        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

      • 효과적인 안경원 임상실습을 위한 실태분석연구

        This study was to analysis of the problems of the clinical field-work in optician's shop for the effective clinical field-work and the curriculum development for the needs industry. The date was collected using a prac-tice diary during clinical field work from 125 college students. The most popular cases of problems in the clinical field-work are ophthalmic dis-pensing technique(48%). The frequency of 'edging &beveling, (20.5%) and fitting, (19.7%) was the highest among the problems in the oph-thalmic dispensing technique. And the frequency of problems were for lensmetry, 16.3%, for insertation, 16.1%, for pattern making & lay-out, 10.1%, for automatic edging machine, 9.9% and for coloring 5.2% The result showed the practice of ophthalmic dispensing technique, especially Tedging &beveling, and fitting, is very important for effec-tive clinical field-work.

      • Protein kinase A 억제제인 KT5720이 글루카곤 매개성 항산화 효소의 발현감소에 미치는 영향

        We reported previously that glucagon decreased alpha- and pi-class glutathione S-transferases (GSTs) and microsomal epoxide hydrolase (mEH) protein levels in primary cultured rat hepatocytes. The present study examines the effects of protein kinase A (PKA) inhibitor, KT5720, on the glucagon-mediated decrease in expression of GSTs and mEH. To assess cell viability, lactate dehydrogenase release and MTT activity were examined in hepatocytes treated KT5720. Cell viability was significantly decreased in a concentration-dependent manner after incubation with KT5720 at the concentrations of I μM or above for 24 h, which was inhibited by the cytochrome P450 inhibitor SKF-525A. In contrast, another PKA inhibitor H89 (up to 25μM) was not toxic to hepatocytes. The glucagon-mediated decrease in expression of alpha- and pi-class GSTs and mEH was completely inhibited by 25μM H89 and attenuated by O.1μM KT5720. This study demonstrates that KT5720 may cause cytotoxicity in rat hepatocytes through cytochrome P450-dependent bioactivation. The present study implicates PKA in mediating the inhibitory effect of glucagon on expression of alpha- and piclass GSTs and mEH.

      • 天安삼거리公園基本計劃에 關한 硏究

        The goal of this study is to propose a plan for the construction of Chonan Samgori park. Present this park is designated a neighborhood park of Chonan city. This park is located on the point of traditionally important traffic point linked to Samnam province. And, this place is where the perspective experience of history and culture, which is a unique folk related activities. The site which covers about 96,000㎡is located about 4 km from the cityhall of Chonan city. This basic idea of this plan is to reconstruct the ancient culture and traditional way of life which is rapidly disappearing, to offer the place of education of vernacular culture, to give the recreation place to be a center where city dwellers release and enjoy. Facilities of this park are divided into five groups according to function such as support facilities, folk facilities, play facilities, educational and cultural facilities, and landscape facilities. Design principles are established as follows; to provide the folk park with identity, effectively to admit the folk-related activities, to maintain a functional linkage between facilities. The frame works for final master plan are Landuse plan, Circulation plan, Planting Plan, Facility plan.

      • Hurdle Technology방법에 의한 식품저장

        Hurdle technology에 의한 방법으로 식품을 저장하는 방법이 논의되었다. 이 방법은 다른 처리방법과 함께 사영하거나하여 미생물이 성장하는데 장애가되는 저해 요인들을 유도하여 한 가지 방법이 아니라 여러방법에 의해 가공 처리하는 방법이다. 이 방법은 냉장이나 냉동 없이도 안정하게 유지되며 가벼운 처리 가공방법이 적용되므로 관능면이나 영양가면에서도 매우 우수한 것으로 나타났다. 이에 기초를 둔 대표적인 식품으로는 F-SSP, A-SSP, pH-SSP, Combl-SSP, IMF 그리고 방사선조사 식품에 다른 처리를 한 것등이 있다. 이 기술방법은 주로 육류제품, 유가공제품, 해산물등에 응용되고 있으나 이를 다른 분야에 적용하는 방안을 앞으로 모색하여 나아가야할 것이다. The concept of hurdle technology is reviewed. This technique is the intelligent use of combinations of different preservation factors or technique in order to achieve multitarget, mild but reliable preservation effect. Hurdle-preserved foods are F-SSP, A_wSSP. pH-SSP, combi-SSP,IMF and irradiated foods with other treatments. This technology is mainly applied meat products, fish, and dairy products. Recently qualities of irradiated foods have been improved by combination of irradiation with other treatments such as cryogenic temperature or heating. The future work for application to various foods is discussed.

        전두근ㆍ근막피판을 이용한 선천성 안검하수의 교정

        Ptosis of the upper eyelid is one of common deformities of the eye region. To obtain the best surgical result in ptosis, it is necessary to select the appropriate operation for each case according to levator function, degree of ptosis, its etiology and other factors. For the correction of the congenital ptosis with very poor or absent levator function, frontalis suspension with various many materials and direct transplantation of the frontalis muscle have been used by many authors. Each of the above has its specific virtues and inherent shortcomings. On the basis of comparative study of the conventional techniques and anatomical study of the eyebrow region, we designed a superiorly based frontalis myofascial flap. The orbital portion of the orbicularis oculi muscle was included in the distal end of the frontalis myofascial flap to lengthen the flap. According to our results, this technique of frontalis myofascial flap has several advantages compared to Song's direct frontalis transplantation : (1) the operation can be done under a local anesthesia at the out-patient department with simple procedure and less bleeding. (2) the forehead wrinkles are to not disturbed, (3) this procedure needs only a single incision and does not disturb the important anatomic structures around the eyebrow region.

      • Larson-Miller 법을 이용한 보일러튜브의 크립 잔존수명에 관한 연구

        The boiler components are operated at high temperature, and their materials are subject to the time dependent creep deformation. The materials eventually reach the fracture stage due to the creep damage, and thus the designer should pay attention to the creep damage, and thus the designer should pay attention to the creep lifetime to prevent the catastrophic fracture of those components. In this study, test specimens were prepared from the primary superheater in Yousu power plant which have been used for 11 years, and the creep tests were performed to measure the remaining lifetime. To predict the remaining lifetime of used materials, an equation was proposed by using the Larson-Miller parameter which can estimate the lifetime of unused materials. The primary superheater was turned out to be risky, and the remaining lifetime was predicted to be about 2 years. The fractography captured by the scanning electron microscope showed that the fracture was transgranular with dimples for all the specimens.

        The hypertrophy of pancreatic islets of pregnant rats is caused by the increased uptake of food due to fetal demand and by the increased hormonal secretions, estrogen and progesterone. These hormones are closely related with the growth of breast, and the maximal growth results from a combination of prolactin and growth hormone in addition to them. Prolactin continues to secrete during lactation after parturition. Expecting the islet changes after parturition in that reason, this experiment is designed to observe the effects of the pregnancy, lactation and suppression of lactation. Those changes are studied quantitatively by calculating the percentage of endocrine tissue, according to the method of Cardell, on sections stained with hematoxylin and eosin. Results obtained are as follows: 1. The islets have normal appearance. But in the full term and lactation groups there are islets, large round or irregular in shape. 2. The percentage of endocrine tissue of the full term is 7.66±3.12 which shows significant increase in comparing with the control. 3. The percentage of endocrine tissue in the group of lactation for 20 days shows 6.86±2.34 which is relatively significant increase (P<.025). 4. In the group of lactation for 40 days the percentage of endocrine tissue shows 7.80±4.19 but it is not significant to compare with the control. 5. in the group of suppressed lactation for 20 days, the percentage is 4.55±1.43 that shows the decrease in comparing with the full term and seems to restore to the control. (P<.025). Above results show not only the hypertrophy of pancreatic islets in pregnancy but do also the same change during lactation, and the possible causes are discussed.

      • 자연산 및 양식산 농어의 영양성분 비교

        The differences in the contents of nutritive components of meat were examined between wild and cultured sea bass. Total amino acid contents of the wild sea bass were higher than that of the cultured one. The main amino acids in the both sea bass were glutamic acid, aspartic acid, leucine and lysine. The wild sea bass contained higher amounts of glycine than in the cultured sea bass. Total free amino acid contents of the cultured sea bass were higher than that of the wild one. In composition of free amino acid, taurine was abundant, and glutamic acid, glycine, alanine and histidine were the major free amino acids. The contents of glycine, alanine and histidine in the wild fish were higher than those in the cultured one, while the content of taurine was lower. In the contents of nucleotides and their related compounds, IMP was dominant, and followed ADP, AMP, in order. The contents of nucleotides and their related compounds in the cultured sea bass were markedly lower than that of the wild sea bass. In fatty acid composition, C16:0, C18:1, C22:6, C16:1 and C20:5 fatty acids were abundant. The wild sea bass showed higher levels in C16:1 and C18:1 fatty acids than the cultured sea bass, while the cultured sea bass contained much amont of C22:6 fatty acid.

        Over the last few years, there has been a growing interest in quantitative representation of the weld pools in order to relate the processing conditions to the driving forces of the weldment produced and to use this information for the optimization of the welding process. A theoretical model offers a powerful alternative to check the physical concepts of the welding process and the effects of driving forces. To solve this problem, a 2-D thermo-fluid model were developed for determining temperature and velocity distribution for the GMA welding process.
