위의 유암종과 선종의 병발 1례

유암종은 전신에 퍼져 잇는 장크롬친화성 세포에서 기원하는데 위의 유암종은 비교적 드문 질환으로 모든 유암종의 1.9~2.2%에 해당하며 위에 발생하는 종양의 1% 미만에서 보고되고 있다. 유암종은 일반적으로 증상이 비특이적이고 면역조직화학검사에 의해서 진단되기 때문에 과거에는 드물게 발견되었으나 최근 내시경 기기 및 술기의 발전과 검사 빈도의 증가 및 면역조직화학검사의 발달로 진단율이 증가 추세에 있으며, 드물지만 유암종에서 선암과 공존하는 예들이 보고되고 있다. 저자들은 심와부 동통을 주소로 내원한 70세 남자 환자의 상부위장관내시경 검사에서 위체상부의 대만곡에서 0.4×0.4 cm 크기의 중간 함몰을 가진 Yamada 1형의 용종과 함께 전정부의 전벽에서 0.8×0.5 cm 크기의 융기된 점막 소견을 보여 조직학적으로 각각 위유암종 및 위선종임을 확진한 후 내시경적 용종 절제술로 치료한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Carcinoid tumors are arisen from enterochromaffin cells, which are scattered throughout the body. These tumors are composed of variable numbers of cells that contain endocrine granules in their cytoplasm, which can be identified by immunohistochemistry and electron microscopy. Gastric carcinoid tumors are relatively rare tumor, the reported incidence being 1.9~2.2% of all carcinoid tumors and less than 1% of all gastric tumors. Sometimes, composite carcinoid tumors and adenocarcinoma is reported. We experienced a case of the coincidence of carcinoid tumor and adenoma at the other site of the stomach in a 70 year old male patient. Gastrofiberscope showed 0.4×0.4 cm sized Yamada type I polyp with central depression on the greater curvature of the proximal body and 0.8×0.5 cm sized focal slightly elevated mucosa on the anterior wall of the antrum. The endoscopic biopsy specimens revealed carcinoid tumor and tubular adenoma with moderate atypism, respectively. The carcinoid tumor and adenoma were removed by endoscopic polypectomy. According to the follow-up gastrofiberoscopy after 7 month later, both lesions were nearly normal mucosa except whitish atrophic change after polypectomy.

노령의 대퇴 전자간 골절 치료에서 압박고 나사못의 안정성 및 유용성

목 적: 노령의 대퇴 전자간 골절에서 압박고 나사못의 안정성에 영향을 주는 인자들간의 상관 관계와 골절 양상에 따른 술후 고관절 기능을 평가 및 분석하였다. 대상 및 방법: 압박고 나사를 이용하여 수술적 치료를 시행한 231명의 환자 중 6개월 이상 추시가 가능하였던 60세 이상 84명의 환자들을 대상으로 했다. 방사선적 계측은 압박고 나사못의 활강 길이 및 내반각, 가압 나사 위치, 정복 상태, 원위 골편의 내측 전위를 측정하였으며 술후 기능은 Clawson의 기능 평가표로 하였고 골절 양상에 따라 방사선 계측과 기능 회복과의 상관 관계를 평가하였다. 결 과: 임상 결과는 불안정 골절에서 양호가 10례, 불량이 23례, 안정 골절에서는 양호 40례, 불량이 11례로 안정 골절에서 임상 결과가 좋았고(p<0.001), 압박고 나사 활강은 10 ㎜ 이상일 때 양호가 4례, 불량이 21례, 10 ㎜ 이하에서 양호가 46례, 불량이 13례로 10 ㎜ 이상 활강시에 술후 결과가 불량했다(p<0.001). 다르 방사선적 계측 요인은 결과에 의미있는 차이를 보이지 않았다. 또한, 불안정 골절 중에서 전위 양상 골절의 경우 10㎜ 이상 활강이 12례, 10㎜ 이하 활강이 8례, 분쇄 양상 골절의 경우는 10㎜ 이상이 11례, 10㎜ 이하가 2례로 분쇄 양상의 불안정 골절에서 10㎜ 이상 활강이 의미있게 많았다 (p<0.001). 결 론: 10㎜ 이상의 지연 나사 활강은 고관절 기능 회복 약화를 유발시킬 수 있으며 불안정 골절 분쇄 양상 골절은 이런 지연 나사의 활강이 과도하게 유발될 수 있으므로 불안정 분쇄 양상 골절에서 압박고 나사의 단독 사용은 재고해 보아야 할 것이다. Purpose: To evaluate the relationship between fracture stability and functional results, and analyze the correlation between stability factors and the outcome in intertrochanteric fracture of the elderly. Materials and Methods: Of the 231 patients, 84 patients with age above 60 were able to follow up for minimum 6 months. We measured the sliding length of the lag screw, varus degree, position of lag screw, reduction status and medialization of distal fragment radiologically. The functional outcome of the treatment was evaluated with the Clawson's result classification and we evaluated the correlation between the radiological results of measurement and the functional recovery depending on the Evans fracture classification. Results: There were good results in 40 cases out of 51 stable fractures, and in 10 cases out of 33 unstable fractures (p<0.001). In case of sliding of lag screw more than 10㎜, good results were obtained in 4 cases, and poor in 21. And in case of sliding less than 10㎜, good results were obtained in 46, and poor in 13. (p<0.001).But there was no relationship between other radiologic factors and clinical results. In unstable type, there were 12 cases with lag screw sliding more than 10㎜ and 10 cases with less than 10㎜. In comminuted type, there were 11 cases with lag screw sliding more than 10㎜ and 2 cases with less than 10㎜(p<0.001). Conclusion: The sliding of lag screw more than 10㎜ may result in poor outcome. As in comminuted unstable pattern, sliding of lag screw might be excessive, the use of compression hip screw alone is not good treatment option.

T Cell Receptor Rearrangement in NKT Cells Differentiated from Cord Blood CD34+ Cells

Objectives: To investigate the time of rearrangement of the TCR gene in the process of NKT cell differentiation from CD34+ human cord blood cells in vitro. Methods : We isolated the CD34+ human cord blood cells and induced the differentiation of NKT cells by liquid culture including IL-15, FL and SCF for 30 days. In order to detect the time of TCR gene rearrangement in differen-tiated NKT cell, we performed PCR for TCR-rearrange-ment excision circles (TRECs) with isolated DNA. Results : Signal joint TREC first appeared on day 4 or day 8 and continuously existed until day 30. Between day 9 and day21, cells showed high output of coding joint TRECs after signal joint rearrangement. Conclusion : In differentiated NKT cells, TCR gene rearrangement started within a week after culture started and mostly occurred in 2 to 3 weeks after culture started. 목적: 제대혈에서 분리한 CD34 양성세포를 사이토카인을 이용하여 NKT 세포로 분화시키는 과정에서 T 림프구의 수용체(TCR) 유전자의 재배열이 어느 시기에 나타나는지 알아보고자 하였다. 방법: 제대혈에서 분리한 CD34 양성세포에 IL-15, Flt3-L와 stem cell factor를 첨가하여 액체 배양하였다. 일주일 간격으로 세포를 수거하여 DNA를 분리하고, 이것을 TCR 유전자 재조합의 결과로 생겨나는 TCR-rearrangement excision circles (TRECs)을 검출하는 시발체(primer)를 사용하여 중합효소연쇄반응을 시행하였다. 결과: 배양 시작 4일째부터 TREC이 검출되어 배양 후 30일까지 검출되었으나, 배양 시작 2~3주에 그 정도가 가장 높았다. 결론: 제대혈에서 분리한 CD34 양성세포에서 분화하는 NKT 세포의 TCR 유전자 재조합은 배양 개시 후 일주일 이내에 시작되어 배양 개시후 2~3주에 활발히 일어난다.

The purpose of this study is to investigate the relations among the maternal social competence, involvement behaviors on the children's peer relations and the children's peer competence and to find out the path of the maternal social competence and involvement behaviors on the children's peer competence affect the children's peer competence and the relative influence. The subjects of this study were 219 children of two kindergartens and three children's houses located in Seoul and Kyoungki-province and their mothers. To make things clear, three kinds of questionnaires were applied. Interpersonnal Competence Questionnaire(ICQ), developed by Buhrmester etc, was used to measure mother's social competence. Parental Involvement Checklist(PIC) was used to measure mother's involvement behaviors on children's peer relation. And Children's Peer Competence Scale, developed by Ju Hee Park etc, was used to measure childrens' peer competence. To analyze the data, the prequency and percentage were produced through the statistics package SAS. The data were analyzed by means, standard deviation, and correlation analysis. And path analysis is also undertaken to find out the path which affects children's peer competence and the relative influence. The results obtained from this study are as follows. Firstly, positive variables affecting the son's peer competence are maternal social competence, involvement behaviors on children's peer relations(supervision, advice and support, adjustment and mediation, the total score of involvement behaviors on children's peer relations). Positive variables affecting the daughter's peer competence is the maternal involvement behaviors on the children's peer relations(monitoring, consultation and support, arrangement and mediation, the total score of the maternal involvement behaviors on children's peer relations). Secondly, the maternal social competence hadn’t an influence on the children's peer competence directly, but it had an influence through the involvement behaviors on children's peer relations indirectly. That is, the maternal involvement behaviors on children's peer relations mediates between the children's peer competence and the maternal social competence. There was mediator function in the maternal involvement behaviors on the son's peer relations(monitoring, consultation and support, arrangement and mediation, the total score of the maternal involvement behaviors on children's peer relations). The other side, there was mediator function in consultation and support, the total score of the maternal involvement behaviors on children's peer relations of the maternal involvement behaviors on the daughter's peer relations.

We investigate the relationship between the ownership structure(institutiona1 investors, individuals investors, and foreign investors) and returns of firms on days when the absolute value of the market's return is 6.5% or more. These days are reffered to as "event-days." Our basic premise is that institutional shareholders react strongly to large market price changes by herding together and moving price. Because they are risk averse managers. A manager's reputation is hurt if everyone makes the same bad decision than if only the manager makes the bad decision. So, The managers follows a rational decision-making process. They do herding behavior. We find that a firm's abnormal return on these days is not related to the percentage of institutional ownership, and foreign ownership. This results means that institutional investors, foreign investors don't herd behavior.

가미과루해백황금탕의 항고지혈증 효과

Gamigwaruhaebaekwhanggum-Tang (GGHWT) have been evaluated for antihyperlipidemic effects on experimental hyperlipidemic rats and mice induced by Triton WR-1339, corn oil and high cholesterol-diet. Especially, GGHWT is formulated with Trichosanthis Fructus, Pinelliae Tuber, Aurantii Immaturus Fructus, Magnoliae Cortex, Allii Macrostemi Bulbus, Cinnamomi Ramulus and Scutellariae Radix. Oral administration of GGHWT at 500 mg/kg/day for 3 days significantly inhibited the increase of serum triglyceride and LDL-cholesterol, liver triglyceride in hyperlipidemic rats induced by Triton WR-1339. And, GGHWT significantly inhibited the increase of serum triglyceride in hyperlipidemic rats induced by corn oil. Also, administration of GGHWT (500 mg/kg, once daily for 1 week, p.o.) prevented the increase of serum total cholesterol, triglyceride, LDL-cholesterol, liver total cholesterol and triglyceride in 1% cholesterol-diet fed mice. These results suggest that GGHWT is effective for the treatment of hyperlipidemia.

80% extract of Gamigwaruhaebaekbaekju-Tang (GGHBT), Gagamgwaruhaebaekbaekju-Tang (GGGHBT) and Gamigwaruhaebaekwhanggum-Tang (GGHWT) remarkably showed inhibitory effects on HMG-CoA reductase, lipid peroxidation of rat liver and LDL oxidation, and DPPH free radical scavenging effect in a dose-dependent manner. Especially, GGHWT which is formulated with Trichosanthis Fructus, Pinelliae Tuber, Aurantii Immaturus Fructus, Magnolia Cortex, Allii Macrostemi Bulbus, Cinnamomi Ramulus and Scutellariae Radix on the basis of Gwaruhaebaekbaekju-Tang listed on the traditional medicinal references showed more effective hypocholesterolemic activities in vitro bioassay than the other prescriptions.
