Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

The purpose of this study was to analyse landing motions applied with diverse directions actually taking place in the field instead of single direction landings in the context of experiments. In doing so, it sought to conduct analysis on the effect of landing directions on shock absorption mechanism at lower extremity joints. In this experiment, 16 male undergraduates majoring at the department of physical education took part (age: 22.6±3.4 yrs, height: 175.0±7.2 cm, weight: 69.3±6.6 kg). The study used eight infrared cameras and one force platform, and using 200Hz and 2000Hz respectively, collected data of images and round reaction forces. This experiment acquired the result as below. 1. As a result of examining the range, hip joint had the largest impact during landing to the right, while knee during landing in front and to the right 2. As a result of examining the ratio of range of motion, hip, knee and ankle had the largest impact during right, front and left landing respectively. 3. As a result of examining the size of work done, hip and ankle joints had the largest impact during right landing, while knee during front and left landing. 4. As a result of examining the proportion of work done, both hip and ankle joints had the largest impact during right landing, while knee had the largest impact in the order of right, front and left landings. This study identified that landing to the right could cause the largest load on lower extremity joints.

<P>Porous polydioxanone (PDO)/polyvinyl alcohol (PVA) scaffolds were fabricated by blending PDO with a small amount of PVA to improve the hydrophilicity and cell/tissue compatibility of the scaffolds for tissue engineering applications. PDO/PVA scaffolds with different PVA compositions up to 10 wt% were fabricated by a melt-molding particulate-leaching method (non-solvent method). The prepared scaffolds exhibited highly porous, uniform open-cellular pore structures. The PDO/PVA scaffolds with PVA compositions more than 5 % were easily wetted in cell culture medium. The hydrophilized PDO/PVA (5 wt%) scaffold showed better cell adhesion and growth than the control hydrophobic PDO scaffold. The PDO/PVA (5 wt%) scaffold also showed faster tissue infiltration into the scaffold than the PDO scaffold. It seems that 5 wt% addition of PVA to PDO to fabricate PDO/PVA scaffolds is enough for improving the hydrophilicity and cell/tissue compatibility of the scaffolds.</P>

Biodegradable polymers have been widely used to fabricate porous scaffolds for three-dimensional (3-D) cell cultures to regenerate tissue-based artificial organs. Until now, poly(L-lactic acid), poly(glycolic acid), and poly(lactic-co-glycolic acid) as synthetic biodegradable polymers have been most widely used to fabricate 3-D scaffolds. However, these polymers have limitations for some applications because of their brittleness, hydrophobicity, and low cell adhesiveness. Recently polydioxanone (PDO) has been interested in the applications for tissue regeneration since this polymer has good elasticity as well as biocompatibility and biodegradability. However, porous scaffold fabrications from PDO were limited due to its poor solubility (solvents to dissolve PDO are few). In this study, we firstly fabricated porous PDO scaffolds (disc and meniscus shapes) using a melt-molding particulate-leaching method developed by our laboratory. This method does not need any solvents during the fabrication process. To improve the hydrophilicity and cell compatibility of the scaffolds, PDO was blended with polyvinyl alcohol (PVA) with different ratio. The hydrophilicity, morphology, porosity, and mechanical properties of the prepared PDO scaffolds were investigated. The PDO/PVA (5 wt%) blend scaffold was found to have sufficient hydrophilicity, mechanical strength, and elasticity for tissue engineering applications including meniscus regeneration.

Purpose: Few studies have examined whether bioengineering can improve fecal incontinence. This study designed to determine whether injection of porous polycaprolactone beads containing autologous myoblasts improves sphincter function in a dog model of fecal incontinence. Methods: The anal sphincter of dogs was injured and the dogs were observed without and with (n = 5) the injection of porous polycaprolactone beads containing autologous myoblasts into the site of injury. Autologous myoblasts purified from the gastrocnemius muscles were transferred to the beads. Compound muscle action potentials (CMAP) of the pudendal nerve, anal sphincter pressure, and histopathology were determined 3 months after treatment. Results: The amplitudes of the CMAP in the injured sphincter were significantly lower than those measured before injury (1.22 mV vs. 3.00 mV, P = 0.04). The amplitudes were not different between dogs with and without the injection of autologous myoblast beads (P = 0.49). Resting and squeezing pressures were higher in dogs treated with autologous myoblast beads (2.00 mmHg vs. 1.80 mmHg; 6.13 mmHg vs. 4.02 mmHg), although these differences were not significant in analyses of covariance adjusted for baseline values. The injection site was stained for smooth muscle actin, but showed evidence of foreign body inflammatory reactions. Conclusion: This was the first study to examine whether bioengineering could improve fecal incontinence. Although the results did not show definite evidence that injection of autologous myoblast beads improves sphincter function, we found that the dog model was suitable and reliable for studying the effects of a potential treatment modality for fecal incontinence.

<P>Meniscus is the most commonly injured structure in the knee joint. Resection of the meniscus as well as the torn menisci is known to induce the degeneration of the articular cartilage. Replacement of the resected meniscus by allograft is limited by its availability and potential disease transmission. Artificial prostheses are being tried in an attempt to regenerate the meniscal tissue and we developed the biodegradable porous polycaprolactone(PCL) scaffold, which acts as a temporary scaffold to enable the regeneration of a new tissue in time. We report the results of rabbit implantation model. Biodegradable PCL scaffold coated with type I collagen with pores sized 100~150 +m and with compression modulus 400 kpa were fabricated by melt-molding particulate- leaching method. The molds were made using the native meniscus of the rabbit. Medial meniscus of right knee was partially removed through arthrotomy, leaving anterior 1/5 of the meniscus, after sectioning medial collateral ligament. The implant was attached to the peripheral capsule and remnant anterior meniscus with sutures. The medial meniscus of the left knee was removed and served as a control without replacement. The regenerated meniscus was harvested at 4 & 12wks after implantation. In addition to the routine histology of the tissue regenerated and remnant scaffold, junction between the normal meniscus and the regenerated tissue, and cartilage surface degeneration was observed. After 4 and 12 weeks the scaffolds, although considerable amount of the materials remained, were largely filled and covered with fibrous tissue which was assumed to be derived from synovial tissue of peripheral capsule. The tissue grossly resembling the native meniscus was maintained and spindle shaped cells with extracellular matrices were observed histologically. Neither cells with chondrocytic phenotype nor distinct cartilage matrices were observed until 12 weeks. The bonding between the regenerated tissue and the peripheral synovial capsule was firm and solid in all cases. The tissue bridges between the native meniscus and the regenerated tissue were found in 9/10 operated knees. Articular surface degeneration was not different between experimental and control groups except one case. More or less, the extrusion of the meniscus was found in almost all knees. This study revealed that meniscal replacement with PCL polymer prosthesis was feasible and led to adequate tissue formation. Long term studies on adaptive remodeling will be required.</P>
