        The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.

        The incidence of aseptic meningitis infection is ensuing and threatening the health of children. Enteroviruses are the major agents of aseptic meningitis and identification of virus has been a clue to diagnosis and epidemiology. The outbreak of aseptic meningitis occurred in Pusan, 1998. Patients were concentrated from April through November. Children were more susceptible than adults. Among 306 cases of specimens from stool, throat swab tested, only 7.2% were positive on virus isolation, 12 cases from stool and 10 from throat, respectively. All isolated 7 serotypes of viruses represented cytopathic effect on cultured cells. Three types of echovirus 6, 25, 30 and coxsackievirus B2, B3, B4, B6 were identified by neutralizing anti body test. Isolated coxsackievirus and echovirus were observed by an electron microscope with negative staining.

        Multiple bilobar liver matastases (MBLM) are the main cause of low resectability in the colon cancer liver metastases. The authors experienced one case of initially non-resectable colon cancer liver metastases. He was curatively and safely treated with a two-stage hepatectomy using the new method of future remnant liver volume growing. A 54-year-old man was referred to our department with the sigmoid colon cancer combined with MBLM, which were checked in two small metastatic lesions in the left lobe and five large sized lesions in the right lobe in the computed tomogram (CT). A laparoscopic assisted anterior resection was primarily performed. We performed the 1^(st) stage hepatectomy 3 weeks after the colon resection. Intra-operative Ultrasonogram (US) found 9 small superficial metastatic lesions in the left lobe. All that lesions were completely removed by non-anatomical wedge resection. An occlusion ballon catheter was placed in the right portal vein through a small branch of the inferior mesenteric vein at that time. The future remnant liver volume was sufficiently increased 3 weeks after the 1^(st) hepatectomy. A right hepatectomy was safely performed 22 days after the 1^(st) hepatectomy. The patient received a regional chemotherapy (interleukin2 based immuno-chemotherapy through hepatic artery) for 4 months, then received 9 cycles of systemic chemotherapy (biweekly Oxaliplation, leucovorin, plus 5-fluorouracil) without any recurrence evidence.

      • 급성 충수염으로 진단된 예측 불가 염증성 맹장 종물에 대한 외과적 고찰

        Purpose: An unexpected inflammatory cecal mass of uncertain etiology was encountered during surgery for presumed appendicitis. In this case, the surgeon couldn't exclude differential diagnosis which were severe appendicitis involving the cecal region, diverticular disease, other inflammatory bowel diseases, and neoplasm. Therefore, this finding leads to a therapeutic dilemma for the surgeon, when making surgical decisions during the operation. The aim of this study was to resolve this dilemma, a retrospective review was undertaken in this study. Methods: A retrospective study was performed to review the management of this problem at Soonchunhyang University Chunan Hospital for eight and half years from July 1991 to December 1999. A review of the emergency operation records identified 50 patients who had undergone ileocecal resection or right hemicolectomy for an inflammatory ileocecal mass of uncertain etiology when operated on for probable appendicitis. Results: Altogether, 39 patients underwent ileocecal resection, and 11 patients underwent right hemicolectomy. The final pathologic diagnosis was appendiceal abscess in 32 patients (63%), cecal diverticulitis in 7 patients (14%), neoplasm in 6 patients (12%), appendiceal phlegmon in 3 patients (6%), ileal tuberculosis in 1 patient (2%) and Anisakiasis in another patient (2%). The neoplasm of the 6 neoplasm patients was ascending colon cancer for 4 patients and cecal cancer for 2 patients. There was no mortality in this group. Conclusion: Although most inflammatory cecal masses are caused by benign disease, all cases in which the intraoperative diagnosis is unclear, any pathologic diagnosis including neoplasm cannot be ruled out. So this suggest ileocecal resection or right hemicolectomy to remove a possible underlying malignancy, and to decrease morbidity and mortality.

        Most of the parasites in nemathelminthes and platyhelrninthes have highly developed reproductive organs as 'compared with their digestive organs, and deposit heavily protein-rich eggs. One female adult of Ascaris lumbricoides lays more than 200, 000 eggs per a day, a female of hookworm more than 20, 000 eggs and more than 4,000 eggs from an adult of leaf-like liver fluke with length of 1. 0 to I.5cm. It is considered that these eggs-depositing parasites may take nutrients not only through degenerated mouth part but also through any other absorption mechanism, especially for the compensation of protein loss due to egg deposition From this viewpoint, an autoradiographic study was carried out for investigating the intake mechanism of protein into the above-mentioned parasites. Toxocara canis and Ancylostonza caninum were infected to the dogs, and Clonorchis sinensis was infected to the rabbits for the present study. A serum albumin for radioisotope, 1311-RISA, was also injected intravenously into each host, which was sacrificed 48 hours after injection The parasites were removed from the sacrificed hosts, and then the quantitative distribution of radioactivity were counted by Well Type Scintillation Counter from the parasites. The results are as follows; 1. Toxocara canis The dog ascaris harboring freely in the intestinal lumen of the host takes protein from the host through the cuticle as well as the primitive mouth :_. Part. Absorption amount of, serum protein was 1.2m; Ci per gm. Of body weight of the parasite and the absorption rate .was. 0.6%. The results indicate that the parasites take protein mainly from the contents of intestinal' lumen regardless of direct absorption from the intestinal wall of the host. 2. Ancylostoma caninum Canine hookworm intakes serum protein from the host only with its mouth part. Absorption amount .bf serum protein was 100.8 mpCi per gm. Of body weight of a parasite and 50.4% of absorption rate. Absorption amount of serum protein was actually 85 times higher than that of Toxocara canis. It is showed that blood-sucking through the mouth part with 3 pairs of teeth, is enough for protein metabolism in the canine hookworm. 3. Clonorchis sinensis The liver fluke inhabiting in the bile duct of the host liver, intook serum protein of the host with the oral sucker and integument. Absorption amount of serum protein was 60.0 mpCi per gm. Of body weight of the worm. The absorption rate (30.0%) was 50 times higher than that of Toxocara canis. It is suggested that serum protein is intaken from bile juice and wall of the host bile duct through the integument as well as the oral sucker of the liver fluke.

      • 중추에서 Prostaglandin계가 Renin-angiotensin System에 미치는 영향

        Role of prostaglandins on the renin-angiotensin system in the central nervous system was examined in normotensive and 2-kidney, 1 clip (2K1C) hypertensive rats. The experiment was done under thiopental (50 ㎎/㎏, IP) anesthesia. Captopril and indomethacin were injected into the right lateral cerebral ventricle. Arterial blood pressure and heart rate were recorded from the femoral artery. Intracerebroventricular (ICV) captopril (1 ㎎) caused a decrease in mean arterial pressure in both normotensive and 2K1C hypertensive rats. The depressor response to captopril was more sensitive in hyper-tensive rats than in normotensive rats. Indomethacin treatment (ICV, 200 ㎎) altered the depressor response to captopril neither in normotensive nor in hypertensive rats. These results suggest that the cardiovascular effect of renin-angiotensin system in the central nervous system may not be mediated via prostaglandin systems in normotensive and 2KlC hypertensive rats.

        삼상 순환 유동층 생물반응기에서 열전달 계수

        직경이 0.102m이고 높이가 1.0m인 기체-액체 순환 및 삼상 순환유동층 생물반응기의 상승관에서 열전달 특성을 고찰하였다. 기체 및 액체의 유속 그리고 생물막 담체의 체류량이 상승관 내부의 수직 열원과 반응기 사이의 열전달 계수에 미치는 영향을 결정하였다. 기체-액체 순환 반응기에서 열전달 계수는 기체의 유속이 증가함에 따라 증가하였으나 액체 유속의 증가에 따라서는 약간 증가하였다. 삼상 순환유동층 생물반응기에서 열전달 계수는 기체의 유속과 유동 생물막 매체의 체류량이 증가함에 따라 증가하였으나 액체의 유속이 증가함에 따라서는 약간 증가하였다. 본 연구의 실험 범위에서 이들 열전달 계수는 실험 조작 변수의 상관식으로 잘 나타낼 수 있었다. Heat transfer characteristics have been investigated in the riser of gas-liquid circulation and three-phase circulating fluidized-bed bioreactors whose diameter and height are 0.102m and 1.0m, respectively. Effects of gas and liquid velocities and holdup of fluidized biofilm media on the heat transfer coefficient between the immersed vertical heater and the bed have been determined. The heat transfer coefficient in the gas-liquid circulation reactor increases with increasing gas velocity, but increases only slightly with increasing liquid velocity. The h(heat transfer coefficient) value in the three-phase circulating fluidized-bed bioreactor also increases with increasing gas velocity or holdup of fluidized biofilm media, but it increases only slightly with liquid velocity. The value of heat transfer coefficient has been well correlated in terms of operating variables.
