To investigate the prognostic factors for the survival of transplanted kidney in patients with end-stage renal failure, 59 cases of renal transplantation from September 1986 to Feburary 1997 in Chungnam National University Hospital were analyzed retrospectively. The results were as follows: 1)The mean age of recipients was 33.8 years and that of doners was 38.9 years. The male to female ratio of recipients was 2.5:1, and that of donors was 1.03:1 2) Living related donore(LRD) were 79.6% and living non-related donors were 20.3%. The HLA-identical donors(ID) in LRD were 18.7% and HLA-haploidentical donors(HID) were 61.0%. In living non-related donors(LNRD), mean matched HLA-AB antigens were 1.56 and mean matched HLA-DR antigens were 0.56. 3) The average 5-year patient survival fate was 94%, and average 5-year graft survival rate was 70%. The 5-year graft survival fate of HLA-ID was 100%, and those HLA-HID and LNR were 70% and 36% respectively. 4) Total 33 episodes of acute rejection were found in 45.8% of transplanted patients. The number of acute rejection episode did not show significant difference between LRD and LNRD(P > 0.05). There was no significant difference in MLC between two groups(p > 0.05). 5) In comparison between the 18 patients who lost their graft function in 5 years and 17 patients who are maintaining graft function for more than 5 years, MLC was significantly lower in patients with functioning graft than of patients with non-functioning graft (p < 0.05). The number of rejection episode was also lower in patients with functioning graft than that of the patients with non-functioning graft(p < 0.05). However, there were no significant difference in recepient and donor age and history of pre-transplantation donor specific transfusion between two groups. With the results above, we can speculate that adequate donor selection according to good matched HLA typing and low MLC is very important for graft survival in renal transplantation. Prevention and treatment of acute rejection in renal transplantation is also na important factor for graft survival.

소형 Gun Drill Machine 개발에 관한 연구

This compact Gun Drilling Machine was developed to improve productivity and economical efficiency for small and midium enterprise tool makers. To produce specialize tools that gun drilling works should be required prior to making deep holes. Gun drill machines are so expensive and big burden for small tool makers so that works used to execute through outside orders but it was required lot of cost too. Most of gun drill machines are providing for high volume and large capacity enterprises. In order to support for small and medium enterprise purpose that compact gun drill machine was designed and developed. It could be improved products quality, productivity and saving manufacturing cost using this machine.

절단된 안면신경의 일차문합술

Because the facial nerve innervates the muscles of facial expression, injury to this nerve may cause social, psychological, and functional disability. For this reason, a transected facial nerve must be restored appropriately by any one of a variety of approaches. The choice of anastomosis should be based upon the cause, location of transection and the length of time since paralysis was first noted. Although there are numerous methods of anastomosis, none will result in complete recovery. And thus, the method must be carefully selected and tailored to suit each patient. During the 20 months between July 1988 and February 1990. We have been operated (immediate end to were epineurial repair) in 20 patients. 15 of 20 aged 23 to 60. There were 13 males and 2 females. The nerve injury was unilateral in all cases. One of 15 received a nerve graft harvested from the greater auricular nerve. 12 of 15 Stensen's duct injuries were associated with served zygomatic branch and/or buccal branch. So an end to end anastomosis of the duct was performed simultaneously with anastomosis of the nerve. Failure to repair the duct resulted in a salivary fistula in two cases. To show the success of the procedure, EMG(electromyogram) and NCV(nerve conduction velocity) were done at least 6 months following surgery. The findings have been considered good to excellent and the reinnervated muscles have showed symmetric movements in all cases.

세 도시의 생활 쓰레기 발생량의 장래 추이 예측

This study centers on a long-term prediction for the generation of municipal solid wastes for 3 cities such as Pusan, Taejon and Cheongju. For the long-term prediction several statistical models based on time or population were proposed and utilized. The MSW discharge per day from 3 cities predicted is in the range of 12,100~14,600 ton for Pusan, and 3,100~3,800 ton for Taejon, and 2,200~2,600 ton for Cheongju in the year of 2008, respectively. However, the volume-based charge system since January 1995 has made a great effect on the generation of MSWs. Unfortunately, there has been few data to consider this effect for the prediction. Therefore, there has been an urgent need to collect new baseline data for the more realistic long-term prediction.

아리스토텔레스의 덕론에 의거한 도덕과 수업지도 방안

The purpose of this study is to make an approach to the moral education based on Aritotle's theory of virtue. Aristotle accepts that people generally view happiness as the end of life. He says happiness must be an activity of virtue. In Aristotle virtue is particularly the activity of reason and of rationally ordered habits. Aristotle distinguishes the moral virtues and the intellectual virtues. Moral virtues are more directly concerned with good living. Intellectual virtues make the intellect a better instrument of knowledge. Virtue is a habit of choosing the mean between the extremes of excess and defect in action, and this mean is determined by reason guided by the intellectual virtue of prudence. We need take notice of common good, justice, and friendship as major virtues in the theory of Aristotle. As for Aristotle, the happiness of the individual and that of the community are identical. He also sees that justice tends to tends to produce and preserve happiness, and friendship refers to the truest form of justice. Keeping those in mind, this study constructs some instructional plans of moral studies in the primary school. Two themes in the textbooks of fourth and sixth grade, common good and justice, are extracted.

2007년 대한감염학회 권장 성인 예방접종 권장안

In 2007, the Korean Society of Infectious Diseases recommended an immunization schedule for adults residing in Korea. This recommendation includes four categories: standard immunization for healthy persons, supplementary immunization for persons with underlying illnesses, recommendations for health-care personnel and for international travelers. The Standard Immunization Schedule consists of universal vaccination (tetanus-diphtheria/tetanus-diphtheria-pertussis (Td/Tdap); hepatitis A vaccine for persons aged 20s; human papilloma virus vaccine for women under 26 years old; meningococcal vaccine for military recruits and college students in dormitories; influenza vaccination for persons aged 50 years or more and pregnant women; and pneumococcal vaccination for persons aged 65 years or more) and catch-up vaccination (hepatitis A vaccine for persons aged 30s; hepatitis B vaccine, varicella and MMR vaccines for persons who have uncertain specific immunities). The Supplementary Immunization Schedule recommends 4 vaccines, i.e., pneumococcal, influenza, meningococcal, and Haemophilus influenzae type b, appropriate to the underlying illnesses, and confirmation of the presence of antibodies to hepatitis A and hepatitis B. The Recommendation for Health-Care Personnel includes universal vaccination (influenza and pertussis) and catch-up vaccination (hepatitis B, measles-mumps-rubella and varicella for seronegative health-care personnel). The Recommended Immunization for International Travelers focuses reinforcement of vaccinations for healthy persons and includes certain immunizations necessary for international travel (yellow fever, meningococcal, plaque, tick-borne encephalitis, and cholera). Rat의 복강내에 투여한 5-fluorouracil-polyglycolic acid 제형이 장기에 미치는 영향에 관한 연구

For more effective intraperitoneal chemotherapy, the authers made 5-fluorouracil-polyglycolic acid composite disks(FU-PGA disks) with 5-fluorouracil and polyglycolic acid, a biodegradable polymer. We inserted the FU-PGA disk into the peritoneal cavites of Sprague-Dawley male rats, the control and three experimental groups; one FU-PGA disk insert group(300mg/kg), two FU-PGA disk insert group(600mg/kg), and three FU-PGA disk insert group(900mg/kg). The control group received a similar number of PGA disk inserts. A pharmacokinetic study was performed to measure the 5-fluorouracil concentration in the peritoneal fluid, blood, and tissues(liver, kidney and heart) at 24 hours, 72hours and 168 hours after insertion of the FU-PGA disk. Light and electron microscopic studies were done. The results were as follows: 1) The number of white blood cells and platelets decreased after FU-PGA insertion. The degree was proportional to the duration and amounts inserted. The change in the number of red blood cells varied slightly. 2) Light microscopically, slight changes were noted at 168 hours in the 3 disk insert group (900mg/kg). Mild fatty change and hepatocyte degeneration around the central veins of the liver were noted, with vacuolar degeneration in the proximal convoluted tubules of the kidney. 3) Electron microscopically,the liver showed focal increases of lipid droplets in the hepatocytes, and irregular nuclear membrane with focal nucleolar segregation of the fibrillar and granular elements. Also the double membranous structure of the hepatocytes and Kupffer cells was widened with bleb formation. The kidney showed wide separation of the nuclear double membrane in the proximal convoluted tubular cells and mesangial cells, with vacuolation and myelin figure formation in the mitochondria of the proximal convoluted tubular cells, at 72 hours and 168 hours in the 2 to 3 FU-PGA insert groups. The heart showed focal loss of cristae,vacuolation and myelin figure formation in the mitochondria of the 2 to 3 FU-PGA insert groups. In conclusion, despite the large amont of FU-PGA inserted, the histopathological changes in the liver, kidney and heart were slight, and consonant with the very low amount of 5-fluorouracil concentrations detected in the liver, kidney and heart. The above results suggest that the FU-PGA composite can serve as a new device for releasing drugs in a controlled manner and easily targeted to intraperitoneal organs. This device can improve the efficacy of intraperitoneal chemotherapy for gastric cancer.

진행된 HIV 감염 환자에서 십이지장의 미란성 병변으로 발현한 Mycobacterium genavense 감염 1예

Mycobacterium genauense는 최근에 알려진 미코박테륨으로서, 식욕 부진, 체중 감소, 설사, 림프절 병증 등 파종성 Myocobacterium avium-intracellulare complex 감염과 유사한 임상상을 보이며, 위장관 침범을 잘 하는 것으로 알려져 있으나 아직 국내에서 증례는 보고된 바가 없다. 저자들은 진행된 HIV 감염 환자에서 십이지장의 미란성 병변으로 발현한 Mycobacterium genavense 감염 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. 환자는 상복부 동통과 발열, 설사로 내원하였다. 십이지장의 미란성 병변에서 시행한 조직검사에서 항산균이 가득 차있는 거품세포가 관찰되었으며, 이 검체에서 16S rRNA 유전자의 중합연쇄반응 검사를 시행하여 Mycobacterium genavense를 확인하였다. Mycobacterium genavense, first identified in 1990, is known as a pathogen that mimics disseminated Myocobacterium avium-intracellulare complex (MAC) infection with particular propensity for the gastrointestinal tract. In Korea, no case with the organism has been reported. Herein we report a case of Mycobacterium genavense infection that manifested with erosive lesion of duodenum in a patient with acquired immune deficiency syndrome. The patient presented with epigastric pain and fever, diarrhea. Duodenal biopsy showed histiocytic infiltration with numerous acid-fast bacilli. Identification of the mycobacterial isolate by the polymerase chain reaction restriction analysis of 16S rRNA gene revealed Mycobacterium genavense.

절골술을 이용한 안면교정

The most important factor in the beauty of the face is the harmony of the facial skeleton and soft tissue. The postoperative result is not always satisfying after only classic soft tissue surgery. The reason for dissatisfaction is disharmony of the facial skeleton. Recently, the development of maxillofacial surgical techniques enable disharmony of the facial skeleton to be successfully corrected. The authors have had experience with 33 cases of congenital and acquired facial deformities, from March of 1986 to February of 1989, and have followed them from 6 months to 2 years. We used an intraoral approach in most cases and got good results. There were four wound disruptions which were treated conservatively and resolved without sequelae.
