        한국인 직무 스트레스 측정도구의 개발 및 표준화

        Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

      • 개 비장 질환의 미세세침흡입술을 통한 세포병리학 검사 4례

        To evaluate the accuracy and diagnostic significance of cytopathological diagnosis of canine splenic disorders, cytopathological examination throgh fine-needle aspiration(FNA) referred to Veterinary Medical Teaching Hospital of Chungnam National University were retrospectived. Splenic neoplasm, hemolymphatic neoplasm, hyperplasia, extramedullary hematopoiesis and splenitis were diagnosed canine splenic disorders through cytopathological examination. The patient should be identified what coagulation profile are normal before aspiration, because complication such as hemorrhage could happen. The 4 cases were identified symmetric, asymmetric splenomegaly on abdominal ultrasonography and then were performed ultrasound-guided FNA. The 4 cases were diagnosed hemangiosarcoma(2), hyperplasia(1) and extramedullary hematopoiesis(1) by clinical signs, hematological examination, and cytopathological examination. Cytopathological examination through FNA was cost-effective, no gneral anesthesia, no complication, and non-invasive method of diagnosis for canine splenic disorders.

      • 고양이의 만성 췌장염 치료 증례

        A 5-year-old Russian blue cat was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with the history of anorexia, lethargy, weight loss(6.5 to 4.8 kg) 2 weeks ago. Abdominal ultrasonography showed the following : increase of echogenicity of liver, irregular shape of gall bladder, hyperechoic region around duodenum. Pancreatitis, cholangitis, hepatic lipidosis was diagnosed by the ultrasonography. Anti- inflammatory therapy, antibiotic therapy was administered. Abdominal ultrasonography is a efficient available diagnostic tool but serum fPL test reveals the highest sensitivity and specifity result for feline pancreatitis. SNAP fPL test is newly released that can check pancreatitis within 10 minutes by same princple of Spec fPL test used in Gastrointestinal Laboratory of Texas A & M Univerrsity.

      • 부산교육대학교 교과교육 사이버 문답교실 과학 영역의 질문 분석을 통한 활용상의 문제점과 개선 방안

        The purpose of this study was to find out potential problems associated with cyber questioning and answering system of science areas of Busan National University of Education and to suggest ways of improvements of its system. The total of 145 questions raises in the system were critically analyzed in terms of six categories such as including misconception, task performance, simple information, detailed information, application of knowledge, and synthesis of information. The major results are as follows: First, most questions raised through the questioning and answering system were lower levels of cognition, which is incompatible to the intention of the system. Second, the questioners, in general, were not recognized the intention of the system. Third, the questions were often repeated. Fifth, there was a lack of interactions between the questioner and responder. Sixth, there was systematic problems, that is, the system had no places to notify the information on questioners and to check the degree of satisfaction of questioners on the questions they raised. Based on these results, we suggest several ways for improvements of the systems, including the providing of opportunities to interaction and immediate feedback to the question.

      • 고선량 방사선 조사후 치아수복재의 특성변화

        To investigate the effect of high dose irradiation on the Vickers hardness number(VHN), color change and wear rate of dental filling materials, esthetic filling material specimens were studied. Six kinds of composite resins and two kinds of glass ionomers(GI) were used. One(CLF) was chemically cured composite resin, and five(HCX, HPL, PHB, POF, PRT and RZI) were light cured composite resins. One(FJT) of GI was chemically cured GI and one(FLC) was light cured GI. The filling material specimens were made according to the manufacture's instructions and were finally polished with Microcloth(Buehler). Half of the specimens were irradiated with 100 Gy radiation using a linear accelerator(Mitsubishi, 6 MV). VHN and CIELAB color were measured before and after irradiation, and wear test were performed for nonirradiated specimens by brushing with toothpaste, From the experiment , the following results were obtained. 1. The CIELAB color difference was significantly different depending on the material(p<0.01), and FLC showed the highest color difference (CIELAB ΔE) of 9.452 and CLF showed the lowest value of 3.642. 2. The VHNs of nonirradiated specimens were 12.22∼73.58 and those of irradiated specimens were 25.53∼84.53. The VHNs of irradiated specimens were significantly higher than those of nonirradiated specimens(p<0.05) except RZI, FJT and FLC. 3. The wear rate of irradiated specimens of RZI, FLC were significantly lower than those of nonirradiated specimens(p<0.05). 4. The color difference after brushing of irradiated specimens were higher than those of nonirradiated specimens except CLF.

        Bortezomib과 Dexamthasone으로 치료한 골수외 형질세포종 4예

        Despite the use of aggressive local and systemic treatment including autologous stem cell transplantation in multiple myeloma, extramedullary recurrences are common and the prognosis of these patients is poor. Many novel drugs such as thalidomide, lenalidomide and bortezomib improve the response of treatment of multiple myeloma, but some reports failed to describe thalidomide has effect in extramedullary plasmacytoma. Recent data report on the successful treatment plasmacytomas with bortezomib in patients with advanced multiple myeloma. We treated 4 relapsed or refractory extramedullary plasmacytomas with bortezomib at our institution. We recognized all these extramedullary plasmacytomas decreased and showed more than partial response. This report lends support to the efficacy of bortezomib in the treatment of plasmacytoma and describes the safe use of bortezomib. Responses may, however, be of short duration. Therefore, despite our limited experience, we propose that bortezomib may be considered a therapeutic option for such patients who have risk of radiation therapy

        인천 인현동 호프집 화재 피해자 분석

        Purpose : This study was conducted to develop field triage, transportation, distribution, and prehospital care at a fire disaster by analyzing the victims of the fore that broke out at a bar in Incheon. Method : We analyzed the cases of the victims of a fire in Incheon in Oct. 1999. We determined the primary care hospital, the arrival time, the burn size, the outcome, and the injury type from the medical records, the concerned organ records, and interviews with concerned persons. Result : The total number of victims was 137: 56 prehospital deaths, 1 hospital death, and 80 survivals. The Pearson correlation coefficient between the burn size and the severity was -0.175. There were 121(89.6%) cases of inhalation injury, 59 (43.7%) cases of flame burns, 66 (48.9%) cases of hypoxic brain damage, and 16 (11.9%) cases involving other types of injury. Conclusion : The causes of death of the fire victims were inhalation injury and hypoxic brain damage due to CO poisoning and other toxic inhalants. We propose the use of a simple triage and rapid treatment(START) system and a reassessment the delayed category in fire disasters.

        Fibroblast Growth Factor 11 Inhibits Hepatitis B Virus Gene Expression Through FXRα Suppression

        Fibroblast growth factor 11 (FGF11) is a member of the intracellular FGF family, which shows different signal transmission compared with other FGF superfamily members. The molecular function of FGF11 is not clearly understood. In this study, we identified the inhibitory effect of FGF11 on hepatitis B virus (HBV) gene expression through transcriptional suppression. FGF11 decreased the mRNA and protein expression of HBV genes in liver cells. While the nuclear receptor FXRα1 increased HBV promoter transactivation, FGF11 decreased the FXRα-mediated gene induction of the HBV promoter by the FXRα agonist. Reduced endogenous levels of FXRα by siRNA and the dominant negative mutant protein (aa 1–187 without ligand binding domain) of FXRα expression indicated that HBV gene suppression by FGF11 is dependent on FXRα inhibition. In addition, FGF11 interacts with FXRα protein and reduces FXRα protein stability. These results indicate that FGF11 inhibits HBV replicative expression through the liver cell-specific transcription factor, FXRα, and suppresses HBV promoter activity. Our findings may contribute to the establishment of better regimens for the treatment of chronic HBV infections by including FGF11 to alter the bile acid mediated FXR pathway.

        Core promoter mutation of nucleotides A1762T and G1764A of hepatitis B virus increases core promoter transactivation by hepatocyte nuclear factor 1

        Hepatitis B virus (HBV) infection highly increases the risk for liver cirrhosis and hepatocellular carcinoma (HCC). The clinical manifestation of HBV infection is determined by the mutual interplay of the viral genotype, host genetic factors, mode of transmission, adaptive mutations, and environmental factors. Core promoter activation plays a critical role in the pre-genomic RNA transcription of HBV for HBV replication. The mutations of core promoter have been implicated in HCC development. We had obtained HBV genes from Myanmar HBV infectants and identified gene variations at the core promoter region. For measuring the relative transactivation activity on core promoter, we prepared the core-promoter reporter construct. Both of A1762T and G1764A mutation were consistently found in the HBV genes with hepatocellular carcinoma. The A1762T/G1764A mutation was corresponding to K130M/V131I of HBx protein. We prepared the core promoter- luciferase reporter construct containing the double A1762T/G1764A mutation and the K130M/V131I HBx protein expression construct. The A1762T/G1764A mutation highly was responsive to core promoter transactivation by HBx, regardless of HBx mutation. The A1762T/G1764A mutation newly created hepatocyte nuclear factor 1 (HNF1) responsive element. Ectopic expression of HNF1 largely increased the HBV core promoter containing A1762T/G1764A mutation. In addition, hepatic rich fatty acid, palmitic acid and oleic acid, increased K130M/V131I HBx level by core promoter activation. These results provide biological properties and clinical significance of specific HBV core promoter mutants related with HCC development.
