        We have usually experienced bone conduction impairment in chronic otitis media, but it is unclear that raised bone conduction threshold is due to the reflection of cochlear dam- age and / or Carhart effect ( an artificial elevation of bone conduction threshold due to conductive defect ) or both. The purpose of this study is to verify the influence of the artificial elevation of the bone conduction threshold in chronic otitis media and to estimate Carhart effect. A total of 336 cases of tympanoplasty with unilateral chronic otitis media without any other causes of sensorineural hearing loss was studied. The average threshold levels of the bone conduction in diseased ears ranged from 18.5dB at 4kHz to 12.1dB at 250Hz The longer duration of the disease, the larger perforation size, the more severe damage of the middle ear structure and the presence of the cholesteatoma made bone conduction threshold increased. The postoperative thresholds of bone conduction were Improved in 36 cases (40.4%) of the 89 cases that underwent operation with type I tympanoplasty technique without electrie drilling. The degree of the maximal improvement was 14.3dB at 2000Hz and the minimum was 8.8dB at 250Hz. but these result did differ from a statistically significant degree. The result suggest that the bone conduction threshold in chronic otitis media is elevated artificially by conductive defect and improved by successful reconstructive surgery.

      • 가와사끼 병 환아에서 이상뇌파와 신경학적 합병증 발현과의 연관성에 대한 고찰

        목적 : 가와사끼 병의 뇌파 소견 및 다른 임상인자들과의 연관성을 알아보고자 하였다. 방법 : 1994년 1월부터 1995년 12월 사이에 순천향대학 천안병원 소아과에 입원한 23명의 신경학적으로 정상인 가와사끼 병 환아에게 뇌파검사를 실시하여 이를 검사시기, 성별, 연령, 혈소판, 간기능 검사치, 그리고 동맥류 유무등의 임상인자들과 비교하였다. 결과 : 이상뇌파가 23명중 52.2%인 12명에게서 관찰되었으며, 이상뇌파로는 예파 및 극파, 극·서파 복합 양상과 전반적인 배경파의 서파양상이 나타났다. 뇌파의 검사시기가 진단부터 1주 이내인 경우에 이상뇌파의 빈도가 그 후보다 의미있게 높았으나, 그 밖의 다른 임상인자와의 연관성은 별 의미가 없었다. 결론 : 가와사끼 병 초기에 이상뇌파가 나타나는 것은 비록 신경적 증상은 없지만 중추 신경계가 영향을 받는다는 간접적인 증거로 생각할 수 있어 향후 신경적 합병증 발생 여부에 대한 면밀한 관찰이 필요하다고 생각된다. Purpose : This study is to find out if there is any association between EEG abnormalities in Kawasaki disease with other clinical factors. Methods : We performed EEG in 23 Kawasaki disease patients without any neurologic sign who admitted in Soonchunhyang Chunan Hospital from 1994. 1. to 1995. 12. and compared it with sex, age, platelet count, liver function test and presence of coronary artery aneurysm. Results : Abnormal EEG findings were observed in 12 of 23 patients(52.2%) and as for abnormal EEG findings, sharp, spike, spike & slow wave complex, and generalized slow background activity were noted. When EEG was performed within a week after the diagnosis, the incidence of abnormalities of EEG increased significantly but there was no correlation with other clinical factors. Conclusion : EEG abnormalities in the early stage of Kawasaki disease indicate that although there is no abnormal neurologic signs clinically, central nervous system might be affected and there should be a close observation for the developement of neurologic complication.

        LED를 이용한 복합레진의 광조사시, 중합수축의 속도와 양, 미세경도에 관한 연구

        This study evaluated the effectiveness of the light emitting diode(LED) units for composite curing. To compare its effectiveness with conventional quartz tungsten halogen (QTH) light curing unit, the microhardness of 2mm composite, Z250, which had been light cured by the LEDs (Ultralume LED2, FreeLight, Developing product D1) or QTH (XL 3000) were compared on the upper and lower surface. One way ANOVA with Tukey and Paired t-test was used at 95% levels of confidence. In addition, the amount of linear polymerization shrinkage was compared between composites which were light cured by QTH or LEDs using a custom-made linimeter in 10s and 60s of light curing, and the amount of linear polymerization shrinkage was compared by one way ANOVA with Tukey. The amount of polymerization shrinkage at 10s was XL3000 > Ultralume 2, 40, 60 > FreeLight, D1 (P<0.05) The amount of polymerization shrinkage at 60s was XL3000 > Ultralume 2, 60 > Ultralume 2, 40 > FreeLight, D1 (P<0.05) The microhardness on the upper and lower surface was as follows; <본문 참조> It was concluded that the LEDs produced lower polymerization shrinkage in 10s and 60s compared with QTH unit. In addition, the microhardness of samples which had been cured with LEDs was lower on the lower surfaces than the upper surfaces whereas there was no difference in QTH cured samples.

        The Working Group on the revision of Clinical Research Methodology for Acupuncture met in Seoul, Republic of Korea from 24 to 26 August 2005. The main objectives of the meeting were to review the regional publication guidelines for Clinical Research on Acupuncture, to make necessary revisions in the guidelines, to make recommendations on further collaboration and activities in the field of research on acupuncture, and to discuss scientific evidence-based approaches in the clinical research on acupuncture. Fourteen members from the eight Member States, one secretariat staff from the WHO Regional Office for the Western Pacific and ten observers from the Republic of Korea attended the meeting. The members presented their papers to review the current status of clinical research on acupuncture. The drafts of proposed revision to the previous guidelines for clinical research methodology on acupuncture were discussed extensively. The issues covered during the discussion included: the definition of the new terms; reorganization of clinical research design; revision and update of contents (e.g., control group); introduction of Institutional Review Board (IRB) on the section of ethical approval; additional outcomes including health-related quality of life (HRQOL), qualitative measures, etc. In the course of these discussions, the working group developed the revised guidelines for clinical research on acupuncture and made recommendations for promoting the dissemination of the revised guidelines.

      • 비강 및 부비동 종양에서의 종양혈관형성에 관한 면역조직화학적 연구

        Background and Objectives : Angiogenesis is essential for the growth of solid tumors. Microvessel density(MVD), which represent a measure of tumor angiogenesis, have been found to have prognostic significance in many tumor types for predicting an increased risk of metastasis. However, the significance of angiogenesis in sinonasal malignant tumor remains controversial. The purpose of this study was to asses the tumor vascularity using the highly specific endothelial cell marker CD34, and the expression of its related protein p53, in sinonasal malignant tumors. Materials and Methods : The study samples were obtained after surgical removal of 27 cases of inverted papilloma(IPs), 5 cases of IPs with malignant transformation(IP-MT), 16 cases of squamous cell carcinoma(SCC) in sinonasal cavity. Microvessels were stained with anti-CD34 and p53 expressions were assessed by immunohistochemical staining. Results : The degree of tumor angiogenesis in squamous cell carcinoma group was higher than in inverted papilloma group, and which reached at highest level in IP-MT(p<05). The p53 expressions were increased as tumor progressed, which was statistically significant also(p<05). Significant correlation between MVD and p53 expressions was observed in IP(p<05) but there were no correlation in IP-MT and SCC. Conclusions : These results suggest that overexpression of p53 and MVD in the sinonasal tumors seemed to be associated with tumorigenesis but further studies on tumor angiogenesis of sinonasal tumors are needed before it can be adopted as a prognostic marker.

      • 흰쥐 腦의 中心扁桃核에서의 Substance P 및 Enkephalin 함유구조의 형태학적 특징

        흰쥐 뇌의 중심편도핵에서의 substance P (SP) 및 Enkephalin (ENK) 함유신경세포 및 축삭종말의 분포를 관찰하기 위하여 peroxidase antiperoxidase법을 행하여 다음의 결과를 얻을 수 있었다. SP함유신경세포는 중심편도핵의 medial zone에 분포하였고 SP함유축삭종말은 lateral capsular zone에서 고밀도로 관찰되었다. SP함유신경세포는 원형 내지는 타원형이었고 2∼3개의 근위수상돌기(proximal dendrite)를 가졌다. ENK함유신경세포 및 축삭종말은 공히 central 및 lateral capsular zone에 존재하였다. ENK함유 신경세포는 직경이 약 17-24㎛ 정도였고 3∼4개의 근위수상돌기를 관찰할 수 있었다. The central amygdaloid nucleus is rich in a number of neuropoptides. The present study examines the distribution and morphological characteristics of substance P (SP) and leucine-enkephalin (ENK) containing neurons in this nucleus. The results obtained were as follows: SP immunoreactive cells were observed within the medial zone and SP immunoreactive axon terminals were heaviest within the lateral capsular zone. The immunoreactive cells were round or oval shape (diameter, 15-20㎛) and had two or three proximal dendrites. ENK immunoreactive cells and axon terminals located within central and capsular zones. The immunoreactive cells were round or oval shape (diameter, 17-24㎛) and had three or four proximal dendrites.

      • 장애인 스포츠권(權)과 법적 필요성에 관한 연구

        This research intend to discuss the necessity so that the disabled can find the right for their sports activity and think of their basic right such as the legal ideas, the reality, the functions and the contents of the physical education and sports, and the necesstiy to build up the laws. And then it come to a conclusion as follows. First, the laws of the sports for the disabled should contain the guarantee of the social approach right and the equal of opportunity to them. They must be a value conception possible when the social environment is improved that the real disabled can make an access freely. And the reality that they can approach should be made through the process for their welfare because it isgenerally shaped on the noraml people. Second, the sporting rights of the disabled should be carried on in a survival place and need to make sports laws in an organizational size to make an harmony with the modern society and have a rational relation with several social institutions. Third, to make normal social life, the disabled should be able to get education first, work, move freely in society and approach to the sports facilities, some information, and tele-communication tools that they can enjoy sports, leasure and pleasure life. The convience facilities should be installed that they can move most shortly in using them and guarantee that they can make use of the public building, facilities and information as equally as the normal people do. Fourth, the legal knowledge to cope with regnition change about the physical education and sports, and the continuous research of their laws should be needed and be developed. From now on, we don't mandate the legal problems of the physical education and sports to the legal profession but we ought to think of and correct the indulgence and ingnorance of the disabled's right for the sports. With the critical and rational insight and knowledge, many studies and activities about the laws of the physical education and sports that can be with the disabled should be made.

        정신분열증 환자에서의 카페인 투여 급성반응

        This study was conducted to investigate the acute effects of caffeine in chronic schizophrenia. Eighteen DSM-Ⅲ diagnosed chronic schizophrenic patients and sixteen healthy subjects were orally administered caffeine, 10㎎/㎏, in a double blind placebo controlled design after being free of caffeine for a minimum of 2 weeks. The effects of caffeine were assessed by employing Brief Psychiatric Rating Scale, Hamiltion Anxiety Scale, and measures of blood pressure, pulse and plasma caffeine level, prior to the challenge and 60 minutes afterwards. The results were as follows : 1) Activation factor(tension, excitement, mannerism and posturing) of BPRS was worsened significantly by caffeine challenge in the schizophrenic patients. 2) Caffeine produced significant increase in somatic anxiet factor score of HAMA in the healthy subjects. 3) There was no significant difference in blood pressure and pulse rate between caffeine and placebo-group except sitting diastolic BP in the healthy subjects, which was higher in experimental group. There findings suggest that caffeine tend to exacerbate symptoms of psychosis but do not increase anxiety in the schizophrenic patients compared with the healthy subjects demonstrated decreased sensitivity to the anxiogenic effects of caffeine.
