Background: This study is aimed at describing the outcomes of the management of the patients, caregivers, and healthcare workers (HCWs) who are exposed to the pandemic influenza (H1N1 2009) virus and at evaluating the adequacy in exposure management and infection control. Methods: From July 2009 to January 2010, for 7 a month period, we managed patients and healthcare workers without any respiratory protective devices, who came within 1 m distance of H1N1-positive individuals for more than 1h and performed a 1-week follow-up. Results: The total of 157 cases with exposure to pandemic influenza (H1N1 2009) virus and exposed individuals of 907 were reported. Of the exposed individuals who were under management, 15 were confirmed to be infected with the infection rate being 1.7%. The confirmed individuals did not have a secondary infection after the exposure. Rates of infection of the exposed patients and healthcare workers were 1.8% and 1.6%, respectively, and these figures were not statistically significant. Conclusion: The exposure management results at the hospital revealed that the infection had spread by contact with individuals who were positive for the infection. The high incidence of early exposure to the virus warrants the need to ensure the use of protective equipment and the adoption of assertive teaching methods that have long lasting effects.

In this paper, the modified design equation of shielding effectiveness was presented to predict more accurately the shielding effectiveness of honeycomb structure with hexagonal waveguide. The design equation of shielding effectiveness in honeycomb was represented from adding shielding effectiveness of single lattice to shielding effectiveness of infinite array of single lattice. This paper proposed the generalized design equation of shielding effectiveness by analyzing basis lattice of hexagonal waveguide which composes honeycomb structure and infinite array structure of basis lattice. To provide the validity of the modified design equation of shielding effectiveness in this paper, comparison with other available date using 3D EM commercial software is made.

Purpose: Antimicrobial resistance monitoring could be a useful source of information for treating and controlling nosocomial infections. We analyzed antimicrobial resistance data generated by Korean Hospitals and by a commercial laboratory in 2005 and 2007. Materials and Methods: Susceptibility data for 2005 and 2007 were collected from 37 and 41 hospitals, respectively, and from one commercial laboratory. Intermediate susceptibility was not included in the calculation of resistance rates. Results: Methicillin-resistant Staphylococcus aureus (MRSA) (64%), third-generation cephalosporin-resistant Klebsiella pneumoniae (29%), fluoroquinolone-resistant Escherichia coli (27%), Pseudomonas aeruginosa (33%), and Acinetobacter spp. (48%), and amikacin-resistant P. aeruginosa (19%) and Acinetobacter spp. (37%) were prevalent in hospitals in 2007. A gradual increase of vancomycin-resistant Enterococcus faecium and imipenem-resistant Acinetobacter spp. was observed. Higher incidences of thirdgeneration cephalosporin-resistant E. coli and K. pneumoniae and imipenemresistant P. aeruginosa were found in the commercial laboratory than in the hospitals. Conclusion: Methicillin-resistant S. aureus, third-generation cephalosporin-resistant K. pneumoniae, and fluoroquinolone-resistant E. coli, P. aeruginosa and Acinetobacter spp. remain prevalent in Korea, while the incidence of vancomycin-resistant E. faecium and imipenem-resistant Acinetobacter spp. has increased gradually. The higher prevalences of third-generation cephalosporinresistant E. coli and K. pneumoniae, and imipenem-resistant P. aeruginosa in the commercial laboratory are a new concern.

나선식 CT를 이용한 간의 전이성 선암의 조영전, 후기의 조영양상 및 종괴 발견율의 비교

Purpose : To evaluate the contrast enhancement patterns and mass detection rate of metastatic adenocarcinomaof the liver in the early and late phase of spiral bolus dynamic CT. Materials and Methods : Two-phase spiralbolus dynamic CT of the liver was performed on 34 patients with clinically or pathologically proved metastaticadenocarcinoma of the liver after bolus administration of 120 mL of contrast material intravenously. CT scanningwas started at 45 seconds and 6 minutes after beginning of injection in 23 patients, and at 35 seconds and 3minutes in another 11 patients. The enhancement patterns of the tumors were classified into 6 types compared withattenuation of surrounding normal liver parenchyma, as diffusely high, peripherally high, centrally high, mixed,iso, diffusely low. The enhancement patterns of 76 tumors in 34 patients were analysed. Results : In the earlyphase, peripherally high attenuation was seen in 34(45%) and diffusely low attenuation in 23(30%). In the latephase, low attenuation was seen in 27(36%) followed by iso and mixed in 14(18%), in each, and peripherally high in13(17%). The appearances of enhancement pattern from the early to the late phase were variable. 12(16%) werediffusely low in both phases, 11(14%) were peripherally high in the early phase and diffusely low in the latephase, and 10(13%) were peripherally high in both phases. Number of tumors lesions detected were more in the earlyphase in 12(35%)patients, and in the late phase in 3(9%)patients. Conclusion : Metastatic adenocarcinoma of theliver showed variable enhancement patterns in the early and late phases of spiral bolus dynamic CT, although mostcommon pattern was peripherally high in early phase, and diffusely low in the late phase. The detection rate ofmasses was higher in early phase than late. The two-phase spiral bolus dynamic CT should be good method indetection and differential diagnosis of metaststic adenocarcinoma of the liver.
