      • 위선암에서 Cdx2와 p53의 표출

        Gastric carcinoma is one of the frequent malignant tumors in Korea. It is well known that the expressions of oncogenes have relationship with the morphologic features, tumor progression, depth of invasion and lymph node metastasis. It has been reported that the expression of Cdx2 gene has not only related with histologic types and progression of the tumor, but also prognosis of the patients. The p53 modulates cell proliferation and overexpression of the gene results in malignant transformation of the normal cells and effects on the prognosis of the patients as well. The expressions of Cdx2 and p53 with the clinopathologic parameters were evaluated by immunohistochemical methods in 129 cases of gastric carcinoma. The Cdx2 and p53 were expressed each in 51.7%and 77.7% respectively in the gastric carcinomas. The Cdx2 was expressed more frequently in intestinal type than in diffuse type (P=0.005), and also expressed more frequently in moderate to well differentiated tumor than in poorly differentiated tumors (P=0.001). In the advanced carcinoma, Cdx2 was expressed significantly according to the invasion depth (P=0.017). In the early gastric carcinoma,the expression rate of the Cdx2 was higher in intestinal type than in diffuse type (P=0.008). The expression rate of p53 was higher in advanced carcinoma than in early carcinomas (P=0.040).

      • 만성육아종성염증에서의 T-림프구, B-림프구 및 자연살세포의 분포에 관한 비교 연구

        Chronic granulomatous inflammation, such as tuberculosis, leprosy, parasitic infestation foreign body granuloma, is charecterized by the formation of granuloma. Especially, leprosy shows much difference of immune reaction and prognosis according to subtypes. Tuberculoid leprosy shows adequate immune reaction and good prognosis with localized lesion, but lepromatous leprosy shows immune nonresponsiveness and poor prognosis with frequent dissemination. Borderline leprosy is intermediate type beteween tuberculoid and lepromatous leprosy. Numerous studies have been performed to understand the cell mediated immune response on leprosy and tuberculosis, but the immune mechanism of natural killer cell on granulomatous inflammation is not understood well. Therefore, to understand distribution of T-lymphocyte, B-lymphocyte, and natural killer cell on granulomatous inflammation, the author collected 32 cases, such as tuberculosis 12 cases; leprosy 9 cases, cysticercosis 5 cases, and foreign body granuloma 6 cases, of formalin fixed, paraffin embedded tissue obtained by biopsy, performed immune histologic reaction by avidin-biotin complex method using MT1, LN2, and anti-Leu7. Results obtained are as follows : 1. In tuberculosis, there were numerous MT1 positive cells. In contrast, relatively few LN2 positive cells wese observed. 2. In the inner and outer areas of granuloma, MT1 positive cells were more abundant in tuberculoid and borderline leprosy than in lepromatous leprosy. LN2 positive cells were also demonstrated. 3. In cysticercosis, LN2 positive cells, rather than MT1 positive cells, were distributed in the periphery of granuloma. In the foreign body granuloma, many MT1 positive cells, but not LN2 positive cells were found. 4. Anti-Leu7 positive cell was observed in all cases of tuberculoid and borderline leprosy and not in all cases of tuberculosis. 5. Cysticercosis and foreign body granuloms showed infiltration of anti-Leu7 positive cells in a few cases. The above results suggest that, in tuberculoid and borderline leprosy, natural killer cell is involved in the defense mechanism.

      • 위암에서의 CEA의 분포에 관한 연구

        Carcinoembryonic antigen(CEA) isolated from adenocarcinoma of human digestive system and fetal or embyonic gut tissue is one of tumor associated antigen. In recent years, extensive study in tissue of various condition has been reported but study in gastric cancer, common malignant tumor in korea, was rare. Therefore, to understand positive rate of CEA according to degree of differentiation of gastric cancer, the author collectd serial 95 cases of formalin fixed, paraffin embedded stomach obtained by subtotal resection, performed immunoenzyme histologic reaction by Avidin-Biotin-Complex method using anti-human CEA rabbit serum and analyzed their histologic reation. Results obtained are as followings 1. CEA positive reaction to anti-CEA antibody was noted in 26(77%) of the 95 cases with gastric cancer. 2. CEA postive reaction to anti-CEA antibody was noted in 26(84%) of the 33 cases with well differentiated gastric cancer. 3. CEA positive reaction to anti-CEA abtibody was noted in 20(74%) of the 27 cases with moderately differentiated gastric caner. 4. CEA positive reaction to anti-CEA antibody was noted in 27(73%) of the 37 cases with poorly differentiated gastric cancer.

        코로나-19 감염증 유행기간 동안 권역 내 응급의료기관별 및 일반적 특성에 따른 의료진의 직무스트레스, 우울감, 불면증, 피로감 비교분석

        Objective: The objective of this study was to investigate job stress, depression, insomnia, and fatigue of coronavirus disease 2019 (COVID-19) medical staff and analyze the differences according to their general characteristics. Methods: The study enrolled 98 workers at emergency medical institutions with COVID-19 treatment facilities located in Gangwon Province. An online survey was used to protect personal information. Data were analyzed using the statistical program SPSS version 28. The differences in job stress, depression, insomnia, and fatigue of COVID-19 medical staff were verified by t-test and analysis of variance. Results: Analyzing job stress revealed that the average of the lowest relationship conflict stress for each sub-factor was 2.22, the average stress for job demand was the highest at 3.78, and the average whole job stress was 3.04. Contrarily, the average for depression was 1.69, insomnia 2.96, and fatigue 3.07. No statistically significant differences were observed for job stress, depression, insomnia, and fatigue when considering the type of workplace, gender, and occupation of COVID-19 medical staff. However, statistically significant differences were obtained in some variables including age, family members, working period, COVID-19 confirmed, and COVID-19-related education experience. Conclusion: In this study, differences in job-related stress, depression, insomnia, and fatigue were confirmed considering the general characteristics of COVID-19 medical staff. In particular, the results of this study are significant since the difference in perceived stress in performing COVID-19 tasks was identified through empirical analysis by considering sub-factors of job stress.

        사람 위선암과 HeLa 세포에 관한 저에너지 마이크로파 고정효과의 조직화학 및 전자현미경적 연구

        여러 가지 장점이 있음에도 일정성이 유지되지 않는 다는 단점이 있는 마이크로파 고정 효과를 "저에너지 마이크로파"를 사용하여 보완하고자 하였다. 이를 위해 사람 위선암(gastric adenocarcinoma)을 저 에너지 마이크로파(low energy of microwave, LEM)로 고정하여 미세구조와 항원-항체반응 수준에서 그 효과를 관찰하고자 하였다. LEM으로 고정한 시료에서 항원-항체반응은 monoclonal 생쥐-항-사람-p53 (IgG2b, kappa)과 토끼-항-사람-c-erbB-2로 처리하여 결과를 냉동 절편과 비교하였다. 암세포 특이 항원은 LEM으로 처리한 시료에서 발색반응 산물의 확산이 적은 것으로 관찰되었고 보다 쉽게 인지될 수 있었다. LEM으로 고정한 위선암 조직의 미세구조는 보존된 것으로 보였으나 그 고정효과는 2차 원인에 의해 손상되는 것으로 보였으므로 이를 보상하기 위해서 LEM 고정 후 저농도 화학 고정액으로 재차 처리하여 다소의 일정성을 유지할 수 있었으나 위선암 조직은 더 낮은 마이크로파 에너지 요구성이 있는 것으로 생각할 수 있었다. 따라서 암세포가 요구하는 "최소 마이크로파 요구 범위"를 밝히기 위해서 배양 HeLa 세포를 더 낮은 에너지의 마이크로파와 저농도 화학고정액으로 처리한 결과 HeLa세포 미세구조가 보존되는 등 비교적 좋은 효과를 볼 수 있었다. 이 결과로 보면 LEM 조사(照射)로서 생체 저분자 및 수용성 단백질이 crosslink되는 효과가 있는 것으로 보았고 이를 저농도 화학 고정액으로 재차 고정하는 방법으로서 미세구조 및 항원성 보존 효과가 있는 것으로 사료되었으므로 이에 대하여 논의하고자 하였다. Human gastric adenocarcinomas are fixated with low energy of microwave (LEM) to study fixation effects in level of ultrastructure and antigenicity of the cancer. For the Ag-Ab reactions , the LEM fixated sdenocarcinomas are incorporated with monoclonal mouse anti-human p53 (IgG2b, kappa) and rabbit anti human cerbB-2. The retrieval of antigenicity are easily recognizable in the LEM fixated sections compared with that of frozen sections which show often diffused colour reactions. And the LEM fixation methods have preserved ultrastructures of the adenocarcinoma, but it was often difficult to maintain constancy in fixation effects. For the constancy, LEM was coupled with low concentration of chemical fixatives, such as glutaraldehyde (<1%) and $OsO_4$ (<0.5%). The results were acceptable, but there are tendencies that the adenocarcinoma requisitioned rather weak microwave energy to come into the optimal fixation effects. Therefore , cultured HeLa cells were fixated with lower energy of microwave than that used to the adenocarcinoma. The ultrastructures of the single HeLa cell have been preserved. The results may imply that a different energy levels of microwave are requisitioned in accordance with kinds of cells and tissues for the optimal fixation effects. It is reported and discussed that the fixation methods of LEM used in this work could be applied routinely to conceal a insufficient diffusion rate of chemical fixatives into some kinds of cancer without compromising the ultrastructures as well as to improve antigenic quality of frozen sections.
