        파킨슨 환자의 사상체질 및 UPDRS 분포 연구

        Objective: In order to find Sasang constitutional therapies on Parkinson's disease and to make a fundamental basis for clinical application, this study was performed . Method : We recruit thirty five person as the disease group and we test them by QSCCII. also, We estimate them by UPDRS scale. Results : In the distribution of sasang constitution among parkinson's disease patients and controls, we can know that in the distribution of sasang constitution among parkinson's disease patients, Taeumin has a large division. but we don't find out significantly difference statistically. In the distribution of UPDRS Std. score of sasang constitution, we find out significantly difference statistically. Conclusions : Unfortunately, we fail to lead significantly difference statistically in the sasang constitution among parkinson's disease patients and controls. But, in the distribution of UPDRS Std. score of sasang constitution, we find out significantly difference statistically. Consequencely, it is necessary that further study on the theme in the more sample

        太衝 陽陵泉의 針자극이 특발성 파킨슨 환자의 UPDRS 및 HRV Parameter에 미치는 영향

        Introduction : This study was designed to evaluate the effect of acupuncture on Unified Parkinson's Disease Rating Scale (UPDRS) and Heart Rate Variability (HRV) of patients with Parkinson's disease Methods : Subjects were voluntarily recruited through newspaper and internet advertisement. All the subjects were confirmed as idiopathic parkinson's disease by a neurologist. Acupuncture was applied 2 times a week for four weeks by oriental medical doctor at Kyung-hee University hospital. Acupuncture points used were GB34 and LR3. The patient's symptoms were assessed before and after 4 weeks of treatment by UPDRS and HRV. HRV was measured for 5 minutes before and after 4 weeks of treatment. Results : The results were as follows; 1. In both groups, UPDRS scores were significantly improved after 4 weeks compared to the pre-treatment. 2. After 4 weeks of treatment UPDRS score differences between the two groups were insignificant. 3. After 4 weeks of treatment HRV parameter scores showed statistically insignificant differences between the two groups. SDNN parameters of the Experimental group were significantly increased. Conclusion : This study suggests that acupuncture treatments can be applicable to improve symptoms in the patients with idiopathic Parkinson's disease. Further study on the acupuncture and HRV in the patients with Parkinson's disease is recommended.

        형방지황탕(荊防地黃湯)으로 호전된 초기 파킨슨병환자의 두통(頭痛) 변비(便秘) 치험례

        Objectives The purpose of this case study is to report significant improvement of headache, constipation and a slight improvement of tremor after treatment with Hyeongbangjihwhang-tang in a Soyangin patient with Parkinson's disease.Methods This Soyangin patient is treated by Hyeongbangjihwhang-tang and acupuncture therapy. The progress was evaluated using the Unified Parkinson Disease Rating Scale before and after treatment. Secondary results assessments include Global Assessment Scale, change of patient's omniloquilism, Xerostomia and Cold hands & Feet Syndrome.Results and Conclusions The patient's symptoms of headache, constipation and tremor were improved after using Hyeongbangjihwhang-tang. and UDPRS score decreased in Part Ⅰ, Part Ⅱ, and Part Ⅲ. In conclusion, this study shows that Hyeongbangjihwhang-tang can be effective treatment for Soyangin patient with early-stage Parkinson''s disease. Overall, Sasang medicine can be a solution of the symptoms caused by Parkinson's disease. Through Sasang medicine we can contribute Parkinson's disease Patient's quality of life get better.

        소양인 파킨슨병 환자의 한양방 병행치료 장기 경과관찰 보고 3례

        Objective This case study was about a long-term clinical effects after using Korean medicine treatment and western treatment in three Soyangin patients with Parkinson's disease. In this study, we reported significant improvement of non-motor and motor symptoms of these patients after Sasang constitutional medicine treatments for more than a year. Method The patients were identified as Soyangin and treated with Yangguksanhwa-tang, Hyeongbangdojeok-san and acupuncture. The Hoehn & Yahr scale, Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale (UPDRS) were used to assess the overall function of the patients. And the Global Assessment Scale (GAS) was used to assess the change of chief complaints after the treatments. Result and Conclusion The non-motor and motor symptoms of Parkinson's disease improved after Sasang constitutional medicine and western medicine treatments for more than a year. Furthermore these patients' chief complaints improved without increasing of amount of western medicine (levodopa). In conclusion, this study showed that Sasang constitutional medicine with western medicine can be effective treatment for long-term clinical care of Parkinson's disease.

        루이소체 치매로 추정되는 이차성 파킨슨증 환자의 Non-Motor Symptom Scale(NMSS)로 평가한 비운동성 증상을 한약과 침의 복합치료로 호전시킨 증례보고 1례

        Parkinson's syndrome is a degenerative brain disease that presents characteristic motor symptoms of tremor, rigidity, and gait disturbance. In addition to these motor symptoms, Parkinson's syndrome also presents non-motor symptoms (NMSs) such as sleep disturbance and cognitive decline. NMSs reduce patient's quality of life and psychosocial functioning and cause economic burden on the patient, so appropriate evaluation and treatment are required. Lewy body dementia is one of the several diseases belonging to Parkinson's syndrome. Its symptoms such as cognitive function, memory impairment, and hallucinations occur with Parkinsonism. Although drug therapy is being used with drug treatment to treat non-motor symptoms, it has limitations such as side effects, which stimulated interest in other complementary treatment methods such as oriental medicine treatment, dance, and yoga. The patient in this case complained of tremor in the right upper extremity, muscle hypertension and pain, and persistent vision, memory, and cognitive decline. The patient was diagnosed with probable Lewy body dementia. The patient was hospitalized for 4 months and received acupuncture and herbal medicines. After treatment, the patient's NMS scale scores decreased from 90 to 63, and the Unified Parkinson's Disease Rating Scale scores (summed I, II, and III) decreased from 17 points to 8 points. The Beck Depression Inventory score decreased from 22 points to 13 points. In addition, the patient's subjective evaluation revealed improvement. In this case, a patient diagnosed with probable Lewy body dementia who did not respond to the standard treatment and did not want to take medications showed improvement in not only motor symptoms but also NMSs after integrative Korean medicine treatment.

        Validation Study of the Official Korean Version of the Movement Disorder Society-Unified Parkinson’s Disease Rating Scale

        Background and Purpose The Movement Disorder Society-Sponsored Revision of the Unified Parkinson’s Disease Rating Scale (MDS-UPDRS) is widely used for estimating the symptoms of Parkinson’s disease. Translation and validation of the MDS-UPDRS is necessary for non-English speaking countries and regions. The aim of this study was to validate the Korean version of the MDS-UPDRS. Methods Altogether, 362 patients in 19 centers were recruited for this study. We translated the MDS-UPDRS to Korean using the translation-back translation method and cognitive pretesting. We performed both confirmatory and exploratory factor analyses to validate the scale. We calculated the comparative fit index (CFI) for confirmatory factor analysis, and used unweighted least squares for exploratory factor analysis. Results The CFI was higher than 0.90 for all parts of the scale. Exploratory factor analysis also showed that the Korean MDS-UPDRS has the same number of factors in each part as the English version. Conclusions The Korean MDS-UPDRS has the same overall structure as the English MDSUPDRS. Our translated scale can be designated as the official Korean MDS-UPDRS.

        Relationships between upper extremity performance and unified Parkinson's disease rating scale-motor exam

        Objective: Clinical measures that quantify upper extremity function are needed for the accurate evaluation of patients and to plan an intervention strategy. The purpose of this study was to examine the relationship between the Unified Parkinson's Disease Rating Scale (UPDRS)-Motor Exam and upper extremity performance as a quantifying clinical tool of upper extremity function in persons with Parkinson's disease. Design: Cross-sectional study. Methods: Thirty-two idiopathic Parkinson's Disease persons participated in this study. To investigate the relationship between the UPDRS-motor exam, Box and Block test (BBT), and Action Research Arm Test (ARAT) by two physical therapists. The examination took up to 1 hour, and the participants were invited to rest between each clinical measure in order to minimize the effects of fatigue. Clinical measures were assessed while the subjects were in the "on" phase of their medication cycle, generally 1-3 hour after taking their anti-Parkinson's medications. Results: In more affected side, the UPDRS-motor exam was significantly negative correlated with the BBT (p<0.05) but it was not significantly correlated with the ARAT. In less affected side, only positively correlation was significantly shown between BBT and ARAT (p<0.05). On the other hand, between BBT and ARAT were not significantly correlated with the UPDRS-motor exam. Conclusions: The UPDRS-motor exam is effective tool which was significantly correlated with manual dexterity in more affected upper extremity. But The UPDRS-motor exam is not effective tool in less affected upper extremity.

        특발성 파킨슨병 환자에 대한 한의치료의 효과 : 후향적 연구

        Objectives: The purpose of this study was to investigate the effects of Korean medicine on patients with idiopathic Parkinson’s disease.Methods: The charts of 47 patients diagnosed with idiopathic Parkinson’s disease in the Department of Internal Korean medicine, Stroke and Neurological Disorders Center, Kyung Hee University Hospital, Gangdong between August 2012 and July 2016 were reviewed. The Unified Parkinson’s disease rating scale (UPDRS) was administered before and after treatment with Korean medicine.Results: Thee average UPDRS Ⅱ+Ⅲ, UPDRS Ⅱ, and UPDRS Ⅲ of the 47 patients decreased significantly from 22.26±15.15, 10.19±7.53, and 12.06±8.35, respectively, pretreatment to 16.96±13.63, 7.47±6.44, and 9.49±7.73, respectively, post-treatment. The average postural instability and gait difficulty (PIGD), tremor, and bradykinesia also significantly improved post-treatment.Conclusions: These results provide evidence that Korean medicine can improve the activities of daily living and motor function of patients with idiopathic Parkinson’s disease. In particular, Korean medicine may be effective for the treatment of PIGD, tremor, and bradykinesia.

        기립성 저혈압이 있는 진행된 파킨슨병 태음인 환자 치험 1례

        Objectives This case study is about a Taeeumin patient with advanced Parkinson's disease identified as Dry-heat (Joyeol) pattern. In this study, we report significant improvement of non-motor and motor symptoms after treatment with Cheongsimyeonja-tang.Methods The patient was identified as Taeeumin Dry-heat (Joyeol) pattern and treated with Cheongsimyeonja-tang and acupuncture. The Unified Parkinson Disease Rating Scale (UPDRS) was used to assess the overall functions of the patient. And the global assessment scale (GAS) was used to assess the improvements of dizziness, bradykinesia and tremor after the treatment.Result and Conclusion The UPDRS total score decreased from 138 points to 86 points after 5 weeks treatment. And symptoms of dizziness caused by hypotension, bradykinesia, and tremor showed significant improvement in GAS after the treatment. Furthermore, sleep disturbance and constipation were reported to be improved after the treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeeumin patient with advanced Parkinson's disease having orthostatic hypotension.
