        Satellite upper platform is optimized by response surface method which has non-gradient, semi-glogal, discrete and fast convergency characteristics. Sampling points are extracted by design of experiments using Central Composite Method and Factorial Design. Response surface also is generated by the various regression functions. Structure analysis is execuated with regard to static and dynamic environment in launching stage. As a result response surface method is superior to other optimization method with respect to optimum value and cost of computation time. A confidence also is varified in the various regression models.

      • 아연결핍이 흰쥐에서 metallothionein의 발현과 카드뮴의 흡수 및 분포에 미치는 영향

        Zine(Zn) is an essential element in biological process, however inadequate Zn status in general population have been recognized. To update the knowledge for Zn-cadmium (Cd) interaction, we studied the intestinal uptake and transport, and the expression of metal transporter proteins (divalent metal transporter 1, DMT1; metal transporter protein 1, MTP1; zinc transporter 1, ZnT1; metallothionein 1, MT1) in duodenum after Cd exposure using Zn deficient animal model. Rats were fed Zn deficient (ZnD, 0.5~1.0mg Zn/Kg) or Zn supplemented(ZnS, 50mg Zn/kg) diet for 4 weeks, and followed single administration of ^(109)CdCl2 orally. the body Zn status and tissue Cd concentration were determined at 24 hrs after Cd administration. Total body burden of Cd and Cd absorption index(Al, %) were estimated based on the tissue Cd analyzed. DMT1, MTP1, ZnT1 and MT1 mRNA were analyzed by using RT-PCR method. Feeding of Zn deficient diet for 4 weeks produced aa reduced body weight gain and a depletion of body Zn. Tissue Cd concentration, body burden of Cd and Cd absorption index were higher in the ZnD diet fed rats than the ZnS diet fed rats. Especially, Cd concentration in the small intestine (duodenum, jejunum and ileum) and the colon of FeD diet fed rats were higher markedly than in the FeS diet group. The expression levels of DMT1, MTP1 and ZnT1 mRNA in FeD diet fed rats were similar to the FeS diet. The level of MT1 mRNA expression was significantly lower in the FeD than the FeS diet fed rats. Taken together, theses results indicate that Zn deficiency in diet induce an increased intestinal absorption and tissue retention of Cd, and down-regulate the MT1 expression in the intestine which might be play a part of role in Cd absorption and transport in mammalian. These findings suggest that deficiency of essential metal could be enhanced the toxicity of toxic, non-essstial metals through the metal-metal interaction.

        In present study, we fractionated polysaccharides from Croton tiglium and investigated their hepatoprotective effects on CCI₄ intoxication. Polysaccharide fraction of which molecular weight is over 300,000(HP) showed the most potent hepatoprotective effects on CC1₄ intoxication. Lipid peroxidation, BAST and sALT were used as parameters to evaluate the liver damage. Glucose, xylose and arabinose were found to be monosaccharides composing sugar moiety of HP.

        흰쥐 뇌에서 전기경련 충격에 의한 CREB 인산화의 발달단계에 따른 변화

        연구목적 : 이 논문에서는 뇌에서 전기경련 충격(Electroconvulsive shock. ECS)에 의한 전사인자 CREB의 인산화를 발달단계에 따라 알아봄으로써 정신장애의 신경발달학적 이해를 위한 생물학적 기반지식을 얻고자 하였다. 방 법 : 생후 7. 14. 21일 및 성년 흰쥐에 ECS를 가하고 시간별로 해마 및 소뇌 조직을 얻어 CREB의 인산화를 알아보기 위해 특이 항체로 면역블롯을 실시하였다. 결 과 : 발달단계에 따라 해마에서는 CREB이 감소한 반면 소뇌에서는 CREB이 증가하였다. 기저상태의 CREB 인산화는 해마와 소뇌에서 생후 7일에 비해 14일 이후 증가하였는데, 소뇌의 경우 CREB의 양증가와 비례하였다. ECS 후 CREB인산화 증가는 해마에서는 생후 21일 이후에 나타났으나, 소뇌에서는 생후 7일은 물론 성년에서도 나타나지 않았다. 결 론 : CREB 매개 신호전달은 발달단계별 및 조직별 활성의 차이를 나타내었으며 해마에서는 생후 21일 이후 ECS에 의해 활성화되었지만, 소뇌에서는 그렇지 않았다. 해마에서 ECS에 의한 CREB 인산화 증가는 c-fos 유전자의 발현과 관계있으리라고 생각되지만, CREB의 Ser-133 인산화 만으로는 발달단계 및 조직에 따른 c-fos 발현의 특이성을 설명할 수 없었다. Objectives : In order to understand the biological basis of neurodevelopmental perspectives of mental disorders, the authors investigated the developmental and regional changes in the phosphorylation of the transcription factor CREB following the electroconvulsive shock(ECS) in rat brain. Methods : Rats of various age groups(7, 14, 21 days postnatal and adults) were given ECS and their hippocampi and cerebella were dissected at specified time points. The content of CREB and phosphorylated CREB were measured by immunoblot analysis. Results : The amount of CREB increased in the hippocampus and decreased in the cerebellum according to the age. Baseline levels of CREB phosphorylation in both tissues were increased from postnatal 14 days, and it was proportional to the amount of CREB protein in the cerebellum. In the hippocampus, ECS increased the phosphorylation of CREB at postnatal 21 days, but in the cerebellum, ECS did not increased the phosphorylation of CREB in any age group. Conclusion : CREB mediated signal transduction pathways showed developmental and tissue-specific changes. ECS increased the phosphorylation of CREB in the hippocampus by postnatal 21 days, but not in the cerebellum. CREB activation is supposed to be related with the induction of c-fos after ECS in the hippocampus. However, the Ser-133 phosphorylation of CREB could not completely explain the developmental and tissue specificity of c-fos induction.

        소아에서 간질발작의 임상양상과 뇌파검사의 관계

        목적:발작의 분류는 발작의 원인, 예후를 추정하거나 항경련제를 선택하는 데 있어 매우 중요하며 뇌파검사는 발작의 종류를 구분하고 적절한 치료를 하기 위해 시행하는 중요한 검사이다. 저자들은 소아에서 임상적 발작이 확실한 경우 간질발작의 임상양상과 뇌파검사 간의 일치 정도를 알아보고자 한다. 방법:2000년 1월 1일부터 2008년 9월 30일까지 1회 이상의 간질발작을 주소로 가천의대 길병원 소아청소년과에 방문한 환자 중 본원에서 처음으로 뇌파검사를 시행한 환자를 대상으로 하였다. 발작의 임상양상은 목격자와 본인의 설명을 기록한 의무기록지를 기준으로 하여 부분발작과 전신발작으로 분류하였고 뇌파검사는 발작간간질양방전을 기준으로 정상과 비정상으로 분류하였으며 각성과 수면상태 모두에서 시행하였다. 결과:총 461명의 환자의 뇌파검사 시의 평균 연령은 6.7세였고 남자는 247명, 여자는 214명이었다. 전체 환자에서 발작의 임상양상은 부분발작이 310명, 전신발작이 187명이었다. 각성뇌파검사에서 비정상은 158명으로 부분발작파가 118명, 전신발작파가 59명, 두 가지 발작파가 한 환자에서 나온 경우가 19명이었고, 수면뇌파검사에서는 비정상이 239명으로 부분발작파가 196명, 전신발작파가 77명, 두 가지 발작파가 한 환자에서 나온 경우가 34명이었으며 전체 환자에서는 비정상이 273명으로 부분발작파가 216명, 전신발작파가 97명, 두 가지 발작파가 한 환자에서 나온 경우가 40명이었다. 간질증후군은 전체 환자 중 90명에서 관찰되었다. 146명(41.5%)의 부분발작과 44명(23.4%)의 전신발작 및 79명(87.8 %)의 간질증후군이 뇌파소견과 일치하였다. 결론:영유아 및 소아에 있어 발작을 진단할 때에는 발작양상 분 아니라 정확한 뇌파소견을 세심하게 고려하여야 한다. Purpose:Electroencephalography(EEG) is an essential method carried out for classifying seizures and taking appropriate treatment. The aim of this study is to investigate the concordance between clinical findings of epileptic seizures and EEG in children. Methods:We enrolled 461 patients from those who visited Gil hospital from January 1, 2000 to September 30, 2008 with the chief complaint of epileptic seizure more than once and checked their first EEG at the same hospital. The clinical findings of seizure were based on the charts and interictal waking and sleep EEGs were done. Results:The mean age of the patients was 6.7 years old. 497 epileptic seizures occurred and its clinical finding included 310 of partial seizures and 187 of generalized seizures. In 315 waking EEG 158 were abnormal including 118 of partial seizures, 59 of generalized seizures and 19 of both seizures, in 431 sleep EEG 239 were abnormal including 196 of partial seizures, 77 of generalized seizures and 34 of both seizures, and in waking and/or sleep EEG 273 were abnormal including 216 of partial seizures, 97 of generalized seizures and 40 of both seizures. Epileptic syndromes were observed in 90 patients. 146(41.5%) of partial seizures, 44(23.4%) of generalized seizures, and 79(87.8%) of epileptic syndromes accorded with EEG. Conclusion:When diagnosing seizures in children, we must consider not only clinical findings but also accurate EEG findings.

        The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.

        Fecal Transplantation using a Nasoenteric Tube during an Initial Episode of Severe <i>Clostridium difficile</i> Infection

        <P>The incidence of <I>Clostridium difficile</I> infection is increasing worldwide, and its severity and resulting mortality are also on the rise. Metronidazole and oral vancomycin remain the treatments of choice, but there are concerns about treatment failure and the appearance of resistant strains. Furthermore, antibiotic therapy results in recurrence rates of at least 20%. Fecal transplantation may be a feasible treatment option for recurrent <I>C. difficile</I> infection; moreover, it may be an early treatment option for severe <I>C. difficile</I> infection. We report a case of severe <I>C. difficile</I> infection treated with fecal transplantation using a nasoenteric tube during an initial episode. This is the first reported case of fecal transplantation using a nasoenteric tube during an initial episode of <I>C. difficile</I> infection in Korea.</P>

        Backgrouond: Aggregatibacter aphrophilus (A. aphrophilus) is a gram negative, capnophilic, fermentative, coccobacillus that is occasionally isolated from patients with infective endocarditis. It has also been reported to cause brain abscess, empyema, meningitis, osteomyelitis and joint infection. Herein we report the first case of brain abscess caused by A. aphrophilus in Korea. Case: A 26-year-old man visited hospital for a headache during three weeks. He presented fever, nausea and vomiting for three days. His blood pressure was 108/69 mmHg and his body temperature was 36.9 ℃ at that time. His past medical history was unremarkable. Brain magnetic resonance imaging (MRI) was performed and revealed the 4 cm sized abscess in the frontal lobe with profound perilesional edema and subdural empyema. He received empirical intravenous antibiotic treatment with ceftriaxone 2.0 g every 12 hours, vancomycin 1.0 g every 12 hours and metronidazole 500 mg every 8 hours. On the 4th hospital day, he had decompressive craniectomy and drainage of the abscess. On the 8th hospital day, A. aphrophilus was identified in the culture of abscess. Vancomycin and metronidazole were discontinued and ceftriaxone monotherapy was maintained because it was susceptible to the 3rd generation cephalosporin. On the 11th hospital day, transthoracic echocardiography was performed, but there was no evidence of endocarditis. After a eight-week course of intravenous ceftriaxone therapy, follow-up MRI revealed almost resolved abscess. Intravenous ceftriaxone was changed to oral cefixime 100 mg every 12 hours and he was discharged. He treated with oral cefixime for 12 weeks in outpatient clinic until the complete resolution of brain abscess on MRI.
