        내시경적 역행성 담췌관조영술 후 발생한 급성췌장염의 발생인자로 여러 가지 인자들이 관여하지만 특히 담췌관조영술 시행 후 과도한 췌선방 조영화에 의한 중증의 췌장염 증례를 경험하였기에 보고하는 바이다 Endoscopic retrograde cholangiopancreatography (ERCP) has been considered as a valuable tool for the diagnosis and management of disease of the pancreas and biliary tree. Post-ERCP pancreatitis is the most frequent and clinically significant complication following ERCP. But a few complication associated with this procedure are unforeseeable. Specially, acinarization as a result of a careless cooperation between operator and assistant may be influenced in complications of ERCP. Severe post-ERCP pancreatitis is rare complication, and its prognosis will be poor. Therefore, careful process and good relationship between operator and assistant in ERCP may be prevented this complication, but is not clear. Neglect of acinarization may be resulted in more aggravation of pancreatitis. We have cared for one patient with severe post-ERCP pancreatitis with pseudocyst after water-soluble contrast media spreading into the acini (acinarization) via ERCP in resolving phase idiopathic pancreatitis.

        한국인 직무 스트레스 측정도구의 개발 및 표준화

        Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

        한국 여성운동과 걸스카우트운동 : 과거·현재·미래 Yesterday, Present and Tomorrow

        One of the major implications of women's movements seems to lie in the fact that it tends to promote women's consciousness-raising and, thus, heighten social status of women in general. It would be very much the case when discussion centers around the emergence of Korean women's movements in modern sense. Due to the socio-political atmospheres of the lathe 19th century Korean society, its basic characteristics owe to strong tones of patriotic nationalism under the slogan of "learning is might enough to deal with the imperialistic foreign powers of the time." As such, ever since the introduction of women's enlightenment movements of the time, Korean women's movements have provided for the advancement of egalitarian society. However, assessing the effectiveness of programs that are practiced by the existing women's organizations in Korea, it deserves mention in that most programs are for the adult female members, assuming it would only be applicable to those adults. In this regard, it can be said that the Korean Girl Scout organization has worked a unique role to make up such voids in women's movements - most programs developed and exercised by the Girl Scout are reserved for those young girls. For instance, special activities like social services, field campaigns, and international gatherings would definitely work a profound impact in the formation of gender-role stereotypes of the youth. The present study tries to examine the Korean Girl Scout movement being the case of women's movement organization. Discussion will cover such topics as; historical reviews of Korean Girl Scout movement, principal organizational goals and its practice in real life, and internal dynamics of members between "staffs" and the ordinary members themselves-on perceptions of the movement, evaluation of leaderships, and the future orientations of the Girl Scout, etc. Various research methods are employed. Book reviews, in-depth interviews, questionnaires, and participant observations are utilized, when the respective measurement is required. Major findings of the study can be summarized as follows: (1) Numbers of Girl Scout membership have declined since 1980, while revealing its utmost pick upto mid-70s. In terms of regional distributions of the members, both Seoul and Kyung-Nam Branches are the most prosperous. Whereas commemoration ceremonies and camping works are considered most impressive programs, membership trainings and field works are the lowest. About 70 percent of the members show their length of activities in Girl Scout less than one year. Most members are involved in the activities based on self-decision in order to have social services, club memberships, or self actualizations. They are satisfied with most of the programs, especially when participating in the social service programs. (2) As for the staff members, about half of the leaders in the headquarters are newly appointed. Whereas majority of the 'field' staffs are young (in their 20s, or 30s), other leaders are considered old (most of them are in their 40s, 50s, or 60s). Half of the leaders are involved in the activities for less than 5 years. They are recruited by either colleagues' or office seniors' recommendations. Reasons to participate in the movement are to have self-actualized or to have opportunity for social services. All in all, leaders are satisfied with the Girl Scout activities mainly because they can see their youth members progressing and developing. (3) Several issues for the better organizational managements seem to be related. prevalence of aspirational differences between leaders and members tends to cause psychological burdens. Other problematics such as lack of free communication networks, tensions due to cohort differences, authoritarian leadership styles, and financial difficulties would hinder Girl Scout movement more promising and popular to all. Based on the research findings, two major suggestions might be in order. First, underlying rationales of the Girl Scout movement should be reoriented. A special emphasis on the creation of self-supportive 'androgynous individuals' would be the case (in this regard, an intensive program targeted for those pre-school-aged members would be the most effective). As a way to achieve the strategic reformulation, reliance on the interdisciplinary program (for example, coordination with the university) will be one possibility. Secondly, an organizational management should be sufficiently considered. provisions (tangible and intangible) for the 'field leaders' being the key leader group in the Girl Scout movement should be afforded enough to keep their participations voluntary and continuous.

        To evaluate the health behavior and medical utilization patterns of residents in Choongju rural area is useful for district health authority and health facilities in identifying the problems and setting the priorities. The reason is that rural area in Choongju city has a relatively few health resources except public health subcenters and public health posts. We evaluated the health behavior, health status, and medical utilization pattern of people in Choongju rural area using the household survey in summer, 1995. From 12 Myuns, randomly selected 6 Myuns first, and then applied convenient sample method. The residents had many health problems. 98(32%) out of the 307 respondents had some health problems, most of which were musculoskeletal and gastro-intestinal problems. A total of 218 persons(72%) thought that it was individual responsibility. Because the Lay referral system was poor and many of residents had to make the decisions about their health problems and medical seeking by themselves. When they were ill, 34% of respondents answered that they would visit hospitals first, and 24% would visit health-subcenter. But, when illness was severe, 72% of respondents answered that they would use hospitals at first. We suggest that public health facilities in rural area should be activated since they play the major role in promoting the health of rural residents. Health programmes such as health education, counseling, and disease prevention program, are also necessary especially for most of elderly persons.

        The Longitudinal Trend of Cardiac Surgery in Korea from 2003 to 2013

        Background: The purpose of this study was to investigate longitudinal changes of the utilization of operational and surgical medical care inside and outside a metropolitan area over 10 years, analyzing the residential areas of patients and the locations of medical facilities for major cardiovascular surgery. Methods: Data analysis was conducted by classifying the addresses of patients and the locations of medical care facilities of metropolitan cities and provinces, using data from the National Health Insurance Corporation from January 2003 to December 2013. Results: There is serious concentration of major heart surgery to medical facilities in Seoul; this problem has not improved over time. There were differences in percentages of surgical procedures performed in the metropolitan areas according to major diseases. In the case of Busan and Daegu provinces, at least 50% of the patients underwent surgery in medical facilities in the city, but there are other regions where the percentage is less than 50%. In the case of provinces, the percentage of surgical procedures performed in medical facilities in Seoul or nearby metropolitan cities is very high. Conclusion: Policies to strengthen the regional capabilities of heart surgery and to secure human resources are required to mitigate the concentration of patients in the capital area. Many regional multi-centers must be designated to minimize unnecessary competition among regional university hospitals and activate a win-win partnership model for medical services.

        자가면역성 갑상선 질환을 동반한 한국인 제1형 당뇨병 환자에서 항GAD65 항체의 측정 : 면역침전후 Western Blotting법 Immunoblotting followed by Immunoprecipitation

        Background : Type 1 diabetes mellitus is frequently associated with other autoimmune diseases. The broad concept of polyendocrinopathies takes into consideration that patients affected by at least one endocrine disease may have another autoimmune disorder or express specific autoantibodies. Anti-glutamic acid decarboxylase autoantibodies, now recognized as one of the major serological markers for type 1 diabetes has been reported to be higher in type 1 diabetes patients with autoimmune thyroid diseases (ATD) than in those without ATD. The objective of the present study was to evaluate the prevalences of GAD65 antibodies applying a newly developed assay(anti-GAD65) in type 1 diabetes patients with and without ATD. Methods : We developed a new anti-GAD65 assay after mammalian expression of a recombinant GAD65 antigen. Since the detection of anti-GAD65 is rather complicated and insensitive due to inherent antigenic difference of antibody recognition in conventional assays, we applied this new approach in measuring anti-GAD autoantibodies and compared the result with ICA and anti-GAD measurement using the purified porcine GAD (anti-GAD) in 109 cases of type 1 diabetes, 29 of whom had concomitant ATD (mean age at diagnosis: 7.9 yr, mean duration of type 1 diabetes: 4.5 yrs). Results : The overall prevalence of anti-GAD65 antibodies was 65%(71 of 109) in patients with Korean type 1 diabetes. Prevalences and titers of anti-GAD65 had not changed much after controlling for the duration and the status of concomitant ATD. In contrast, the prevalence of anti-GAD was 56%(61 of 109), while that of ICA(+) WAS 36% in type 1 diabetes patients. We found significant, but not strong association of anti-GAD65 either with anti-GAD(r=0.4, p$lt;0.01) or with ICA(r=0.6, p $lt; 0.001). Conclusion : From this, we could assess that autoantibodies are present at comparable sensitivity and specificity in Korean type 1 diabetes patients. This anti-GAD65 assay, another immunologic marker for type 1 diabetes might also confer disease susceptibility among Koreans, but no increase in the prevalence or in the titer in patients with ATD may suggest that this marker is unlikely to give much benefit, for the detection of the overlapping disease of type 1 diabetes and ATD(J Kor Soc Endocrinol 15:190-203, 2000).

        Clinical Utility of Quantitative CT Analysis for Fissure Completeness in Bronchoscopic Lung Volume Reduction: Comparison between CT and Chartis™

        Objective: The absence of collateral ventilation (CV) is crucial for effective bronchoscopic lung volume reduction (BLVR) with an endobronchial valve. Here, we assessed whether CT can predict the ChartisTM results. Materials and Methods: This study included 69 patients (mean age: 70.9 ± 6.6 years; 66 [95.7%] males) who had undergone CT to assess BLVR eligibility. The ChartisTM system (Pulmonox Inc.) was used to check CV. Experienced thoracic radiologists independently determined the completeness of fissures on volumetric CT images. Results: The comparison between the visual and quantitative analyses revealed that 5% defect criterion showed good agreement. The ChartisTM assessment was performed for 129 lobes; 11 (19.6%) of 56 lobes with complete fissures on CT showed positive CV, while this rate was significantly higher (40 of 49 lobes, i.e., 81.6%) for lobes with incomplete fissures. The size of the fissure defect did not affect the rate of CV. Of the patients who underwent BLVR, 22 of 24 patients (91.7%) with complete fissures and three of four patients with incomplete fissures (75%) achieved target lobe volume reduction (TLVR). Conclusion: The quantitative analysis of fissure shows that incomplete fissures increased the probability of CV on ChartisTM, while the defect size did not affect the overall rates. TLVR could be achieved even in some patients with relatively large fissure defect, if they showed negative CV on ChartisTM.

      • Modulation of the effective density and refractive index of carbon nanotube forests via nanoimprint lithography

        <P><B>Abstract</B></P> <P>The unique properties of carbon nanotubes (CNTs) allow them to be used in various optical applications, such as ultra-dark surfaces, bolometers, metamaterial cloaks, and anisotropic absorbers. In particular, organization of CNTs with controlled density at the sub-micrometer scale could enable new strategies to engineer optically active surfaces. Here, we present a new strategy to engineer the density-dependent optical properties of CNT forests by patterning of catalyst film via nanoimprint lithography (NIL) followed by atmospheric pressure chemical vapor deposition (CVD) synthesis of CNTs. Via this approach, we demonstrate atmospheric pressure growth of CNT structures with widths of 80–350 nm. These structures form self-supporting arrays with height exceeding 500 μm, representing aspect ratios well over 1000:1. Optical attenuation measurement places the density of NIL patterned forests to be a fraction of the density of unpatterned CNT forests, confirming that the CNT density is effectively controlled by the catalyst coverage. The infrared absorbance measurements corroborate the density control, and Kramers-Kronig analysis shows that the refractive indices of the NIL patterned CNT forests are tunable in the range of 1–1.8.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

      • <i>Physalis peruviana</i> L. inhibits ovalbumin-induced airway inflammation by attenuating the activation of NF-κB and inflammatory molecules

        <P><I>Physalis peruviana</I> L. (PP) is well known for its various properties, including its antioxidant property. In our previous study, the protective effects of PP against cigarette smoke-induced airway inflammation were confirmed. The purpose of the present study was to evaluate the anti-inflammatory effect of PP against ovalbumin (OVA)-induced airway inflammation. Treatment with PP inhibited the numbers of eosinophils and the levels of inflammatory cytokines, including interleukin (IL)-4, IL-5 and IL-13, in the bronchoalveolar lavage fluid (BALF) of animal models with OVA-induced allergic asthma. PP also significantly decreased the production of total immunoglobulin E in the serum. Lung sections stained with hematoxylin and eosin revealed that the influx of inflammatory cells was decreased in the lungs of mice treated with PP compared with cells in the OVA group. The increased expression levels of monocyte chemoattractant protein-1 (MCP-1) and T cell marker KEN-5 were also reduced following PP treatment in the lung tissues compared with those in the OVA group. The PAS staining results showed that PP attenuated the overproduction of mucus in the lung. Additionally, western blot analysis revealed that PP significantly downregulated the activation of nuclear factor-κB/p38 mitogen-activated protein kinase/c-Jun N-terminal kinase, and upregulated the expression of heme oxgenase-1 in the lungs. In an <I>in vitro</I> experiment, PP effectively reduced the levels of LPS-stimulated MCP-1 in a concentration-dependent manner. Taken together, these results indicate that PP has considerable potential in the treatment of allergic asthma.</P>
