        Siriraj Stroke Score 따른 뇌졸중 환자 감별의 적합성

        Background: The differentiation between hemorrhagic(HS) and norhemorrhagic(NHS) stroke is the most important first step in the management of acute stroke because clinical management of the two disorders differs substantially. Neuroimaging studies are useful in diagnosing and distinctioning between HS and NHS. The use of clinical variables, such as Siriraj stroke scores(SSS), has led to good sensitivity, specificity and predictive values. The aim of our study was to evaluate the use of the SSS in the Korean population and assess whether it could aid to expedite treatment decisions. Methods: We reviewed 111 cases of stroke patients admitted to our hospital via the emergency department over a 6 months period from July to December 1998. Levels of consciousness, vomiting, headache, and atheroma markers used in the SSS were applied to these patients who met the criteria for a stroke. Results: Of the 111 patients, the SSS classified 83 with sensitivities of 81.1%(NHS) and 73.3%(HS) and positive predictive values of 84.3% and 68.8%, respectively. The overall accuracy rate was 78.3%. Conclusion: Our results suggest that the SSS is not reliable in distinguishing stroke types in the Korean population. Definite neuroimaging studies are needed prior to thrombolytic therapy.

        Normal feet have the longitudinal arch for the distribution of the weight. The longitudinal arch is known lateral and medial longitudinal arch. Collapsed arch of the foot is flat foot and it is the cause of the foot pain. Of course the low arch does not always make a foot problem in sports and daily activity. Longitudial foot arch is visible in the standing foot lateral radiograph, Harris mat and visual observation. In Radiographic study the lateral Talo-lst Metatarsal angle is known in standing foot lateral film. But the longitadinal medial arch is not correctly obtained in Talo-lst Metatarsal angle. So this study for Calcaneo-lst metatarsal angle in standing lateral foot radiograph is more valuable angle in medial longitadinl arch. And we do the study on difference in the longitadinal arch in normal weight versus overweight person.

      • 한국 진도견의 serum protein 및 lipoprotein 변동에 관한 연구

        The concentration of serum proteins and lipoproteins in Korean Jindo dogs were examined from the time of after birth to 150 days of age. Eight(4 females and 4 males) Jindo dogs were used in this study. Concentration of serum proteins and lipoproteins were fractioned by cellulose acetate electrophoresis. The results are summarized as follows; 1. The total serum protein concentrations at 1 days(4.6±0.4 g/dl) after birth were lower than in adult dogs, thereafter increased rapidly at 3 days(7.1±0.6 g/dl) of ages, and there was no significant changes between sexes. 2. The concentrations of serum protein from the time of after birth to 150 days of age were 0.35±0.1-8.5±0.2 g/dl and 0.5±0.1-0.8±0.2 g/dl, respectively. The γ-globulin concentrations were not detected 1 days after birth and increased rapidly after pups ingested colostrum(after 3 days of age), thereafter there was no significant changes of γ-globulin concentration after 10 days of age. 3. The concentrations of serum α, β-lipoprtein were not detected 1 days after birth and increased rapidly after pups ingested colostrum(at 3 days of age), from the time of after birth to 150 days of age were 49.1±11.6-94.1±16.4 ㎎/dl and 11.7±6.5-102.0±10.8 ㎎/dl, respectively. 4. the γ-lipoprtein concentrations no detected 1 days after birth and increased rapidly after pups ingested colostrum(at 3 days of age, 11.6±2.5 ㎎/dl) and decreased gradually during 50 days of age, and thereafter increased rapidly during 100-150 days of age(71.8±4.8-77.2±4.1 ㎎/dl).

        전기화상 환자에 대한 임상적 고찰

        Background: Good documentation of electrical injuries at the time of presentation is very important to emergency management, so this study was designed to investigate the clinical characteristics and the outcomes of patients with electrical injury. Methods: A review of 75 cases of electrical injuries admitted to our hospital via the emergency department over a 4 year period from 1996 to 1999 was conducted. Results: There were 49 patients with high-voltage injuries and 19 patients with low-voltage injuries. All but 4 patients were males, with a mean age of 29.5 years. The most common type of injury was electrothermal burn. The most common point of entry was hand. The average size of burn wounds was 14.5% TBSA in the high-voltage group and 2.5% in the low-voltage group. Forty-nine(72.1%) of the injuries were work related. The number of patients with compartment syndrome was 19, and fasciotomies were performed in all but one patient. Myoglobinuria was noted in 22 patients, but no patient developed acute renal failure due to myoglobinuria. In the high-voltage group, 10 limb amputations were performed. Complication were observed in 12 patients. The most common complication was neurological injury. The average length of hospital stay was 50.7 days in the high-voltage group and 13.8 days in the low-voltage group. The overall mortality rate was 4.3%. Couclusion: Prevention of electrical injuries is very important. Education and compliance with safety measures, as well as basic knowledge and precaution in dealing with electricity, are essential to avoid these injuries.

      • 농흉과 종격동염이 합병된 viridans streptococcus에 의한 인후농양 1예

        최근 저자들은 viridans streptococcus에 의한 인후농약으로 인하여 발생한 종격동염 및 농흉을 보인 47세 여자 환자를 적절한 배농과 항생제로 치료한 국내 최초의 1예를 치험하였기에 문헌 고찰과 함께 보고하는 바이다. Since the antibiotic era, retropharyngeal abscess became a rare disease entity nowadays. But it may often progress to lethal complications involving vital structure if diagnosis or treatment is delayed. We experienced a case of empyema and mediastinitis complicating retropharyngeal abscess caused by viridans streptococcus in a 47-year-old female patient with diabetes mellitus. She was successfully treated with appropriate antibiotic therapy combined with percutaneous abscess drainage. To our knowledge, such a complication of retropharyngeal abscess has not been reported in Korea yet and we present this unusual case herein followed by a review of the related literature.

      • 전침 자극에 의한 오디 괄약근 운동성 변화에 관한 연구

        Background/Aims: This study was designed to evaluated the effects of electroacupuncture on the sphincter of Oddi (SO) motility in humans and to correlate the manometric findings with plasma cholecystokinin (CCK) levels. Methods: Eleven patients (M: F= 5: 6) who had various kinds of biliary disorders were enrolled. SO motility was monitored with a conventional low-compliant continuously perfused technique using ERCP (n=9) or percutaneous transhepatic cholangioscope (n=2). After baseline monitoring for phasic wave contractions of SO, electroacupuncture was applied at a specific acupoint, GB 34, in these 11 patients. A nonspecific acupoint 5 cm away from GB 34 was selected as a control. Manometric parameters of SO were also checked in 6 subjects during stimulation of the control acupoint. CCK plasma levels were measured during the time course of electroacupuncture stimulation. Result: All the manometric parameters including the basal pressure of SO, amplitude, frequency and duration of phasic wave contraction of SO were significantly decreased (p< 0.05) during electroacupuncture stimulation. The inhibition of SO contractility was accompanied by increased CCK plasma levels. After removal of electroacupuncture stimulation, restoration of amplitude and duration to basal condition were noted. A tendency towards the return of SO contractility was also observed in basal pressure and frequency. Stimulation of the control acupoint, however, did not affect the SO contractility and plasma CCK levels. Conclusions: Electroacupuncture stimulation of acupoint GB 34 showed reversible inhibition of SO contraction in humans. We speculate that the response of SO to electroacupuncture stimulation may be mediated by some neurohormonal mechanisms including CCK release.       • 한글 문서 구조 분석에 관한 연구

        Although information technology has been developed fast, large amount of machine printed document images are not properly handled due to immature document layout analysis techniques. Previous research has focused on character recognition itself to improve character recognition accuracy. Recently, however, some efforts are made to analyze complex document layout automatically. In this paper, we propose document layout analysis and understanding method for various Hangul document images. Skew detection is performed using projection profile and connected component layout information. Top-down and bottom-up methods are combined to analyze document layout. After text region and non-text region determination, table is further processed to extract row, column, and cell position. We got very high performance for skew correction, zone detection, and table processig from experiments with various Hangul document images.

        H9c2 심근 세포주에서 외인성 nitric oxide가 허혈에 의한 세포 독성에 미치는 영향

        Background: Nitric oxide(NO) is known to have protective effects on an ischemic heart and to exert triggering effects on ischemic preconditioning. However, the effects of NO during the ischemic period have not been investigated. To investigate the role of exogenous nitric oxide in a model of ischemic heart cell death, we studied the effects of ischemic preconditioning and ischemia in a normal and an ischemic buffer. Methods: Rat cardiac myoblast cells(H9c2) were cultured in a normal and an ischemic buffered medium. For the ischemic culture of heart cells, the cells were cultured in a dessicator with GasPak for 5 hrs. In ischemic preconditioning, the cells were pretreated with ischemic buffer for 5 min and then perfused with normal medium for 30 min. For the measurement of the cytotoxicity, a MTT(3-4-Sdimethylthiazol-2-yl-2,5-diphenyl tetrazolium bromide) assay was performed. A DAPI(4',6-diamidino-2-phenylindole dihydrochloride) staining procedure and a flow cytometry analysis were performed to confirm apoptotic cell death by ischemia. Results: Cell viability, as determined by using a MTT assay, showed that the preconditioned group treated with NO showed more cell death than with the not-preconditioned groups in both normal and ischemic buffers. But, In normal medium and not-preconditioned groups, NO showed protective effect according to the concentrations(100,1000μM) . No treatment with NO produced the different results. In normal medium, the protective effect of ischemic preconditioning was demonstrated, but no protective effect of ischemic preconditioning could be seen in the case of the ischemic buffer. The DAPI staining and flow cytometry analysis of heart cells showed characteristic apoptotic features. Conclusion: NO added in the ischemic phase had deterious effects on heart cells. Ischemic preconditioning was more harmful than ischemia alone. The toxicity of the cells was characteristic apoptosis.

        In the course of Internet proliferation, many World Wide Web-related technologies are examined for possible growth and evolution. The use of World Wide Web-based technologies has further fuelled the demand for World Wide Web-based applications. In this paper, We describe the design and implementation of image marking system that help user manipulate their own image. The user can edit, print and merge their own image with other offered image using World Wide Web. Also the user don't need to buy a required software at a high price but only use World Wide Web that offer the service.
