Granisetron is known as the most selective 5-HT3 receptor antagonist and is reported to be effective in the prevention of acute emesis induced by cisplatin based chemotherapy. From June 1885 to December 1995, 32 solid tumor patients who were scheduled to receive cisplatin based combination chemotherapy were randomized either to granisetron or MDL(Metoclopramide, Dexamethasone, Lorazepam) prophylaxis. We evaluated 32 patients; 17 patients received granisetron 3mg, while 15 patients received MDL. The male to female ration was 12:5(Granisetron group) versus 12:3(MDL group), and moan age was 44.6 yrs(Granisetron group) versus 46.5 yrs(MDL group). Of 17 granisetron treated patients, 76.5%(13/17) obtained complete or major response in prevention of acute emesis, while 73.4%(11/15) of patients receiving MDL on first 24 hours of chemotherapy(P>0.05). No severe side effects were recorded in both groups. In granisetron group, mild headache and constipation were noted. In MDL group, the most common side effect was sedation(4/15, 26.7%), but extrapyramidal reaction was not noted in our patients. These results show that granisetron is an effective antiemetic in the control of cisplatin induced nausea and vomiting, and can be administered safely with minimal side effects.

항암제 처리한 백혈병 세포주에서의 Apoptosis 발현

The bcl-2 proto-oncogene encodes a 26 kD protein that promotes cell survival by blocking apoptosis. The bax protein is a member of the bcl-2 familly, now known to form heterodimers with the bcl-2 protein. The ratio of bax to bcl-2 is be critical in determining the fate of the cell in response to stimuli that can induce apoptosis. Extract of Pulsatilla Koreana (SB-31) showed promising antitumor activity in vitro with Topo I inhibitory action. In the present study, the relationship between apoptosis and the apoptosis related proteins, bcl-2 and bax were investigated in human leukemic cell lines HL-60, U-937 and CEM-CM3. All anticancer drugs(adriamycin, etoposide, camptothecin, SB-31) induced extensive apoptosis in HL-60, U-937 cells and CEM-CM3 cells. The expression of bcl-2 and bax protein were determined in cell lines by western blotting before and after incubation with anticancer drugs at different time points. 1) In HL-60 or U-937 cell lines, down regulation of bcl-2 and up-regulation of bax were found after incubation with ADR, VP-16 or camptothecin. 2) In HL-60 or U-937 cell lines, no significant change in bcl-2 or bax protein expression resulted after incubation with SB-31. 3) In CEM-CM3 cells, virtually no change was noted in bcl-2/bax expression after incubation with ADR, VP-16, camptothecin or SB-31. It is suggested that different leukemic cell lines use different pathways of apoptosis activation and a given cell may utilize different pathways of apoptosis activation in response to different cytotoxic agents.

만성 신부전 환자에서의 말초혈액 T-임파구 아형의 분포

To evaluate the immunologic status of uremic patients, T-cell subsets were measured in 12 patients with end-stage renal disease undergoing long-term maintenance hemodialysis. The T-cell subsets were measured by indirect fluorescent staining of peripheral lymphocytes with specific monoclonal antibodies(OKT_4, OKT_8). The results were as follows: 1. The mean level of OKT_4^+ in uremic patients was significantly lower than that of control subjects. 2. The mean level of OKT_4^+ cells in male uremic pafients was signifcantly lower than that of male control subjects, while the mean level of OKT_8^+ cells in female patients was significantly lower than that of male patients. 3. The mean level of OKT_4^+ cells in uremic patients whose age was above 40 years was lower than that of control subjects. 4. The mean level of OKT_4^+ cells in uremic patients dialysed below 1 year was significantly lower than that of patients dialysed over 1 year. 5. The mean level of OKT_4^+ cells in uremic patients transfused more than 5 units of blood was significantly lower than that of patients transfused below 5 units of blood. On the basis of the above tesults, it is suggested that the cellular immunodeficiency in uremic patients undergoing long-term maintenance hemodialysis may result from the reduction of helper T-cells in peripheral blood.

各種 腫瘍 患者에서의 血淸 ferritin에 對한 硏究

Serum ferrtin, total hemogram, serum iron, total iron binding capacity and transferrin saturation were determined in 71 cases of various malignancies and 50 control subjects. One case of colon cancer with liver metastasis showed greatest elevation in serum ferritin (1804ng/ml) and elevation of ferritin disproportionate to that of blood iron indices were found in 75%, 57.3%, 50.0%, 44.0%, 33.0%, 13.3% respectively of acute leukemia, lung cancer, colon cancer, hepatoma, lymphoma and stomach cancer. Serum ferritinemia was in no relation with hemoglobin, serum iron, and transferrin saturation in each malignancy group. Serum iron and transferrin saturation in stomach cancer group was significantly reduced to that of normal control. (p 0.01) For evaluation of iron storage in malignancy patients, bone marrow iron staining rather than serum ferritin or serum iron profile is suggested. Measurement and serial determination of serum ferritin and its isotypes (isoferritin) would be of value in the diagnosis and evaluation of response to treatment in malignant neoplasia.

Binary Random Power Approach to Modeling Asymmetric Conditional Heteroscedasticity

A class of asymmetric ARCH processes is proposed via binary randompower transformations. This class accommodates traditional nonlinear modelssuch as threshold ARCH (Rabemanjara and Zacoian (1993)) and Box-Coxtype ARCH models(Higgins and Bera (1992)). Stationarity condition of themodel is addressed. Iterative least squares(ILS) and pseudo maximum likelihood(PML) methods are discussed for estimating parameters and relatedalgorithms are presented. Illustrative analysis for Korea Stock Prices Index(KOSPI) data is conducted.

A Doubly Robustified Estimating Function for ARCH Time Series Models

We propose a doubly robustied estimating function for the estimationof parameters in the context of ARCH models. We investigate asymptoticproperties of estimators obtained as solutions of robust estimating equations.A simulation study shows that robust estimator from specied doubly ro-bustied estimating equation provides better performance than conventionalrobust estimators especially under heavy-tailed distributions of innovationerrors.
