백서 심근경색모델에서 시간경과와 경색의 크기에 따른 심자도의 변화

Background: Magnetocardiogram (MCG), which records the changes of magnetic fields generated by the heart`s electrical activity, theoritically can provide unique data for clinical application. To date, MCG has been investigated only at a single time point after myocardial infarction (MI) with severe left ventricular dysfunction in rats. The purpose of the present study was to investigate sequential changes of MCG after MI and to evaluate effects of infarct size on MCG. Methods: Acute MI were induced by the permanent ligation of left coronary artery in 22 rats. Magnetic fields were recorded just above a rat with Nb Superconducting Quantum Interference Device (SQUID) gradiometer inside a magnetically shielded room. MCG was measured before and immediately after surgery and it was subsequently recorded at the time points of 1, 4 and 6 hours post operatively. MCG was also measured at 1, 3, 7 and 21 days after surgery. Results: Elevation of ST segment and appearance of pathological Q wave on the MCG were evident immediately after the ligation of coronary artery and persisted to 6 hours after MI. On MCG, ST segment was depressed and T wave was inverted from 1 day after MI. In rats with small-and moderate- sized MI (infarct size〈30%), ST depression returned to near the isoelectric level and Q wave disappeared from 7 days after MI. However, ST depression and Q wave were still present in rats with larger infarct (infarct size≥30%). Conclusion: Evolutional changes of MCG were well-recognized up to 21 days after MI. Furthermore, the infarct size can be expressed by the extent of Q wave and ST segment depression on MCG. Taken together, these data indicate that MCG is a helpful modality for the diagnosis, evaluation of infarct size and follow up after MI.(Korean J Med 62:42-48, 2002)

Korean Talchoom`s modern life esthetics I Choi, Chan-yeoul/Kim, Mi-hyun This paper is going to interpret the Korean Talchoom Pan and Talchoom performer with the Deleuze`s idea of ``Life``, Also we`ll compare Artaud`s theater of cruelty with Talchoom through the concept of life, These kind of discussions will be the job that will revive the cultural characteristics of our traditional Talchoom. By revealing the modern meanings of Talchoom and comparing it with the Western modern theaters, we will see Talchoom as the play that is still alive at the 21st century, not as a taxidermied stuff beyond history, And by these point of views, I think Talchoom is the most valuable source in making our Korean theaters 21st century prospect. The grounds of Deleuze, who is the best of modern becoming-ontology, are the grounds of life. Deleuze`s grounds heads to the sea of life by escaping from the unique disease: transcendence of the West. The life philosophy of Deleuze, which is heading to the transcendental field of life, overthrows the shape philosophy of Plato. Separated from the existing shape philosophy, Deleuze attaches Nietzsche and Bergson and explains the world and art without appealing any transcendent things. I will have the perspective of not seeing the Talchoom Pan as any of these following things: an imitation of any kind of transcendent things, an expression of any composition agent, planning agent, audience agent and social organizations or the reflection of the social contradictory relationship. That will be an action of denying any kind of identities of shape that are imposed to the Talchoom Pan, and also will be an action of denying the essence of shape that guarantees those identities, And this means also, possessing the Talchoom Pan, at the perspective of perfect creation, through the ``dynamical process`` of energy, power and spirit. Deleuze sees that these dynamical fields of power and energy, and the sea of spirits are the field of life. The transcendental horizon that constitute Talchoom performer is time, On the horizon of time, a Talchoom performer makes a new ``himself`` through facing objectives, and by that he loses his identity. The recovery of this loss demands self modifying and this demand makes him repeat questions about himself. Only through these repeating, a Talchoom performer can reconstitute himself from dissolution, Talchoom performer is a possessor of an ability to make new identities through the struggle with those dissolution processes, so they are an existence that are established between differentiation and sameness, To Artaud, play is his life and the world is the stage. Artaud`s theater of cruelty, which creates dramas of new life, is a play of a process of repeating a new life through death, not a play with decided rules or results. The ``death`` here is not a negative thing. It means the process of making a new life through facing lifes that are not organic. Artaud`s idea of ``body without organs`` is the most important idea in solving the theater of cruelty with the concept of life. The actors of the theater of cruelty, which is a modern play of the West, and the Talchoom performers of our traditional play Talchoom, in other words, performers, puts high values on bodies and physical tension more than language and psychological tensions based on language, And they are not just people who represents the intents of the writers and directors. They have the same thing in common that they are becoming a creative ``piece of art`` themselves now-here at the play by new self-making, So this way our Talchoom and the modern play of the West goes together.

Gastrointestinal stromal tumor (GIST) is the most common non-epithelial neoplasm arising in the gastrointestinal tract, but this tumor is rarely seen in association with type l neurofibromatosis (NFl) Generally, the majority of GISTs are isolated neoplasm and they are sporadically found in the stomach, but the GISTs in NFl patients are usually multiple and usually found in the small intestine. We report a case of multiple GISTs in the jejunum of a 63-year-old woman diagnosed as NFl accompanied by complication of gastrointestinal bleeding, In this case, Patient had multiple cafe-au lait spots and neurofibroma on skin and had freckling on axilla and groin, and then, we made a diagnosis of NFl. Gastrointestinal bleeding is controlled by resection of multiple GISTs. Generally, only 3-5% of all gastrointestinal bleeding comes from the small bowel. Causes of small intestinal bleeding are angiodysplasia, neoplasm, NSAIDs induced ulcer etc. but, If patients have NFl accompanied by complication of gastrointestinal bleeding, GISTs on small bowel must be considered.

Underwater weighing has been regarded as one of the most reliable methods of determining body composition, fat free mass. However, it requires time and is cumber some for screening a large number of subjects. Reliable, rapid and screens are paening techniqu. The purpose of this study was to assess body composition of elite Archery Mayers using bioelectrical impedance method (BI). The BI data acquisition system (Art Haven 9, Kyoto, Japan) was used to determine the BI of the upper and lower, limbs, BI of the forearm upper arm lower leg, and thigh on the right and left side were determined in 19 elite Archery players using a specially designed BI acquisition system. The BI is a rapid, noninvasive, and relatively inexpensive method for assessing body composition in field settings. Some sophisticated methods of BI can also be utilized in laboratory settings. To use BI methods appropriately in either setting, testers need to understand the basic assumption and principles, as the potential sources of measurement error, for this method. The present study indicate that BI analysis may be useful to predict body composition including, body fat, bone, and muscle volume.

항원 흡입에 의한 비특이적 기관지과민성: II. 화분

피부단자검사, RAST 및 항원 기관지유발검사(allergen bronchial provocation test, ABPT)결과 쑥, 잔듸, 나무화분에 감작된 기관지 천식 환자 13명을 대상으로 이들 화분이 비특이적 기관지과민도(non-specific bronchial hyperresponsiveness, NSBR)에 미치는 영향을 알아보고, ABPT시의 조기 및 후기 천식반응(early, late asthmatic response, EAR, LAR)이 NSBR 생성에 관여하는지와 각각 화분의 항원성을 비교하고자, 감작된 화분으로 ABPT를 시행하고, 이를 전후하여 메타콜린 기관지유발검사(methacholine bronchial provocation test, MBPT)를 시행하여 아래와 같은 결과를 얻었다. 기저 폐기능상 기도 폐색을 보인 환자는 없었으며, 알레르기성 비염만을 호소하였던 환자 11명 중 4명(31%)은 MBPT에 양성반응을 나타내었으나, ABPT에는 11명 모두 양성반응을 보인 천식 환자였다. ABPT상 3명은 EAR, 2명은 ILAR, 8명은 LAR이였다. EAR만을 나타낸 경우는 ABPT 24시간 후 PC20이 감소하지 않고, 오히려 다소 증가하였으나, ILAR을 나타낸 환자들은 24시간 후 PC20가 감소하여 3일 후 기저치로 환원된 반면, LAR에서는 24시간 후 PC20가 현저히 감소하여, 2주 내지 6주 후에야 기저치로 환원되었다. LAR을 보인 환자들 중 쑥에 감작된 환자들이 목초 화분에 감작된 환자에 비해 Pc20의 감소기간이 유의하게 길었다. 이상의 결과를 종합해 보면 화분 흡입에 의한 LAR에 의해 NSBR이 악화되며 지속기간이 상당히 긴 것을 알 수 있다. 또한 화분 중 쑥의 항원성이 목초보다 상당히 강한 것도 알 수 있다. 더불어 화분에 감작된 알레르기성 비염 환자들의 대부분에서 천식이 잠재해 있음도 알 수 있다. 그러므로 향후 천식의 증상이 없는 화분에 감작된 비염환자라 할지라도 MBPT 및 ABPT를 시행하여 정확한 진단을 해야겠다. 본 연구에서 종래의 다른 연구 결과들과는 달리 쑥의 경우 ILAR의 경우도 NSBR을 악화시킴을 관찰하였는데, ILAR과 LAR의 차이가 단순한 기도폐색의 정도 차이인지, 기도 내에서 염증반응에 차이가 있는지는 향후 기관지 페포세척이나 조직 검사등을 통해 밝힘으로써, ABPT에 의한 NSBR의 발생 및 악화의 기전을 보다 확실히 규명할 수 있을 것으로 사료된다. Bronchial hyperresponsiveness is a fundamental characteristics in bronchial asthma. The pathogenesis of bronchial hyperreactivity has not been clearly understood, yet. We studied non-specific bronchial hyperresponsiveness with methacholine inhalation sequentially afterallergen provocation in pollen such as, alder, grass and mugwort-sensitzed allergic asthma and/or allergic rhinitis to investigate if there is any relation between allergen exposure and bronchial hyperreactivity, and compare the allergenicity of each pollen. 3 of 13 study subjects showed early asthmatic reaction, but 8 of them showed late(dual) asthmatic reaction, and the other 2 showed intermediate late asthmatic reaction. Non-specific bronchial hyperreactivity was not increased in early asthmatic responder, however, it was increased upto 45 days after allergen inhalation in all late asthmatic responder and in intermediate late reactor, upto 3 days. In mugwort-sensitized late asthmatic responder NSBR decreased much longer than any others. Taken together, pollen-induced late asthmatic reaction is one factor which contributes NSBR in pollinosis patients.

除草劑 Paraquat가 미생물의 증식에 미치는 영향

Experiments were carried out to understand the inhibitory effect of chemicals, especially that of a herbicide paraquat(1,1'-dimethyl-4,4'-bipy-lidilium dichloride) against microbial growth. Bacillus megaterium (ATCC 1787) was used as the indicator microorganism in the experiment. Among the eighteens of tested chemicals(insecticides 7, germicide 7 and 4 herbicides), six kinds of germicide and paraquat revealed an aparent repression against growth of the indicator organism at the concentrations pf ten ppm. Paraquat at the concentration above 1 x 10^-3M drastically repressed not only the growth of bacterial cells but yeast cells. Re-growth of the bacterial cell was inhibited in the concomitant presence of 1 x 10^-3M paraquat within eight hours of continuous cultivation, however, it was possible when con-centration of the herbicide was lowered 1 x 10^-4M.

삼출성 늑막액에서 양악성 감별지표로서 CEA, TPA, SCC Ag 측정의 의의

상피 세포에서 기원한 대표적인 종양표지자인 carcinoembryonic antigen (이하 CEA로 약함)은 삼출성 늑막액 환자의 양악성 감별에 보고적으로 사용되고 있다. CEA 이외에 혈청에서 양악성감별의 보조적 지표로 알려진 tissue polypeptide antigen (이하 TPA로 약함)과 squamous cell carcinoma antigen (이하 SCC Ag으로 약함)을 혈청과 늑막에서 동시 측정하여 늑막삼출액의 악성 감별에 어느 정도의 임상적인 유용성이 있는지를 알아보기 위하여 이 연구를 시행하였다. 1997년 1월 1일부터 동년 8월 31일까지 계명대학교 동산의료원에 입원한 환자들 중 삼출성늑막액을 가진 61명을 대상으로 하여 혈청과 늑막액에서 CEA, TPA, SCC Ag의 수치를 방사면역법으로 측정하였다. 각각의 조양표지자들은 악성과 양성군으로 구분한 뒤 분석하였으며 악성군이 28례, 양성군이 33례이었다. 그리고 진단양성기준치를 설정한 뒤 종양표지자들의 특이도, 민감도를 산출하였고 상기 지표들을 종양표지자와 늑막액 세포검사르 조합한 경우에도 산출하여 비교 분석하였다. 혈청 CEA 와 TPA는 각각 7.0 ng/ml, 80.0 ng/ml, 늑막액 CEA와 TPA는 각각 50. ng/ml, 4700.0 ng/ml로 진단양성기준치를 설정하였을 때 특이도를 낮추지 않으면서 가장 높은 민감도를 보였다. 늑막액 세포검사와 동시에 혈청 TPA 도는 늑막액 CEA를 측정하였을 때 특이도는 떨어뜨리지 않으면서 민감도를 높이는 좋은 조합인 것으로 나타났으며 혈청 CEA 및 TPA수치를 늑막액 세포검사와 도시에 시행하였을 때 특이도를 떨어뜨리지 않으면서 가장 높은 민감도를 얻었다. 늑막액 세포검사가 음성인 경우에도 혈청 CEA와 TPA를 동시에 측정하여 높은 민감도와 특이도를 얻을 수 있었다. 따라서 CEA와 TPA는 늑막삼출액의 양악성 감별 진단 유용한 보조적 지표로서 사용할 수 있을 것으로 판단된다. Carcinoembryonic antigen(CEA), the most widely used tumor marker was measured in pleural fluid of patients with exudative pleural effusions in order to differentiate malignant from benign effusions. This study was performed to find out if there is any clinical utility in differential diagnosis of malignancy by measuring simultaneously CEA, tissue polypeptide antigen(TPA) and squamous cell carcinoma antigen(SCC Ag) in serum and pleural fluid. The study population was 61 patients with exudative pleural effusions who were admitted to Keimyung University Hospital from January 1 to August 31, 1997. Each CEA, TPA and SCC Ag level in serum and pleural fluid were measured using radioimmunoassay method. These patients were divided to malignant and benign group. Malignant group consists of 28 cases and benign group consists of 33 cases. And the sensitivity and specificity of each tumor marker was obtained using cut-off value and that combining tumor markers and pleural fluid cytology were also obtained and analyzed. When the cut-off value was applied to CEA and TPA in serum using 7.0 ng/ml and 80.0 ng/ml respectively, the highest sensitivity was obtained without specificity being lowered. The same result was obtained when the cut-off value was applied to CEA and TPA in pleural fluid using 5.0 ng/ml and 4700.0 ng/ml respectively. When CEA in pleural fluid or TPA in serum were measured in combining with pleural fluid cytology sensitivity was increased without decreasing specificity than measured in pleural fluid cytology alone. When CEA in serum and TPA in serum were measured in combining with pleural fluid cytology simultaneously, the highest sensitivity was produced without decreasing specificity than measured in any other combinations. In addition, when serum CEA and TPA in serum were measured in the negative group of pleural fluid cytology, high sensitivity and specificity were obtained. These data suggest that CEA and TPA can be used as useful tumor markers for the differential diagnosis of malignancy and benign condition in patients with exudative pleural effusions.

온라인 교사공동체에 대한 질적 사례 연구 : '여교사 아이사랑 카페' 사례

본 연구는 인터넷 커뮤니티 공간에서 여교사들의 상호작용 과정의 특징을 밝히고자 하는 연구이다. 이를 위해 한 인터넷 커뮤니티 공간인 '여교사 아이사랑 카페'를 대상으로 질적 사례 연구를 수행하였다. 본 연구 결과, 여교사들은 '여교사 아이사랑 카페'를 통해, '교직생활 나누기', '엄마 역할 배우기' 등을 경험하는 것으로 나타났다. 본 연구를 통해, 여교사들은 인터넷 커뮤니티 공간에서 자신의 고민을 나눌 뿐만 아니라 적극적으로 정보를 찾아 나가며, 자신의 전문성 신장을 위한 통로로 활용하기도 한다. 여교시들끼리의 공감대를 형성하고, 경력이나 연령에 상관없이 더욱 수평적인 관계를 형성하는 모습을 보인다는 연구결과를 도출하였다. The purpose of this study was to analyze the characteristics of woman teachers' communication in a internet community. For that purpose, we did the qualitative case study on 'woman teachers lovong-child cafe' which is an internet community. We found that woman teachers were experiencing the 'sharing the teaching life' and 'learning the mom' s role' in the internet community. From this research, We concluded as following. 1) woman teachers are not only sharing their worries, but also looking for information in the internet community. 2) Woman teachers also are utilizing the internet community for their professional. 3) Woman teachers are making the sympathy in the internet community. 4) Woman teachers are interacting based on horizontal relationship in the internet community.
