        In vivo study for the hemostatic efficacy and foreign body reaction of a new powder-type polysaccharide hemostatic agent

        Purpose: Various hemostatic agents have been introduced in therapy as postoperative bleeding is a poor prognostic factor for postoperative outcomes. These products can be divided into those that directly promote the hemostatic cascade and those that physically form a barrier by absorbing blood. The latter, powder-type hemostatic agents have the advantages of being inexpensive and more absorbable with less foreign body reactions (FBRs) and are applicable to a relatively wide area. This study was conducted to verify the safety and efficacy of a newly invented polysaccharide product (OOZFIX, Theracion Biomedical), which improves blood absorption and hemostatic effects. Methods: Two separate animal experiments were performed. The first evaluated FBRs histologically at 3 days, 2 weeks, and 4 weeks, after implantation of OOZFIX in rats, and the second compared hemostatic performance of OOZFIX and Arista AH (Bard) in the porcine liver punch biopsy model. Results: We found minimal FBRs in the 3-day group and no reactions in both the 2-week and 4-week groups after implantation of hemostatic agents. The time to hemostasis of OOZFIX was not significantly different from that of Arista AH (median [interquartile range]: 9 [6–10] minutes vs. 8 [6–10] minutes, respectively; P = 0.522). When comparing the serial bleeding grade tendency, there was no statistical difference between OOZFIX and Arista AH (P = 0.656). Conclusion: OOZFIX caused a minimal FBR that disappeared within 2 weeks in vivo, and its hemostatic performance was comparable with that of an existing agent, Arista AH. Further clinical studies are required in the future.

        Actinomycosis is a chronic suppurative infection, whitch infiltrates mucosa-associated tissues. Thoracic, pelvic and cervicofacial infections of actinomyces are not uncommon, but endobronchial actinomycosis is rarely reported. We report a case of a 29 year old man who presented with a recurrent pneumonia and chronic cough. Fiberoptic bronchoscopy revealed occlusion of the right. lower medio-basal segment bronchus with exophytic endobronchial mass. The diagnosis was confirmed histologically by demonstration of sulfur granules in the bronchoscopic biopsy of the mass. Intravenous administration of amoxacillin and clindamycin followed by oral amoxacillin and clindamycin therapy resulted in complete recovery.

        The purpose of this experiment was to compare the quality characteristics of soy ice creams prepared with soy protein isolate(SPI) or Jinpum-soybean, and different kinds of sweeteners of maltitol and sucrose, xylitol+sucrose, or malitiol. The viscosity of the ice cream mix containing xylitol+sucrose appeared the highest, and that from Jinpum-soybean was higher than that from SPI. The overrun of soy ice creams containing xylitol+sucrose was the highest, and the next was the samples containing maltitol+sucrose. The overrun values of Jinpum-soybean samples appeared higher than those of SPI samples. As for the case of the SPI samples, the melt-down percent was the highest in the sample of maltitol addition, and the next were maltitol+sucrose addition, xylitol+sucrose addition in order. The melt-down percent of Jinpum-soybean was the highest in the sample of maltitol+sucrose addition, and the next were maltitol addition, xylitol+sucrose addition in order. The higher the viscosity of samples, the lower the melt-down percent. In sensory characteristics, the beany flavor, flavor, mouthfeel, overall quality of Jinpum-soybean samples were evaluated better than those of SPI in general. The samples containing xylitol+sucrose evaluated better in beany flavor, mouthfeel, icy feel, and overall quality. The quality characteristics of the samples prepared with Jinpum-soybeans and xylitol+sucrose were evaluated the best.

        Light-induced isomerization dynamics of a cyanine dye in the modulus-controlled regime

        <P>The <I>trans</I>–<I>cis</I> isomerization of an excited molecule converts light energy into mechanical motion, which interacts cooperatively with its surroundings. To understand such a photodynamic process in solids, we investigated the internal twisting motion of 1,1′-diethyl-2,2′-cyanine iodide (DCI) in a series of poly(alkyl methacrylate) (PAMA) polymers by measuring the Young's moduli of the polymers with atomic force microscopy nanoindentation and the fluorescence lifetimes of the dye with time-correlated single photon counting. We found that the isomerization rate constant obtained from the average lifetime correlated well with the mechanical property of the matrix. Our results show that the light-induced molecular motion lies in the modulus-controlled regime in which the polymer matrix not only provides a rigid environment for the dynamics of the molecules but also participates actively in the motion.         Predicting postoperative total calcium requirements after parathyroidectomy in secondary hyperparathyroidism

        Background/Aims: To prevent hypocalcemia after parathyroidectomy (PTX), parenteral calcium is required in addition to oral calcitriol and calcium. After switching to oral calcium, patients can be discharged from the hospital. The aim of this study was to analyze the clinical characteristics and outcomes of PTX performed at a single Korean center and to investigate the associated laboratory factors used to analyze the total amount of postoperative calcium required. Methods: We enrolled 91 hemodialysis patients undergoing PTX from November 2003 to December 2011. We collected clinical and laboratory data preoperatively, 12 and 48 hours postoperatively, at discharge, and 3 and 6 months postoperatively. Results: In total, 59 patients underwent PTX with autotransplantation (AT), 6 underwent total PTX without AT, 11 underwent subtotal PTX, and 15 underwent limited PTX. Total PTX without AT showed the lowest recurrence rate. At all postoperative time points, the mean levels of serum calcium, phosphorus, and intact parathyroid hormone (iPTH) decreased significantly, compared with preoperative levels; however, alkaline phosphatase (ALP) increased significantly from 48 hours postoperatively to discharge (p < 0.001). On multiple linear regression analysis, the total amount of injected calcium during hospitalization showed a significant correlation with preoperative ALP (p < 0.001), preoperative iPTH (p = 0.037), and Δphosphorus at 48 hours (p < 0.001). We developed an equation for estimating the total calcium requirement after PTX. Conclusions: Preoperative ALP, preoperative iPTH, and Δphosphorus at 48 hours may be significant factors in estimating the postoperative calcium requirement. The formula for postoperative calcium requirement after PTX may help to predict the duration of postoperative hospitalization.

        Familial adenomatous polyposis (FAP) is an autosomal dominant syndrome, typically characterized by multiple colorectal adenomas and increased incidence of colorectal carcinomas if it is not treated. Moreover, a variety of extracolonic manifestation are seen. The prevalence of thyroid tumors developing in patients with FAP is about 1∼2%. Generally, papillary thyroid cancer indicates low mortality but ,however, as people get older, the cancer becomes more aggressive. So, the operation is required. Recently we experienced a case of FAP, presenting with papillary thyroid carcinoma, and reported with a brief review of literatures.

        Amino-Functionalized Mesoporous Silica Particles for Ocular Delivery of Brimonidine

        <P>To treat glaucoma, conventional eye drops are often prescribed. However, the eye drops have limited effectiveness as a result of low drug bioavailability due to their rapid clearance from the preocular space. To resolve this, we proposed amino-functionalized mesoporous silica (AMS) particles as delivery carriers of the glaucoma drug, brimonidine. Because of the presence of mesopores, brimonidine (BMD) could be encapsulated in the AMS with a loading amount of 41.73 μg/mg (i.e., drug loading capacity of about 4.17%) to give the BMD-AMS, which could release the drug in a sustained manner over 8 h. BMD-AMS was also shown to be mucoadhesive due to the presence of both hydroxyl and amino groups in the surface, allowing for formation of hydrogen bonds and an ionic complex with the mucin, respectively. Therefore, when topically administered to rabbit eyes in vivo, BMD-AMS could reside in the preocular space for up to 12 h because of its adherence to the mucous layer. To assess in vivo efficacy, we examined the variance in intraocular pressure (IOP) and brimonidine concentration in the aqueous humor (AH) after applying BMD-AMS to the eye, which was compared with that induced by Alphagan P, the marketed brimonidine eye drops. For BMD-AMS, the duration in the decrease in IOP and the area under the drug concentration in the AH-time curve (AUC) were 12 h and 2.68 μg·h/mL, respectively, which were about twice as large as those obtained with Alphagan P; this finding indicated enhanced ocular bioavailability of brimonidine with BMD-AMS.</P> [FIG OMISSION]</BR>

        상암 새천년주거단지 현상설계에서 나타나는 우리나라의 지속가능한 주거단지계획 적용특성

        The Design competition of Sangam residential area held by SMDC is representative of Korean sustainable housing design. 31 entries were proposed by many well-known architects groups. For that reason, this is a good object to explore the cognition and applications of sustainable housing design in Korea. Through literature studies, it can be said that sustainable housing could be fulfilled when energy, environment, ecology and humanity would be in harmony mutually in a circular system. These four sections are categorized by several factors like passive design, microclimate, traffic, cycle of water, etc. Index was made after analyzing the index of the domestic verifications and foreign sustainable housing examples. And then, all designs of Sangam were explored and compared with foreign examples within this index. These results are followings: The prize-winning designs are specially apt to enlarge the underground built area to remove lots of parking areas on the ground, finally reduce the natural land area. When the frequency of the use of factors was analyzed as a whole, total frequencies of Sangam and foreign ones are similar. In sections, ecology in Sangam shows higher frequency than foreign ones. But in the design methods, Sangam designs are poorer than foreign ones. Therefore it can be said that the principles of sustainable housing are applied roughly and there are many complications and contradictions in determining building forms, floors and uses of natural environment.

      • 고삼 추출물을 이용한 해독물질의 개발

        고삼 메탄올 추출물로부터 분리한 flavonoids를 IR, NMR등의 분광학적인 방법으로 Leachianone A(LA)과 Sophoraflavanone G(SFG)로 동정하였고, 카드뮴의 세포독성효과를 고삼 및 고삼추출물이 방어할 수 있는 지를 검색하기 위하여 NIG 3T3 세포에 고삼 및 LA와 SFG, LA+SFG를 처리하고 MTT assay 및 광학현미경으로 세포의 생존률을 검색하여 다음과 같은 결론은 얻었다. MIT의 흡광도는 카드뮴의 농도에 의존하여 감소하였으며, IC_50인 MTT50은 12.5??이었으며 카드뮴을 IC_50농도로 처리하고 고삼 및 LA, SFG, LA+SFG를 각 화합물의 자체 세포독성을 갖지 않는 농도로 처리한 후 MTT assay로 세포 생존율을 측정한 결과 LA, SFG, LA+SFG 처리군들은 카드뮴 처리군(MTT_50)에 비해 세포생존률이 증가되었으며 이들은 고삼 및 고삼추출물의 농도에 의존적으로 증가되었고 각각의 단일 화합물보다 두 화합물 LA+SFG 병용처리군에서 수복 효과가 더 좋은 것으로 나타났으며 광학현미경적 소견에서도 세포재생이 뚜렷하게 보였다. 이상과 같이 고삼에서 분리한 flavonoids가 카드뮴 독성에 의하여 손상된 NIH 3T3 섬유아세포의 재생효과에 영향이 있는 것으로 판단된다. The aim of this study was to assess the antitoxic compounds, flavanones (Leachinanone A=LA and Sophora flavanone G=SFG), from Sophora flavescents (S. Flavescens). We investigated the possibility of antitoxicity of LA and SFG against NIH 3T3 fibroblasts cell lines using colorimetric MTT[3-(4-5-dimethylthiazol-2-yl)-2-5diphenyl-2H-tetra-azolium bromide] assay. The results were as follows : After cadmium was treated against NIH 3T3 cell lines, we determined IC_50. Accordingly we have examined the detoxification effects of S. flavescens, LA and SFG under cadmium IC_50=12.5?? and was carried out to observe morphological changes by the light microscopic study. In NIH 3T3 cells, Sophora flavescens, LA, SFG and LA+ SFG showed inhibitory effects on the cytotoxicity of cadmium and these detoxication effects increased in proportion to the concentration of these drugs. These results suggest that LA and SFG from S. flavescens retain a potential antitoxic activity.
