Ocular MALT Lymphoma : Report of Two Cases

Lymphoma arising in mucosa-associated lymphoid tissue(MALT) is extranodal marginal zone lymphoma displaying a proliferation of follicle marginal zone-type cells(centrocyte-like cells or monocytoid B-cells), often with surrounding reactive germinal centers and lympoepithelial lesions. This typically expresses pan-B-cell markers and monotypic immunoglobulin but not CD5 or CD10. Ocular MALT consists of the lacrimal gland, lcular soft tissue, orbit, conjunctiva and cornea. Lymphoma arising in thses sites is of very rare occurrence in up to 1.5% of all patients with non-Hodgkin's lymphoma. Herein we report two cases of ocular MALT lymphoma which was very rare in the dermatologic field. Case 1, 73-year-old man, developed the right upper conjunctival swelling measuring about 2.0×2.5cm for about 1 year. Histologically, the excised lesion showed follicular centers and infiltration of centrocyte-like cells of B-cell lineage in the stroma but no lymphoepithelial lesions, consistent with low-grade B-cell lymphoma. Case 2, a 66-year-old female, had a rapid course of about 1.2×1.0cm-sized, soft, wellow mass arising from lacrimal gland of the right upper eyelid for about 1 month. Hostologically, the lesion showed diffuse patterend features admixed with centrocyte-like cells and large cells such as centroblast, compatible with high-grade lymphoma. Case 2 received combined chemotherapy of m-BACOD, with a somewhat improvement.

The purpose of this study was to compare the number and percentage of natural killer cell in schizophrenic and major depressive patients with those in normal healthy subjects 15 Schizophrenic patients, 15 major depressive patients and 16 healthy subjects were included in this study There was no statistical difference in the age and sex between patients and control subjects. All of these patients were drug-free for at lest two weeks before this study. The results of this study were as follows : 1) In schizophrenic patients, the number and percentage of natural killer cell of the group with below-mean score of general psychopathology were significantly lower than those of the group with above-mean score(p<.05). The number but not the percentage of natural killer cell of the group with below-mean BPRS score was significantly lower than that of the group with above-mean BPRS score(p<.05). 2) The number of natural killer cell showed statistically negative correlation with generalpsychopathology score or BPRS score(p<0.05). In conclusion, these findings suggest that the number and percentage of natural killer cell is not significantly changed in schizophrenia and major depressive disorder, but in schizophrenic patients, severe psychopathology in the rating scales is associated with suppression of the number and percentage of natural killer cell.

Public perception concerning water fluoridation and influencing factors were assessed. A mailing survey was conducted in 1999 with a representative sample of the general Seoul citizen (response rate=40.1%), who consisted of 1108 adults aged between 20 and 78. The data were analyzed using the chi-square test and structural equation modeling. 48.3% favored and 11.4% opposed water fluoridation while 40.5% were not certain. The contact on information about water fluoridation was the most significantly influencing factor on the perception concerning water fluoridation while experience of marriage, income and the perception of tap water were also significant. 52.8% expect governmental health authorities to take the decisions about water fluoridation.

Primary aldosteronism is a syndrome characterized by hypertension, hypokalemia, suppressed plasma renin activity, and elevated serum aldosterone levels. Conn first described it 1955 in association with adrenal cortical adenoma(Conn's syndrome). Today, it can be divided into at least six distinctive sub-groups. The diagnosis of primary aldosteronism is usually suspected when the patient presents with poorly controlled hypertension with hypokalemia. The main issues in the evaluation of primary aldosteronism are to differentiate an adenoma from hyperplasia and to localize the adenoma. Basic hormonal studies and computed tomographic(CT) scanning can be used effectively for that differentiation. An adenoma is one of a few potentially curable forms of hypertension, and it is best treated by removing the adrenal tumor. We experienced a case of a typical adrenal adenoma. The patient was a 37-year-old male who had experienced in both his legs over a period of several days a weakness due to hypokalemia. He was diagnosed using basic hormonal studies and adrenal CT scanning. After, he received laparoscopic adrenalectomy and was discharged with improved condition.

There have been several positive criteria for skin prick test, which has been most commonly used in the diagnosis of allergic diseases. Among them, comparing wheal size induced by allergen to that induced by histamine is logical for criteria of skin prick test positivity. We measured the wheal sizes induced by histamine and various causative allergens with five minutes interval, from 5 minutes to 40 minutes, and compared of the time which the wheal size of both reached at peak. The followings were the summary of the findings. 1. Maximal sizes of wheals induced by histamine reached at 12 minutes in average. 2. Maximal sizes of wheals induced by allergens reached at 15 minutes in average. 3. There was no statistical difference between the time of maximal size of wheal induced by histamine and causative allergen.

0.15% V was added to the modified Hadfield steel containing 3.2% Ni and after solution treatment, the steel was isothermally heated in the range 300℃ to 980℃. The precipitation of carbide and pearlitic constituent were tudied by means of microscopys and hardness test. The results obtained were as follows. (1) Carbide is more rapidly precipitated in the range of 600-800℃ and formed in the range of 350-910℃ with increasing heating time. (2) The precipitation of P.C. is more rapid at 700℃ and occurs in the range of 500-700℃ with increasing heating time. (3) The higher the heating temperature is the shorter the length of the acicular carbides becomes. (4) lncreased hardness is mainly caused by acicular carbide. The shape and the distribution of precipitates also bring about the hardness change.

The authors describe two cases of unusual intracranial metastatic adenoid cystic carcinomas. A 42-year-old woman had a right pariental epidural metastatic mass. presumed hematogenous. spread from a primary tumor in the parotid gland. In second case, a 32-year-old man had a intracranial metastatic adenoid cystic carcinoma mimicking calcified psammomatous meningioma on CT scans. We present these unusual cases with a review of relevant literature and a discussion of possible pathogenesis.

Risk Factors for Posttraumatic Seizure The authors evaluate the risk factors for posttraumatic seizures. this prospective study was performed in a series of 470 head injured patient from January 1996 to Decomber 1998. The patients who were dead within 1 week of injury or children from 3 years old and under were excluded. There were 358 male and 112 female with a mean age of 35.2±22.8 years (range, 4-88 years). The results were as follows : 1). Of the 470 patients, pasttraumatic seizure occurred in 48 patients(10.2%): early seizure in 28(6%) and late seizure in 20 patients(4.2%). 2). The clinical risk factors for posttraumatic seizure were poor consciousness at admission, presence of brainstem herniation signs, poor GCS score, and posttraumatic amnesia more than 24 hours after trauma(p<0.001). 3). The radiological risk factors for posttraumatic seizure were subdural hematoma(p<0.001), intracerebral hematoma (p<0.02), mutiple intracranial CT lesions(p<0.001), or delayed lesions on follow-up CT scans(p<0.001). the incidence of seizure increased according to the severity of diffuse brain lesion(p<0.001), and the seizure rate in mass lesion was higher than that in diffuse lesion(p<0.001). 5). Hypoxia and coagulopathy had a statistically significant influence on posttraumatic seizure(p<0.05). Hypoxia had a infuence on occurrence of late seizure(p<0.05). 6). The incidence of posttraumatic seizure signiticantly increased in surgical group compared with coservative group(p<0.001) and significantly increased in patient who underwent decompressive craniectomy compared with those who underwent craniotomy(p<0.001) 7). Preventive administration of antiepileptic drugs decreased the incidence of early seizure, but did not affect on the reduction of late seizure. 8). Multivariate logistic regression analysis showed the subdural hematoma, GCS score, delayed lesion, and intracerebral hematoma in order of importance for seizure risk. in conclusion, the risk factors affecting on the occurrence of seizure were clinical status at admission, CT lesions, and severity of diffuse brain inJury. Considering that hypoxia affected on the late seizure, it is necessary to detect and treat it immediately after trauma. In addition, the patients who underwent decompressive craniectomy should be carefully followed to evaluate the risk of late seizure. Even though preventive administration of antiepileptic drugs had a tendency to decrease the incidence of early seizure, it is unclear whether administration of antiepileptic drugs can reduce the incidence of posttraumatic seizures. Prospective study will be needed in the selected patients.
