        공동주택내 이산화질소(NO2) 및 휘발성유기화합물(VOCs) 노출에 따른 건강 위해성 평가

        Indoor air quality has become a topic of interest and concern. Especially changes in construction design and the increased use of synthetic products may result in an increasing of complaints and health effects about the quality of indoor air at home. In this study, nitrogen dioxide(NO2) and volatile organic compounds(VOCs) within new and established apartments on the basis of 4 years of building year were measured every 3 days consecutively during 60 days. We selected each 10 house in Seoul, Asan and Daegu, respectively, and produced risk numbers for hazard quotients, and predicted increases in incidence of cancer. The calculations were made for the adult with default exposure values and also made for a worst case scenario using Monte-Carlo simulation as describing the reasonable exposure(RME). Mean of Monte carlo analysis by benzene, in the construction under 4 years (male: 9.2105, female: 1.0×104) and over 4years (male: 6.8×105, female: 8.3×105) exceeded 106 of permitted standards in US EPA, RME of Monte carlo analysis. In construction under 4 years (male: 9.9×103, female: 9.6×103) and over 4years (male: 9.8×103, female: 7.8×103) exceeded 104 of maximum permitted standards in US EPA. The hazard index of non-carcinogenic pollutants by nitrogen dioxide, toluene, m,p-xylene and o-xylene, both male and female in apartment constructed under 4 years and over 4 years was found less than the permitted standards of hazardous health effects in CTE. Significant cancer risks and non-cancer hazard quotients were predicted in under 4 years of building year.

        8년간(1992~1999) 산업안전보건연구원에 의뢰된 직업병 심의 사례 분석

        Objectives : Pneumoconisis and noise-induced hearing loss (NIHL) have been reported as main occupational diseases by the Special Health Examination. The Industrial Accident Compensation Insurance has reported various work-related diseases, however, these two diseases occupied almost a half of compensated cases. Therefore, it was not well known about the status of occupational diseases other than pneumoconiosis, NIHL, and cardio-cerebrovascular accident (CVA). This study was conducted to analyze claimed cases as an occupational disease, that was requested to the Korea Industrial Safety and Health Agency (KOSHA). Methods : The local office of the Korea Labor Welfare Corporation (KLWC) has asked the KOSHA for confirmation of claimed cases as an occupational disease. We analyzed 379 cases requested from KLWC, the Ministry of Labor, employers, unions and occupational health agencies from 1992 to 1999. Results : Male was 80.7 % of the requested cases. Their mean age was 42 years old and 75.5 % of them were more than 35 years old. The requested cases were increased rapidly from 25 cases in 1992 to 108 cases in 1999 and the accept rate was 50.7 %. The majority of requested cases were respiratory diseases(22.4%), cancers(18.5%), Neuropsychiatric problems(14.5%), and musculoskeletal problems(13.5%). The accept rate was high in reproductive, respiratory, musculoskeletal and digestive disorders and low in neuropsychiatric, renal and otologic problems and occupational cancers. 73.6% of them were caused by chemical agents, especially 28.5 % were by organic solvents. 67% of them were clinically confirmed at university hospitals. A half of the cases were from KyongIn area, even the request came from the whole country. Conclusions : A claim was common in workers whose age was over 35 years old and exposure history was over 10 years. The respiratory diseases and neuropsychiatric disorders were still main problems in occupational health and occupational cancers was increasing even though its accept rate was not high yet.

        The occurrence of a mixed tumor containing papillary thyroid carcinoma (PTC) and primary squamous-cell carcinoma (SCC) is rare because there is no squamous epithelium in the thyroid gland. Reported herein is a 30-year-old female with mixed PTC and primary SCC of the thyroid presented as thyroid incidentaloma. Fine-needle aspiration biopsy of the thyroid nodule revealed the presence of malignant thyroid cells. The histopathological examination following total thyroidectomy yielded two mixed, morphologically distinct histotypes that included PTC and SCC. After total thyroidectomy, the patient underwent radioactive iodine therapy. No recurrence or metastasis occurred during the 20-month follow-up period after the operation.

        Synergistic Renoprotective Effect of Melatonin and Zileuton by Inhibition of Ferroptosis via the AKT/mTOR/NRF2 Signaling in Kidney Injury and Fibrosis

        According to recent evidence, ferroptosis is a major cell death mechanism in the pathogenesis of kidney injury and fibrosis. Despite the renoprotective effects of classical ferroptosis inhibitors, therapeutic approaches targeting kidney ferroptosis remain limited. In this study, we assessed the renoprotective effects of melatonin and zileuton as a novel therapeutic strategy against ferroptosis-mediated kidney injury and fibrosis. First, we identified RSL3-induced ferroptosis in renal tubular epithelial HK-2 and HKC-8 cells. Lipid peroxidation and cell death induced by RSL3 were synergistically mitigated by the combination of melatonin and zileuton. Combination treatment significantly downregulated the expression of ferroptosis-associated proteins, 4-HNE and HO-1, and upregulated the expression of GPX4. The expression levels of p-AKT and p-mTOR also increased, in addition to that of NRF2 in renal tubular epithelial cells. When melatonin (20 mg/kg) and zileuton (20 mg/kg) were administered to a unilateral ureteral obstruction (UUO) mouse model, the combination significantly reduced tubular injury and fibrosis by decreasing the expression of profibrotic markers, such as α-SMA and fibronectin. More importantly, the combination ameliorated the increase in 4-HNE levels and decreased GPX4 expression in UUO mice. Overall, the combination of melatonin and zileuton was found to effectively ameliorate ferroptosis-related kidney injury by upregulating the AKT/mTOR/ NRF2 signaling pathway, suggesting a promising therapeutic strategy for protection against ferroptosis-mediated kidney injury and fibrosis.

        Hemorrhagic fever with renal syndrome (HFRS) frequently cause abdominal pain with tenderness; therefore this is occasionally misdiagnosed as surgical abdomen. A 20-year-old male presented with abdominal pain for 2 days. He also had suffered from fever since 7 days ago. Abdominal computed tomography showed ascites along both paracolic gutters, mesentery and paravesical spaces. Hantaan virus infection was demonstrated by serology. After conservative treatment, the patient with HFRS spontaneously improved. We report a case of HFRS presented acute focal abdominal pain of acute abdomen.

      • 혈청 Cystatin C와 크레아티닌에 근거한 사구체 여과율의 비교

        Background : In recent years, cystatin C (CysC) was proposed as a new marker for the glomerular filtration rate (GFR) due to a constant serum level. The present study analyzes whether creatinine based GFR prediction equations might be replaced by simple prediction equations based on serum concentrations of CysC in healthy young Korean men. Methods : We studied 145 apparently healthy young Korean men aged 19-29 years who visited The Armed Forces Yangju Hospital (Gyeonggi Province, Repulic of Korea) for health check from November 2008 to January 2009. Creatinine-based GFR was estimated by the formula of modified MDRD, Crockcroft Gault (CG). CysC-based GFR was estimated by the formula of Larsson, Hoek, Filler, Grubb, Le Bricon, Orebro-cyst (DAKO), Orebro-cyst (Gentian). Linear regression analysis was performed to evaluated correlations between GFR estimating formulae based upon serum CysC and serum creatinine. Results : Among CysC based GFR, Hoek's GFR is most similar with CG GFR and abbreviated MDRD GFR. Significant correlations were noted between CysC based GFR and creatinine based GFR. Conclusion : Creatinine based GFR prediction equations for healthy young Korean men might be replaced by simple prediction equations based on serum concentrations of CysC (especially Hoek' equation). 배경 : 혈청 cystatin C는 외부 요인에 영향을 받지 않고 일정한 값을 유지하므로 사구체 여과율에 대한 새로운 표지자로 제시되고 있다. 본 연구는 젊은 성인 남성에서 cystatin C에 근거한 사구체 여과율이 혈청 크레아티닌에 의한 사구체 여과율을 대체할 수 있는지 조사하였다. 방법 : 2008년11월부터 2009년1월까지 병원을 방문한 평균 연령 21세의 젊은 성인남성 145명을 대상으로 하였다. Cystatin C에 근거한 사구체 여과율은 Larsson, Hoek, Filler, Grubb, Le Bricon, Orebro-cyst (DAKO), Orebro-cyst (Gentian) 공식을 이용하였고 혈청 크레아티닌에 근거한 사구체 여과율은 변형된 MDRD, Crockcroft Gault 공식을 이용하여 산출하였다. Cystatin C에 근거한 사구체 여과율과 혈청 크레아티닌에 근거한 사구체 여과율은 선형 회귀분석을 통해서 연관 관계를 확인하였다 결과 : Cystatin C에 근거한 사구체 여과율 산출 공식중에서 Hoek 공식에 의한 사구체 여과율이 혈청 크레아티닌에 근거한 사구체 여과율과 의미있는 상관관계를 보였다. 결론 : 젊은 성인 남성에서 Cystatin C에 근거한 사구체 여과율은 혈청 크레아티닌에 의한 사구체 여과율을 대체할 수 있을 것으로 생각된다.

      • 수종 Laminate Veneer용 도재간의 색차 비교

        The shade of prosthetic materials is very important for a successful restoration esthetically harmonized with the natural tooth. the purposes of this study were to evaluate the color differences that can exist between different brands of procelain laminate veneer systems with identical shade designations and to evaluate the color variations after luting with resin cement. For this purpose, the following experiments were done:4 kinds of porcelain laminate veneer specimens were fired to the same thickness and a resin cement of the same shade was luted to each porcelain laminate veneer specimen. Afterwards, the color of each specimen was determined and analysed by the colorimeter. The results were as follows: Corresponding shades of different brands of porcelain laminate veneer systems produced significantly different colors(p<.05). Color variations were found after luting with resin cement to the porcelain laminate veneer specimens(p<.05).

      • 제올라이트(NaA형, NaX형)의 이온교환성에 관한 연구

        A study has been made of the exchange of Ag^+, Li^+, and Ba^++, Ca^++, Sr^++ ion by using synthetic zeolite-13X and zeolite-NaA. The zeolite used in this work was supplied as beads by Aldrich Chemical Company, Inc., and the formula of unit cell is M_x/n[AlO₂)_x(SiO₂)_y]zH₂O. X-ray diffraction analysis and Fourier transform infrared spectroscopy indicated that the crystallinity of zeolites was retained though sodium ion in zeolite exchanged with the cation. The amount of ion exchanged was measured by the determining of the amount of cation remained in solution by Inductively coupled plasma, and surface area, adsorption isotherm, Ion exchange kinetics. The selectivity of ions was in the order Ag^+>Li^+>Sr^++>Ca^++>Ba^++.

        농업용수 수질기준의 문제점 및 개선대책 : 외국사례와의 비교를 통해

        우리나라 농업용수 수질기준과 다른 국가 또는 농업관련 국제기구의 수질기준에 대한 비교분석을 통해 우리나라 농업용수 수질기준의 문제점 도출과 해결방안을 모색코자 본 연구를 수행하게 되었으며, 그 결과를 요약하면 다음과 같다. 1. 우리나라 농업용수 수질기준은 관개용수 수질기준이 아니고, 수원보호측면의 수질기준이라 할 수 있다. 2. 우리나라 수질기준중 유기물지표가 하천과 호소에 따라 BOD와 COD로 이원화되어 있고, 특히 COD의 경우 산화제로 KMnO4를 사용함으로써 효과적인 유기물지표로 활용할 수 없는 실정이다. 농업용수로의 활용측면에서는 K₂Cr₂O_(7)을 이용한 COD를 기준으로 도입하는 것이 유기물 분해능 및 수중의 유기물지표로 활용하고 있는 국제적 데이터 일치를 위해서도 타당하다. 3. 우리나라 농업용수 수질기준은 캐나다와 FAO에서 제시하고 있는 염류에 대한 기준은 거의 없는 실정이다. 또한 양이온에 대한 기준도 없다. 염류의 기준이 모든 이온 존재의 가능성을 의미하므로, 실험치 및 외국문헌에 의한 1.0dS/m수준을 한계농도로 선정해야 한다. 4. FAO와 캐나다에서 사용하고 있는 축산용수에 대한 수질기준이 없다. 5. 관개용 농업용수가 농업(작물)에 미칠 수 있는 영향을 평가하기 위해서는 수원별로 수질기준이 다른 것이 큰 모순이다. 따라서 관계부처에서는 부처간의 이해를 떠난 수질기준을 설정해야 한다. 6. 우리나라 농업용수 수질기준의 모순점을 극복하기 위해서는 농업의 안전성확보측면 위해 관개용수 수질기준과 축산용수 수질기준을 조속히 제정해야 할 것이다. Present problems and possible solutions on the Water Quality Standards for Agriculture in Korea, were identified in the study. Because the Ministry of Environment made the laws and standards without consulting the agri-scientists, among others, the water quality standards formulated were not suitable standards of FAO, Japan, Canada, and the likes. The BOD and COD are the organic materials indicators. In case of rivers, BOD are set for the indicator. And at reservoirs. COD are set. Especially. COD was analysed using KMnO₄It is not suitable oxidizing agent for COD. Because the oxidic capacity is very low. So, in most developed country, K₂Cr₂O_(7) is usually used to analyze COD. And the salts and cations are not belonged to the Korean Water Quality Standards for Agriculture. The water quality standards for agriculture have big differences among lakes, rivers and groundwater. To apply different standards by resources to evaluate the relationship between water and crops is conflicting. So, to overcome the these problems, the suitable water quality standards for irrigation and livestock should be set.
