        복시를 주소로 내원한 에드하임-체스터병 1예

        Purpose: We present a case of Erdheim-Chester disease (ECD) with diplopia. Case summary: A 56-year-old woman came to the hospital with a 6-week history of diplopia on left lateral gaze. The right eye showed mildly limited adduction. Humphrey automated perimetry demonstrated inferior bitemporal quadrantanopia. Orbital and brain magnetic resonance imaging revealed well-defined orbital masses in both intraconal orbits with homogenous enhancement, as well as multiple masses of homogenous signal intensity in the brain. Systemic evaluation showed involvement of the long bones, and retroperitoneum, but no involvement of the heart, or lungs. Incisional biopsy of the right orbital mass was performed. Histopathological examination showed numerous lipid-laden histiocytes and few multinucleated Touton giant cells. Immunohistochemical staining showed positivity for CD68, but negativity for CD1a, and ECD was therefore diagnosed. The patient received treatment with radiation therapy and interferon-α, but died due to sepsis secondary to urinary tract infection after 2 months. Conclusions: Except exophthalmos, diplopia may be the only initial symptom of an orbital mass. Although rare, the possibility of ECD should be considered in the differential diagnosis of both retrobulbar and orbital masses with diplopia. J Korean Ophthalmol Soc 2014;55(2):283-288

        Methanol extracts from 450 plants were screened for muttidrug-resistance reversing activity using drug sensitive KB-3-1 and multidrug-resistant KB-Vl cells. Among them, the extracts of Cynanchum wilfordii, Torilis japonica, Celastrus orbiculatus, Melia toosendan and Teminialia chebula strongly potentiated vinblastine cytotoxicity in KB-Vl cells. But their cytotoxicities to both sensitive KB-3-1 and resistant KB-Vl cells were in the same order of magnitude. These results indicate that the above samples would contain the active principles which do not exert their ativity solely by cytotoxicity.

        홍삼사포닌 투여의 다낭성난소에 의한 불임 치료효과 및 기작연구

        사람에서의 PCO와 몇몇 측면에서 유사한, 실험적으로 유발된 다낭성난소(polycystic ovary, PCO)는 장기간 작용하는 estradiol valerate (EV)에 의해 유발된다. 랫드에서 스테로이드로 유발한 PCO 모델에서 홍삼 총사포닌의 역할에 대한 우리의 이전연구는 전침이 난소의 nerve growth factor (NGF) 농도를 조절하는 것을 검증하였다. 실제로 PCO와 관련된 난소 기능부전의 병리기전에서 신경성 요소의 관련성은 난소로의 교감신경유출(sympathetic outflow)의 증가로 인해 높아진다. 따라서 본 연구에서는 치료제로서의 GTS 투여가 PCO를 유발한 랫드에서 교감신경 활성을 조절할 것이라는 가설을 시험해보고자 하였다. 본 연구에서는 스테로이드로 유발한 PCO에 내재하고 있는 병태생리학적 과정과 연관된 NGF 단백질과 NGF mRNA를 분석함으로써 이루어졌다. PCO가 유발된 랫드에서 EV 대조군은 oil 대조군과 비교하였을 때 유의적으로 높은 NGF mRNA의 발현을 나타내었으며, GTS 투여군은 EV 대조군에 비해 유의적으로 낮은 NGF mRNA의 발현을 나타내었다. 그러나 NGF 단백질은 oil 대조군에 비해 EV 대조군과 GTS 투여군에서도 유의적인 변화를 발견할 수 없었다. 이러한 결과는 EV가 설치류에서 낭종 형성과 무배란을 유발하는 병리학적 단계의 구성요소일 수 있는 난소의 신경향성 단계(neurotrophic state)를 조절한다고 생각할 수 있다. Experimental induction of polycystic ovary (PCO) resembling some aspects of human PCO syndrome was produced using the long-acting compound estradiol valerate (EV). Our previous study on the role of Korean red ginseng total saponins (GTS) in a steroid-induced PCO rat model demonstrated that electro-acupuncture modulates nerve growth factor (NGF) concentration in the ovaries. In fact, the involvement of a neurogenic component in the pathology of PCO-related ovarian dysfunction is preceded by an increase in sympathetic outflow to the ovaries. In the present study, we tested the hypothesis that therapeutic GTS administration modulates sympathetic nerve activity in rats with PCO. This was done by analyzing NGF protein and NGF mRNA expression involved in the pathophysiological process underlying steroid-induced PCO. EV injection resulted in significantly higher ovarian NGF mRNA expression in PCO rats compared to control rats, and PCO ovaries were counteracted by GTS administration with significantly lower expression of NGF mRNA compared to EV treated ovaries. However, NGF protein was unaffected in both EV and GTS treated ovaries compared to control rats. These results indicate that EV modulates the neurotrophic state of the ovaries, which may be a component of the pathological process by which EV induces cyst formation and anovulation in rodents.

        Focal Segmental Glomerulosclerosis(FSGS) is one of the underlying pathology of congenital and infantile nephrotic syndrome. There is no ultimate curative treatment except renal transplantation. We have experienced a case of infantile nephrotic syndrome In a 10 month old boy who presented with proteinuria and hematuria. His elder brother also suffered from nephrotic syndrome and died at the age of 18 months due to sepsis. We have diagnosed this patient with clinical manifestations, laboratory data and pathologic findings which was done by open renal biopsy. The patient expired 54 days after admission because or progressive uremia and sepsis.

        대학 캠퍼스 내 무인 식품 매점 운영에 대한 고객 요구도 분석

        대학구성원의 식생활 개선을 위해 24시간 운영하는 '무인 식품 매점' 운영에 대한 고객 요구도를 총 329명을 대상으로 온라인 설문조사를 한 결과 조사대상자 중 남자는 31.3%, 여자는 68.7%였고, 1인 가구는 전체의 54.1%였으며, 아침식사 결식률은 39.2%였고, 섭취가 부족한 식품군은 과일류 (35.6%), 채소류 (32.8%) 순으로 높았다. 점심식사는 배달음식을 이용하는 경우가 32.2%로 가장 많았고, 점심식사로 선호하는 메뉴는 한식 (74.2%), 일품요리 (34.3%), 일식 (31.9%), 패스트푸드 (28.0%), 중식 (17.0%)의 순이었다. 점심식사와 점심식사 장소에 대한 전체적인 만족도는 평균 3.91점/5점이었고, 전체적인 만족도는 직접 조리해서 먹는 경우가 유의적으로 가장 높았고, 편의점에서 식사하는 경우가 가장 낮았으며 (p < 0.001), 가족과 식사하는 경우가 친구나 혼자 식사하는 경우보다 유의적으로 가장 높았다 (p < 0.001). 무인 매점 이용 의향은 평균 3.98점/5점으로, 직접 조리하는 경우 (3.76점)가 유의적으로 가장 낮았고, 편의점에서 식사하는 경우 (4.38점)가 유의적으로 가장 높았다 (p < 0.05). 무인 식품 매점 희망 이용 시간대는 점심시간 (54.1%), 저녁시간 (46.2%), 오후 간식 시간 (41.9%) 순이었고, 구입 희망 식품은 도시락류 (62.0%), 샌드위치류 (56.2%), 컵과일류 (54.4%), 샐러드류 (51.7%), 김밥류 (50.5%) 순이었다. 판매 예정 식품의 구입 희망 평균 가격은 도시락류는 4,991원, 샐러드류가 3,403원, 컵과일류는 2,745원, 샌드위치류는 2,609원, 김밥류는 2,235원이었다. 대학교 내 무인 매점 경영자는 무인 식품 매점의 성공적인 운영을 위해서 고객의 요구를 실시간 파악하여 고객만족을 극대화할 수 있는 효과적인 마케팅 전략을 수립해 나가야 할 것이다. Purpose: This study was performed to analyze customer needs for the operation of unmanned food stores on a university campus. Methods: A total of 329 responses were collected from customers who had purchased food at the stores. Statistical analyses were conducted using the SPSS program for frequency analysis, χ2-test, t-test, one-way analysis of variance, and Duncan's multiple range test. Results: On average, the overall satisfaction score for lunch and the eating location was 3.91 out of 5 points. Overall satisfaction was significantly higher for home cooking, and lower for eating at convenience stores (p < 0.001), and the score for eating with family was significantly higher than for eating alone or with friends (p < 0.001). On average, the intention to use an unmanned store received a score of 3.98 out of 5 points, while 'home cooking (3.76)' was significantly lower than other eating locations and eating at 'convenience stores (4.38)' was significantly higher than other eating locations (p < 0.05). The desired time of use for unmanned food stores was 'lunch (54.1%)', 'dinner (46.2%)', and 'afternoon snack (41.9%)', and the desired foods were 'doshirak (62.0%)', 'sandwich (56.2%)', 'fruit cup (54.4%)', 'salad (51.7%)', and 'kimbap (50.5%)'. There were significantly more opinions that single-person households would use unmanned food stores for dinner more than family households (p < 0.05), and significantly more willingness to purchase fruit cup (p < 0.05). The average prices (Korean won) that the subjects were willing to pay for the purchase of food were 4,991 for doshirak, 3,403 for salad, 2,745 for fruit cup, 2,609 for sandwich, and 2,235 for kimbap. Conclusion: For the successful operation of the unmanned food stores on the university campus, the store manager should grasp the customer's needs in real-time and establish an effective marketing strategy.

        섬유주세포에서 베바시주맙과 라니비주맙이 일산화질소합성효소의 발현에 미치는 영향

        Purpose: To compare the effects of bevacizumab and ranibizumab on the expression of eNOS in cultured human trabecular meshwork cells (HTMC). Methods: Primarily cultured HTMC were exposed to 0.5 mg/mL bevacizumab and ranibizumab using 1% serum-containing media for 30 minutes. Expression of eNOS mRNA was assessed with RT-PCR. Additionally, after exposure to 20 ng/mL of vascular endothelial growth factor (VEGF) and 0.5 mg/mL bevacizumab and ranibizumab, the production of nitric oxide was assessed with Griess assay. Results: VEGF increased the production of nitric oxide in HTMC. Bevacizumab and ranibizumab decreased the expression of eNOS mRNA and production of nitric oxide (<em>p</em> < 0.05) in HTMC. The decrease in eNOS mRNA expression and production of nitric oxide was not significant between bevacizumab and ranibizumab (<em>p</em> > 0.05). Conclusions: In HTMC, both bevacizumab and ranibizumab decreased the expression of eNOS mRNA with no significant difference observed between the two drugs.         Expression of transferrin gene in various organs of rat was studied using rat transferrin cDNA. The hybridization method of [35S]-labeled transferrin cDNA with transferrin mRNA in cytoplasmic preparations was used to measure the level of transferrin mRNA. The rat from 15-day old fetus to 21-day old postnatal were employed as an animal model. In the liver, the level of transferrin mRNA increased with increasing age. However, the level of transferrin mRNA in brain was significantly lower than that in liver and the level did not increase with age.

        Diabetes is one of the important risk factors in cardiovascular disease associated with atherosclerosis, and cardiovascular disease is the leading cause of death in patients with diabetes mellitus. Recent randomized placebo-controlled cardiovascular outcome trials of all new antidiabetic drugs have linked SGLT-2 inhibitors and GLP1-agonists to not only increased cardiovascular stability but significant reduction of cardiovascular disease. These results have led to preferential selection of the most effective and beneficial antidiabetic drugs with the evidence of cardiovascular safety and efficacy. Herein, we address cardiovascular stability and the effectiveness of antidiabetic drugs, focusing on recently developed ones. (Korean J Med 2021;96:85-91)

        적응적 채도 스트레칭을 이용한 단일 영상 안개 제거

        채도 기반 안개 제거 방법은 속도가 빠르고, 화소 단위로 전달량을 결정하기 때문에 우수한 방법으로 알려져 있다. 그러나, 이 방법은 고정된 채도 스트레칭 방법을 사용하기 때문에, 다양한 채도를 가지는 안개 영상을 적응적으로 제거하지 못하는 단점을 가진다. 본 논문은 적응적 채도 스트레칭 함수를 이용한 효과적인 안개 제거 방법을 제안한다. 먼저 전달량을 구하기 위하여 최대값을 이용하는 HSV 색 공간의 채도를 사용한다. 원 영상의 채도는 안개 영상의 채도에 따른 적응적인 채도 확장 함수를 이용하여 추정한다. 제안 방법은 안개의 양에 따라 채도를 적응적으로 추정할 수 있는 장점을 가진다. 실험 결과를 통하여 제안 방법이 기존의 방법과 비교하여 주관적 및 객관적으로 우수한 성능을 나타냄을 보였다. A saturation-based image dehazing method is known to be fast and efficient because it determines the transmission map on the basis of pixel unit. However, because this dehazing algorithm exploits the fixed saturation stretching function, this method has a disadvantage in that it cannot adaptively apply on hazy images having various saturations. This paper presents an effective single image dehazing method using an adaptive saturation stretching function. First, we exploit the saturation in the HSV color space, which uses the maximum pixel value. The saturation of scene raidiance is estimated using an adaptive saturation stretching function according to the mean of saturation of hazy image. The proposed has the advantage of adaptive estimation of the saturation of scene radiance according to the amount of haze. The experimental results show that the proposed method outperforms the existing methods subjectively and objectively.
