        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

        CFRP로 보강된 콘크리트 보의 온도 변화에 따른 영향 분석

        In this study, the behavior characteristics of specimen strengthened with CFRP plate were analyzed according to the change of temperature. CFRP plate itself has a good resistance at the high temperature, but epoxy used as a adhesive is lost its bonding strength at the relatively low temperature. Therefore, this study carries out experiment on the beams slot-bonded with CFRP plates in order to maintain the successful bonding strength of epoxy at high temperature. It is presented that the range of glass transition temperature is 60~80℃ and RC beams slot-bonded with CFRP plate shows more increasing resistance and failure load than that of interface bonded at the high temperature.

        단일 단계 접착제의 변연 미세누출

        This study evaluated the marginal microleakage of five single step adhesives. Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were prepared on both buccal and lingual surfaces of extracted human molar teeth. Prepared teeth were randomly divided into five groups and restored using one of the single step adhesives and composite resins: Prompt L-Pop/Filtek Z-250 (Group 1), AQ Bond/Metafil CX (Group 2), One-Up Bond F/Palfique Toughwell (Group 3), Futurabond/Admira (Group 4), Xeno III/Spectrum TPH (Group 5). The restored teeth were thermocycled. Microleakage was assessed by dye penetration using 2% methylene blue dye solution. The teeth were bisected buccolingually and evaluated for microleakage under steromicroscope. The data were statistically analysed by Kruskal-Wallis test and Mann-Whitney tests. The results of this study were as follows; 1. Microleakage of enamel margins in group 3 was statistically higher than that in groups 1, 2, 4, 5 (p < 0.05) 2. Microleakage of dentin margins in group 1 was statistically higher than that in groups 2, 5, and that in group 3 was statistically higher than that in groups 2, 4, 5 (p < 0.05). 3. Dentin marginal microleakage was higher than enamel marginal microleakage in all experimental groups. In conclusion, Prompt L-Pop showed the least leakage at enamel margin, and AQ Bond showed at dentin margin in this study. Marginal miroleakage in dentin was higher than that in enamel.

        Some authors suggest that certain types of surfaces are the origin of such injuries. A few years after the first medical concerns about surfaces were voiced, publications of biomechanical measurements apperared, describing accleration, force, and impact measurement on different types of surfaces. In many sport activities, surfaces can be under very high dynamic load. This was the reason for the development of various methods for impact simulation, like the development of various methods for impact simulation, like the artificial athlete. Furthermore, it is important to collect information about the hardness of new and already existing surfaces in sport arenas. The idea on which this measuring system is based Is as follows: The stiffness of the material can be computed from the kinematics measurd at touch down of a rigid body onto a material sample. The results show the following The result for the artifical surfaces(Synpave ace) is surprising. It is known that these surfaces are much harder than synpave rebound classic, synpave spring. This finding suggests that it may be possible that the subjective impression is used as one criterion in the selection of landing(or style) strategies. The number of subjects in this experiment is too small to make statistically significant conclusions. It is shown analytically that when an object a deformable surface, the acceleration it experiences is inversely propotional to its mass. In future, it need to stress that the interaction between shoe and surface is important, and this aspect has now become well accepted. Considering biomechanical aspect in player's injury, it request Korean Standards for synthetic playing surfaces in sport like ASTM(America Society for Testing & Materials) standards of America, DIN 18035 standards of Germany, BSI standards of U.K.

      • 중년층 여성의 하체 균형을 위한 스커트원형 연구 : 비만체형의 경우

        The purpose of this study was to develop a basic skirt pattern for the somatotype standardization of the Korean middle-aged women. Eight women were selected as subjects who were more than 1.8 Rohrer's index on the basis of their physical characteristics in Kwangju area and Chonnam area, ranging in their age from 40 through 60. The measuring methods used for a total of 30 item were direct measurement and in-direct measurement. The statistical methods used for the analysis of the data were mean, standard deviation in order to get data to make new pattern. The measure of dart was calculated by the analysis of the horizontal section polymerized picture. Emphasis of the try-on test was placed on the comparision of anthropometric data before and after trying on the basic skirt. The sensory evaluation was applied to evaluate the new pattern by comparing it with the most accepted conventional pattern. The results were as follows : 1) According to the composite reliability coeffcient, all the items showed significant differences(p〈0.001) between the conventional pattern and the new pattern, with the new pattern having higher scores. 2) Drafting method of the new pattern proved to be superior to the conventional one especially in the fitness at waist marginal part, hip marginal part, the position and the amount of darts and lateral line.

        Cyclophosphamide 투여 흰쥐에서 비타민 A가 창상파열력에 미치는 영향에 관한 실험적 연구

        Cyclophosphamide lays restraints upon production of fibroblast and collagen fiber and reduce wound burst strength by acting on fibroblast in acute inflammatory phase. Vitamin A is known to increase inflammatory reaction by exerting an effect upon inflammatory cells in the beginning of the wound healing process. It shows contrary effect to cyclophosphamide. In order to study the influence of vitamin A over wound healing delay induced by cyclophosphamide in rats, we observed and compared the wound burst strength of cyclophosphamide and vitamin A combined treated group with those of control and cyclophosphamide treated group. Wound burst strength of each group was measured in the skin segments of 2×4cm obtained from the operation sites at the 3rd, 5th, 7th and 14th postoperative days after making 5cm incision and suture at the back of 72 rats. Results of the experiment are as follows; 1. Wound burst strength in control group increases slowly as days go by and shows trends of sudden increase between the 5th and 7th postoperative days. 2. Wound burst strength in cyclophosphamide treated group is markedly less than that in control group. 3. Wound burst strength in cyclophosphamide and vitamin a combined treated group shows similar level to that in control group. 4. Body weight in cyclophosphamide treated group reduces gradually as days go by. But there's no weight reduction in cyclophosphamide and vitamin A combined treated group. Judging from these results, we found the fact that vitamin A could prevent or recuperate the side effects of cyclophosphamide, delay of wound healing and worsening of general condition.

        불소도포가 상아질 접착에 미치는 영향

        AbstractThe purpose of this study was to investigate the effect of stannous fluoride on the dentin bonding with three kinds of commercially available dentin bonding systems containing diffe- rent adhesive monomers. Dentin specimens with exposed labial dentin prepared from freshly extracted bovine mandibular anterior teeth were divided into experimental and control groups. The specimens of experimental groups were bonded with dentin bonding systems and composite resins including All bond 2 & Bisfil, Scotchbond Multi-Purpose & Z100, and Denthesive II & Charisma after 2% stannous fluorided application for 5 minutes and washing for 1 minute. The specimens of control groups were bonded with the same dentin bonding systems and composite resins as used in the experimental groups. After bonded specimens were stored in 37? distilled water for 24 hours, the tensile bond strength and cohesive failure rate were measured, and then the pretreated dentin surfaces and the fractured dentin surfaces were examined under scanning electron microscope.The results were as follows :Mean bond strength of stannous fluoride applied groups of All bond 2, Scotchbond MP, and Denthesive II were 2.5MPa, l.lMPa, and l.lMPa respectively, and those of control groups were 7.5MPa, 8.1MPa, and 4.6MPa. Bond strength values of stannous fluoride applied groups were significantly lower than those of the control groups(pOn SEM observation of the fractured dentin-r'esin interface, while most of the specimens of stannous fluoride applied groups showed adhesive failure mode, those of All bond 2 and Scotchbond MP control groups showed mainly adhesive-cohesive mixed failure mode, and mainly adhesive failure mode in Denthesive II control group.

      • GLUTARALDEHYDE 처리한 가견 절단 치수조직의 중장기간 관찰

        성견의 상하악치아 총 84개을 대상으로 생활치수절단술을 시행한후, 2% 및 4% GA희석액을 실험군으로, Dycal을 대조군으로 적용하여 4개월 경과후 그치수반응을 광학현미경으로 관찰한 바 다음과 같은 결과를 얻었다. Dycal군을 제외한 전 실험군 표본은 치수 전방에 걸친 광범위한 염증세포의 침윤을 나타내었으며 그 정도는 미만성에 미세 농양 형성까지 다양하였고 GA 2%군에서 보다는 GA 4%군에서 더 심한 양상이었다. 2차 상아질의 형성은 치수벽 전면에 걸쳐 각 실험군 모든 표본에서 현저하여 치수강이 상당히 협소화되어졌으며 그 정도는 Dycal군에서 가장 왕성하였고 다음은 GA 2%군, 4%군 순이었다. 특히 Dyucal군에서는 2차 상아질에의한 상아교의 형성이 명확했다. This experimental study was performed to evaluate the pulpal response to the glutaraldehyde diluted to 2% and 4%, comparing with the response to the Dycal as control group, in 84 teeth of dogs with routine vital puloptomy. The animals were sacrificed after 4 months following pulpotomy and each specimen was prepared for histopathologic examination with light microscope. The results were as follows; The specimen of experimental groups, 2% glutaraldehyde dressing group and 4% glutaraldehyde dressing group, showed diffuse inflammatory infiltrate with or without microabscess formation in all experimental periods. In general the inflammatory infiltrate was severer in 4% glutaraldehyde dressing group than in 2%. On the contrary, the control group, Dycal dressing group, showed no inflammatory infiltrate in all specimens. The reparative dentin formation along dentinal walls resulting in narrowing of pulpal cavity was marked in all experimental and control groups. The peripheral deposition of calcified tissue was most favorable in Dycal dressing group and was followed in descending order of 2% and 4% dressing groups. The dentin bridge was not formed except for Dycal group showing complete formation.

        상아질 표면의 건조에 따른 습윤상태가 레진세멘트의 접착에 미치는 영향

        AbstractThe purpose of this study was to evaluate the effect of wetting condition made by drying time on bonding of resin cement to dentin.Freshly extracted bovine teeth were grinded to expose flat dentin surfaces. After the exposed dentin surfaces were treated with pretreatment agents and water rinse, each wetting condition of dentin surfaces was made according to drying times and methods including slight blow bry for 1-second by air syringe, blow dry for 20-second by air syringe, and 12-hour dry in desiccator respectively, and then, previously made composite resin specimens were bonded onto each conditioned dentin surface of the specimen using Panavia-2l(Kura- ray Co.), Bistite(Tokuso Co.), and ChoiceCuse with All bond-2, Bisco Inc.) resin cement according as manufacturer's instruction.Bonded specimens were stored in 37? distilled water for 24 hours, then the tensile bond strength was measured, cohesive failure rate was calculated, and fractured dentin surfaces and acrylic rod sides were examined under scanning electron microscope.The result were as follows ;In the group of bonding with Panavia-21 resin cement, higher tensile bond strength was seen in 12-hour dry group than in 1-second and 20-second dry group(pIn the group of bonding with Bistite resin cement, higher tensile bond strength was seen in 1-second dry group than in 20-second and 12-hour dry group(pIn the group of bonding with Choice resin cement, no significant differences of bond strength under given drying time were seen.Cohesive failure rates derived from the groups of bonding with Panavia-21 and Choice resin cement were increased with the increase of tensile bond strength in each drying time.On SEM examination of fractured surface, adhesive failure mode with fractured resin tags was mostly seen in wet condition v/ith 1-second drying time in the group of bonding with Panavia-21 resin cement, mixed failure mode with shortened and fractured resin tag was seen in the group of bonding with Bistite resin cement, and regardless of drying time, and cohesive-adhesive mixed failure mode with fracture of 'Hollow' typed resin tags was mainly seen in the group of bonding with Choice resin cement.

        The Actinomyces species are normal flora of oral cavity and upper intestine characterized as gram-positive, non-spore-forming anaerobic or mjcroaerophilic rods. Actinomycosis is a chronic suppurative and granulomatous disease characterized by abscess, sinus, abundant granulation and mass. The appendix is the commonly involved site among abdominal actinomycosis. Abdominal actinomycosis is often difficult to diagnose before operation because of no reliable clinical features and infrequency of the disease. We report two cases of abdominal actinomycosis which were manifested as acute appendicitis. Those were detected Incidentally tv the pathologist after routine appendectomy. Both patients were successfully treated with appendectomy followed by proper antibiotics use.
