      • 위암환자의 복강내에 투여한 Mitomycin C-Carbon Particle의 Mitomycin 용출에 관한 연구

        Locoregional recurrence is the most common type of recurrence in surgical operation of gastric adenocarcinoma, and peritoneal dissemination is one of the most difficult problems in advanced gastric adenocarcinoma treatment. Because the peritoneal cavity is the most common site of the first recurrence after gastric cancer resection, intraperitoneal chemotherpy seems a logical choice for cancer chemotherapy. The Mitomycin C(MMC) adsorbed by the activated charcoal particles(CH) is relatively released when the drug concentration surrounding the carbon particles becomes low in the peritoneum of the peritoneal cavity. For the intraperitoneal chemotherapy on the advanced gastric adenocarcnoma, mitomycin C adsorbed on activated carbon particles was administered in the peritoneal cavity just before abdominal wall closure. The closed drainage tubes were inserted in the peritoneal cavity and clamped for tuo hours after completion of operation. MMC concentrations were serially measured in peritoneal fluid, plasma and urine at 2hour, 48 hour, 72 hour and 168 hour following its administration in order to study the efficacy of the MMC-CH as a drug delivery system. There were minimal toxicities in born marrow, liver, and gastrointestinal system after intraperitoneal MMC-CH administration. The data of this study suggested that MMC-CH may have a somewhat more beneficial effect than surgery alone when administered in optimal dose and schedules, but the MMC concentration of the peritoneal fluid was not sufficient to eradicate remnant cancer cells, and effective duration of maintenance was only below 24 hours in the peritoneal fluid and plasma.

      • PMDA-ODA 폴리이미드의 합성 및 특성에 관한 연구

        Pyromellitic dianhydride(PMDA) 와 Oxydianiline(ODA)를 N, N-dimethylacetamide (DMAc)에 녹여 Polyamic acid(PAA)를 2단계 열이미드화법으로 제조하였다. 제조된 폴리이미드 막의 이미드화도는 FT-IR에 의하여 분석되었다. 또한 PAA의 농도를 10~30wt%로 변화시키며 합성하여 열이미드화 시킨 후 농도에 따른 단면의 상태를 SEM사진을 통하여 관찰하였다. 농도 변화에 따른 밀도와 T_g의 변화는 혼합액체부상법과 DSC로 분석되었다. 열처리 온도를 증가시킬수록 이미드화도는 증가되었고 PAA의 농도가 증가할수록 필름의 단면이 치밀해짐을 알 수 있었다. 또한 제조된 PMDA-ODA 폴리이미드의 T_g는 215℃ 이었다. Polyamic acid(PAA) was synthesised with Pyromellitic dianhydride(PMDA) and Oxydianiline (ODA) in N, N-dimethylacetamide(DMAc). PMDA-ODA polyimide membranes were obtained by two step thermal imidization, and the degree of imidization of polyimide membranes was observed by the FT-IR. After the thermal imidization through the concentration of varied synthesis of PAA, 10~30wt%, SEM photographies of section state were observed. The density and glass transition temperature on the varied concentration were replace with investigated test of density and Differential Scanning Calorimetry (DSC). With increasing thermal treatment temperature, the degree of imidization was increased. While the concentration of PAA increases, the section of membrane was densed. The glass transition temperature was maintained to be constant(215℃) within the PAA concentration range.

        한국 재원 정신분열병환자의 사회경제적 특성에 관한 역학적 조사 : 일일 시정조사 Point Prevalence Study

        1984년 2월 27일 현재 전국 정신과 병·의원의 입원환자를 대상으로 일일시정조사를 시행하였다. 자료분석 때까지 도착한 자료중 ICD-9의 진단기준에 의하여 정신질환의 범주에 속하는 2,895명을 1:1 쌍쌍대응법으로 분석하여 거주지별로 정신분열병군과 대조군간의 사회경제적 특성을 비교하였다. 분석결과는 다음과 같았다. 1. 정신분열병환자의 결혼상태는 두 지역 모두에서 독신이 많았고 odd ratio는 각각 5.33과 3.56이었다. 농ㆍ어촌의 정신분열병환자가 독신이 더 많았다. 2. 교육수준은 양 군에서 뚜렷한 차이는 없었다. 3. 직업은 도시의 정신분열병환자에서 무직이 많았고 odd ratio는 1.90이었다. 4. 경제수준은 도시의 정신분열병환자에서 낮은 층이 많았고 odd ratio는 1.65이었다. 5. 그외 가족구조와 종교, 부모의 결혼상태에서는 유의한 차이를 보이지 않았다. Aiming at suggestion the possible ways to improve psychiatric care, rehabilitation, and mental health services in Korea, this nation-wide hospital study was intended to explore the socioeconomical status of mental in-patients. This point prevalence study was conducted to patients who were in psychiatric wards at Feb. 27th. 1984 using mailing questionnaire. The number of hospitals and clinics answered was 88 and the patients, 3,300. Among them, we selected the 2,895 patients who were included in mental disorders (290-319) by ICD-9. Finally we compared the socioeconomical status between schizophrenia and non-schizophrenia in rural and urban area with the method of Fleiss. The results were as follows; 1. The relative proportions of single persons in schizophrenia were higher in both areas, and their odds ratios were 5.33 and 3.56, respectively. There were more single schizophrenics in rural than urban. 2. In educational levels, there were no significant differences between schizophrenia and control. 3. In urban area, the jobless persons were more in schizophrenia than control, and its odd ratio was 1.90. 4. The urban schizophrenics were significantly lower than the urban control in economical levels, and its odd ratio was 1.65. 5. About family structure, religion, and marital status of parents, there were no significant differences between schizophrenia and control.

        Since Chinese vice Premier Teng Hsiao-pin recovered his power there has been Much change in chinese political sphere such as domestic affairs, economics, military, culture and foreign relations as well. Therefore this paper aims at trying to figure out an inclination of recent Chinese politics and to examin how does inclination effect the relation-ship with the major powers surounding Korean peninsular. Untill the mid 1970's Mao's teaching had been a only dominant doctrine in the field of Chinese political ideology but recently it shows us some symptoms of reassessment of Mao's thought under the Hua ana Teng's leadership. Therefore the theoretical basis of Sino-Soviet dispute lanunched by Khrushchev's destalinization and peaceful coexistence will be gradually discolored by it's pragmatic professionalism which inclined to bring industrialization and modernization to china. In the economic aspect china has blaimed foreign investment as a comparador capital during more than two decades. Now they are willing to accept foreign investment and technical co-operation program from western countries including U.S. and Japan for the fulfilment of their recent modernization program While China defines Russia as a main enemy she is willing to maintain reliable relationship with U.S. and to have technical assistance of nuclear power and highly developed electronic equipment for modernization of their military. Thus Chinese political inclination has effected role of the major powers. In other words China needed to promote relation with U.S., Japan and Western European countries and U.S.-Sino relation stimulated Japan to normalize relation with China. During this process Russia has maid enormous efforts to hinder the normalization of Sino-Japan relation and Russia on the other hand presented favourable attitude to the Taiwan and South Korea. It means there has been a Keen competition among Major powers and China more likely to promote relations with U.S. Japan and western European countries in the near future.

        백서 뇌에서 전기경련 충격에 의해 발현이 조절되는 새로운 유전자

        연구목적: 이 연구는 백서 뇌에서 전기경련 충격(Electroconvulsive shock, ECS)에 의해 발현이 조절되는 새로운 유전자를 검색하고, 발달단게 및 뇌 조직에 따른 발현양상을 관찰하기 위하여 시행되었다. 방법: 생후 7일, 21일된 어린 웅성 백서에 전기경련 충격을 가한 후 mRNS differential display-PCR(DD-PCR) 기법을 시행하여 발현의 차이를 나타내는 유전자를 검색한 후 서열분석을 실시하였다. 생후 7일 및 21일된 백서의 뇌와 성년 백서의 대뇌피질, 해마를 이용한 Northern blot 분석을 시행하여 검색된 유전자의 ECS 처치 후 시간경과에 따른 유전자 발현양상을 관찰하였다. 결과: ECS 처치군과 위충격군간에 발현의 차이를 나타낸 유전자 4개를 검색할 수 있었으며, 이중 ECS에 의해 발현이 증가되는 1개의 유전자를 선정하여 서열분석을 실시한 결과 EST 서열만이 보고된 새로운 유전자임이 밝혀졌다. 이 유전자는 생후 7일된 백서의 뇌에서는 ECS 처치 후 180분에 발현양의 증가가 최고치에 도달하였으나, 생후 21일된 백서의 뇌와 성년백서의 대뇌피질 및 해마에서는 360분 경과시까지 지속적으로 발현 양이 증가하였다. 결론: 이같은 결과는 ECS에 의해 생체 내에서 유전자 발현의 조절이 이루어짐을 시사하며, 향후 이러한 유전자의 세포 내 기능에 관한 연구를 시행함으로써, ECS에 의한 생체의 반응과 치료기전에 대한 이해를 넓힐 수 있을 것으로 기대된다. 중심단어:전기경련 충격·DD-PCR·유전자·백서 뇌. Objectives: This study was performed to identify genes regulated by electroconvulsive shock (ECS) and to observe the pattern of expression of genes according to different developmental stages and brain regions. Method: ECS(130V, 0.5 sec) was given to male Sprague-Dawley rats with age of postnatal day 7 and 21(P7, P21 respectively). After screening genes regulated by ECS with mRNA diffe-rential display-PCR(DD-PCR), we selected one clone among them and observed the induction of this gene after ECS by time-dependent Northern blot analysis of rat brain of P7, P21 and adult rat cortex and hippocampus. Results: By DD-PCR method, we have identified four clones whose expression was regulated by ECS. Among them, one(CP 10-2) was proved to be a new gene by sequencing and BLAST search. Its expression was increased after ECS in P7, P21, and adult rat brain. The expression of CP 10-2 reached peak level at 180 minutes after ECS in P7 rat brain, but was further increased until 360 minutes after ECS in P21 and adult rat brain. Conclusion: In this study, a new gene was identified in rat brain which showed up-regulated expression in response to ECS. Cloning and characterization of this new gene would be helpful to elucidate the effect of ECS in rat brain. KEY WORDS:Electroconvulsive shock·Differential display-PCR·Gene·Rat brain.

      • 수술후 경막외강에 주입한 ketamine의 진통효과에 관한 연구

        Author divided 60 patients who got lower abdomen or lower extremity surgical operation into three groups. Each group consisted of 20 patients. We injected ketamine 10㎎, 20㎎, and 30㎎ respectively into the epidural space when the patient complained of pain postoperatively and observed the postoperative analgesic effect of ketamine. Finally, we concluded the following : 1) The onset time of analgesic effect. : The analgesic effect appeared within 10 minutes in 3 groups and the significant difference among them was not seen. 2) The duration of analgesic effect : The duration of analgesia was relatively short with less than 1 hour on an average in all 3 groups and was not constant. 3) Cardiopulmonary system. : The significant change of blood pressure, pulse and respiratory rate was not seen in all three groups and and the difference among them also was not seen. 4) Side effects. : Sedative action(16%), nausea(10%), dizziness(5%), headache(3.3%) From above results, the epidural administration of ketamine is not suitable for postoperative pain management because of a lot of side effects as well as short and inconstant duration of analgesic effeet.

        광섬유 격자 다중화 스트레인 센서 시스템

        광섬유 센서는 구조물에 대한 비파괴적인 측정이 가능하고, 전자파에 의한 간섭이 발생하지 않으므로, 전자파 장애의 영향을 무시할 수 있는 장점이 있다. 또한, 구조물 건조시 콘크리트 같은 대상체에 광섬유를 내장시킬 경우에는 검사시 대상체를 파괴시키지 않고서도 대상체의 손상여부와 역학적 거동을 측정 및 해석할 수 있는 비파괴 검사 기술이다. 특히 광섬유 브래그 격자 센서는 그러한 대상체에 대한 비파괴 검사를 수행하는데 가장 적합한 센서이다. 광섬유 브래그 격자 센서는 그러한 대상체에 대한 비파괴 검사를 수행하는데 가장 적합한 센서이다. 광섬유 브래그 격자는 특정파장의 빛을 반사 또는 제거시키는 특성을 지니고 있으며, 스트레인 같은 물리량이 광섬유 브래그 격자에 가해지면, 반사되는 빛의 중심파장이 이동하여 이를 통해 물리량을 측정할 수 있다. 정ㆍ동적 스트레인을 측정할 수 있는 광섬유 브래그 격자 센서는 건축물이나 토목구조물 등의 안전 진단(health monitoring)을 위해 사용되고 있으며, 최근에는 도로나 교량과 같은 토목 구조물로의 응용에 있어서 필수적인 동적 스트레인의 측정에 대해 그 관심이 집중되고 있다. 본 연구에서는 패브리=페로(Fabry-Perot) 필터를 이용하여 다중점에서 동적 스트레인을 측정할 수 있는 센서 시스템을 제작하였으며, 제작된 센서 시스템을 모의 구조물(외팔보)에 적용하여 모의 구조물에 가해지는 정적, 동적 스트레인을 측정하였다. 측정 결과는 기존의 전기적 센서와 유사하였다. An optical fiber sensor is capable of nondestructive measurement of a structure and it has an advantage of the immunity to electromagnetic interference because light is not affected by electromagnetic wave. In addition, if optical fibers are buried in an object like a concrete, this sensor can analyze defects and physical status of the object without disassembling it. Especially, the fiber Bragg grating sensor is a promising optical fiber sensor capable of nondestructive test of such an object. A fiber Bragg grating has the characteristics of reflecting or blocking light of a specific wavelength. If we apply physical quantity like strain to the fiber Bragg grating, the center wavelength of the reflected light is shifted and then we can find the physical quantity applied to the fiber Bragg grating by measuring the center wavelength shift of the reflected light. The fiber Bragg grating sensor capable of static and dynamic strain measurement is being used in health-monitoring of buildings, structures, etc. Recently increasing is interest in dynamic strain measurement inevitable to the civil structures such as roads and bridges. In this study we implemented the optical fiber sensor system which can measure dynamic strain at multiple points using Fabry-Perot wavelength demodulation. And we measured the static and dynamic strain using this sensor system with a test structure(cantilever). Measurement results were similar to those obtained with the conventional electrical measurement methods.

      • 생체분해성 망막압정을 이용한 망막고정에 대한 실험적 연구

        Biodegradable retinal fixation devices obtain mechnical fixation of the retina with desirable chorioretinal scarring and with the potential for local, sustained release of antimetabolites and steroids to inhibit proliferative vitreoretinopathy. We manufactured a biodegradable retinal tack with barb that was designed in order to prevent intrusion from implantation of retinal tacks. This study was carried to evaluate the efficacy for retinal fixation and the capability for sustained release of drugs with a newly designed biodegradable retinal tack Biodegradable retinal tacks were made of polymers of glycolic acids and were designed with barbs in a shape to prevent the disinsertion. Biodegradale retinal tacks are divided into 3 parts, a conical portion that is inserted into the sclera, a cylinder portion that remains in the vitreous, and a neck portion between the pin and the cylinder. The tapered conical end was manufactured to allow easy insertion through the retina and choroid into the sclera. A cylinder portion was manufactured with a tapered angle that fixes firmly into the orifice of 19 gauge spinal needle. A neck portion, 0.4 mm in diameter, was designed to prevent disinsertion from following implantation of retinal tack. The applicator was a 19 gauge spinal needle and its orifice was prepared to 15°angle to accept the tapered cylinder portion of the retinal tack. The retinal tacks, secured in the needles, were passed through the formed vitreous and inserted into the retina, choroid, and sclera and were released by pushing the internal needle, usually within 2-3mm of the medullary ray of the posterior rabbit retina A retinal tack was placed in each of 8 pigmented rabbit eyes. Slit-lamp biomicroscopy, indirect ophthalmoscopy and fundus photography were performed periodically from 1 day to 8 weeks after surgery. Eight eyes were enucleated and studied by light microscopy at 8 weeks. Biomicroscopic evaluation of the animals revealed edemas adjacent to the retinal surfaces immediately after insertion of the biodegradable retinal tacks in all the animals. These edemas disappeared after 1 week. The first noticeable change in the size of retinal tacks was shown after 2weeks. The size of the retinal tacks gradually got smaller, decreasing to about one-half at 4 weeks and about one-third at 8 weeks. All retinal tacks remained in inserted places without any movement for an 8 week period. On light microscopy, epiretinal proliferations were seen to extend into the vitreous cavity. Cellular capsules that lined the inner aspect of the scleral defect caused by tack insertion were found. However the adjacent retina had a normal cytologic appearance and architecture in all specimens. We manufactured a biodegradable retinal tack that is designed to prevent intrusion from implantation of retinal tacks. All biodegradable retinal tacks reduce in size with time, but no retinal tacks extruded from the inserted place. The newly designed biodegradable retinal tack can be used for retinal fixation and may be used as a vehicle for the introduction of pharmacologic agents to prevent the cellular events that promote proliferative vitreoretinopathy.
