        Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

        노인운동의 변화단계에 따른 의사결정 균형에 대한 연구

        Purpose : This study was carried out to identify the factor of decisional balance for exercise. This was composed of a set of variables at the level of decision making when the elderly decide to do exercise. It was used to classify and identify the characteristics of the stages of change for exercise to which the elderly belong, and was used to identify the variables of decisional balance which influence the stage of change for exercise in the elderly. Methods : Convenient samples of 198 subjects over age 60 in Seoul(mean age=70) were selected from community living, mentally competent older adults and the data was collected from April 1, 1999 to May 31, 1999. The research instrument was the Decisional Balance Measure for Exercise(Marcus & Owen., 1992), Stage of Change Measure(Marcus et al, 1992). The data was analyzed by SAS Program. Results: 1. According to stage of change measure, without missing data, 191 subjects were distributed in each stage of change for exercise : 50 subjects(26.1%), 7 subjects(3.6%), 52 subjects(27.2%), 4 subjects(2%), and 78 subjects(40.8%) belonged to the pre-contemplation stage, contemplation stage, preparation stage, action stage and maintenance stage. 2. Factor analysis identified 3 factors of decisional balance as appropriate factors for excise of the elderly and named by researchers; 1)'Perceived Physical-psychological benefit', 2)'Perceived Physical-psychological burden', and 3)'Perceived time burden'. 3. The analysis of variance showed that the two components Perceived Physical - psychological benefit(F=45.95, p=.0001), and Perceived Physical-psychological burden(F=26.52, P=.001) were significantly associated with stage of change. 4. Through the discriminant analysis, it was found that both 'Self Perceived Physical-Psychological benefit' and 'Perceived Physical-Psychological burden' were the influential variables in discriminating the three stages of change(pre-contemplation, preparation, and maintenance). Conclusion: Results are consistent with the application of the Transtheoretical model, which has been used to understand how people change health behaviors. Even though this study is a cross-sectional, not a longitudinal study, the findings of this study give useful information for exercise intervention about especially the factors relating to decision making for exercise of the elderly in the different stages of change of exercise.

        Causes and Predictive Factors Associated with "Diagnosis Changed" Outcomes in Patients Notified as Tuberculosis Cases in a Private Tertiary Hospital

        Background: The aim of our study was to evaluate the "diagnosis changed" rate in patients notified as tuberculosis (TB) on the Korean TB surveillance system (KTBS). Methods: A total of 1,273 patients notified as TB cases on the KTBS in one private tertiary hospital in 2011 were enrolled in the present study. Patients were classified into three groups: "diagnosis maintained", "diagnosis changed" (initially notified as TB, but ultimately diagnosed as non-TB), and "administrative error" (notified as TB due to administrative errors). Results: Excluding 17 patients in the "administrative error" group, the "diagnosis maintained" and "diagnosis changed" groups included 1,097 (87.3%) and 159 patients (12.7%), respectively. Common causes of "diagnosis changed" were nontuberculous mycobacterial (NTM) disease (51.7%, 61/118), and pneumonia (17.8%) in cases notified as pulmonary TB, and meningitis (19.5%, 8/41) and Crohn's disease (12.2%) in cases notified as extrapulmonary TB. Being older than 35 years of age (odds ratio [OR], 2.18) and a positive acid-fast bacilli stain (OR, 1.58) were positive predictors and a TB-related radiological finding (OR, 0.42) was a negative predictor for a "diagnosis changed" result via multivariate logistic regression analysis in pulmonary TB cases. Conclusion: Because of a high "diagnosis changed" rate in TB notifications to the KTBS, the TB incidence rate measured by the KTBS may be overestimated. Considering the worldwide trend toward increased NTM disease, the "diagnosis changed" rate may increase over time. Thus, when reporting the annual TB notification rate in Korea, the exclusion of "diagnosis changed" cases is desirable.

      • 産學蓮繫에 依한 醫學技術敎育 改善方案에 關한 硏究

        The field of medical laboratory science, like many other allied health professions, is currently experiencing a number of noteworthy changes. One of the obvious changes is the multitude of new tests and instruments being rapidly introduced into the practice of laboratory sciences. As results of these technological innovations, the duties and responsibilities of laboratory manpower have also been changed. Requisition to the development of competency based curicula in the medical laboratory science is the translation of competency statement into terminal performance objectives so that the achievment of career-entry competence can be measured. In order to study laboratory tasks in depth, and to provide data for logically reconstructing job roles, a task inventory based on professional practices for their respective areas of the clinical laboratory had been investigated. Results of this task analyses could be in identifying objectives necessary to develop a task-oriented curriculum. The implications of the result of this study are summarized in the following. 1. For the purpose of updating and practicing curriculum for medical technology established curriculum total credit hours should be increased within the limits of the possibility. 2. The curriculum should provide opportunities for students to apply theory in to practice through correlated and supervised instiution in clinical practice areas. 3. The teaching program must be well organized on the educational curriculum comprised of humanities and related science subjects. 4. While the student/instructor ratio in the medical laboratory sciences courses should be appropriate more balanced ratio is emphasized. 5. Based on the data studied, it can be concluded that some major courses are lacking its uniformities. 6. It should ascertained that the affiliation between academic institutions and hospital facilities must be compulsory for enhancing competency oriented education.

        Ecological comparison of Mongolian oak (Quercus mongolica Fisch. ex Ledeb.) community between Mt. Nam and Mt. Jeombong as a Long Term Ecological Research (LTER) site

        Species composition, frequency distribution of diameter classes, species diversity, and stem vitality of woody plants were analyzed in a Mongolian oak (Quercus mongolica Fisch. ex Ledeb.) forests in permanent quadrates of Mt. Nam and Mt. Jeombong, which were installed for Long Term Ecological Research (LTER). The principal objective of this study was to clarify the ecological characteristics of both sites by comparing the Mongolian oak communities established in Mt. Nam surrounded by urban area and in Mt. Jeombong as a natural area, to accumulate the basic data for long-term monitoring, and furthermore to predict possible changes in vegetation due to climate change. The species composition of the Mongolian oak community on Mt. Nam differed from that of Mt. Jeombong. Such differences were usually due to Sorbus alnifolia, Styrax japonicus, Oplismenus undulatifolius, Ageratina altissima and so on, which appeared in higher coverage in Mt. Nam. Species diversity of the Mongolian oak community in Mt. Nam was lower than that in Mt. Jeombong. This result was attributed to the fact that the Mongolian oak community in Mt. Nam is under continuous management and was dominated excessively by S. alnifolia, and S. japonicus, which were originated from artificial interference and chronic air pollution. As the results of analyses on the frequency distribution of diameter classes of major tree species and the transitional probability model based on Markov chain theory, the Mongolian oak community in Mt. Nam showed a possibility of being replaced by a S. alnifolia. Considering that this replacement species is not only a sub-tree but is also shade-intolerant, such a successional trend could be interpreted as a sort of retrogressive succession. The Mongolian oak community established in Mt. Jeombong differed from the community in Mt. Nam in terms of its probability of being continuously maintained.

        Roles of Na<SUP>⁢</SUP>-Ca<SUP>2⁢</SUP> Exchange in the Negative Force-Frequency Relationship

        <P> Frequency-force relationships (FFR) were studied in electrically field stimulated rat left atria (LA) by reducing the stimulation frequency from resting 3 Hz to test frequencies (0.1-1 Hz) for 5 minutes. The twitch amplitudes of LA elicited the typical negative staircases with 3-phased changes: the initial rapid increase, the second decrease and the following plateau at test frequencies. Verapamil (3⁓10<SUP>⁣5</SUP> M) pretreatment elicited frequency-dependent suppression of the twitch amplitudes, exaggerating the negative staircase. Monensin pretreatment enhanced not the peak but the plateau amplitudes in a concentration-dependent manner. When the Na<SUP>⁢</SUP>-Ca<SUP>2⁢</SUP> exchange was blocked by Na<SUP>⁢</SUP> and Ca<SUP>2⁢</SUP> depletion in the Krebs Hensleit buffer (0 Na<SUP>⁢</SUP>-0 Ca<SUP>2⁢</SUP> KHB), the twitch amplitudes increased in a frequency-dependent manner, changing the negtive staircase into the positve one. Meanwhile, the 0 Na<SUP>⁢</SUP>-0 Ca<SUP>2⁢</SUP> KHB applicationinduced enhancement was strongly suppressed by caffeine (5 mM) pretreatment. Only dibucaine among the local anesthetics increased the basal tone during frequency reduciton. There were no differences in <SUP>45</SUP>Ca uptakes between 0.3 Hz and 3 Hz stimulation except at 1 min when it was significantly low at 0.3 Hz than 3 Hz, illustrating net Ca<SUP>2⁢</SUP> losses. Monensin pretreatment enhanced the rate of this Ca<SUP>2⁢</SUP> loss. Taken together, it is concluded that Na<SUP>⁢</SUP>-Ca<SUP>2⁢</SUP> exchange extrudes more SR released Ca<SUP>2⁢</SUP> out of the cell in proportion to the frequency, resulting in the negative rate staircase in the rat LA.
