Background: The purpose of this study was to document the incidence and clinical characteristics of patients with tuberculosis (TB) in combat and auxiliary police, living in a group, in Korea where the incidence rate of active TB in a general population is higher than in Western countries. Materials and Methods:We retrospectively reviewed the medical records of all tuberculosis patients diagnosed at National Police Hospital from January 2002 through December 2004. Results:In 2002-2004, a total of 156 cases of tuberculosis were identified with the mean (Standard deviation) age of 20.6 (±1.0) years. Of these, 134 (85.9%) patients were registered as new cases, 11 (7.1%) as relapse, 2 (1.3%) as failure, 5 (3.1%) as treatment-after-default cases whereas 4 (2.6 %) patients were not included in any categories. Average annual new TB rate and smear-positive TB rate were 86.5/10^(5) and 17.4/10^(5) person-years, respectively. In 12 of 31 smear-positive cases, time from onset of symptoms to diagnosis was more than 30 days. Two multidrug-resistance TB cases were identified and two suspected outbreak episodes of TB had occurred during 3 years. Conclusion:There was no statistically significant difference in the incidence rate of new cases of TB between the general population aged 20 to 29 years and combat and auxiliary police in Korea. 배경 : 국내 결핵의 발생은 감소 추세에 있으나 여전히 후진국형 발생양상을 보이고 있다. 특히 집단생활을 하는 젊은이들에서 발생하는 결핵의 유행이 간헐적으로 알려지고 있어 우려를 낳고 있다. 이에 저자들은 최근 전경대원의 결핵 발병 양상에 대해 알아보고자 본 연구를 시행하였다. 재료 및 방법 : 2002년 1월부터 2004년 12월까지 3년간 경찰병원에서 결핵 진료를 받은 전경대원의 의무기록을 후향적으로 분석하여 연도별 결핵 신환발생률, 도말양성결핵 신환율, 진단 지연, 집단발병, 다제내성결핵을 조사하였다. 결과 : 연도별 결핵 신환자 수(10만명당 발생추정치)는 2002년 50명(83.6명), 2003년 42명(83.7명), 2004년 42명(93.5명)이었고 도말양성 폐결핵 신환자 수는 2002년 10명(16.7명), 2003년 11명(21.9명), 2004년 6명(13.4명)이었다. 3년간 도말양성 폐결핵 환자는 31명이 있었고 그 중 12명은 호흡기 증상 발생 30일이 지나서 진단을 받았다. 다제내성 결핵은 2명이 있었으며 결핵의 집단발병이 의심되는 사례는 2차례 있었다. 결론 : 전경대원과 20-29세 연령군 일반인의 결핵 신환발생률, 도말양성 폐결핵 발생률의 통계적으로 유의한 차이는 없었다.

Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

This study was carried out to evaluate the anticancer effect of guava leaf extracts and its fractions. The chemical compositions of the active extracts were also determined. In the present study, we set out to determine whether the anticancer effects of guava leaves are linked with their ability to suppress constitutive AKT/mammalian target of rapamycin (mTOR)/ribosomal p70 S6 kinase (S6K1) and mitogen-activated protein kinase (MAPK) activation pathways in human prostate cancer cells. We found that guava leaf hexane fraction (GHF) was the most potent inducer of cytotoxic and apoptotic effects in PC-3 cells. The molecular mechanism or mechanisms of GHF apoptotic potential were correlated with the suppression of AKT/mTOR/S6K1 and MAPK signaling pathways. This effect of GHF correlated with down-regulation of various proteins that mediate cell proliferation, cell survival, metastasis, and angiogenesis. Analysis of GHF by gas chromatography and gas chromatography-mass spectrometry tentatively identified 60 compounds, including <TEX>${\beta}$</TEX>-eudesmol (11.98%), <TEX>${\alpha}$</TEX>-copaene (7.97%), phytol (7.95%), <TEX>${\alpha}$</TEX>-patchoulene (3.76%), <TEX>${\beta}$</TEX>-caryophyllene oxide (CPO) (3.63%), caryophylla-3(15),7(14)-dien-6-ol (2.68%), (E)-methyl isoeugenol (1.90%), <TEX>${\alpha}$</TEX>-terpineol (1.76%), and octadecane (1.23%). Besides GHF, CPO, but not phytol, also inhibited the AKT/mTOR/S6K1 signaling pathway and induced apoptosis in prostate cancer cells. Overall, these findings suggest that guava leaves can interfere with multiple signaling cascades linked with tumorigenesis and provide a source of potential therapeutic compounds for both the prevention and treatment of cancer.
최근 십전대보탕의 실험적 연구 및 치험례에 대한 고찰(국내 학술지를 중심으로)

ObjectivesThe aim of this review is to analyse the study tendency in papers related with sibjeondaebotang (Shiquan dabutang) which are published in Korea from 2000 to 2013.MethodsWe searched the four electronic database (NSDL, RISS, Korean traditional knowledge portal, OASIS) and checked relevant Korean journals from 2000 to 2013. We classified the papers by publish date, speciality, study method, and field of study, and ana-lysed the study tendency.Results1. 4 papers were published annually on average. 2. After classifying papers by the speciality of journal, continuous study was followed not only in korean medicine, but al-so in many specialities like Dietetics, Biotechnology and Pharmacology. 3. In study meth-ods, clinical case was 29%, in vivo was 26%, and in vitro was 26%. 4. After classifying pa-pers by field of study, beneficial effect was 62%, toxicity was 14%, qualitative analysis was 9%, and adverse effect and pharmacology was 3%. 5. In beneficial effect, it is effective in antioxidation, treatment and prevention of neurologic disease, skin disease, gynecologic disease and so on. It has also an effect in enhancing the immune system, improving the dysfunction of organs, and it can also be used for anticancer and anti-metastatic purpose.ConclusionsThese results suggest that Sibjeondaebotang (Shiquan dabutang) can be used as cure medicine, not just as herbal tonic, but there are not sufficient evidence based papers, so there should be further studies in order to establish Sibjeondaebotang as a cure medicine. Aims: Entecavir has been widely used for treatment-naive chronichepatitis B patients. However, about 20% of patients show partialvirologic response (PVR) after 2 year of entecavir therapy [Yoon, etal, 2011〕. If the HBV DNA continued to be detected, underlying liverdisease may progress, and the risk of hepatocellular carcinoma canbe increased. Therefore, switching to more potent antiviral therapymay be needed.In this study, we compared the efficacy of switching to tenofovirwith continuing entecavir in patients who shows PVR to entecavir.Methods: This is an investigator initiated open label randomized controlledtrial (NCT01711567). Primary end point was a virologic responserate 12 months (VR, HBV DNA < 20 IU/mL).We included chronic hepatitis B patients receiving entecavir 0.5 mgmore than 12 months with detectable HBV DNA over 60 IU/mL, butno resistance to entecavir.Results: A total of 45 patients were enrolled. Twenty two patientswere randomized to tenofovir and 23 patients to entecavir arm.Baseline characteristics were not significantly different between thegroups.After 12 month of treatment, VR rate were significantly higher intenofovir group compared with entecavir group by per protocol analysis(55% vs 20%, P = 0.022) as well as intention-to-treat analysis(50% vs 17.4%, P= 0.020). At month 12, the mean HBV DNA levelwas lower (1.54 vs. 2.01 log IU/mL, P = 0.011) and the degree ofHBV DNA reduction was greater (-1.13 vs. -0.67 log IU/mL, P = 0.024)in tenofovir group than entecavir group, respectively. Proportion ofpatients with normal ALT and HBeAg loss/seroconversion rate werenot different between the groups.Conclusions: In chronic hepatitis B patients with PVR to entecavir, switchingto tenofovir would be a better strategy to achieve optimal response.

<P><B>Abstract</B></P> <P>Commensal bacteria interact with mucosal immune or epithelial cells to modulate the immune system. Probiotics have been proposed as a novel preventive or therapeutic method for immunological diseases because of their ability to alter intestinal microflora. Herein, lactobacilli isolated from kimchi were tested for their capacity to modulate intestinal allergy in mice. Lactic acid bacteria suppressed IL-4, IL-5, IL-13, and IL-17A cytokine production in splenocytes from OVA-sensitized mice <I>in vitro</I>. In a mouse model of food allergy, lactobacilli significantly inhibited the expression of Th2-associated gene products such as GATA3, IL-4, IL-5, IL-9, and IL-13 in the small intestine. Moreover, pro-inflammatory cytokine levels were lower in the lactobacillus group. Lactic acid bacteria also modulated mast cell infiltration and activation. Serum IgE/IgG2a and IgG1/IgG2a ratios were altered to alleviate Th2 responses. In conclusion, lactobacilli from kimchi effectively regulated intestinal allergy by suppressing Th2 responses and modulating mast cell function and recruitment.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Lactobacilli isolated from kimchi suppress Th2 cytokine production <I>in vitro</I>. </LI> <LI> Lactobacilli significantly inhibit Th2 responses in food allergy mice. </LI> <LI> Pro-inflammatory cytokine levels are lowered by administration of lactobacilli. </LI> <LI> CJLP133 and CJLP243 regulate mast cell infiltration in jejunum tissue. </LI> <LI> CJLP133 and CJLP243 adjust immune responses in food allergy mice. </LI> </UL> </P>

This paper deals with the ellipse permanent magnet machines for the minimization of torque ripple based on electromagnetic field theory. On the basis of a magnetic vector potential and a two dimensional (2-D) polar system, analytical solutions for flux density due to permanent magnet (PM) and current are obtained. In particular, the analytical solutions for mathematical expressions of magnets with different circumferential thicknesses can be solved introducing improved magnetization modeling techniques. The analytical results are validated extensively be nonlinear finite element solutions, a reduction of torque ripple can be achieved.
