents of each groups were as follows: S2 - sugammadex 2 mg/kg, S1 - sugammadex 1 mg/kg, SN - sugammadex 1 mg/kg + neostigmine 50 μg/kg + glycopyrrolate 10 μg/kg, N - neostigmine 50 μg/kg + glycopyrrolate 10 μg/kg. The time to recovery of the train-of-four (TOF) ratio was checked in each group. Results: The time to 90% recovery of TOF ratio was 182.6 ± 88.9, 371.1 ± 210.4, 204.3 ± 103.2, 953.2 ± 379.7 sec in group S2, S1, SN and N, respectively. Group SN showed a significantly shorter recovery time than did group S1 and N (P < 0.001). However, statistically significant differences between the S2 and SN groups were not be observed (P = 0.291). No hypersensitivity reactions occurred in all groups. Conclusions: For the reversal from rocuronium-induced moderate neuromuscular blockade, the combined use of sugammadex and neostigmine may be helpful to decrease the recovery time and can also reduce the required dosage of sugammadex. However, the increased incidence of systemic muscarinic side effects must be considered.
스마트 사이니지에 활용이 가능한 사용자 거리별 영상 제어기술 연구

광고 및 홍보를 위하여 현재 많이 사용하고 있는 사이니지 기술은 대형건물 옥상이나, 도로 주변에 설치되고 있으며, 그에 따른 광고효과를 가지게 되었다. 또한 많은 곳에 설치되고 운용되고 있고 상시 광고 영상을 제어하기 때문에 에너지을 많이 사용하여 경제적으로 많은 비용이 들어가게 되었다. 스마트 사이니지에 사용자와 인터렉션이 가능한 사이니지 기술 개발이 필요하다. 따라서 본 논문에서는 스마트 사이니지에 활용이 가능한 사용자 거리별 영상 제어 기술을 제안한다. 또한 사용자 인지에 따라 콘텐츠를 제어하거나디스플레이를 제어하는 기술을 제공하고자 한다. 이에 대한 기술을 설계하고 이를 구현하여 성능을 검증하였다 Recently, Signage technology is widely used for advertisements and promotional announcements located on the rooftop or around large buildings and near the roads as well. In addition in many places it is installed and operational financially costly because it uses a lot of energy to control the advertising images. Smart Signage technology developed interaction between the user and the possible need in signage. In this paper, we propose a specific user distance image control technologies that can be used in smart signage. Also, to control the content according to the user whether or provide a technique for controlling the display. Designing technology for it, and the performance was verified by implementing them.
전기경련요법에서 remifentanil이 혈역학에 미치는 효과

Objective: Electroconvulsive therapy (ECT) is often associated with transient hypertension and tachycardia. Acute hemodynamic change may produce serious complications. Remifentanil is a potent ultra short acting opioid and has been found to reduce blood pressure and heart rate. The goal of this study was to evaluate effects of remifentanil on seizure duration, recovery parameters and hemodynamic changes during ECT. Methods: Ten ASA Ⅰ, Ⅱ patients enrolled in this prospective, randomized trial, each receiving more than four electroconvulsive therapy. Patients were randomized to receive placebo, group P (propofol 1mg/kg+0.9% normal saline 1ml, n=10), and propofol with remifentanil, group R (propofol 1mg/kg+remifentanil 1ug/Kg n=10). Supplemental dose of propofol was given as required to achieve loss of consciousness. We recorded hemodynamic parameters, seizure duration, and recovery parameters. Results: Seizure duration and recovery parameters were similar in group R and group P. Blood pressure and Hear rate after ECT were significantly lower in group R. Conclusions: Remifentanil(1ug/kg) attenuated the acute hemodynamic response to ECT under propofol anesthesia without adversely affecting the duration of seizure activity and recovery profile. Adjunctive use of remifentanil may be good alternative regimens for ECT.
