      • 결장직장암 환자의 임상적 고찰 : 조선대학교 부속병원의 10년 간의 결과

        Background and Objectives: The colorectal cancer rates 4th in the total cancer prevalence in Korea. The clinical patient demographics were considered with the incidence rate of the colon and rectal cancer. The clinicopathological factors were analyzed for the 5 year survival rate. Materials Methods: Of the 282 patients who were operated from Jan. 1, 1992 to Dec. 31, 2001 and diagnosed by pathological biopsy as colo-rectal cancer, 234 patients were followed up. The age, sex, location of the cancer, TNM stage, operation method, histopathological classification, metastatic region, complications after operation and the survival rate were investigated. Results: Of the 234 patients, male and female consisted of 120 cases (51.3%) and 114 cases (46.7%), with an average age of 61.6 years. The average follow up period was 52.3 months and the peri operative mortality was 5.6%. The site of the cancer was most frequent at the rectum with 127 cases (54.3%). The patients were classified according to the TNM stages with 38 cases of stage 1 (16.2%), 98 cases of II (41.9%), 72 cases of III (30.8%) and 26 cases of IV (11.1%). The 5 year survival rate in the age group older than 50 was 63.3 % and 73.5% in the group younger than 50. In regard to the operation type, the 5 year survival rate was 65.4% for elective operation and 65.8% for emergency operation. The 5 year survival rate for well differentiated, moderately differentiated, poorly differentiated cancer, and mucinous adenocarcinoma was 73.7%, 60.2%, 58.3%, and 92.6%, respectively. The 5 year survival rate of each of the TNM stage is 81.1%, 74.8%, 57.0%, and 11.5% for Classes I, II, III, and IV, respectively. Conclusion: The clinical factors were analyzed for the 5 year survival rate with no statistical significance for the age, sex, and location of the cancer. The TNM stage and histopathological differentiation were statistically significant. The colon cancer showed better prognosis compared to the rectal cancer.

        운동신경원성 질환을 동반한 전두측두엽치매의 임상양상과 신경심리소견

        Backgrounds:Frontotemporal dementia (FTD) is rarely associated with motor neuron disease (MND). This comorbidity (FTD-MND), a subtype of FTD, results in progressive dementia and muscle weakness. Among the few available reports of series of patients, however, there have been controversies about the clinical course of FTD-MND. This study, the first report of a series of FTD-MND patients in Korea, investigated demographic and clinical features, clinical course, and neuropsychological findings of nine patients with FTD-MND. Methods:Nine FTD-MND patients (2 men and 7 women with mean age 55.6±7.2 years) were selected among 45 FTD patients who met the FTD criteria proposed by the Lund and Manchester Groups. Their clinical and neuropsychological findings were analyzed retrospectively. Results:Mean age of onset was 54.3±8.0 years. The time interval from onset to death was 27±11.3 months. Presenting symptoms were personality change, hyperphagia, nonfluent speech or motor weakness. In most cases (8/9), dementia preceded the muscle weakness;in only one patient the muscle weakness preceded the cognitive decline. Regarding the muscle weakness, bulbar symptoms occurred earlier than limb weakness in four patients;bulbar and limb weakness occurred simultaneously in another four patients;in the remaining one patient limb weakness preceded bulbar weakness. Neuropsychologic tests showed no specific patterns but diffuse dysfunction in all cognitive domains. Conclusions:Our study suggests that FTD-MND predominantly affects the bulbar muscles rather than the limb muscles in early stage and symptoms related to MND occurs later than those related to FTD. The time from diagnosis to death was 10.6±6.5 months, suggesting that FTD-MND is a rapidly progressive disease. This rapid clinical course may account for our neuropsychological findings that showed general cognitive deficits father than predominant frontal dysfunctions.

        교과과정의 조직은 체계적이고 뜻이 있는 순서로 학생들을 가르치기 위해 지식을 어떻게 조직화할 것인지와, 과목들 사이에서 공통된 내용은 통합시켜 중첩되는 일이 없도록 조정할 필요가 있다. 이에 현재 본 대학 간호과에서 운영되었던 97학년도의 17개 전공과목들의 교과과정의 내용을 비교, 분석하여 중복을 줄이며 순차적 지식의 습득을 도모하는 새로운 통합교과과정의 틀을 개발, 제시함으로써 보다 나은 전문직 간호교육이 이루어지도록 하고자 하였다. Most curricula are considered how to teach the student in systematic and meaningful orders. And it is necessary to coordinate the duplicate contents in major subjects in nursing. So this study was designed to set up a new comprehensive curriculum through comparing and ananlyzing each seventeen major subjects in Nursing Department of Kyung-Bok College. Therefore, the purpose of this study is to present the new comprehensive curricula for promoting the efficiency and effectiveness in diploma degree course of nursing education.

      • 인체 자궁암 세포에서 천연 성분이 P-당단백질의 활성에 미치는 영향

        Multidrug resistance (MDR) of cancer cells is, at least in part, associated with the overexpression of P-gly-coprotein (P-gp). Many studies hve demonstrated that natural compounds obtained from fruits, vegetables, teas and medicinal plants may modulate P-gp activity. The objective of the present investigation was to examine the effect of seven natural compounds on the P-gp activity in human uterine sarcoma cell line, MES-SA/DX5. Daunomycin uptake was significantly increased by biochanin A and silymarin (p<0.0001) whereas it was reduced by morin (p<0.01). The efflux of daunomycin from the cells was significantly inhibited by biochanin A, morin, cephalotaxine, berberine (p<0.05) and silymarin (p<0.0001). Biochanin A, berberine and silymarin significantly decreased IC_(50) value of daunomycin (p<0.05) while morin increased it (p<0.05). These results suggest that some natural compounds such as biochanin A and silymarin may inhibit P-gp funciton and can by developed as MDR reversing agents to improve the efficacy of chemotherapeutic drugs when administered concomitantly.

        Are Spinal GABAergic Elements Related to the Manifestation of Neuropathic Pain in Rat?

        Impairment in spinal inhibition caused by quantitative alteration of GABAergic elements following peripheral nerve injury has been postulated to mediate neuropathic pain. In the present study, we tested whether neuropathic pain could be induced or reversed by pharmacologically modulating spinal GABAergic activity, and whether quantitative alteration of spinal GABAergic elements after peripheral nerve injury was related to the impairment of GABAergic inhibition or neuropathic pain. To these aims, we first analyzed the pain behaviors following the spinal administration of GABA antagonists (1Ռg bicuculline/rat and 5Ռg phaclofen/rat), agonists (1Ռg muscimol/rat and 0.5Ռg baclofen/rat) or GABA transporter (GAT) inhibitors (20Ռg NNC-711/rat and 1Ռg SNAP-5114/rat) into naïve or neuropathic animals. Then, using Western blotting, PCR or immunohistochemistry, we compared the quantities of spinal GABA, its synthesizing enzymes (GAD65, 67) and its receptors (GABA<sub>A</sub> and GABA<sub>B</sub>) and transporters (GAT-1, and -3) between two groups of rats with different severity of neuropathic pain following partial injury of tail-innervating nerves; the allodynic and non-allodynic groups. Intrathecal administration of GABA antagonists markedly lowered tail-withdrawal threshold in naïve animals, and GABA agonists or GAT inhibitors significantly attenuated neuropathic pain in nerve-injured animals. However, any quantitative changes in spinal GABAergic elements were not observed in both the allodynic and non-allodynic groups. These results suggest that although the impairment in spinal GABAergic inhibition may play a role in mediation of neuropathic pain, it is not accomplished by the quantitative change in spinal elements for GABAergic inhibition and therefore these elements are not related to the generation of neuropathic pain following peripheral nerve injury.

        Original Articles : Effect of aldosterone on the amplification of oncolytic vaccinia virus in human cancer Lines

        Background/Aims: JX-594 is an oncolytic virus derived from the Wyeth vaccinia strain that causes replication-dependent cytolysis and antitumor immunity. Starting with a cross-examination of clinical-trial samples from advanced hepatocellular carcinoma patients having high levels of aldosterone and virus amplification in JX-594 treatment, we investigated the association between virus amplification and aldosterone in human cancer cell lines. Methods: Cell proliferation was determined by a cell-counting-kit-based colorimetric assay, and vaccinia virus quantitation was performed by quantitative polymerase chain reaction (qPCR) and a viral plaque assay. Also, the intracellular pH was measured using a pH-sensitive dye. Results: Simultaneous treatment with JX-594 and aldosterone significantly increased viral replication in A2780, PC-3, and HepG2 cell lines, but not in U2OS cell lines. Furthermore, the aldosterone treatment time altered the JX-594 replication according to the cell line. The JX-594 replication peaked after 48 and 24 hours of treatment in PC-3 and HepG2 cells, respectively. qPCR showed that JX-594 entry across the plasma membrane was increased, however, the changes are not significant by the treatment. This was inhibited by treatment with spironolactone (an aldosterone-receptor inhibitor). JX-594 entry was significantly decreased by treatment with EIPA [5-(N-ethyl-N-isopropyl)amiloride; a Na+/H+-exchange inhibitor], but aldosterone significantly restored JX-594 entry even in the presence of EIPA. Intracellular alkalization was observed after aldosterone treatment but was acidified by EIPA treatment. Conclusions: Aldosterone stimulates JX-594 amplification via increased virus entry by affecting the H+ gradient. (Korean J Hepatol 2011;17:213-219)

        이뇨제를 사용한 급성 신손상 환자에서 FEUrea의 진단적 유용성

        목적: 임상적으로 FENa가 일과성 신손상 (T-AKI)과 지속성 신손상 (P-AKI)을 감별하는 데 많이 사용되지만 이뇨제를 사용한 환자에서 FENa는 유효 혈류량 결핍 상태에서도 증가하게 되어 진단적 정확성이 떨어진다고 알려져 있다. 본 연구에서는 이뇨제 투여상황에서 FE(Na)와 비교하여 FE(Urea)의 진단적 유용성에 대하여 알아보고자 하였다. 방법: 107명의 급성 신손상 환자를 임상적 특성에 따라 일과성과 지속성 신손상 군으로 나누고 이를 다시 이뇨제 투여 유무에 따라 재분류하였다. ROC 곡선에 따라 계산된 cutoff value에 따라서 일과성 신손상을 정의하였고 이뇨제 투여로 인한 cutoff value의 변화에 따른 민감도와 특이도를 비교하였다. 결과: AKI가 발생한 107명의 모든 환자중 검사 이전에 경험적으로 이뇨제를 사용한 경우가 67명으로 63%를 보였고, 일과성 신손상군의 경우는 52명 중 27명으로 52%를, 지속성 신손상군의 경우는 55명 중 45명으로 73%를 보였다. ROC curve에 따라 cutoff value를 FE(Na)≤1.5 FE(Urea)≤30으로 하였을 때 T-AKI를 진단하는 데 사용된 두 값의 민감도와 특이도는 모든 환자군에서 FE(Na)가 81%, 98%를, FE(Urea) 가 94%, 82%를 보였다. 이뇨제 비투여군에서는 FE(Na)가 96 %, 100%를, FEUrea가 92%, 87%를 보였으며, 이뇨제 투여군에서는 FE(Na)가 63%, 98%를, FE(Urea)가 96%, 83%를 보였다. 결론: 이뇨제를 사용한 경우 P-AKI를 진단하는데 FE(Urea)도 FE(Na) 정도의 진단적 유용성을 가진다. Purpose: Although fractional excretion of sodium (FE(Na)) has been used to distinguish transient-acute kidney injury (T-AKI) from persistent-AKI (P-AKI), the availability of FE(Na) in the diagnosis of T-AKI is reported low in patients with diuretics use. We compared the diagnostic performance of fractional excretion of urea (FE(Urea)) with that of FE(Na) in patients with diuretics use. Methods: One hundred seven AKI patients were classified as having T-AKIor P-AKI according to the clinical context. Each group was again subdivided according to exposure to diuretics. According to the cut off value generated by receiver operating characteristic (ROC) curves, sensitivity and specificity of FE(Na) and FE(Urea) were compared with each other. Results: The numbers of patients administered with diuretics were 67 out of total 107 AKI patients (63%), 27 out of 52 (52%) of T-AKI patients, and 40 out of total (65) 55 (73%) of P-AKI patients. When the cutoff value of T-AKI was defined as FE(Na) ≤1.5 and FE(Urea) ≤30 according to the ROC curves, sensitivity and specificity of FE(Na) were 96% and 100% in non-diuretics group, and 63% and 98% in diuretics group, respectively. Sensitivity and specificity of FE(Urea) were 92% and 87% in non-diuretics group, and 96% and 83% in diuretics group, respectively. Conclusion: FE(Urea) is as good as FE(Na) at distinguishing T-AKI from P-AKI in patients administered with diuretics.

        수온에 따른 영산강 식물플랑크톤군집 변동

        In order to clarify the ecological properties of phytoplankton community, the distribution of phytoplankton and the relation of water temperature and size-fractionation measurements were studied from November 2004 to August 2005 in Youngsan River, Korea. A total of 265 phytoplankton species was identified. It consists of 48 genera and 123 species (46%) of Chlorophyceae, 27 genera and 89 species (34%) of Bacillariophyceae, 12 genera and 25 species (9%) of Cyanophyceae, respectively. From size fractionation analysis, nanophytoplankton (2~20 μm) dominated from early spring to early summer, and microphytoplankton (20~200 μm) from summer to winter. The relationship between chl-α and nanophytoplankton showed high correlation coefficient value (r2=0.93) from Najudaegyo site. The correlation coefficient values between water temperature and nanophytoplankton were low except Dongkangdaegyo site which showed high value (r2=0.73).
