Research topics: This study analyzes the factors influencing the decline in nurses' perceived quality of medical services. Research background: This study aims to analyze how nurses' perceptions of staffing, workload, and work appropriateness affect their evaluation of the medical service quality within their affiliated medical institutions. Differences from prior research: While previous studies on the perception of the quality of medical services focused on nurses in specific medical institutions, such as nursing hospitals, this study offers a comprehensive perspective and analysis from nurses across various medical institutions. Research method: We conducted a regression analysis and mediating effect analysis on 19,194 third-shift nurses using the 2022 data from the "Survey on Working Conditions at Medical Institutions,” which is conducted annually by the "Korean Health & Medical Workers’ Union(KHMU)." This study examined "changes in medical service quality" as the dependent variable, “manpower level” as the key explanatory variable, and "work intensity" and "work appropriateness" as the mediating factors. Research results: Staffing shortages in medical institutions positively affect the perceived decline in medical service quality, suggesting a causal relationship. Additionally, negative perceptions of overall work evaluation, such as work intensity and work appropriateness, were found as mediators between perceived labor shortage and declining medical service quality. Contribution points and expected effects: Drawing from a survey of nearly 20,000 nurses in diverse medical fields, this study is expected to contribute to enhancing medical service quality by examining the influences of major variables affecting the decline in medical care.
팬데믹이 간호노동 수행과정에서 우울감에 영향을 미치는 요인 분석 - 코로나19 업무 경험, 직무소진, 의료기관 대응 인식 간의 효과를 중심으로

This study aimed to understand what factors contributed to the worsening of depression in the nursing work process during the COVID-19 pandemic. For this purpose, we examined the mechanisms through which COVID-19-related work experiences, job burnout, and the response system established by medical institutions collectively promoted nurses' depression levels. The results showed that job burnout had a significant positive effect on depression levels, as did COVID-19-related work experience. In addition, perceptions of the responding level of medical institutions in terms of management communication and worker protection measures in the face of the COVID-19 outbreak had a significant effect on nurses’ depression levels. The order of influential effect of key independent variables revealed through multiple regression analysis were: job burnout > lack of labor protection measures > COVID-19-related work experience > lack of management communication. We also found that job burnout had a mediating effect between COVID-19-related work experience and depression levels, and that poor management communication and lack of worker protection measures at medical institutions had a moderating effect between job burnout and depression levels. This study is meaningful in that it comprehensively and multidimensionally identified the causes of worsening psychological depression in the nurses’ working process in the face of the COVID-19 pandemic. However, given the methodological limitations of cross-sectional studies and the possibility that a sufficient sample number of nurses who experienced severe situations during the early stages of the pandemic may not have been included, complementary follow-up studies that overcome the limitations of this study are needed.

We report a case of doxycycline-induced phototoxicity in a 62-years-old male. The patient, had erythematous macules and patches on sun-exposed areas. A Phototest revealed a marked decreased minimal erythemai. dose (MED) to UVA (MEDuva=-10J/cm2). Photopatch tests with 1%, 5% Doxycycline ointment were negative. An oral provocation test was performed, which showed a positive result with a decrease of MED to UVA (MEDuva= 20J/cm2). After the cessation of doxycycline, his skin lesioris were improved markedly with complete loss of photosensitivity. (Korean J Dermatol 1997;35(5): 997-1000)
