        Paricalcitol attenuates lipopolysaccharide-induced inflammation and apoptosis in proximal tubular cells through the prostaglandin E<sub>2</sub> receptor EP4

        Background: Vitamin D is considered to exert a protective effect on various renal diseases but its underlying molecular mechanism remains poorly understood. This study aimed to determine whether paricalcitol attenuates inflammation and apoptosis during lipopolysaccharide (LPS)-induced renal proximal tubular cell injury through the prostaglandin E<sub>2</sub> (PGE<sub>2</sub>) receptor EP4. Methods: Human renal tubular epithelial (HK-2) cells were pretreated with paricalcitol (2 ng/mL) for 1 hour and exposed to LPS (1 μg/mL). The effects of paricalcitol pretreatment in relation to an EP4 blockade using AH-23848 or EP4 small interfering RNA (siRNA) were investigated. Results: The expression of cyclooxygenase-2, PGE<sub>2</sub>, and EP4 were significantly increased in LPS-exposed HK-2 cells treated with paricalcitol compared with cells exposed to LPS only. Paricalcitol prevented cell death induced by LPS exposure, and the cotreatment of AH-23848 or EP4 siRNA offset these cell-protective effects. The phosphorylation and nuclear translocation of p65 nuclear factor-kappaB (NF-κB) were decreased and the phosphorylation of Akt was increased in LPS-exposed cells with paricalcitol treatment. AH-23848 or EP4 siRNA inhibited the suppressive effects of paricalcitol on p65 NF-κB nuclear translocation and the activation of Akt. The production of proinflammatory cytokines and the number of terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling-positive cells were attenuated by paricalcitol in LPS exposed HK-2 cells. The cotreatment with an EP4 antagonist abolished these anti-inflammatory and antiapoptotic effects. Conclusion: EP4 plays a pivotal role in anti-inflammatory and antiapoptotic effects through Akt and NF-κB signaling after paricalcitol pretreatment in LPS-induced renal proximal tubule cell injury.

        Acinetobacter species is a non-fermentative aerobic gram-negative coccobacillus that is an important pathogen found in nosocomial infections. Recently, multi-drug resistant Acinetobacter baumannii (MDR-AB) infections have been increasing and pose a serious problem. Most such infections present as bacteremia, pneumonia, or a wound infection; however, CNS infections are very rare. We herein present a case of ventriculitis caused by MDR-AB in a 37-year old man after a neurosurgical intervention. The patient was successfully treated with intrathecal colistimethate.

        The Association of Visual Impairment With Clinical Outcomes in Hemodialysis Patients

        <▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P>Visual impairment limits people's ability to perform daily tasks and affects their quality of life. We evaluated the impact of visual impairment on clinical outcomes in hemodialysis (HD) patients.</P><P>HD patients were selected from the Clinical Research Center registry a prospective cohort study on dialysis patients in Korea. Visual impairment was defined as difficulty in daily life due to decreased visual acuity or blindness. The primary outcome was all-cause mortality and the secondary outcomes were cardiovascular and infection-related hospitalization.</P><P>A total of 3250 patients were included. Seven hundred thirty (22.5%) of the enrolled patients had visual impairment. The median follow-up period was 30 months. The Kaplan–Meier curve and log-rank test showed that all-cause mortality rates (<I>P</I> < 0.001) as well as cardiovascular and infection-related hospitalization rates (<I>P</I> < 0.001 and <I>P</I> < 0.001) were significantly higher in patients with visual impairment than in patients without visual impairment. In the multivariable analysis, visual impairment had significant predictive power for all-cause mortality (Hazard ratio [HR], 1.77, 95% confidence interval [CI], 1.21–2.61, <I>P</I> = 0.004) and cardiovascular hospitalization (HR 1.45 [1.00–1.90], <I>P</I> = 0.008) after adjusting for confounding variables. Of these 3250 patients, 634 patients from each group were matched by propensity scores. In the propensity score matched analysis, patients with visual impairment had independently significant associations with increased all-cause mortality (HR 1.69 [1.12–2.54], <I>P</I> = 0.01) and cardiovascular hospitalization (HR 1.48 [1.08–2.02], <I>P</I> = 0.01) compared with patients without visual impairment after adjustment for confounding variables.</P><P>Our data demonstrated that visual impairment was an independent risk factor for clinical adverse outcomes in HD patients.</P></▼2>

      • RESEARCH ON WEARING CONDITIONS AND SATISFACTION FOR USER-CENTERED KOREAN FLIGHT SUIT DESIGN

        A flight suit worn by fighter pilots in the Korea Air Force is in the form of coveralls and can be donned/doffed with a slide fastener located on the center front of the body. This all-in-one styled garment encompasses the easy movement of an Air Force pilot in the cockpit (Choi, 2012) as well as protects from flames in an emergency. The standard Korean flight suit was designed based on a modulated American flight suit (Jeon, Park, You, & Kim, 2010); however, it is necessary to develop a new Korean flight suit with a Korean pilot-centered design. This study investigates wearing conditions and satisfaction of Korean flight suit wearers and provides basic data for a user-centered Korean flight suit design. Researchers visited two Korea Air Force wings and interviewed the fighter pilots. The results of the interview were derived through a questionnaire. The main topics of the questionnaire were: demographics, actual wearing conditions, size/fit, subjective perceptions of comfort on mobility, satisfaction of appearance, frequency of pocket usage, improvement requirements and fabric satisfaction. A total 439 questionnaires were collected in January 2015 and 428 used for analysis analyzed (except for 11 female responders).The average age of respondents was 29.12±4.67 years old and their mean work experience was 5.96±4.46 years. Over half of those questioned were captains(52.6%).The results of the questionnaires are summarized as follows. First, only 45.8% of respondents filled their chest size; however, most of them completed their height and weight. There were 52.8% who referred to their height (or used eye measurement)when selecting the flight suit size, even though the actual sizing system for Korean flight suits suggests a height and chest size by choice. It is necessary to offer new guidelines in which the wearer could select their size easier. Second, the summer flight suit appearance satisfaction was significantly lower(p<0.01) than winter flight suits and could infer (based on the pre-interview responses) that the air- summer suit vents might be a factor that lowered the appearance satisfaction. Third, the responders felt crotch and shoulder inconvenience during body motions; therefore, it is important to set quantitative standard of ease based upon the extension of skin surface and anthropometric properties of pilots. Forth, questions on suit fabric indicated that they were generally satisfied with protection characteristics such as anti-electrostatic qualities (3.27±0.78, 5-point Likert scale) while comfort characteristics such as thermal insulation, absorbency and elasticity indicated a relatively low satisfaction (2.39±0.794, 2.99±0.87 and 2.65±0.83 respectively, 5-point Likert scale). Therefore, fabric development is also needed to improve user satisfaction. Fifth, survey respondents had a negative opinion on changing flight sites. They preferred improvements in quality or better ease of length and width. Therefore, we have to find a way to improve flight suit function with respect to current designs rather than pursuing a dramatic change of appearance.

        Low parathyroid hormone level predicts infection-related mortality in incident dialysis patients: a prospective cohort study

        Background/Aims: Parathyroid hormone (PTH) is an important factor influencing immunologic dysfunction, but the effect of PTH level on infection-related outcomes remains unclear in incident dialysis. Methods: We evaluated a multicenter prospective cohort study of 1,771 incident dialysis patients (1,260 hemodialysis and 511 peritoneal dialysis) in Korea. Patients were divided into three groups based on serum intact PTH (iPTH) level. The primary outcomes were all-cause and infection-related mortality and multivariate Cox regression analysis was performed to evaluate the role of iPTH in all-cause and infection-related mortality. Results: During the follow-up period of 27.3 months, 175 patients (9.9%) died, and infection-related death represented 20% of all-cause mortality. Both all-cause mortality and infection-related mortality rates (p < 0.001 and p = 0.003, by logrank) were markedly higher in patients with serum iPTH < 150 pg/mL than in the other groups. Multivariate Cox regression analysis revealed that patients with serum iPTH < 150 pg/mL remained at higher risk for infection-related mortality than patients in the target range of 150 ≤ iPTH < 300 pg/mL, after adjusting for confounding variables (hazard ratio [HR], 2.52; 95% confidence interval, 1.06 to 5.99; p = 0.04). The HR of infection-related mortality in patients with serum iPTH < 150 pg/mL was significantly higher in patients with low serum phosphorus, low Ca × P product, low serum alkaline phosphatase and those older than 65 years. Conclusions: Low serum iPTH level is an independent predictor of infection-related mortality in incident dialysis patients.

        Trends in epidemiologic characteristics of end-stage renal disease from 2019 Korean Renal Data System (KORDS)

        Background: The Korean Society of Nephrology (KSN) has maintained a nationwide end-stage renal disease (ESRD) registry data from Korean Renal Data System (KORDS) since 1985, as the representative registry of ESRD patients in Korea. This review is aimed to update the status of domestic ESRD and to provide evidence on the direction of dialysis therapy. Methods: The KORDS Committee of KSN has collected data on dialysis centers and patients through an online registry program, and the data from 1986 to 2019 were analyzed. Results: The incidence and prevalence of ESRD patients in Korea are increasing. The ESRD population numbered more than 100,000 in 2019, doubling during the 10 years since 2010. The proportion of diabetes mellitus as a major cause of ESRD seems to have reached a plateau. The increasing number of elderly dialysis patients is a constant trend, with more than half for the proportion of patients older than 65 years old in 2019. All-cause mortality decreased for the last approximately 20 years, regardless of sex, age, and cause of ESRD. The 5-year patient survival rate in both hemodialysis and peritoneal dialysis increased from 2001 to 2013. Since 2013, the patient survival rates in peritoneal dialysis were similar to those in hemodialysis. Cardiovascular complications were the leading cause of death in ESRD patients. Conclusion: The incidence and prevalence of Korean ESRD patients have increased over time, although patient survival has also steadily increased. The establishment of a surveillance method to address the major cause of mortality in ESRD patients will help improve outcomes.

      • Paricalcitol Pretreatment Attenuates Renal Ischemia-Reperfusion Injury via Prostaglandin E <sub>2</sub> Receptor EP4 Pathway

        <P>The protective mechanism of paricalcitol remains unclear in renal ischemia-reperfusion (IR) injury. We investigated the renoprotective effects of paricalcitol in IR injury through the prostaglandin E<SUB>2</SUB> (PGE<SUB>2</SUB>) receptor EP4. Paricalcitol was injected into IR-exposed HK-2 cells and mice subjected to bilateral kidney ischemia for 23 min and reperfusion for 24 hr. Paricalcitol prevented IR-induced cell death and EP4 antagonist cotreatment offset these protective effects. Paricalcitol increased phosphorylation of Akt and cyclic AMP responsive element binding protein (CREB) and suppressed nuclear factor-<I>κ</I>B (NF-<I>κ</I>B) in IR-exposed cells and cotreatment of EP4 antagonist or EP4 small interfering RNA blunted these signals. In vivo studies showed that paricalcitol improved renal dysfunction and tubular necrosis after IR injury and cotreatment with EP4 antagonist inhibited the protective effects of paricalcitol. Phosphorylation of Akt was increased and nuclear translocation of p65 NF-<I>κ</I>B was decreased in paricalcitol-treated mice with IR injury, which was reversed by EP4 blockade. Paricalcitol decreased oxidative stress and apoptosis in renal IR injury. Paricalcitol also attenuated the infiltration of inflammatory cells and production of proinflammatory cytokines after IR injury. EP4 antagonist abolished these antioxidant, anti-inflammatory, and antiapoptotic effects. The EP4 plays a pivotal role in the protective effects of paricalcitol in renal IR injury.</P>

        저자들은 종양 표지자 상승과 다발성 복강 내 림프절 비대로 전이성 악성 종양의 감별을 위하여 내원한 환자에서 조직 검사상 양측성 신혈관근지방종의 국소 림프절 침범과 무증상 결절성 경화증의 동반으로 확인된 증례를 경험하여 이를 보고하는 바이다. Angiomyolipoma is a rare, benign, mesenchymal hamartomatous neoplasm consisting of a mixture of adipose tissue, smooth muscle cells, and anomalous blood vessels. The kidney is a common origin site, but extrarenal angiomyolipoma has been reported. Simultaneous involvement of the kidney and regional lymph nodes is uncommon, and may be confused with a metastatic malignant lymph node. Lymph node involvement in angiomyolipoma represents a multifocal invasion of the tumor, rather than metastatic disease. Tumor markers usually associate with malignancy but they have sometimes been detected in benign conditions. We report a case of bilateral renal angiomyolipoma with tuberous sclerosis simultaneously involving regional lymph nodes confused with metastatic malignant lymph node enlargement due to associated elevation of serum tumor markers.
