        In order to show the effect of a vast point pollutant source on air quality of Pusan Thermoeletric Power Plant and its surrounding area, air quality around Pusan Thermoeletric Power Plant was simulated by ISCLT-2 which was supplied by EPA. For this purpose the emission amount of SO_2, NO_2 and TSP was calculated and atmospheric stability was classified for a recent decade(1985~1994) in Pusan. A result of the emission amount showed that much amount of NO_2, NO_2 and TSP are emitted from industrial area. It was clear that NO_2 is much emitted from line source and industrial area. And as a result of classification of atmospheric stability, neutral, stable and unstable state were 58%, 24.1% and 17.9%, respectivly. The result of ai quality simulation by ISCLT-2 showed that Pusan Thermoeletric Power Plant is affecting on the increse of 2.0ppb, 3.0ppb and 5.0㎍/㎥, SO_2, NO_2, and TSP respectively at its surrounding area, site A-3 which was located westward 2.2㎞ distance from Plant

        강박장애 환자에서 Sertraline 치료반응의 예측인자에 관한 연구

        연구목적: 1980년 중반부터 강박장애의 치료로 SSRIs가 널리 쓰이고 있으나 치료반응의 예측인자에 대한 연구는 미비한 실정이다. 본 연구에서는 sertraline 투여후 호전군과 비호전군의 인구학적 특성, 발병연령, 이환기간, 과거의 강방장애에 대한 투약경력, 정신과 질환의 가족력, 증상의 심각도, 임상양상 등의 변수들을 관찰하여 sertraline 치료반응과의 상관관계를 살펴보았으며 치료방응을 예측 할 수 있는 인자를 알아보았다. 방법: 연구 대상은 이대부속 동대문병원 정신과에 입원 또는 외래치료를 받은 환자들 중 정신과 진단 편람제 4판 진단 기준에 의거하여 강박장애로 진단 받은 31명(남자 10명, 여자 21명)이었다. 연구 방법은 sertraline을 하루 50mg씩 투약하면서 Y-BOCS, NIMHOCS, CGI로 강박증상의 변화를 평가하였고, HAM-A로 불안정도의 변화를 측정하여 투약 전 점수와 비교하였다. 결과: 1) Y-BOCS 점수 40%이상 감소와 함께 CGI 점수 2점 이상 감소를 보인 호전군은 대상환자 31명중 17명(54.8%)이었고, 비호전군은 14명이었다. 호전군이 비호전군에 비해 늦은 나이에 발병하는 경향을보였고(p<0.1), 이환기간이 짧았으며(p<0.05), 과거의 강박장애에 대한 투약경력은 적었다(p<0.05), 정신과 질환의 가족력은 두 집단간에 차이가 없었다. 2) 호전군에서 투약 전 Y-BOCS의 강박행동 점수가 유의하게 낮았으며(p<0.05), 기타 다른 평가척도에서는 두 집단간에 유의한 차이가 없었다. 3) 평균 Y-BOCS 점수는 투약 전에 비해 sertraline 투약 12주째 43.5%가 감소하였다. 투약 전과 비교하여 투약 2주째부터 불안을 평가하는 HAM-A 점수가 유의하게 감소하였고, 투약 4주째부터 Y-BOCS, NIMHOCS, CGI 점수가 유의하게 감소되기 시작하여 12주까지 지속되었다.(p<0.01). 4) 호전군에서 오염에 대한 강박사고와 씻음의 강박행동의 유의하게 많았으며(p<0.05), 셈의 강박행동은 없었다(p<0.05), 병적 의심은 비호전군에서 많은 경향을 보였다(p<0.1). 결론: 이상의 결과를 종합하면, 질병의 이환기간이 짧으며 과거의 강박장애에 대한 투약경력이 적고 투약 전강박행동이 심하지 않으며, 주된 임상양상이 오염에 대한 강박사고와 씻음의 강박행동인 경우 sertra-line에 좋은 치료반응을 보이지만, 셈의 강박행동이 있는 경우에는 치료반응이 좋지 않다고 예측할 수 있다. Objectives: The pharmacologic treatment of obsessive-compulsive disorder(OCD) with serotonergic antidepressant agents is well established in clinical psychiatry on the basis of serotonin hypothesis. Studies on predictive factors of treatment reponse to OCD are relatively scarce. The author compared the demographic factors, age of onset, length of illness, family history of mental illness, previous drug history for OCD, severity and type of symptoms between the drug reponders and non-responders to find out the predicitive factors that may be related to treatment response. Methods: The subjects were 31, both inpatients and outpatients who met the diagnostic criteria for OCD by DSM-IV and scored 20 or higher on the Yale-Brown Obsessive-Com-pulsive Scale(Y-BOCS) completed 12-week trial of sertraline. Y-BOCS, National Institute of Mental Health Obsessive-Compulsive Scale(NIMHOCS), Clinical Global Impression Scale(CGI) were scored to measure the changes of obsessive-compulsive symptoms, and Hamilto Anxiety Scale(HAM-A) was measured at each visit. Results: 1) The responders, defined as achieving at least a 40% decrease in the Y-BOCS total score and more than a 2 point decrease in CGI score, were 17(54.8%), and non-responders were 14(45.2%). Responders tended to show later onset(p<0.1), and shorter duration of illness(p<0.05), and less previous drug therapy for OCD(p<0.05) than non-responders. There were no significant differences in family history of mental illness between the two groups. 2) The basline Y-BOCS compulsive score was significantly lower in responders(p<0.05), although there was no significant difference in the basline severity of other measures. 3) The obsession of contamination and washing behaviors were more frequent, but compulsive counting was not shown, in responders(p<0.05). The pathological doubt tended to be more common in nonresponders(p<0.1). Conclusions: In summary, the patients with shorter duration of illness, less prior drug ther-apy for OCD, and less severe compulsive behaviors, and the obsession of contamination and washing behaviors are expected to be better outcome with sertraline, whereas compulsive counting are the predictor of poor response.

        Analysis items of this study were 1,1-dichloroethene, dichloromethane, chloroform, 1,2-dichloroethane, 1,1,1-trichloroethane, benzene, carbontertrachloride, toluene, ethylbenz ene, m,p-xylene, styrene, o-xylene. Equipment of analysis was purge & Trap/HP-6890GC/5973MSD, GC operating condition was carrier gas 99.999% Helium-0.7㎖/min, Trap meterial Tenax/SG?Charcoal, Column HP-PONA(Crosslinked Methyl Siloxane, 50m×0.2mm×0.5㎛), Oven program 35℃(10min)-4℃/min 150℃-1 5℃/min 200℃(5min), Aux 280℃, MS Source 230℃, MS Quad 150℃ Result of RSD% measurement was chloroform 9.37%, 1,2-dichloroethane 5.79%, 1,1,1-trich loroethane 12.56%, benzene 4.31%, carbon tetrachloride 15.35%, toluene 3.89%, ethylben zene 5.69%, m,p-xylene 5.65, styrene 4.02%, o-xylene 2.94%. Result of analysis showed that 1,1,1-trichloroethane, carbontetrachloride, dichloromethane were not detected, 1,1-dichloethane was detected level of 0.19ppb, 0.26ppb in 1st sampling point 6, 7, 1,2-dichloromethane 0~7.25ppb, benzene 0~0.9ppb, toluene 0/11~1/79ppb, ethyelbenzne 0~2.15ppb, m,p-xylene 0~2.32ppb, styrene 0~0.8ppb, o-xylene 0~0.68ppb.

        비만 개선 효과를 지닌 탄수화물 및 지방 흡수 억제 기능성 식이조성물 개발

        In a previous study, we developed a new food additive as an egg yolk antibody (IgY) against carbohydrate digestion enzymes for the regulation of blood glucose level and weight control. The IgY delayed and decreased the increment of blood glucose level after administration of sucrose in human being by 30% in 20∼30 min. We also developed a lipase inhibitor as a water extract of two kinds of herb, Platycodon grandiflorum and Solanum melongena. Twenty three volunteers were subjected to the intake of the egg yolk IgY plus the herbal extracts for 50 days. In average, the treated subjects appeared to lose 1.96 ㎏ of body weight and 3.4 ㎏ of body fat mass during the treated period. Furthermore, Panniculus adiposus and breech size were significantly decreased during the experimental period. Above results suggested that the administration of the dietary additives composed of egg yolk IgY and natural herbal extract improve the obesity by the decrement of body weight and body fat mass.

      • 전방 도달법과 수장부 T자형 금속판 고정을 통한 원위 요골 골절의 치료

        목적 : 원위 요골 골절에 대해 전방 도달법을 이용한 관혈적 정복 및 수장부 T자형 금속판 내고정술의 방사선학적 및 임상적 결과를 분석하여 문헌 고찰과 함께 보고하고자 한다. 대상 및 방법 : 도수 정복이 되지 않았거나, 심한 분쇄 골절로 인해 도수 정복 상태가 유지되지 못했거나, 부적절한 관절면의 정복 등의 이유로 원위 전완부 전방 도달법을 이용한 관혈적 정복 및 수장부 T자형 금속판을 이용한 내고정술을 시행 후 최소 1년 이상 추시 관찰이 가능하였던 19예를 대상으로, 수술 전 후의 방사선학적인 평가와 최종 추시시의 기능적 평가를 Green과 O'Brein의 방법을 이용하여 평가하였다. 결과 : 수술 직후 모든 예에서 관절면의 해부학적 정복을 얻을 수 있었다. 방사선학적 결과에서 요골 관절면의 요골 길이는 수술 전 평균 8.8 ㎜ (±4.8 ㎜)에서 수술 후11 ㎜ (±3 ㎜)로, 요골 경사각은 평균 15˚ (±5.7˚)에서 20˚ (±5˚)로, 전방 경사는 평균 -11˚ (±13˚)에서 7˚ (±4˚)로, 척골 양성 변이는 평균 4 ㎜ (±3 ㎜)에서 0 ㎜ (±1 ㎜)로 향상되었으며, 기능적 결과에 있어서는 우수 9예, 양호 7예, 보통 2예, 불량 1예 이었다. 추시 중 초기 수상이 심하였던 1 예에서는 정복의 소실과 함께 장 무지 굴건의 파열이 초래하였다. 결론 : 원위 요골 골절의 수술적 치료에서 일부 제한된 범위에서만 적응증이 되었던 수장부 금속판 내고정술로 방사선학적 측면과 기능적인 측면에서도 만족할 만한 결과를 얻었으며, 초기 분쇄 골절이 심할 경우에는 정복의 소실을 예방하기 위하여 외고정기를 부수적으로 장착하는 것이 좋을 것으로 생각된다. Purpose : To analyze the radiologic and clinical results of open reduction and volar plating through anterior approach for distal radius fracture. Materials and Methods : We retrospectively analysed that 19 distal radius fracture, which would not be reduced by closed reduction or too comminuted to maintain reduction or articular surface incongruency, were treated by open reduction and volar plating through anterior approach. The results were evaluated by preoperative and immediate postoperative radiographics and clinical results were analysed using Green and O'Brien scoring system at final follow up. Results : All cases achieved anatomical articular surface reduction postoperatively. In terms of radiologic analysis, mean radial length (8.8 ㎜±4.8 ㎜ vs. 11 ㎜±3 ㎜), radial inclination (15˚±5.7˚ vs. 20˚±5˚), volar tilt (-11˚±13˚ vs. 7˚±4˚) and ulnar plus variant (4 ㎜±3 ㎜ vs. 0 ㎜±1 ㎜) were improved. The clinical evaluation revealed 9 excellent cases, 7 good cases, 2 fair cases and I poor case. The reduction loss and flexor pollicis longus rupture was occurred in one patient, who had severely displaced comminute fracture in initial injury. Conclusion : Using volar plating, authors gain good radiologic and clinical results. But, additional external fixation is recommended to prevent further collapse in severly comminuted fractures.

        H9c2 심근 세포주에서 외인성 nitric oxide가 허혈에 의한 세포 독성에 미치는 영향

        Background: Nitric oxide(NO) is known to have protective effects on an ischemic heart and to exert triggering effects on ischemic preconditioning. However, the effects of NO during the ischemic period have not been investigated. To investigate the role of exogenous nitric oxide in a model of ischemic heart cell death, we studied the effects of ischemic preconditioning and ischemia in a normal and an ischemic buffer. Methods: Rat cardiac myoblast cells(H9c2) were cultured in a normal and an ischemic buffered medium. For the ischemic culture of heart cells, the cells were cultured in a dessicator with GasPak for 5 hrs. In ischemic preconditioning, the cells were pretreated with ischemic buffer for 5 min and then perfused with normal medium for 30 min. For the measurement of the cytotoxicity, a MTT(3-4-Sdimethylthiazol-2-yl-2,5-diphenyl tetrazolium bromide) assay was performed. A DAPI(4',6-diamidino-2-phenylindole dihydrochloride) staining procedure and a flow cytometry analysis were performed to confirm apoptotic cell death by ischemia. Results: Cell viability, as determined by using a MTT assay, showed that the preconditioned group treated with NO showed more cell death than with the not-preconditioned groups in both normal and ischemic buffers. But, In normal medium and not-preconditioned groups, NO showed protective effect according to the concentrations(100,1000μM) . No treatment with NO produced the different results. In normal medium, the protective effect of ischemic preconditioning was demonstrated, but no protective effect of ischemic preconditioning could be seen in the case of the ischemic buffer. The DAPI staining and flow cytometry analysis of heart cells showed characteristic apoptotic features. Conclusion: NO added in the ischemic phase had deterious effects on heart cells. Ischemic preconditioning was more harmful than ischemia alone. The toxicity of the cells was characteristic apoptosis.

        Multiple sclerosis and lupus erythematosus are chronic, potentially disabling diseases of unknown cause. It is a rare occasion that the finding of both diseases occurs in one patient. Because both diseases may affect the central nervous system, it may be difficult at times to differentiate symptoms of lupus erythematosus from those of multiple sclerosis. As far as we know, there was no case report of SLE in multilple sclerosis in Korea. We report a case of multiple sclerosis accompanied by SLE.

        Case Reports : Underestimated Sacroiliac Joint Lesion on Computed Tomography in Pelvic Open-book Injury: A Case Report

        The classification of anteroposterior compression (APC) injury type is based on using static radiographs, stress radiographs are known as a useful adjunct in classifying type of APC pelvic injuries. According to a recent article, the intraoperative stress examination has led to a change in the treatment plan in more than 25% of patients on 22 patients presumed APC type I (symphyseal diastasis <2.5 cm) injuries. Here authors present a case demonstrating a necessity of intraoperative stress test for excluding concealed posterior ring disruption.

      • Suppression of bipolar conduction via bandgap engineering for enhanced thermoelectric performance of p-type Bi<sub>0.4</sub>Sb<sub>1.6</sub>Te<sub>3</sub> alloys

        <P><B>Abstract</B></P> <P>Substitutional doping is known to be effective when used to enhance the thermoelectric figure of merit <I>zT</I>, and this is generally explained as resulting from a reduction in the thermal conductivity caused by an additional atomic-scale defect structure. However, a comprehensive analysis of the substitutional doping effect on the electrical and thermal properties together has not been undertaken, especially when the bipolar thermal conductivity becomes serious. A previous study by the authors also showed that the <I>zT</I> of Bi<SUB>0.4</SUB>Sb<SUB>1.6</SUB>Te<SUB>3</SUB> thermoelectric alloys was enhanced by indium (In) doping due to the reduction of the total thermal conductivity. Here, we more closely analyze the electrical and thermal transport properties of a series of indium (In)-doped p-type Bi<SUB>0.4</SUB>Sb<SUB>1.6-x</SUB>In<SUB>x</SUB>Te<SUB>3</SUB> (x = 0, 0.003, 0.005, 0.01) using both the single-parabolic-band model and the Debye-Callaway model in an effort to investigate the origin of the observed thermal conductivity reduction more closely. The bipolar contribution to the total thermal conductivity was estimated exclusively based on a two-band model based on a single-parabolic-band model. Furthermore, the lattice thermal conductivity was calculated using the Debye-Callaway model while taking additional In substitutional defects into consideration. The calculations indicated that the significant suppression of bipolar thermal conductivity was achieved as a result of the increased bandgap in Bi<SUB>0.4</SUB>Sb<SUB>1.6</SUB>Te<SUB>3</SUB> caused by In doping. Additional point defects from In doping also reduced the lattice thermal conductivity, but not as much as the bipolar thermal conductivity did. The study suggests that the suppression of bipolar conduction by means of a bandgap modification can be an effective approach for enhancing <I>zT</I> further via a simple In-doping process in Bi<SUB>0.4</SUB>Sb<SUB>1.6</SUB>Te<SUB>3</SUB>.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Thermal conductivity reduction in In-doped Bi<SUB>0.4</SUB>Sb<SUB>1.6</SUB>Te<SUB>3</SUB> is analyzed. </LI> <LI> Bandgap increase by In doping suppresses bipolar conduction significantly. </LI> <LI> Extra point defects by In doping reduces lattice thermal conductivity. </LI> <LI> The <I>zT</I> enhancement in In-doped Bi<SUB>0.4</SUB>Sb<SUB>1.6</SUB>Te<SUB>3</SUB> is mainly due to bipolar suppression. </LI> </UL> </P>

        Treatment of Unstable Pelvic Ring Injuries

        Pelvic fractures are classified according to the stability of the pelvic ring. Unlike stable pelvic fractures, which heal without complications, unstable fractures may lead to pelvic ring deformities, which cause severe complications. An orthopedic surgeon must determine the stability of the pelvic ring by radiography and physical examination of the patient in order to ensure early, prompt treatment. This article includes anatomy of the pelvic ring, classification of pelvic ring injuries, its treatment algorithm, and corresponding cases involving unstable pelvic ring injury.
