Actinomycosis is a chronic suppurative infection, whitch infiltrates mucosa-associated tissues. Thoracic, pelvic and cervicofacial infections of actinomyces are not uncommon, but endobronchial actinomycosis is rarely reported. We report a case of a 29 year old man who presented with a recurrent pneumonia and chronic cough. Fiberoptic bronchoscopy revealed occlusion of the right. lower medio-basal segment bronchus with exophytic endobronchial mass. The diagnosis was confirmed histologically by demonstration of sulfur granules in the bronchoscopic biopsy of the mass. Intravenous administration of amoxacillin and clindamycin followed by oral amoxacillin and clindamycin therapy resulted in complete recovery.

상지의 분절성 운동 마비 증상을 나타낸 대상포진 감염 1예

Herpes zoster is a viral disease that produces inflammatory lesions in the posterior root ganglia with pain and skin eruptions of affected ganglia. It has been rarely associated with motor paralysis of extremity. But involvement of motor nerve is in a small percentage(1-5%) in the herpes zoster infection. A 61-year-old woman presented with painful grouped vesicular eruptions on the right posterior neck and shoulder area. And she complained of weakness of shoulder joint and arm with painful skin lesions. Neurologic examination showed weakness of right shoulder muscles. EMG/NCV studies suggest cervical radiculopathy(C5, 6, 7). The cervical spine MRI study and CSF examination were carried at seven days after symptoms. The CSF study showed pleocytosis(WBC=144), increased protein level(103mg%), and positive antibody of HZV-Ig G & M. The motor weakness finding cleared forty days later. We report a patient of herpes zoster infection with segmental motor paresis of right shoulder muscles and positive CSF HZV-Ig G & M.

암치료에 있어서 체질과 항암효과에 관한 한의학적 연구

As one of the chemical and experimental study on cancer treatment in Korea oriental medical hospital. Furthermore, we carried out the cytotoxicity test of extracts of herbal medicine. The results are as follows : About Clinical Aspects of the Oriental Medical Cancer Treatment - (1) Clinical experience is that 420 Oriental medical hospital were surveyed. Number of cancer patient treated by one oriental medical doctor was under 10(about 60%) or from 10 to 20(about 30%). (2) Diagnostic Procedure is that more than 80% of cancer patients were diagnosed by medical doctor of western hospital. (3) Therapeutic Method is that combination of oriental and western medical treatment was used in oriental medical hospital. (4) Relationship between cancer and constitutions is that to the relationship between cancer and constitutions, about 75% responded and most were considered to be Um. About Experimental Study - to evaluate the direct cytotoxicity of hot water and methanol extracts of 34 oriental medical presciptions on stomach cancer cell, SRB assay was carried out. (1) The IC50 value of water extract of Samsoum was 19㎍/㎖ and that of methanol extract was 15㎍/㎖, so that we confirmed Samsoum has a strong direct cytotoxicity. (2) The IC50 values of water extract of Coptidis Rhizoma, methanol extract of Caesalpiniae Lignum and Draba Semen were 10㎍/㎖, 19㎍/㎖ and 12.5㎍/㎖ respectively, so that we confirmed Coptidis Rhizoma, Caesalpiniae Lignum and Draba Semen have a strong direct cytototoxicity. (3) The IC50 value of combinatory prescription of Rhei Rhizoma, Forsythie Fructus and Caesalpiniae Lignum was about 40㎍/㎖.

Despite the use of aggressive local and systemic treatment including autologous stem cell transplantation in multiple myeloma, extramedullary recurrences are common and the prognosis of these patients is poor. Many novel drugs such as thalidomide, lenalidomide and bortezomib improve the response of treatment of multiple myeloma, but some reports failed to describe thalidomide has effect in extramedullary plasmacytoma. Recent data report on the successful treatment plasmacytomas with bortezomib in patients with advanced multiple myeloma. We treated 4 relapsed or refractory extramedullary plasmacytomas with bortezomib at our institution. We recognized all these extramedullary plasmacytomas decreased and showed more than partial response. This report lends support to the efficacy of bortezomib in the treatment of plasmacytoma and describes the safe use of bortezomib. Responses may, however, be of short duration. Therefore, despite our limited experience, we propose that bortezomib may be considered a therapeutic option for such patients who have risk of radiation therapy

Familial adenomatous polyposis (FAP) is an autosomal dominant syndrome, typically characterized by multiple colorectal adenomas and increased incidence of colorectal carcinomas if it is not treated. Moreover, a variety of extracolonic manifestation are seen. The prevalence of thyroid tumors developing in patients with FAP is about 1∼2%. Generally, papillary thyroid cancer indicates low mortality but ,however, as people get older, the cancer becomes more aggressive. So, the operation is required. Recently we experienced a case of FAP, presenting with papillary thyroid carcinoma, and reported with a brief review of literatures.

Mirizzi's syndrome is a rare complication of the gallstone disease, characterized by narrowing of the common bile duct (CBD) due to chronic extrinsic compression of an impacted gallstone in the cystic duct or the neck of the gallbladder. The impacted gallstone may erode into the bile duct, causing cholecystocholedochal fistula. The patients generally have history of repeated attacks of jaundice and abdominal pain. But there is no report about asymptomatic Mirizzi syndrome with cholecystocholedochal fistula which were found incidentally. So herein we present our clinical experience with a case of asymptomatic Mirizzi syndrome with cholecystocholedochal fistula in a young female.

RAW 264.7 세포에 대한 중국산 천연 광물성 섬유 TAFMAG의 독성효과

Objectives : This study was designed to evaluate cytotoxicity of TAFMAG, which is a trade name of natural mineral fiber mined and produced in China. Methods : The cytotoxicity of TAFMAG was evaluated by measuring iron content, lipid peroxidation, erythrocyte hemolysis, and cytotoxicity in vitro. These results were compared with the data of chrystotile and wollastonite as a positive and negative control, respectively. Results : There was significant increase of Fenton activity in TAFMAG and chrysotile with dose-response pattern. The iron chelating agent, desferrioxamine, significantly decreased Fenton activity of the particulates except wollastonite. TAFMAG and chrysotile fibers significantly increased malondialdehyde concentration from lipid peroxidation of the red blood cell membrane. In erythrocyte hemolysis test, TAFMAG & chrysotile had stronger effect on erythrocyte hemolysis than wollastonite with the concentration of 1,000g/ml. Furthermore, TAFMAG was more hemolytic than chrysotile with the concentration of 5,000g/ml. There was a significant cytotoxic effect in TAFMAG and chrysotile on RAW cell compared with wollastonite. Conclusions : In vitro study suggested that TAFMAG may have a similar health hazard as usual asbestos.

Analysis of Carotenoid Content of Three Types of Chilis Distributed in the South Korea Market

In this study, the content of carotenoid (beta-carotene, alpha-carotene, and beta-cryptoxanthin) in three types of chilis was analyzed by HPLC equipped with an UV-Vis detector (UVD). Alkaline hydrolysis (saponification) was performed for carotenoid analysis. The accuracy and precision of analytical method was evaluated by certified reference material (Multivitamin/Multielement Tablets). The homogenized sample was saponified with a 60% KOH solution and then extracted by solvent extraction, carotenoid was detected with a UVD of 450 nm and quantitatively analyzed. The accuracy and precision of the analysis method were evaluated by Multivitamin/Multielement Tablets. As a result of the analysis, In the three cooking methods (raw, blanched, and steamed), carotenoids were detected in the order of red chili, Cheongyang chili, and korean shishito pepper. The content of alpha-carotene in steamed cheongyang chili, Korean shishito pepper from 8.8,1.3 μg/100 g. The highest and lowest beta-carotene contents were detected in steamed red chili, raw Korean shishito pepper. Specifically, beta-cryptoxanthin of 9.0, 8.2 μg/100 g was detected in blanched, steamed Korean shishito pepper.

Synthesis and Assembly of Porphyromonas gingivalis Fimbrial Protein in Potato Tissues

<P>Periodontal disease caused by the gram-negative oral anaerobic bacterium Porphyromonas gingivalis is thought to be initiated by the binding of P. gingivalis fimbrial protein to saliva-coated oral surfaces. To assess whether biologically active fimbrial antigen can be synthesized in edible plants, a cDNA fragment encoding the C-terminal binding portion of P. gingivalis fimbrial protein, fimA (amino acids 266-337), was cloned behind the mannopine synthase promoter in plant expression vector pPCV701. The plasmid was transferred into potato (Solanum tuberosum) leaf cells by Agrobacterium tumefaciens in vivo transformation methods. The fimA cDNA fragment was detected in transformed potato leaf genomic DNA by PCR amplification methods. Further, a novel immunoreactive protein band of ~6.5 kDa was detected in boiled transformed potato tuber extracts by acrylamide gel electrophoresis and immunoblot analysis methods using primary antibodies to fimbrillin, a monomeric P. gingivalis fimbrial subunit. Antibodies generated against native P. gingivalis fimbriae detected a dimeric form of bacterial-synthesized recombinant FimA(266-337) protein. Further, a protein band of ~160 kDa was recognized by anti-FimA antibodies in undenatured transformed tuber extracts, suggesting that oligomeric assembly of plant-synthesized FimA may occur in transformed plant cells. Based on immunoblot analysis, the maximum amount of FimA protein synthesized in transformed potato tuber tissues was approximately 0.03% of total soluble tuber protein. Biosynthesis of immunologically detectable FimA protein and assembly of fimbrial antigen subunits into oligomers in transformed potato tuber tissues demonstrate the feasibility of producing native FimA protein in edible plant cells for construction of plant-based oral subunit vaccines against periodontal disease caused by P. gingivalis.</P>
