        Breast carcinoma is common malignant tumor in korea and many oncogens have been found in tumor cells. bcl-2 prevent apoptosis and located in mitochondria, endoplasmic reticulum, and nuclear membrane. To evaluate the expression of bcl-2 according to histopathologic features and the relation between bcl-2 and prognosis in breast carcinoma, we have performed immunohistochemical staining, using mouse anti-bcl-2, for 27 cases of benign disease and 49 cases of breast carcinoma. The results were as follows; 1) The expression rates were 77.9% in benign disease and 55.1% in breast carcinoma. 2) In benign disease, expression rate of fibroadenoma, atypical ductal hyperplasia, and intraductal papilloma were 85.7%, respectively, and adenosis 50%, tubular adenoma 0%, and lactating adenosis 100%. But there were no significant difference. 3) In breast carcinoma, expression rate of ductal carcinoma in situ was 100%(3/3) and infiltrating carcinoma 52.2%(24/46), but there were no significant difference. 4) Expression rate of bcl-2 according to histologic grade on breast carcinoma was 68.8% of grade Ⅰ, 54.2% of grade Ⅱ, and 33.3% of grade without significant difference. Expression rate of lymph node metastatic cases was 43.8% and non-metastatic cases 66.8% without significant differnece. Above results showed that bcl-2 expression in breast carcinoma may be related to good prognosis of patients. But further continued study should be followed with more cases.

      • 이온주입이 금속의 일함수에 미치는 영향(Ⅰ)

        이온 주입이 일함수 차이에 미치는 영향을 연구하기 위하여 낮은 에너지 이온 가속기, 전자총과 기타 일함수 차이를 측정하기 위한 장치를 설계, 제작하였다. 제작된 이온 가속기의 집속 특성은 이론적인 값과 일치하였다. 즉, 집속 렌즈의 확대율이 이론값은 2.0인데 비하여 1.2로 상당히 작은 값이었다. 또한 집속 렌즈의 전압 V_L과 추출 전압 V_e에 의한 집속 특성 역시 이론값과 매우 잘 일치하였다 . 제작된 장치를 사용하여 다결정 Cu와 Ni의 일함수 차이를 측정한 결과 0.45eV임을 알았다. 그리고 전자선을 이용하여 억제 전위차로 일함수를 측정할때 입사 전자의 반사율을 고려해야 함을 알았다. Low energy ion accelerator, electron gun, and apparatus for measuring work function difference are designed and constructed in order to study the effect of ion implantation on the work function. The focusing properties of accelerator is coincided with theoretical results; the magnification of Einzel lens is found to be 1.2, while theoretical value is 2.0, and the focal properties as a function of V_L/V_e are in good agreement with theory. Using these systems, the work function difference between polycrystalline Cu and Ni is found to be 0.45eV, and the experimental retarding potential plots indicate close agreement with the theoretical values modified by electron reflection.

      • N-tert-Butyl-2-(1-acetoxy-2-fluoro-1-butyl) benzenesulfonamide 의 결정 및 분자구조

        The crystal structure of N-tert-Butyl-2-(1-acetoxy-2-fluoro-1-butyl) benzenesulfonamide has been determined from single crystal X-ray diffraction study ; C16H24FNO4S, Orthorhombic, Pbca, a=14.652(5)Å, b=18.477(2)Å, c=13.261(2)Å, α=β=γ=90°, V=3590.1(14)ų, T=296(2)K, Z=8, Mok α(λ=0.71069Å). The molecular structure was solved by direct method and refined by full-matrix least squares to a final R=5.55% for 1628 unique observed F0>4σ(F0) reflections and 208 parameters. The conformation of the molecule is stabilized by an intramolecular N(19)-H(19)...O(10) hydrogen bond.

      • 이온 주입이 금속의 일함수에 미치는 영향(Ⅱ)

        낮은 에너지 가속기를 제작하여 순도가 99.99%인 다결정 Ni에 Li^+ 이온을 7 keV의 에너지로 각 시료에 대한 이온주입율이 1.3×10^16atoms/㎠, 4.0×10^15atoms/㎠, 2.0×10^15atoms/㎠, 1.0×10^15atoms/㎠, 2.0×10^14atoms/㎠ 가 되도록 주입시킨 후 전자총과 억제 전위차형 에너지 분석기를 이용하여 일함수의 변화를 측정하였다. 이온 주입율이 2×10^15atoms/㎠보다 작을 때는 일함수가 1.9 eV까지 감소하였고 이온주입율이 4.0×10^15atoms/㎠보다 많을 때는 일함수가 2.8 eV까지 증가하였다. 이것은 Topping의 이론 및 Bauer의 실험결과와 일치함을 보였다. The changes in the work function of polycrystalline Ni with a purity of 99.99% due to Li^+ ions implanted using a low energy ion accelerator and retarding potential energy analyser(RPEA) have been studied. The implantation energy of the Li^+ ions is 7 keV and the implanted ion dose for the various samples are 1.3×10^16 atoms/㎠, 4.0×10^15 atoms/㎠, 2.0×10^15 atoms/㎠, 1.0×10^15 atoms/㎠ and 2.0×10^14 atoms/㎠. When the ion dose is less than 2×10^15 atoms/㎠ the, work function decreases to 1.9 eV, but when the ion dose is more than 4×10^15 atoms/㎠, the work function increses to 2.8 eV. These results are in good agreement with Topping's theory and Bauer's experiment.

        한국의 발톱진균증 환자의 삶의 질에 관한 연구

        Background: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that is a refractory disease which may cause a deleterious effect on patiets quality of life (QOL). Objective: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the chandes of QOL after treatment. Methods: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after sytemic antifungal treatment. Responses to the questionnaire were scored by 5-point scale (1∼4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptorn and functional impact, patients views concerning treatment, and relationship with doctor. Results: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS : from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients view concerning treatment (AS: from 1.34 to 1.02). 4. The drgree of patients satisfaction at the therapeutic effect was very high- 62.4% (immediately after treatment) and 65.8% (9 months affer initiation of treatment) of patients answered excellent or good. Conclusion: This study confirms that toenail onychomycosis has significant impact on the overall QOL of patients. Also the effect of antifungal therapy on patients QOL were watisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis. [Kor J Med Mycol 3(1): 115∼124]

        Restaging of Rectal Cancer with MR Imaging after Concurrent Chemotherapy and Radiation Therapy.

        <P>In patients with rectal cancer who have received concurrent chemotherapy and radiation therapy (CCRT) before surgery, magnetic resonance (MR) imaging has low accuracy in prediction of the pathologic stage owing to overstaging or understaging. The factors related to this problem include fibrosis, desmoplastic reaction, edema, inflammation, and viable tumor nets at a fibrotic scar from a previous tumor. Preoperative diagnosis with MR imaging of histologic variants of rectal adenocarcinoma, especially mucinous adenocarcinoma, is important because these variants tend to have a poor response to CCRT. In addition, these variants manifest with high signal intensity on T2-weighted images after CCRT; this finding makes it difficult to differentiate residual tumors from remaining mucin pools. MR volumetry and functional MR imaging may be helpful in prediction and assessment of tumor response to CCRT. Awareness of post-CCRT changes helps radiologists achieve appropriate restaging of irradiated rectal cancer with MR imaging and can lead to a reduction in understaging or overstaging. It is important to obtain and compare both pre- and post-CCRT images before interpreting the post-CCRT images.</P>

      • Real-time pulse measurement of nano-scale field effect transistors: cancellation of displacement current and extraction of coupling capacitance.

        <P>Real-time pulse measurements of nano-scale field effect transistors (FETs) are reported. We demonstrate the direct monitoring of the real-time current of bottom-up assembled silicon nanowire FET and top-down fabricated gate-all-around silicon nanowire FET, both with the diameter of approximately 50 nm. We demonstrate that the displacement current can be cancelled out from the measured pulse responses. On the other hand, the displacement current also can be utilized to obtain the coupling capacitance between the gate and source of the FETs.</P>

        Midazolam 이 안압에 미치는 영향

        Intraocular tension may be acutely changed by many drugs and by various physiologic events. An acute rise in intracoular tension may be catastrophic if it occurs when the globe is open and leads to expulsion of contents. Most general anesthetics cause a decrease in intraocular pressure, although a few causes increased intraocular pressure. Midazolam is a 1,4-benzodiazepine derivtive synthesized by Walser and Freyer in 1975. Eariler studies with midazolam have demonstrated its efficacy for induction of anesthesia and premedication. It is also desirable to know if all anesthetic agents which produce general anesthesia and which are pharmacologically different affect intraocular pressure in a similar manner. Therefore investigation of the influence of midazolam on intraocular pressure in 25 patients was undertaken at the Dept. of anesthesiology, Hanyang University. All patients had no known eye abnormalities. The patients were not premedicated. In all partients the intraocular pressure was measured before induction of anesthesia, after instilling a 0.5% tetracaine into the conjunctival sac. A second reading was taken after induction of midazolam (0.2 mg/kg of body weight) and a third after injection of succinylcholine (1 mg/kg of body weight) and a fourth after endotracheal intubation. A Schiotz tonometer with a 5.5 gm and a 7.5 gm weight was used. In addition to the tonometric determinations, the bood pressure, pulse rate and respiratory rate were recorded before and after induction of midazolam. An attempt was tried to keep the intraocular pressure changed as many and to minimize the other factors affecting intraocular pressure. To achieve this, supine position and constant gea flow was maintained. Special care was taken to avoid pressure on the patients eyes and to maintain a fully patent airway to prevent respiratory disturbances leading to straining and increased venous pressure. Endotracheal intubation was performed with the aid of succinylcholine to avoid cough or laryngospasm. The results of the observation with the above mentioned method were tested by student t-test statistically. Each patient acted as his own control. There was a fall in intraocular pressure in 17 patients among 25 parients (average 1.8 mmHg), but no significant change followed by the use of midazoam. The blood pressure variations were between 10 and 40 mmHg, during the course of anesthesia and could not be related to intraocular pressure changes. Intraocular pressure chagnes had no relation to pulse and respiratory rate variations. This finding indicated that benzodiazepine as a class of drugs have well described muscle relaxant properties that are primarily central (supraspinal) rather than peripheral (myoneural) in action. There was a rise in intraocular pressure in 19 patients among 25 patients, followed by the use of succinylcholine and 23 patients among 25 patients, after endotracheal intubation. According to Feldman and Crawley, diaxepam potentiated the myoneural blocking effects of gallamine and antagonizes the effects of succinylcholine. Nevertheless Dretchen demonstrated that the clinical doses of diaxepam did not potentiated the muscle relaants. Our finding showing no apparent succinylcholine interaction with midazolam are consistent with the finding of Dretche.
