신부전이 동반된 당뇨병 환자에서 발생된 침습성 모균증 : 수술과 Liposomal amphotericin B 및 GM-CSF 병합 요법에 의한 성공적인 치험 1예

저자들은 신부전이 동반된 당뇨병 환자에서 발생된 부비동형 모균증에 대하여 수술과 함께 liposomal amphotericin B 및 GM-CSF의 복합치료를 하여 성공적으로 치료한 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Mucormycosis (zygomycosis) primarily affects diabetic or immunocompromised patients and typically progresses rapidly, necessitating surgical excision and antifungal therapy with amphotericin B. Large doses of amphotericin B are needed for cure, but it has the risk of causing significant renal toxicity. The recent development of liposomal amphotericin B allows antifungal therapy to be administered with potentially improved efficacy and reduced nephrotoxicity. We have experienced a case of paranasal mucormycosis successfully treated with surgery, liposomal amphotericin B and GM-CSF. A 59-year-old male suffering from diabetes mellitus for 6 years was admitted with pain at left maxillary area. He was diagnosed as mucormycosis after cytologic exam on the necrotic nasal mucosa, which showed typical hyphae. He have had diabetic nephropathy with macroproteinuria and had rapidly rising serum creatinine levels with the amphotericin B treatment: creatinine levels reverted to basal level with the use of liposomal amphotericin B. Despite surgical excision and continued antifungal therapy, his infection was not effectively controlled. Therefore, GM-CSF was administered additionally to improve phagocytic activity of leukocytes. He was finally cured after receiving a combination of aggressive surgery, liposomal amphotericin B and GM-CSF. To our knowledge, this is the first detailed clinical description of the treatment of mucormycosis with liposomal amphotericin B in Korea.

위암 환자에서 세포성 면역 기능에 관한 연구

We performed a variety of lymphocyte stimulation tests, delayed hypersensitivity skin tests, and enumeration of several lymphocyte subpopulations in 21 patients with gastric cancer who did not receive any form of anticancer therapy, and in 20 healthy controls. The gastric cancer patients had significantly decreased number and total score of positive reactions in delayed hypersensitivity skin tests compared with the healthy controls. The percent of CD_(4) positive lymphocytes in the gastric cancer patients was significantly decreased compared to the normal control subjects, but it had no correlation with the total score of delayed cutaneous hypersensitivity reactions. The ratio of helper to suppressor cells was lower in the cancer group. The uptake of 3H-thymidine was markedly depressed in cancer patients when stimulated with various mitogens. There was little correlation between any of the stimulation tests and any of the lymphocyte subpopulation proportions to delayed hypersensitivity cutaneous reactions. Optimal proliferative response was found in lymphocytes stimulated with 10 micrograms of concanavalin-A and 10 microgram of phytohemagglutinin. Advanced stage patients had greatly depressed delayed hypersensitivity skin reactions and proliferative responses to mitogens compared with localized diseases. These results suggest that gastric cancer patients have depressed cellular immune functions, which mainly result from the decreased helper cells and defects in functional proliferative response to mitogens. Interleukin-2 and interferon-gamma restored the in vitro proliferative response of lymphocyte in patients with gastric cancer.

멘토링의 철학적 구조와 인간관계를 토대로 한 반성적 멘토링 모형 개발

이 연구의 목적은 반성적 멘토링 모형의 이론적 토대를 구축함으로써 체육교사의 수업전문성 함양을 위한 멘토링 모형을 개발하거나 적용하는 데 작은 지침을 제공하는 데 있다. 먼저 멘토링의 철학적 토대로서 Schon의 ``반성적 실천가``, 이홍우의 ``마음의 중층구조``, 후설의 ``현상학적 환원``이라는 개념을 안내했다. 두 번째는 멘토와 멘티의 인간관계 세 가지를 비유적으로 제시했는데 먼저 교사·학생의 관계로서 멘토와 멘티는 동일하게 체육수업전문성을 탐색하는 일을 하고 있다는 것을 밝혔으며, 이어서 교류교육적 관계로서 멘토와 멘티는 서로 부족한 부분을 가르칠 수 있다는 것을 강조했고, 마지막으로 줄탁동시의 관계로서 멘토의 열정과 멘티의 욕구가 동시에 수반되어야 멘토링이 성사될 수 있음을 드러냈다. 세 번째는 반성적 멘토링의 흐름을 멘토와 멘티의 만남이 이뤄지는 ``이해``의 단계, ``계획→실행→반성``의 순환과정과 함께 멘토와 멘티가 활발하게 상호작용을 하는 ``몰입``의 단계, 1년 동안의 멘토링을 되돌아보며 개선점을 모색하는 ``반성``의 단계 등과 같이 세 단계로 나누어서 멘토링의 리듬을 제시했다. The purpose of this study is to explore the theoretical grounds of reflective mentoring model for enhancing teaching professionalism of PE teachers. The contents of this study are summarized as follows. Firstly, in philosophical grounds of mentoring, conceptions such as Sch?n`s ``reflective practitioner``, Lee`s ``twofold structure of the mind``, and Husserl`s ``phenomenological reduction`` are presented. Secondly, the three metaphors for describing relationships between mentor and mentee are presented: (a) ``relationship of teacher and student`` means that both mentor and mentee are exploring aspects of teaching professionalism of physical education teachers together. (b) ``relationship of exchanging their roles`` means that a mentee is able to teach his or her mentor about something he or she knows better than a mentor does. c) ``relationship of JulTakDongSi`` means that mentoring requires both mentor`s passion for mentoring and mentee`s need for learning to teach PE. Lastly, three rhythms of mentoring have emerged: (a) ``understanding``, at the beginning of the mentoring, which consists of knowing each others` historical and social background, (b) ``immersion``, at the middle of the mentoring, which includes mentor and mentee`s active interaction accompanied with the cyclic process of ``plan→execute→reflect``, (c) ``reflection``, at the end of the mentoring, which enables mentors and mentees to change their roles into learners` ones.

혼합형 자가면역성 용혈성 빈혈을 동반한 전신성 홍반성 루푸스 1예

A 17-year-women developed mixed warm and cold antibody type autoimmune hemolytic anemia associated with systemic lupus erythematosus. The patients was admitted to our hospital for syncope. Jaundice was observed. Urinalysis revealed protein and hemoglobin. Her hemoglobin was 4.6g/dl and reticulocytes were 10%. Total bilirubin was elevated, while haptoglobin and complement were abnormally reduced. ANA and mitochondrial Ab were detected. The direct and indirect Coombs test were positive. The direct antiglobulin test was positive ; IgG and C3d were detected on the red cell surface. The cold agglutinin(CA) titer was 128. The patients was initially treated with high dose steroid therapy. However the patients did not responded to high dose steroid therapy. We treated the patients with plasmapheresis and synchronized cyclophosphamide therapy. Clinical remission of hematologic complications was achieved in this patients. We report a case of mixed type autoimmune hemolytic anemia with SLE that responded plasma pheresis and synchronized cyclophosphamide therapy.

중량물 운반시 잡기유형에 따른 근전도 분석

The purpose of this study was to examine EMG changes of the body according to method the holding during carrying object. The experiments were conducted with utilizing EMG analyses for five male and five female adults as subjects. The results were as followed. 1. The higher muscle action potential of vertical holding was shown than that of right holding when carrying objects in MMH work in proportion to the weight of the object. The came from the subjects using their brachial muscles much to maintain the angles of their wrist joints and elbow joints for keeping their stable postures while walking. 2. The higher muscle action potential of right holding was shown in right and left elector-muscles of their spines(L1-L2, L4-L5) while carrying in all stages of weights except 25kg objects than that of vertical holding. These results were caused from more use of brachial muscles than elector-muscles of spines in carrying weight objects. So vertical holding is more effective in carrying weight objects at a short distance than right holding.

CORBA기반의 미들웨어를 이용한 DBMS 통합 시스템의 설계 및 구현

Database management systems(DBMS) are widely used in most of business information systems, but have various hardware and software platforms. It means that there are difficulties of developing applications which require to access existing multiple heterogeneous DBMS. Developers have to write different applications for each system environments and rebuild all applications whenever the system environment has changed. In this paper, we propose 3-tier system model based on the CORBA distributed object framework in order to solve the above problem and enhance the usability of existing codes. We demonstrate the usefulness of the proposal model by developing an integrated catalog-searching module in on-line shopping mall system and evaluated the system performance.

정보화시대의 공동체 : 정보화의 진전과 기업·소비자 관계 변화를 중심으로

In this study, we questioned about the possibility of the formation of the new community like relationship at the market on the contrary to the traditional market relationship. We conjectured that kind of new community like relationship may possibly arise because of wide spread of new information technology at market behavior of both consumer and producer side. Traditional buyer-seller relationship was characterized as an anonymous one-shot relationship, however, recent changes were mainly emphasized the rich mutual interactions from both sides. Producers needs more consumer side informations as well as consumers begin to participate on production side at more organized level. Some theoretical studies predict that more rich social relationship will appear at the market since the adoption and wide spread of new information technology. We assume that main functions and ingredients of the community are first, the coordination mechanism which reduces conflicts among community members and second, functional efficiency which meets the common needs of community and third, communicational rationality which enhances the integrity of community members. We surveyed about 100 small and medium sized firms located at Pusan and Kyung-Nam area in order to find out whether firms have community like relationship with their consumers. We fine that most of firms realize the impotance of consumer networks, however most of firms consider this kind of network as a marketing strategy and a source of product design instead of more community oriented characteristics like mutual trust building and social interactions.

병행 객체 지향 언어에서 추상형 상태 기반 상속 인터페이스를 이용한 상속 변칙의 해결

Inheritance and concurrency are the primary feature of object oriented languages, and are especially important for code re-use. They provide maximum computational power and modeling power through concurrency of objects. But, concurrent objects and inheritance have conflicting characteristics, thereby simultaneously use of them causes the problem, so called inheritance anomaly, which requires code redefinition of inherited methods to maintain integrity of objects. In this paper, to solve the inheritance anomaly problems we introduce concept of state abstraction, in which internal states of encapsulated objects are made available from a part of object's external interface. And we design the abstract states based inheritance interface mechanisms which methods are inherited efficiently. In our scheme, we can solve the typical inheritance anomaly problems.

백서의 카드뮴 중독시 마늘, D-penicillamine 및 N-acetyl-DL-penicillamine의 방어효과에 관한 연구

In recent studies, they have insisted that the Korean garlic which contains a large amount of disulfide and sulfhydryl compounds and selenium, has a remarkable protective effect against heavy metal poisoning. The mechanism of the effect, however, has still not been proved in detail. Authors tried to compare the protective effect of the garlic on cadmium poisoning with those of N-acetyl-DL-penicillamine (APEK) and D-penicillamine (PEN). For the purpose, the experiment was performed in two steps of treatment. In first step, Experiment Ⅰ, CdCl₂ (1 mg/Kg of body weight) was injected into the peritoneal cavity of rats with experimental agents such as garlic, APEX or PEN for 7 days, and after that only the experimental agents were administered per os to each group of rats for 23 days respectively. In second step, Experiment Ⅱ, CdCl₂ alone was injected into the peritoneum for 7 days and 24 hours after that the experimental agents were administered for 23 days in the same way. The dosage of experimental agents was doubled in last 1 week of 23 days in the second step. For the treatment with the experimental agents, garlic was fed with 6.4% mixture pellets, and APEN (60 mg/Kg/day) and PEN (40 mg/Kg/day) were dosed with water. After the treatment, cadmium levels in liver, kidney, testis and bone were measured and the histopathological examination of testis was done. The results of the study were as follows; 1. When the experimental agents were administered from the beginning of cadmium injection (Experiment I), cadmimum levels in each organs of garlic or APEN-dosed group were markedly lower than those of cadmium-alone-dosed group and also of PEN-dosed group. In kidney, cadmium level of PEN-dosed group was higher than that of cadmium-alone-dosed control group. The extent of damage due to cadmium in the histopathological examination of testis, was mild in garlic or APEN-dosed group compared with the FEN-dosed group. The degree of damage in the latter group was similar with that of cadmium-alone-dosed group. 2. When the experimental agents were administered 7 days latter the cadmium alone was injected (Experiment Ⅱ), the cadmium levels in each organ were decreased little and the histopathological damage in testis was not recovered by the administration of experimental agents. We can conclude, therefore, that the protective or preventive effect of garlic in terms of its potency would be similar with that of APEX and superior to that of PEN. However, it was also revealed that the above mentioned experimental agents including garlic was not so effective as we expected for the treatment of cadmium poisoning if we administer them after the intoxication happened.
