Literally, the mean of Tae-gye is highest brook. And on the basis of the acupuncture theory, Tae-gye point is the Soo earth point and source point of the Kidney Channel as well. In addition, Tae-gye point is one of the Yang-Returning Nine points. We were trying to study bibliographically on the Tae-gye point because Tae-gye point can be used very broadly across the symptoms in the theoretical aspect described above. As a results, we found out that according to the classical books of acupuncture, Tae-gye point is entering point of the Meridian Water as a source point and can be used at both Kidney-Sufficient Syndrom and Kidney-Deficient Syndrom. And Tae-gye can be applied to the disease of kidney or bladder that is urogenital symtoms, Also, Tae-gye is an useful option of tooth-ache, asthma, indigestion, constipation, edema etc whose cause is related with decrease of Kidney ki.

Familial adenomatous polyposis (FAP) is an autosomal dominant syndrome, typically characterized by multiple colorectal adenomas and increased incidence of colorectal carcinomas if it is not treated. Moreover, a variety of extracolonic manifestation are seen. The prevalence of thyroid tumors developing in patients with FAP is about 1∼2%. Generally, papillary thyroid cancer indicates low mortality but ,however, as people get older, the cancer becomes more aggressive. So, the operation is required. Recently we experienced a case of FAP, presenting with papillary thyroid carcinoma, and reported with a brief review of literatures.

기존의 특수건강진단과 작업환경측정을 통한 직업병 관리가 진폐증, 소음성난청 등의 소수 특정질환에 국한되고 실제 직업병 발생 규모 파악이나 신종 직업병의 발견에 한계를 보인다는 사실은 산업의학전문가들 사이에서도 공감을 이루고 있다. 미국과 영국 등에서의 직업성질환 감시체계에 대한 경험은 우리 나라의 직업성질환 감시체계 구축에도 새로운 자극제가 되면서, 1998년이후 인천, 대전, 여천, 구미, 부울경 지역에서 지역 직업성질환감시체계를 산업보건관리의 중요한 시스템으로 구축하려는 노력이 확산되고 있다. 새로이 구축되어지는 이들 지역 직업성질환 감시체계는 감시하고자 하는 대상질환, 활용 가능한 인적자원 및 자료원, 지역 의료체계의 특수성 등에 따라서 목적과 방법을 달리하면서 독특한 형태로 발전을 하고 있다. 각 지역단위 감시체계들이 그 상황에 맞게 독특한 목적과 전략들을 발전 시키면서도, 향후 발전할 국가적인 차원의 직업성질환 감시체계 구축을 위하여 직업성질환 감시의 기본 전략 등을 공유하는 등의 노력이 필요하다. 환례 정의 및 기본적인 등록 서식의 공유, 직업성질환 감시 자료원의 발굴, 공동의 정보 네트워크 및 직업성질환 감시 데이터베이스 구축 등 직업성질환 감시활동을 지원하기 위한 여러 기초 인프라 구축에 힘을 모아야 할 것이다. 우리 나라에서 직업성질환 감시체계를 성공적으로 구축하기 위해서는 수집된 자료의 질 관리를 위한 직업성질환 감시의 원칙 제정과 감시 전략의 공유 등이 필요하며, 전국적인 직업성질환 감시체계의 하부구조라고 할 수 있는 지역감시체계의 기초 토대 마련과 강화작업이 절실하게 필요하다.

<P><B>Abstract</B></P> <P>Cerulein pancreatitis mirrors human acute pancreatitis. In pancreatic acinar cells exposed to cerulein, reactive oxygen species (ROS) mediate inflammatory signaling by Janus kinase (JAK) 2/signal transducer and activator of transcription (STAT) 3, and cytokine induction. Docosahexaenoic acid (DHA) acts as an agonist of peroxisome proliferator activated receptor γ (PPARγ), which mediates the expression of some antioxidant enzymes. We hypothesized that DHA may induce PPARγ-target catalase expression and reduce ROS levels, leading to the inhibition of JAK2/STAT3 activation and IL-6 expression in cerulein-stimulated acinar cells. Pancreatic acinar AR42J cells were treated with DHA in the presence or absence of the PPARγ antagonist GW9662, or treated with the PPARγ agonist troglitazone, and then stimulated with cerulein. Expression of IL-6 and catalase, ROS levels, JAK2/STAT3 activation, and nuclear translocation of PPARγ were assessed. DHA suppressed the increase in ROS, JAK2/STAT3 activation, and IL-6 expression induced nuclear translocation of PPARγ and catalase expression in cerulein-stimulated AR42J cells. Troglitazone inhibited the cerulein-induced increase in ROS and IL-6 expression, but induced catalase expression similar to DHA in AR42J cells. GW9662 abolished the inhibitory effect of DHA on cerulein-induced increase in ROS and IL-6 expression in AR42J cells. DHA-induced expression of catalase was suppressed by GW9662 in cerulein-stimulated AR42J cells. Thus, DHA induces PPARγ activation and catalase expression, which inhibits ROS-mediated activation of JAK2/STAT3 and IL-6 expression in cerulein-stimulated pancreatic acinar cells.</P> <P><B>Highlights</B></P> <P> <UL> <LI> DHA mediates catalase expression via activation of PPARγ. </LI> <LI> DHA inhibits ROS-mediated activation of JAK2/STAT3. </LI> <LI> DHA inhibits oxidative stress-induced IL-6 expression. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
Structural Analysis of Conductive Polypyrroles Synthesized in an Ionic Liquid

Nano-structured conducting polypyrroles were synthesized in the ionic liquids (ILs) based on 1-alkyl-3-methylimidazolium family with tetrachloroferrate as an anion (Cn mim [FeCl4] with n = 4, 8, and 12). The polypyrrole nanostructures synthesized in ILs were formed as spherical shapes. For ionic liquids with alkyl side chain length C4, C4 mim [FeCl4], the size of particles was ranged around 60-nm with a relatively narrow size distribution. As the length of alkyl chain increases, the particle sizes become larger and their distributions become wider. The self-assembled local structures in the solvent ionic liquids are likely to serve as templates of highly organized nano-structured polymers. The length of the alkyl chain in ionic liquids seems to affect these local structures.

Background: Solid malignancies are associated with the development of Pneumocystis jirovecii pneumonia (PJP). This study aimed to evaluate the risk factors for PJP among patients with lung cancer. Methods: This retrospective case-control study compared patients who had lung cancer with PJP (n = 112) or without PJP (n = 336) at Severance Hospital between January 2013 and December 2016. The patients were matched according to age, sex, histopathology, and stage. PJP was considered present when the patient had (i) positive PCR or direct immunofluorescence results for pneumocystis, (ii) clinical symptoms and radiological abnormalities that were consistent with a pneumonic process, and (iii) received targeted PJP treatment. Results: The development of PJP was associated with radiotherapy (RTx), concurrent chemoradiotherapy (CCRTx), lymphopenia, and prolonged high-dose steroid therapy (20 mg of prednisolone equivalent per day for ≥3 weeks). Multivariate analysis revealed independent associations with prolonged high-dose steroid therapy (odds ratio [OR]: 2.02, 95% confidence interval [CI]: 1.07-3.80; p=0.029) and CCRTx (OR: 1.77, 95% CI: 0.99-3.16; p=0.054). Steroid use was frequently related to RTx pneumonitis or esophagitis (29 patients, 43.3%) and cancer infiltration symptoms (24 patients, 35.8%). Sixty-nine of the 112 patients with PJP (61.6%) died during their PJP treatment. Conclusion: Prolonged high-dose steroid therapy and CCRTx were risk factors for PJP development among patients with lung cancer. Clinicians should consider PJP prophylaxis for those high-risk patients.
The Effect of Germination Temperature on Malt Quality of Malting Barely ‘Hopum’

The purpose of this study was to investigate malt characteristics according to the germination temperatures of tow-row variety ‘Hopum’. Hopum has been widely cultivated in Korea for beer-making. In this study, hopum material with germination rate of 97% was used and was malted at five temperatures, 10, 16, 18, 20, 25℃. The higher the germination temperature, the lower the malt yield, and EBC (European Brewery Convention) color index of the wort was darker. There was no significant differences in sugar content, pH and Kolbach index of malt germinated at a germination temperature of over the 16℃. Both α-amylase and β-amylase was induced in malts germinated at all temperatures. β-glucan remaining excessively in the malt makes it difficult to filter the wort during brewing, lowers the filtration rate, and causes problems such as haze and sediment formation in beer. In this experiment, the contents of β-glucan was lower as the germination temperature was higher. We intend to expand the diversity of beer brewing using domestic barley varieties by providing information on quality malt with different germination temperatures.
The change of serum inflammatory markers and cytokines following anti-tuberculosis drug therapy.

Introduction: Active pulmonary tuberculosis (TB) is associated with cell-mediated immunity. Multiple cytokines and inflammatory markers are known to have roles in control TB infection. In this study, we aimed to investigate the change of multiple cytokines and inflammatory markers in active TB patients following anti-TB drug therapy. Methods: Patients with active TB were prospectively recruited between December 2010 and February 2013 at Severance Hospital, in Seoul, South Korea. Treatment was based on the standard regimen of rifampicin (10 mg/kg body weight [BW]/day), isoniazid( 5 mg/kg BW/day), ethambutol (15-25 mg/kg BW/day),and pyrazinamide (15-30 mg/kg BW/day). Blood samples were collected from active TB patients (total n=18) at before (T0), after 2 months (T2), and after 6 months (T6) of anti-TB treatment. We measured the levels of IFN-γ, IL-2, IL-12, IL-10, IL-13, and TNF-αin the supernatant from the QuantiFERON-TB Gold In-Tube assay (QFT-GIT) and WBC count, neutrophil count, lymphocyte count, platelet count, red cell distribution width (RDW), mean platelet volume(MPV), neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR). Results: Compared to baseline levels, IL-10, TNF-α in the supernatant from QFT-GIT, WBC count, neutrophil count, platelet count were produced in significantly lower amounts in response to the treatment. As for IFN-γ, IL-2, IL-12, IL-13 responses, the decrease of median values were not statistically significant. Also the results showed that the IL-10/ IFN-γ ratio of supernatant of QFT-GIT and neutrophil to lymphocyte ratio (NLR)after treatment significantly decreased compared to the baseline, whereas the IL-2/ IFN-γ ratio of supernatant of QFT-GIT increased after treatment. Conclusions: In conclusion, IL-10, TNF-αin the supernatant from QFT-GIT, WBC count, neutrophil count, platelet count decreased following anti-TB drug therapy. And also the IL-2/IFN-γ ratio, IL-10/IFN-γ ratio of supernatant of QFT-GIT, neutrophil to lymphocyte ratio (NLR) may be used as biomarkers to evaluate the effectiveness of drug therapy in active TB patients.
