토끼에서 근육주사시 입자 크기에 따른 amoxicillin의 비교 약물동태학

To investigate the pharmacokinetic difference between the two amoxicillin (AMX) particles in rabbits after intramuscular injection (i.m.), both of AMX-S (particle size: 10 ㎍) and AMX-L (particle size: 100 ㎍) were injected into New Zealand White rabbits (1.2±0.3 ㎏) at a dose rate of 10 ㎎/㎏ of body weight. In this study, serum AMX concentrations were detected by microbiological assay with E. coli BE 1186 which shows high antibiotic sensitivity. After i.m. administration, AMX-S and AMX-L were best fitted as 1-compartment model with the absorption and elimination phase. The biological half-life (T_1/2, _k10) of AMX-S is 4.06±1.09 h and that of AMX-L 4.76±0.69 h. The serum maximal concentration time (T_max) of AMX-S and AMX-L were 0.88±0.17 h and 0.77±0.11 h, respectively. Maximal AMX concentration (C_max) (AMX-S: 5.71±0.62 ㎍/㎖, AMX-L: 5.04±0.25 ㎍/㎖) in serum showed a significant difference (p<0.05). In terms of bioequivalance, however, there was no difference between the two AMX's after i.m. injection in the basis of C_max and AUC.

목적 : 기관지내시경 검사에서 흔히 사용되는 국소마취제인 lidocaine과 내시경시 지혈목적으로 사용되는 thrombin과 epinephrine이 각종 균주에 미치는 항균효과에 대해 알아보고자 하였다. 재료 및 방법 : 균주는 가톨릭대학교 성모병원에서 2004년 3월부터 2004년 9월까지 임상검체 에서 동정된 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 각각 42, 42, 42, 43주를 대상으로 하였다. Lidocaine, thrombin, epinephrine 감수성 검사는 National Committee for Clinical Laboratory Standards (NCCLS, 2002)의 기준에 따랐다. 결과 : Lidocaine은 S. aureus, S. pneumoniae, P. aeruginosa에서 MIC_(50), MIC_(90) 모두 20,000 ㎍/mL 이었다. K. pneumoniae는 각각 10,000 ㎍/mL이었다. Thrombin은 S. aureus와 P. aeruginosa에서 MIC50 500 lU/mL 과 MIC_(90) 500 IU/mL 이상이었고, K. pneumoniae에서는 MIC_(50)과 MIC_(90)이 모두 500 lU/mL이상이었으나 S. pneumoniae에서는 MIC_(50)과 MIC_(90)은 125 IU/mL이었다. Epinephrine은 K. pneumoniae, S. pneumoniae에 대한 MIC_(50), MIC_(90)가 모두 >500 ㎍/mL이었고, S. aureus와 P. aeruginosa에 대한 MIC_(50), MIC_(90)가 모두 500 ㎍/mL이었다. 결론 : 기관지 내시경 검사에서 흔히 쓰이는 lidocaine, thrombin, epinephrine 등의 약제들이 호흡기 질환의 흔한 병원균인 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 균주들에 대해 항균 효과가 있을 수 있어 세균배양검사에 영향을 미칠 수 있겠다. Background : We performed this stody to find out about antimicrobial effect of lidocaine which is commonly used local anesthetic, and thrombin and epinephrine used for hemostasis during bronchoscopic procedures. Materials and Methods : The microorganisms that were cultured from specimens obtained during bronchoscopy were Staphylococcus aureus (n=42), Streptococcus pneumoniae (n=42), Klebsiella pneumoniae (n=42), and Pseudomonas aeruginosa (n=43) collected from St. Mary's Hospital, from March to Sep 2004 were used for susceptibity testing. Susceptibility to lidocaine, thrombin, and epinephrine were tested according to the National Committee for Clinical Laboratory Standards. Result : MIC_(50) and MIC_(90) of lidocaine for S. aureus, S. pneumoniae, P. aeruginosa were all 20,000 ㎍/mL and that for K. pneumoniae were 10,000 ㎍/mL. MIC_(50) and MIC_(90) of thrombin for both S. aureus and P. aeruginosa was 500 IU/mL and above 500 IU/mL, respectively; that for K. pneumoniae were all above 500 IU/mL and for S. pneumoniae they were 125 IU/mL, MIC_(50) and MIC_(90) of epinephrine for K. pneumoniae and S. pneumoniae were above 500 ㎍/mL; that for S. aureus and P. aeruginosa were 500 ㎍/mL. Conclusion : We observed possible antimicrobial effect of lidocaine, thrombin, and epinephrine in vitro against pathogens such as S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa, which are common respiratory microorganisms. The use of these agants could affect the result of bacterial culture.
국내에서 유행하는 HIV의 전파 경로에 따른 Subtype 분포

Background : Previous data have been reported that subtype B is prevalent in South Korea, but neither the extent nor the proportion of subtypes could be evaluated. This study was designed to analyze the distribution of HIV-1 subtypes, temporal instructions and transmission dynamics between epidemiological groups. Methods : 1,280 Koreans had been diagnosed as HIV seropositive during the period 1985 to 2000. Among them, 134 individuals were selected for this molecular epidemiological study. 134 DNAs were isolated from uncultured or cultured peripheral blood mononuclear cells. V3-V5 (0.7 kb) fragment of HIV-1 env gene was amplified by nested polymerase chain reaction and was sequenced. Results : HIV-1 isolates from thirty-seven homosexuals were all subtype B (100%). On the other hand, 66 isolates from 94 heterosexuals were subtype B (70%) and 28 were non B subtypes (30% : 13 A, 4 C, 2 D, 8 E , 1 G). Only subtype B strains were isolated from 73 males who were infected with HIV inside Korea while 16 B and 20 non B subtype strains were isolated from 36 males who were HIV infected outside of Korea. However, B and non B strains were isolated half and half from females who were infected inside Korea except one. Conclusion : The HIV-1 subtype B strains are prevalent in Korea from the early HIV infection until present in both homo and heterosexuals. Non B strains have been transmitted from men who were infected outside Korea to their spouses and casual partners. So, we need further study to monitor subtype B and non B HIV transmission in epidemiological groups of Korea, (Korean J Infect Dis 33:311∼318, 2001)

Hypereosinophilic syndrome is characterized by persistent blood eosinophilia of 1,500/ul or more in the absence of known causes and multiorgan dysfunction by eosinophil-related tissue damage. In Korea, some cases of hypereosinophilic syndrome with hepatic involvement have been described with prolonged benign clinical courses. Most of them were diffuse or multifocal lesions in imaging studies, and a few case presenting as a single mass has been described. Here we report a case of hypereosinophilic syndrome with hepatic involvement in a 65-year-old man who presented with multifocal mass lesions. By abdominal US and CT scan, multiple, ill-defined nodules were detected in throughout the liver. Liver biopsy revealed severe eosinophilic hepatitis, suggestive of hypereosinophilic syndrome. Hepatic mass lesions improved after steroid administration for several weeks.

<P><B>Abstract</B></P><P>The remarkable amount of public and scientific debate with regard to the safety of genetically modified (GM) crops has resulted in the implementation of mandatory risk assessments of newly developed GM crops, as well as mandatory labeling of GM crops and foods following their commercial release. Among the many currently available GM rice lines, transgenic rice lines that overexpress a trehalose-6-phosphate synthase/phosphatase (<I>TPSP</I>) fusion gene have attracted particular interest from researchers, by virtue of their superior tolerance to a variety of abiotic stresses. The primary objective of this study was to determine the copy numbers of the transgene element, and to identify the genomic sequences flanking the integration site of the transgene element in a selected <I>TPSP</I> transgenic rice line, in order to develop new event-specific detection techniques. The genomic sequences flanking the integration site of the transgene element were identified via an inverse PCR protocol. It was determined that a single copy of the <I>TPSP</I> fusion had been integrated into the transgenic line. An effort to find a rice sequence usable as an internal positive control for the screening of the transgenic rice resulted in the identification of an <I>RBE4</I> gene sequence, which appears as one copy within the rice genome, and is rice-specific. The characterization of the genomic sequences flanking the transgene, as well as the availability of the internal positive control sequence, enabled the design of a qualitative PCR technique and a quantitative TaqMan-based detection method for this transgenic rice line. The results of this study may prove useful with regard to the large-scale screening of this transgenic rice line in the processes of risk assessment and commercialization, as well as in the development of novel methods for the detection of other transgenic rice lines.</P>
