        Accuracy of cardiac output measurements during off-pump coronary artery bypass grafting: according to the vessel anastomosis sites

        Background: During beating heart surgery, the accuracy of cardiac output (CO) measurement techniques may be influenced by several factors. This study was conducted to analyze the clinical agreement among stat CO mode (SCO), continuous CO mode (CCO), arterial pressure waveform-based CO estimation (APCO), and transesophageal Doppler ultrasound technique (UCCO) according to the vessel anastomosis sites. Methods: This study was prospectively performed in 25 patients who would be undergoing elective OPCAB. Hemodynamic variables were recorded at the following time points: during left anterior descending (LAD) anastomosis at 1 min and 5 min; during obtuse marginal (OM) anastomosis at 1 min and 5 min: and during right coronary artery (RCA) anastomosis at 1 min and 5 min. The variables measured including the SCO, CCO, APCO, and UCCO. Results: CO measurement techniques showed different correlations according to vessel anastomosis site. However, the percent error observed was higher than the value of 30% postulated by the criteria of Critchley and Critchley during all study periods for all CO measurement techniques. Conclusions: In the beating heart procedure, SCO, CCO and APCO showed different correlations according to the vessel anastomosis sites and did not agree with UCCO. CO values from the various measurement techniques should be interpreted with caution during OPCAB.         Production of Iron-Binding Peptides from Colostral Whey by Enzymatic Hydrolysis

        Colostral whey prepared from colostrum (pooled from first six post-partum milkings) was heated for 10 min at $100^{\circ}C$ Heated colostral whey was incubated with 1% enzymes (protein equivalent basis) for 15, 30, 60, 90, and 120 min at $50^{\circ}C$. Papain, pepsin, trypsin, and alcalase produced different degrees of hydrolysis (DH), 10.66%, 12.42%, 10.83%, and 25.31%, respectively, at an incubation time of 120 min. The SDS-PAGE reveals that significant amounts of bovine serum albumin (BSA), ${\beta}$-lactoglobulin (${\beta}$-LG), and ${\alpha}$-lactalbumin (${\alpha}$-LA) survived papain digestion. In contrast, pepsin completely removed BSA but not ${\beta}$-LG present in heated colostral whey. Alcalase completely eliminated BSA, ${\beta}$-LG, and ${\alpha}$-LA. This differential hydrolysis was confirmed by reversed-phase HPLC analysis. Using ion-exchange chromatography, fraction-1 (F-1) was obtained from alcalase hydrolysate at a NaCl gradient concentration of 0.25 M. Reversed-phase HPLC chromatograms of alcalase F-1 showed numerous small peaks, which probably indicate that a variety of new peptides were produced. Iron content of alcalase F-1 was 28.94 ppm, which was the highest among all enzyme fractions, whereas iron content of colostral whey was 36.56 ppm. Main amino acids contained in alcalase F-1 were Thr (15.45%), Glu (14.12%), and Ser (10.39%). Therefore, alcalase can be used to generate good iron-binding peptides in heated colostral whey, and the resulting iron-binding peptides could be suitable as a value-added food ingredient for food supplements.

        COPA(cuffed oropharyngeal airway)를 이용한 마취 유도 시 근이완제 사용 유무에 따른 반응

        Background : COPA (cuffed oropharyngeal airway) is a convenient device for airway management in patients undergoing general anesthesia for elective surgery in supine position. It causes less pharyngeal trauma than LMA(Laryngeal mask airway). The purpose of this study was to compare the effect of COPA with and without muscle relaxant. Methods : Forty patients with ASA physical status I and II for elective surgery were randomly assigned to two groups. Anaesthesia was induced with propofol (2 mg/kg) and vecuronium (1.5 mg/kg) and was administered intravenously in Group I but not in Group II. Mask ventilation was done for 5 min with 0: 5 L/min. COPA was placed and heart rate, and systolic, mean, diastolic blood pressure and peak inspiratory pressure were measured at 1 min interval for 5 min. Anaesthesia was maintained with pro- pofol 150 ㎍/kg/min, fentanyl 1 ㎍/kg/hr, O_2 2 L/min and N_2O 2 L/min. Results : There were no significant differences between Group I and Group II in heart rate, systolic, mean and dstolic blood pressure. PIP (Peak inspiratory pressure) was increased and hiccups occurred significantly (p $lt; 0.05) in Group II. Complete airway obstruction occurred in one patient of Group I and severe coughing, body movements and gagging occurred during induction and insertion in Group II. conclusions : With the use of appropriate muscle relaxant, we can use COPA without increased inspiratory pressure or significant complication.         도심 수목원에서의 경관감상과 산책이 인체의 생리적 안정에 미치는 영향

        This research was conducted to investigate the impact of viewing scenery and walking in the urban forest onphysiological relaxation of humans. The experiment was conducted in Hanbat Arboretum located in Daejeon, and thecontrol experiment was conducted in front of Daejeon City Hall. The subjects that participated in the experimentcomprised 24 Korean male university students in their 20s (average age, 21.1±2.5 years), participated in the experimentas the subject, and these subjects were classified into three groups divided into three locations such as the pine forest inArboretum, the pond in Arboretum, and the city. The subjects sat down and viewed scenery for 10 min, and they thenwalked for 15 min. Further, physiological changes were measured using indicators such as heart rate variability (HRV),blood pressure, and pulse rate. As a result, when subjects viewed scenery and walked in the urban forest, a statisticallysignificant increase in the high frequency (HF) power of HRV and decrease in systolic pressure was observed comparedwith when subjects viewed scenery and walked in the city. Through this research, it was found that activities like viewingscenery or walking in Arboretum of the city are effective in increasing the physiological relaxation of the city residents.

        산업현장 추락에 대한 기계학습 알고리즘 기반의 다집단 분류

        본 연구에서는 웨어러블 관성센서를 사용하여 산업현장 추락에 대한 기계학습 기반의 다집단 분류를 시도하였다. 20명의 건강한 성인 남성을 모집하였으며, 15가지 non-FFH 동작과 5가지 low-hazard-FFH 동작 그리고 한 가지 high-hazard-FFH 동작을 3회씩 반복 측정하였다. 5개의 기계학습 기법(SVM, k-NN, LDA, QDA, MLP)에 대하여 two-class 분류성능과 three-class 분류성능을 비교하였다. 분석 결과 SVM이 two-class 분류와 three-class 분류에서 각각 99.5%와 98.2%의 가장 높은 정확도를 보였다. 또한, 세 가지 전처리 기법들(SMOTE, min-max 정규화, univariant feature selection) 역시 알고리즘 성능 향상에 기여함을 볼 수 있었다. In this study, a multi-class classification of industrial fall from height (FFH) based on machine learning was tried using a wearable inertial sensor. Twenty healthy adult males were recruited for the experiment. The experiment protocol consisted of 15 non-FFH, five low-hazard-FFH, and one high-hazard-FFH. Each motion was conducted three times repeatedly. Two-class classification performance and three-class classification performance were compared for five different machine learning techniques (SVM, k-NN, LDA, QDA, and MLP). As a result, SVM showed the highest accuracy of 99.52% and 98.25% in two-class classification and three-class classification, respectively. In addition, the three preprocessing techniques (SMOTE, min-max regularization, and univariant feature selection) contributed to the improvement of algorithm performance.

        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

      • Pseudomonas sp. BCNU 171균에서의 유기용매 shock반응과 heat shock반응의 연관성

        The response of Pseudomonas sp. BCNU 171 to various forms of stress was examined by using O'Farrell two-dimensional gel electrophoresis. The toluene resistance of mid-growth cells of Pseudomonas sp. BCNU 171 grown at 30℃ was enhanced by exposing the cells to a prior toluene shock in the presence of 0.05% toluene for 30 min. The heat tolerance of the cell was also induced by the same condition. Individual protein expression was examined at 30 min and 60 min by silver staining after the addition of 0.05%(v/v) toluene to mid-growing cultures of the strain BCNU 171. The toluene stress response increased the expression of eighteen proteins between 30 and 60 min toluene exposure. At 60 min after toluene addition, a toluene specific protein was detected. A few proteins induced by toluene and heat were overlapped on 2D-PAGE.

        대장직장 종양에 대한 내시경 점막하 절제술의 연대순 임상 결과와 학습 곡선

        Background/Aims: Endoscopic submucosal dissection (ESD) is an effective procedure for en-bloc curative resection of the colorectal tumor. As it requires high technical skills and experience in therapeutic endoscopy, it is important to understand learning curve of ESD technique. The aim of this study was to retrospectively describe the clinical results of ESD and to evaluate learning curve for the ESD of colorectal tumors. Methods: A total of 90 patients with 90 colorectal neoplasms, who had undergone ESD at a tertiary referral hospital from July 2009 to December 2012, were enrolled. The ESD was performed by a single endoscopist. All ESD cases were divided into three periods: first, cases 1-30; second, cases 31-60; and third, cases 61-90. Results: The en-bloc resection rates in third period (100%) was significantly higher than that of the first (93.3%) and second period (80%) (p=0.025). The perforation rate in third period (0%) also significantly decreased compared with that of the first (13.3%) and second period (20%) (p=0.032). To calibrate the difference of tumor size among periods, proficiency was calculated, as the procedure time per specimen area (min/cm2). The proficiency in third period (4.3) was significantly shorter than that of the first (16.8) and second period (10.2) (p=0.004). Conclusions: The learning curve of colorectal ESD in our study shows that at least 60 cases of ESD have to be conducted to acquire sufficient skill of degree without perforation.
