        Authors examined 135 workers(133 males and 2 females) employed in a local road construction and maintenance office lacated in the Pohang area to investigate the presence of pneumoconiosis cases among them. Examination consisted of questionnaire survey, physical examination and chest radiograph. Results are as follows ; 1. Of 135 workers, 8 cases(5.9%) were found to have small opacities of category 0/1 or more on chest radiogtaph. These cases were all males and prevalence was highest in group with age 50 or more(11.1% 3 out of 27 workers) and in froup with 5 to 9 years of employment(9.5%, 4 out of 42 workers). Small Opacities were found only in field workers(8.9%, 8 out of 90 workers) and prevalence of small opacities by job title was 8.8% in overload watchmen(3 out of 34 workers), 8.2% in road sweepers, road repair and maintenance workers(5 out of 56 workers). 2. Prevalence of small opacities on chest radiograph was higher in dusty, outdoor jobs than non-dusty jobs and the difference was statistically significant(P<0.05) 3. There was no statistically significant difference between those with radiological appearance of small opacities and normal subjects in age, educational level, duration of employment, previous dust exposure, past history of respiratory illness and clinical symptoms(P>0.05). 4. Of 8 cases with small opacities on chest radiograph, 1 case had small opacity of category 1/0(s/s) and the other 7 cases had small opacities of category 0/1. Two cases had past history of pulmonary tuberculosis, but radiologic findings were not consistent with tuberculosis. Two cases with less than 5 years of employment had previous history of dust exposure. Although specific etiologic factors were not clear with this study alone, authors suspected that specific geological factors in the Pohang area(soils rich in diatomaceous earth, bentonite and fullers' earth etc.) may have some role in development of small opacities. Above results suggests that follow up investigation for further development and progression of pneumoconiosis and effective measures to prevent dust exposure of road workers are needed.

        골흡수 기전에 관한 연구 : 파골세포의 활성화 기전 MECHANISM OF OSTEOCLAST ACTIVATION

        Although the osteoclast has long been recognized to be the cell responsible for bone resorption, little is known of the mechanisms by which its activity is controlled. Recently, it has been suggested that osteoblasts ─ the bone-forming cells ─ seem to be the target cells of PTH, the bone-resorbing hormone, and mediate osteoclastic bone resorption by producing the coupling factor(s). Because bone tissue consists of several types of cells, isolation of distinct bone cell populations is prerequisite for studying the mechanism of bone resorption in cellular level. This experiment was performed ⅰ) to isolate the metabolically distinct bone cell populations from fetal rat calvaria by sequential enzyme digestion and biochemical characterization and ⅱ) to identify the factor(s) produced by osteoblast that stimulate resorption employing organ culture of bone. Calvaria from rat fetus at 19 day of gestation, were sequentially digested by enzyme solution consisted of collagenase, trypsin and EDTA for 10 (population I), 10(II), 10(III), 20(IV) and 20 minutes (V). Each bone cell population was primarily cultured for 6-7 days and effects of PTH, calcitonin and PGE_2 on acid and alkaline phosphatase activity and cAMP level were determined. Basal level of acid phosphatase in populations released early were higher than late population. In contrast, basal level of alkaline phosphatase was reversed. PTH(0.4 unit/ml) increased the acid phosphatase activity only in population I with no effect on alkaline phosphatase. Calcitonin(150ng/ml) had no effect on acid and alkaline phosphatase activity in all bone cell populations. cAMP level of population IV and V were increased by PTH significantly while CT had no effect in all bone cell populations at all. PGE_2 increased cAMP in all populations, the acid phosphatase activity in population I and alkaline phosphatase activity in population IV and V. Taken together, these results indicate that population IV and V express typical osteoblastic phenotype while population I revealed some characteristics of osteoclast. Bone cell population IV and V were incubated with fresh MEM or MEM containing 0.4U/ml PTH for 2 hours. After 2 hour-incubation, both the control-conditioned media(control-CM) or PTH-conditioned media(PTH-CM) were collected. Both conditioned media were lyophyllized and redissolved as 2 fold concentrate. Ulnae and radii were removed from 19-day old fetal rats, prelabelled by subcutaneous injection of 200μCi ^45CaCl_2 into their mothers on the 17th day of gestation. After 24 hours, media was changed with fresh BGJb media or BGJb media containing 300μl of control-CM or PTH-CM and cultured for 5 days. Effects of control-CM or PTH-CM were observed by the ratios of %-release of ^45Ca between paired control and experimental group. Control-CM obtained from population IV and V had no or very little effect on bone resorption but PTH-CM obtained from population IV and V increased the ^45Ca release significantly after 3 and 5 days of culture. This result provides the evidence indicating that osteoblastic cells mediate osteoclastic bone resorption stimulated by PTH.

        수술-교정 치료 환자의 통제 소재

        교정치료와 악교정 수술을 필요로 하는 환자가 점증하는 현 시점에 비추어, 이에 대한 기술적인 측면의 발전과는 달리 수술-교정 치료 환자에 대한 정신사회학적 혹은 정신심리적 양상에 대한 객관적인 정보는 그리 많은 주목을 받는 편이 아니었다. 수술-교정치료는 과학적인 치료과정이긴 하지만, 어느 정도 심미적 혹은 예술적인 측면을 지니는 선택 치료의 면모를 지닌다. 그러므로 치료에 대한 환자의 주관적인 느낌은 치료 기술이나 치료의 의학적 성공 여부보다 더 큰 영향을 미칠 수 있다. 그러므로, 수술-교정 (혹은 교정) 치료에 대한 환자의 반응을 예측하기 위한 정신심리적인 정보는 치료에 유용한 지표가 될 수 있다고 가정할 수 있다. 통제 소재에 대한 연구는 인간의 다양한 행동을 예측하기 위한 유용한 지표로 알려져 왔는데, 본 연구에서는 42명의 수술-교정치료 환자와 성별/연령별로 표본 추출된 42명의 통상적 교정치료 환자를 대상으로 삼아 정신=심리적 비교 및 분석의 방법으로 두 가지 종류의 내외 통제 소재(internal/external locus of control) 검사를 시행하였다. 그 결과 수술-교정치료군에서만 남자가 여자에 비하여 낮은 외적 통제(높은 내적 통제)의 경향을 보였다. 결론적으로 수술-교정치료 환자의 정신심리적 기반은 성형 수술 환자와는 달리 교정치료 환자와 유사한 경향을 지닌 것으로 생각되었다. Surgical-orthodontic treatment is an increasingly more common dental procedure whose unique psycho-social or psychological feature has not been fairly quantified objectively. Since the treatment of a surgical-orthodontic (or orthodontic) patient is part science and part art, a subjective recognition of a patient about his or her own treatment may be more important than technical success during and after treatment. Therefore, the knowledge of the patient''s underlying psychological status could be useful in the prediction of patient's response to surgical-orthodontic (or orthodontic) treatment. The purpose of this study was to investigate and evaluate the psychological difference between conventional orthodontic patient and surgical-orthodontic patient by using locus of control (LOC) examination. Locus of control scale has been proven to be extremely useful in the prediction of a variety of human behaviors. Two types of locus of control data (I-score, Internal locus of control score ; E-score, External locus of control score) were obtained for 42 surgical-orthodontic patients and randomly selected 42 conventional orthodontic patients (as a control group) matched for age and gender. No statistically significant difference was observed in the scale of internal and external locus of control between the groups of surgical-orthodontic patients, males showed lower E-score (external locus of control score, higher internal locus of control tendency) than females. The results of this study suggested that the psychological background of surgical-orthodontic patients, in contrast with that of cosmetic surgery patients, has a similar disposition with that of orthodontic patients.

        다제내성 그람양성균에 대한 Linezolid(Zyvox^(�))의 시험관내 항균력 비교

        배경 : MRSA, VRE, VRSA같은 다제 내성 그람 양성균의 등장에 따라 glycopeptide를 대체할 새로운 항생제의 개발이 필요하게 되었고 결과적으로 새로운 항생제인 linezolid라는 항생제가 개발되었다. Linezolid는 이전의 항생제와는 다른 새로운 계열의 oxazolidinone으로 경구 이용률이 우수하다. 원내 및 원외감염의 중요한 원인균이 되고 있는 MRSA, VRE에 대한 적절한 경구용 항균제가 없는 국내에서 폐렴 및 피부 연조직 감염에서 경구용으로 사용해 볼 수 있는 약제이다. 본 연구에서는 고대 구로 병원에서 분리된 MRSA, VRE 등을 대상으로 다른 여러 항균제와 비교한 linezolid의 시험관내 항균력을 조사하고자 하였다. 재료 및 방법 : 연구대상은 1998년 1월부터 2000년 12월까지 본원에서 입원 및 외래를 통하여 피부 연조직 감염증 및 호흡기 감염증, 요로감염증으로 진단된 환자들의 가검물로부터 분리된 MRSA 60균주, VRE 43균주, PRSP 25균주를 액체배지 또는 한천배지 희석법을 통하여 linezolid 및 기타 항균제에 대한 최소발육억제농도를 구하였다. 결과 : 실험에 사용한 S. aureus는 모두 MRSA였고 이들은 linezolid에 대해 MIC_(90) 2㎍/㎖(MIC 범위 1-2㎍/㎖), Enterococcus spp는 모두 VRE로 linezolid의 MIC_(90)은 2㎍/㎖로 MIC 범위는 1-4㎍/㎖였다. 한 개의 균주에서 MIC 4㎍/㎖로 중등도 감수성을 보였으나 MIC breakpoint가 (8㎍/㎖인 내성균주는 없고 모두 감수성을 보였다. S. pneumoniae의 경우 penicillin 내성이었고, linezolid MIC_(90) 1㎍/㎖ (MIC 범위 0.5-1㎍/㎖)로 전부 감수성을 보였다. 결론 : Linezolid는 MRSA를 위시한 VRE, PRSP 등의 다제 내성 그람 양성균에 대하여 우수한 시험관내 항균력을 보임을 알 수 있었다. Background : The emergence of multi-drug resistant Gram-positive cocci, such as MRSA, VRE, and VRSA, necessitated to develop new antibiotics, which could replace the glycopeptide. As a result, a new antibiotics named linezolid was developed. Linezolid is different line of oxazolidinones with a good oral bioavailability, compared to other antibiotics. Since appropriate oral antibiotics are not presently available for MRSA, which is a major cause of nosocomial and community acquired infections, the introduction of linezolid will have favorable effect on treatment of infections such as pneumonia or skin infections. In this study, we investigated the antibiotic effect of linezolid on MRSA and VRE isolated from patients who were treated in Korea University Guro Hospital. Material and Methods : By using broth microdilution and agar dilution method we measured minimum inhibitory concentration (MIC) with sixty S. aureus, forty three Enterococcus spp., and twenty five S. pneumoniae isolates from patients who were diagnosed as skin, soft tissue, respiratory, and urinary infections in Korea University Guro Hospital from January, 1998 to December, 2002. Results : All of S. aureus used in this study were MRSA, and MIG_(90) of linezolid was below 2 ㎍/㎖ (MIC ranged between 1-2 ㎍/㎖). All of Enterococcus spp. were VRE, and had MIG_(90) of 2 ㎍/㎖ (MIC ranged between 1 to 4 ㎍/㎖). One of the VRE showed intermediate susceptibility with MIC of 4 ㎍/㎖. However, none was resistant with MIC breakpoint above 8 ㎍/㎖. All of S. pneumoniae were resistant to penicillin, but they were susceptible to linezolid with MICao of 1 ㎍/㎖(MIC range 0.5-1㎍/㎖). Conclusion : In conclusions, linezolid has an excellent in vitro antibiotic effect on multi-drug resistant Gram-positive cocci, such as MRSA, PRSP, and VRE.

        서울 강서지역 1개 대학병원에서 성인 급성 신우신염의 원인균과 항생제 감수성

        목적 : 3년간 서울시 강서지역의 1개 대학병원에서 입원치료를 받은 성인 급성 신우신염 환자를 대상으로 원인균과 항생제 감수성을 조사하고, 향후 급성 신우신염의 초기 경험적 치료 항생제를 제안하고자 하였다. 방법 : 1999년 1월부터 2001년 12월까지 3년간 고려대학교 의과대학 구로병원에 상부요로감염으로 내원한 16세 이상 환자 229명의 의무 기록을 검토를 통하여, 인구학적 특성, 원인균, 항생제 감수성, 초기 항생제 사용 양상과 입원기간을 조사하였다. 결과 : 연구에 포함된 229명을 118명의 단순 신우신염군과 111명의 복잡 신우신염군으로 구분 하였다. 단수 신우신염의 평균 발생 연령은 38.2세와 복잡 신우신염 56.1세로 복잡 신우신염 환자의 연령이 유의하게 높았다(P<0.001). 양군 모두에서 원인균 중 E. coli의 분리율이 가장 높았으며 항생제 감수성 결과 ampicillin, trimethoprim-sulfame-thoxazole에는 높은 내성률을 보였고, ciprofloxacin, gentamicin, cefotaxime에 대한 감수성은 비교적 높았으나 복잡 신우신염의 경우 단순 신우신염에 비하여 ciprofloxacin, gentamicin에 대한 감수성이 다소 낮은 것을 확인할 수 있었다. 평균 입원 기간, 항생제 투여 기간에 있어서도 복잡 신우신염군에서 유의하게 길었다. 결과 : 본 연구의 결과 서울의 강서 지역에서 대학병원급에 치료 의뢰되는 단순 급성 신우신염의 치료에 있어 1차 항생제로 3세대 cephalosporin, aminoglycoside, quinolone 중 한가지를 초기 경험적 치료제로 사용을 고려하여야 할 것으로 사료된다. Background : The purpose of this study is to recommend the initial therapeutic regimen for adult patients with acute pyelonephritis (APN) according to the changes of antimicrobial susceptibility patterns of causative microorganisms isolated from patients with APN. Methods : We reviewed medical charts of 229 APN patients, who had been treated at Korea University Guro Hospital from 1st of January. 1999 to 31st of December, 2001. We investigated the demographic data, clinical findings, durations of hospital treatment, antimicrobial susceptibility patterns of the causative microorganisms and initial antibiotic regimens in patients with APN. Results : In this study, 229 adult patients with APN were classified into simple APN patients (118 patients, 51.5%) and complicated APN patients (111 patients, 48.4%). Mean age of patients with simple APN was 38.2±14.1 years old and that of patients with complicated APN was 56.1±14.9 years old. Mean age of patients with complicated APN was significantly higher than that of simple APN patients (P<0.0001). Escherichia coli was the most common microorganism both in simple APN (96.7%) group and in complicated APN (90.6%) group. Antimicrobial susceptibility of E. coli was at the low level of ampicillin (31%/20%) and trimethoprim-sulfamethoxazole (42.6%/34.2%) in each group. In contrast, ciprofloxacin (11.5%/22.7%), gentamicin (16.4%/22%) and cefotaxime (0%/8.2%) resistance remained at relatively lower level. In comparison of simple APN with complicated APN, ciprofloxacin and gentamicin resistances were higher in complicted APN group. Average duration of hospitalization (5.9±2.3 days/8.2±4.6 days) and duration of antibiotic use (12.1±3.9 days/15.3±10.0 days) were significantly longer in complicated APN. Conclusions: The results of this study suggests that 3rd cephalosporin, aminoglycoside or quinolone antibiotic would considered as one of the initial therapeutic regimen for patients with simple APN in southwestern Seoul.

      • Rocuronium의 애별투여방법이 모지내전근의 이완에 미치는 영향

        Objective : There are controversies in pnmmg technique of rocuronium whether it can or can't shorten the onset time. We want to compare the effects of two different priming doses and intervals of rocuronium. Materials and Methods : After giving propofol, 50 patients were randomly assigned to 5 groups. Group I received a placebo, followed 3 min later by rocuronium 0.6 mg/kg. Priming doses and intervals of Group II, Ill, N, V were 0.06 rng/kg and 2 min, 0.12 mg/kg and 2 min, 0.06 rug/kg and 3 min, 0.12 mg/kg and 3 min, respectively. Total dose of rocuronium was 0.6mg/kg. Train of four (TOF) count or ratio and 1st twitch height of TOF (T1) were monitored with 15 secondsintervals. TOF ratio of each groups just before bolus injection, duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were compared. Results : Duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were decreased in priming groups compared to Group 1 (P < 0.01) In group V, significant depression of TOF ratio just before bolus injection was seen (P < 0.05). Conclusion : Priming technique with rocuronium is effective method to shorten the onset of neuromuscular block. But priming dose of rocuronium itself could cause hypoxia and aspiration in awake patients and should be used carefully. Priming dose of rocuronium 0.06 mg/kg and 2 minutes interval maybe relatively safe method.

      • 선인장 양심실 보조장치의 설계 및 임상적용을 위한 평가

        현재 개발되고 있는 맥동형 이식형 인공심장은 완전이식형 인공심장과 좌심실보조장치 뿐이다. 좌심실보조장치를 장착한 환자의 10~15%가 우심실의 보조를 필요로하고 완전이식형 인공심장의 장착을 위해서는 자연심장을 제거해야하는 상황에서 이식형 양심실보조장치에 대한 필요성이 대두되고 있다. 본 연구진은 완전이식형 인공심장으로 개발된 한국형 인공심장을 개선하여 양심 실 보조장치를 개발하였다. 양심실보조장치는 이동형 작동기식 미케니즘을 이용하고 있으며 에너지 변환장치와 감속기로 구성된 작동기와 혈액주머니, 그리고 내장형 제어기로 구성되어 있다. 선인장 펌프로 명명된 KAH350은 캐뉼라의 연결을 위한 커넥터를 포함하여 길이 177mm, 폭 164mm, 높이 67 이고 무게 780g이며 최대 심박출량은 5L/min 이다. 양심실보조장치로 개발된 선인장펌프는 좌심실보조장치로도 응용이 가능하다. 이식적합성과 생체적합성을 평가하기 위하여 좌심실보조장치로 5회, 양싱실보조장치로 6회의 동물실험을 수행하였다. 각각 최장 28일간 생존하였으며 11회의 동물실험중 장치의 결함에 의한 것이 3회 있었는데 이들은 모두 전자장치의 결함과 방수처리문제였다. 현재 이식적합성과 일박출량을 향상시킨 KAH400모델이 개발중이다. The types of pulsatile implantable artificial hearts that are on the way of development and have been developed are totally implantable artificial heart and implantable left ventricular assist device (LVAD). Approximately 10% to 15% of all patients Implanted with wearable VADs have required right heart support with another device. And it is reluctant to patient who should remove his or her own heart to be implanted with total artificial heart. These situations drive the development of implantable bi-ventricular assist device (BVAD). The Korean BVAD was develophed by modifying the moving actuator type Korean artificial heart. This electro-mechanical BVAD comprises actuator including energy converter and reduction gear train, blood sacs, and internal motor and energy controllers. The KAH350 which is named as 'Cactus Pump' is 177mm in length, 164mm in width, and 67mm in thickness including connectors and nuts. The weight and maximum cardiac output of Cactus Pump is 780g and 5L/min, respectively. The Cactus Pump that was developed as BVAD could be used as LVAD by attaching compliance caps on the 2 ports of one ventricle. The animal experiments were undergone 5 times for LVAD, and 6 times for BVAD. The best records were 28 days survival in both applications. There were 3 times of device failure and they were all associated with electrical connection and hermetic sealing. The KAH400 that has improved anatomical fitting characterisitcs and stroke volume is on development.

        Characterization of an Alkali-extracted Peptidoglycan from Korean Ganoderma lucidum

        The biologically active peptidoglycan was purified form the alkali fraction of the fruiting bodies of Ganoderma lucidum and the composition of the peptidoglycan was investigated by conventional analyses. The alkali-extracted peptidoglycan showed differences in chemical compositions from the water-extracted. The alkali-extracted peptidoglycan contained 6.9% protein and 75.9% carbohydrates composed mainly of $\beta$-glucose, mannose, and $\alpha$-glucose. The molecular weight range of the peptidoglycan was determined as 2,000 kDa-17 kDa. The peptidoglycan is considered to be a hybrid molecule of polysaccaride chains covalently bound as a side chain to the polypeptide core.

      • Gas chromatography-mass spectrometric method for the screening and quantification of illicit drugs and their metabolites in human urine using solid-phase extraction and trimethylsilyl derivatization

        <P>A simple and rapid GC-MS method has been developed for the screening and quantification of many illicit drugs and their metabolites in human urine by using automatic SPE and trimethylsilylation. Sixty illicit drugs, including parent drugs and their metabolites that are possibly abused in Korea, can be monitored by this method. Among them, 24 popularly abused illicit drugs were selected for quantification. Very delicate optimizations were carried out in SPE, trimethylsilylation derivatization, and GC/MS to enable such remarkable achievements. Trimethylsilylated analytes were well separated within 21 min by GC-MS. In the validation results, the LOD of all the analytes were in the range of 2–75 ng/mL. The LOQ of the quantified analytes were in the range of 5–98 ng/mL. The linearity (r<SUP>2</SUP>) of the quantified analytes ranged 0.990–1.000 in each concentration range between 10 and 1000 ng/mL. The mean recoveries ranged from 62 to 126% at three different concentrations of each analyte. The inter-day and inter-person accuracies were within −13.3∼14.9%, and −10.1∼13.0%, respectively, and the inter-day and inter-person precisions were less than 12.9%. The method was reliable and efficient for the screening and quantification of abused illicit drugs in routine urine analysis.</P>
