        금속가공유 사용 근로자에서 발생한 과민성폐장염 1예

        서론: 과민성폐장염의 원인물질로 금속가공유는 외국의 경우 작업장의 집단발병 사례들을 중심으로 많은 보고가 되어 왔으나 우리나라에서는 아직 보고되지 않았으며,저 자들은 금속가공유에 1년간 노출된 근로자에서 발생한 과민성폐장염 1예를 발견하였기에 작업환경측정을 시행한 후 작업관련성을 평가하여 보고하는 바이다. 증례: 64세 남자 환자가 1년간 강관을 절단하는 작업을 한 후 호흡곤란을 주소로 내원하였다 노출력,임상적 소견 및 방사선학적 소견을 토대로 과민성폐장염으로 진단되었다. 작업환경 측정결과 공기 중 금속가공유,엔도 톡신,총 세균 및 곰팡이는 각각 0.531 mg/m³, 6.33 EU/m³ 100 CFU/m³ 및 75 CFU/m³이었고,금속가 공유 내 엔도톡신,총세균 및 곰팡이는 각각 1.5x10⁴EU/mL,4.6x10^(5) CFU/mL 및 1.8×10^(5) CFU/mL이었다. 고찰: 금속가공유 내의 원인항원에 대한 침강항체를 검사하지 못했으나,작업환경 측정결과 금속가공유가 일반적인 허용가능 수준 이상으로 오염되었으며,기존의 과민성폐장염 발생 사례들과 비슷한 수준 또는 그 이상으로 오염되어 있었다. 공기 중 금속가공유 농도 역사 NIOSH의 REL과 ACGIH의 NIC를 초과하고,기존의 사례들에 비해서도 대체적으로 높은 수준 이었다. 금속가공유 이외의 다른 원인에 대한 감별과 기존의 발생사례들과의 노출수준 비교를 통하여,저자들은 환자가 금속가공유에 의한 미생물 항원에 노출되어 과민성폐장염이 발생한 것으로 판단하였다. Introduction: There have been no reports of hypersensitivity pneumonitis caused by metal working fluids in Korea, despite their existence in other countries. Here, we report the first such case, along with an assessment of work-relatedness through exposure assessment. Case report: A 64-year old male patient visited the hospital with dyspnea after metal pipe cutting for about a year. He was diagnosed with hypersensitivity pneumonitis from the evidence of specific exposure history, clinical symptoms, and radiologic findings. The air exposure levels of the oil mist, endotoxin, total bacteria and fungus in the work environment was TWA(8-hr) 0.531 mg/m³, 6.33 EU/m³, 100 CFU/m₃ and 75 CFU/m³, respectively. The concentrations of the endotoxin, total bacteria and fungus within the metal working fluid was 1.5 X 10⁴EU/mL, 4.6 X 10^(5) CFU/mL and 1.8 X 10^(5) CFU/mL, respectively. Discussion: Although the patient did not receive a specific precipitating antibody test, the microbial concentration within the metal working fluid was higher than normal and similar to previous case reports, The oil mist level in the air exceeded the NIOSH REL and ACGIH NIC, and were similar or higher than previous cases. By excluding other causes of hypersensitivity pneumonitis, we concluded that the disease developed from exposure to microbial antigens in the metal working fluid.

        기종성 신우신염은 주로 당뇨병환자나 폐쇄성 요로질환을 가진 환자에 발생하며 특징적으로 신실질 및 그 주위조직에 가스를 형성하면서 심한 조직괴사를 일으키는 매우 드문 급성 화농성 신감염으로 사망률이 높은 질환이다. 이 질환은 1989년 Kelly와 MacCallum에 의해 처음 보고된 이래 외국문헌에 약 90례 정도가 보고되어 있으며 국내에는 28례가 보고되어 있다. 수액 및 전해질 보충, 인슐린 투여를 통한 적절한 혈당조절 및 항생제 투여등의 내과적 치료와 절개배농 및 신적출술등의 수술적 치료 방법이 있다. 내과적 치료로 가스의 감소가 없는 경우에는 즉각적인 수술적 치료를 시행하여야하므로 이 가스변화에 대한 추적관찰이 중요하다 하겠다. 본 저자들은 당뇨병환자에서 발생한 기종성 신우신염 1례를 내과적 요법을 치험하였기에 문헌고찰과 함께 보고하고자 한다. Emphysematous pyelonephritis is rare, life-threatening infection of the renal parenchyma and perirenal tisseue. This disease is characterized by the production of intrarenal and perirenal gas and is frequently encountered in patients with diabetes mellitus or urinary obstruction. We experienced a case of emphysematouse pyelonephritis in a 62 years old women with poorly controlled diabetes mellitus who had been managed with medical theraphy. So we report this case with a review of the referenced literatures.

        한국 정신장애의 역학 조사 연구[I] : 각 정신장애의 유병률

        Objectives : This study aims to estimate the prevalence of the DSM-IV psychiatric disorders in Korean population using the Korean version of Composite International Diagnostic Interview (K-CIDI). Methods : Subjects were selected by taking multi-stage, cluster samples of 7,867 adult household residents, 18 to 64 years of age, in ten catchment areas. Total 78 trained interviewers administered the K-CIDI to the selected respondents, from June 1 to November30,2001. Results : Total 6,275 respondents completed the interview. Some 33.5% of respondents reported at least one lifetime disorder, 20.6% reported at least one-year disorder, and 16.7% reported at least one-month disorder. The most common lifetime disorders were alcohol abuse/dependence (17.24%), nicotine dependence/withdrawal (11.19%), specific phobia (5.16%), and major de-pressive disorder (4.25%). The lifetime prevalence of substance abuse/dependence (0.25%) and schizophrenia (0.16%) was very low. Nicotine and alcohol use disorder showed very high male/female ratio. Mood disorder and anxiety disorder were more prevalent among female than male. Conclusion : The prevalence of psychiatric disorders was high. In comparison with other studies, remarkable differences in distributions of psychiatric disorders across the areas and times were observed.

      • 高速度鋼의 充塡硬化에 對하여

        Pack hardening process is used to prevent the furnace atmosphere from reducing the carbon, and from the oxidation in the surface of High Speed Steel at heating. In this process, steel boxes are used and the work is packed in charred leather, granulated charcoal and similar materials. High Speed Steel is composed of various special elements. These elements are made into carbite by combining with carbon. To get high hardness of High Speed Steel by quenching, the carbite should be melted in austenite into the solid solution. But it is hard for the carbite to be melted into the solid solution. The factors of solid solution in austenite are the heating temperature and time. according to the result of experiments, I got the same hardness as the general quenching method in a high temperature by lengthening the time, instead of making the heating temperature fall in the relation of temperature-time. For example, choosing JIS SKH4B (similar AISI type T6) as a specimen, I obtained the hardness of RC 64-65°b/quenching in pack hardening method at 1100℃, changing the heating temperature and time as the relation between the heating temperature- hardness and the heating time-hardness. I know it is possible to get a high hardness of High Speed Steel at 1100℃, which is 225℃ lower than 1325℃ of the general quenching temperature. In addition to the result above, the pack hardening method is better than the general one, on working of hardening and mechanical property, etc.

        간접흡연이란 담배를 피우지 않는 사람이 흡연자의 담배연기를 간접적으로 마시게 되며, 본인의 의사와는 상관없이 비흡연자가 담배연기에 노출되는 상태이다. 간접흡연의 피해는 모든 사람에게 눈과 코의 자극증상, 목의 통증, 두통, 기침 등을 일으킬 수 있으며, 간접흡연자는 흡연자와 동일한 질병을 앓게 된다고 한다. 우리나라 20세 이상의 성인의 흡연률이 미국이나 유럽보다 높다. 따라서 우리나라 국민들이 간접흡연에 노출될 기회가 더 많을 것으로 생각된다. 이에 간접흡연에 의한 건강문제를 알아보고 간접흡연자를 위한 대책과 흡연자들에게 금연의 중요성을 알리고자 한다. Passive smoking is the involuntary inhalation of tobacco smoke by a person, especially a non-smoker, who occupies an area with smokers or a smoker. Passive smoking may cause eye and nose irritation, sore throat, headache, cough to everyone, and it is known that non-smokers have the same kinds of illness with smokers. Smoking rate among the adults aged over 20 in Korea is higher than that of the US or European conturies. Therefore we suppose that Koreans may be exposured to passive smoking than the people of other countries. It is necessary that people have the knowledge on the heath problems caused by passive smoking and on prevention from the involuntary inhalation of tobacco smoke. I propose that the smokers should be informed the importance of smoking cessation.

      • 지면판위 복수 장애물 주위 유동장의 가시화(Ⅰ)

        地面板위에 모양이 서로 다른 복수 장애물을 바람의 방향과 수직으로 배열(Side-Side)하여 액체4염화 티타늄을 관찰대상 표면에 직접 도색하여 유동장을 효과적으로 가시화 하였다. 장애물 사이의 간격이 좁아지면 틈을 통과하는 흐름은 어느 한쪽으로 傾斜진다는 특징이(Biased Flow) 확인되었고, 지면판위 박리선, Necklace Vortex, 후류등 3차원 유동장 특징이 확인되었다. Flow patterns around two-bluff bodies in side-by-side arrangements on the ground plane are easily visualized using liquid titanium tetrachloride. In two-dimensional flow field, the gap flow between two-bodies became biased to one side for closer spacing. Three-dimensional flow patterns, which have seperated regions, necklace vortices and other dynamic seperations, are dependant on the bluff-body shape and the gap-space ratio.

Because of both conflicts between powerful countries and several ideologies in the early period. and the political interferences, the commercialization of The Olympic Games, and taking illegal drugs for a athlete's game ability in the contemporary period, amateur spirit of The 1st Athens Games has been changed and the naive Olympism has met a crisis. So the necessity of The Olympic Games' reform is being stressed all around the world. Especially, ex-chairmen of IOC, Coubertin, Baillet Latiur. Sigfrid Estrom, Brundage, Lord Killannin has emphasized The Olympic Games' reform should be needed. A lot of intellectual journalists has pointed out the necessity of a reform in IOC and The Olympic Games' system since 1896. But, to be more important is for these voices of reform not to be accepted. So, Jeffery said, "how long does it take athletes of young age around the world to understand that they has been exploited by persons and institutes loyal to the not-sublime aim and he added extremely." If IOC can't find out the fundamental sense and flexibility. The Games had better be abolished. It is natural that The Olympic Games should be changed in accordance with speedy changes in every part around the world. So the world wants to reform The Games keenly. To do it, new Olympic Charter should be made in accordance with both Coubertin's modern Olympic idealism and the 21 century's pragmatism and a result of it, IOC can be reborn as not a powerful one to deal with the Games exclusively but a service organization and The Olympic Games should continue to be held forever in the world.

        Shaphylococcus enterotoxin B와 lipopolysaccharide를 작용시킨 사람 섬유아 세포에서 생성된 Transforming Growth Factor-β₁의 정량적 분석

        TGF-β1 is a potent chemotactic factor for inflammatory cells and fibroblasts. It also stimulates the celluar source and components of extracellular matrix and the production of proteinase inhibitors. Collectively, these biologic activities lead to the accumulation and stabilization of the nascent matrix, which is vital to infection control. The objective of this study is to investigate production of TGF-β in vitro fibroblast culture in the presence of Staphylococcus enterotoxin B(SEB) and/or lipopolysaccharide(LPS) and to elucidate the role of TGF-β1 which may be responsible for infection control. The fibroblasts were originated from gingiva and facial dermis in 26 year-old male patient. In the presence of LPS(0.01㎍, 0.1㎍, 1.0㎍), SEB(0.01㎍, 0.l㎍, 1.0㎍) respectively, cells(5×103ml) were cultivated in vitro. At 1, 3, and 5 days after incubation, cells were counted. Also, cells(2.5×105ml) were cultivated in EMEM with LPS(0.01, 0.1 and 1.0㎍), SEB(0.01, 0.1 and 1.0㎍) respectively and LPS(0.1㎍) and SEB (0.1㎍) in combination for 24,48, and 72 hours respectively. Culture supernatants were harvested at 1, 2, and 3 days after incubation period and triplicate culture supernatants were pooled and TGF-β1 was assayed in duplicate. The results were as follows. 1.In gingival fibroblast induced with SEB and LPS respectively or in combination, the suppression of cell Proliferation occurred very significantly since 3 days after incubation, compared with the control and the production of TGF-β1 occurred very significantly at 1 day after incubation, compared with the control. 2.In facial dermal fibroblast induced with SEB and LPS respectively or in combination. the suppression of cell proliferation occurred very significantly at 1 day after incubation, compared with the control. In SEB exposure, the production of TGF-β1 was decreased very significantly at 1 day after incubation, compared with the control. However, in LPS, SEB and LPS exposure, the production of TGF-β1 was increased very significantly at 1 day after incubation, compared with the control. In conclusion, the concentration of bacterial toxins and the incubation period correlated with cell proliferation and production of TGF-β1 very significantly. The gingival and facial dermal fibroblasts have different phenotype each other The orchestrated understanding of fibroblast proliferation and TGF-β1 production play an important part in host defense against the bacterial Infection and may prevent tissue necrosis such as necrotizing fasciitis and life-threatening syndrome such as multiple organ failure.

        The purpose of this study was to investigate current situations on middle school physical education(PE) teacher's health instruction in Kangwon-do. The results attained from 165 middle school physical education teacher's in Kangwon-do are listed below: 1.Although physical education recognize the absolute necessity of health instruction in middles schools in Kangwon-do, it can't be acknowledged that sufficient instruction is being performed due to numerous other reasons. 2.Mostly, physical education teachers are in change of health instruction, but sometime it is refered to nurse-teachers, 3.For appropriate health instruction, first of all, related teaching materials and training opportunities should be more available. Based on above conclusion, following suggestions are made: 1.Information about health instruction must be sought with a great weight when teachers have any training opportunity. 2.School classroom must equip necessary teaching materials for health instruction. 3.There have to be more opportunity for intensive training to teachers. 4.Health instruction includes subtle subjects like problems relates sex education. Physical education teachers be in change of course but it can asked to nurse teachers if it for a better quality of health instruction.(It should be more rational to request nurse-teachers if a young male physical education teacher is in a girl's school) Also, issues related to independence on health instruction subject must be carefully examined.

        교잡종 찰옥수수에 대한 연구 : Ⅱ. 새로 육성된 찰옥수수 교잡종 "대덕찰"에 대한 농업적 특성 Ⅱ. Agronomic Characteristics of the New Waxy Hybrids "Daedukchals"

        1. 새로 육성된 4개의 대덕찰 찰옥수수 교잡종에 대한 간장은 대조구인 찰옥 1호보다 큰 반면에 개화기는 약 4∼7일 늦었으며, 모든 공시 교잡종의 주당 분얼수는 한 개이하의 무효분얼로 나타났다. 2. 새로 육성된 대덕찰옥수수의 이삭길이와 직경은 대조구인 찰옥 1호보다 크게 나타났으며, 특히 대덕찰옥수수 1호와 2호의 과피두께는 대구조보다 얇았고, 당도 역시 높게 나타나 우수 교잡종으로 평가되었다. Stem height of the new developed "Daedekchals" waxy hybrids were higher than check hybrid. Chalok #1, while days to tasseling were delayed about four to seven days. Number of tillers per plant of the these new hybrids including check were appeared to be one less as non-effective tiller. Ear length and diameter of the new waxy hybrids were longer and thicker than check, respectively. In addition pericarp thickness of the Daedekchal #1 and #2 among new developed waxy hybrids were thinner and sugar content were higher than check.
