      • Enhanced Maturation and Function of Dendritic Cells Using Hydrogel Coated Plate and Antigen Electroporation

        <P>Dendritic cells (DCs) are potent antigen-presenting cells that can be matured <I>in vitro</I> from immature dendritic cells (iDCs) in the presence of several biological agents such as cytokine cocktail, CD40L, TNF-a and antigen loading, which are necessary and achieved using various protocols, such as lipofection, passive pulse or electroporation. However, these DCs maturation protocols may cause with a significant loss of cells because of cellular attachment and spreading during culturing. Some biomaterials that influence adhesion and development of cells have been used in cell culture techniques, and it was thought that they might be applied on the culture of DCs. In this study, we used polyHEMA, which is a hydrogel coating biomaterial that prevents DCs from adherence, and investigated whether hydrogel coating affects the maturation of iDCs. The efficiency in the generation of mDCs was improved through hydrogel coating procedure and a dendritic cell maturation marker, CD83, was significantly increased in hydrogel-coated culture condition. The antigen-loaded mDCs from electroporation were further expressed the CD83. The mDCs generated in the hydrogel-coated culture condition showed more, longer and thicker dendrites, and produced more amounts of cytokines such as IL-12 and IFN-γ. Therefore, it was suggested that the hydrogel-coated culture condition could improve function of mDCs.<fn><P>Cheol-Hun Son and Jae-Ho Bae contributed equally to this work.</P></fn></P>

        국내 혈액질환 환자에서 침습성 아스페르길루스증의 구제치료로서 Voriconazole의 효과와 안전성

        Background: Invasive aspergillosis (IA) is associated with significant morbidity and mortality in patients with hematologic malignancies. We investigated the efficacy and safety of voriconazole (VCZ) when used as salvage therapy for IA in Korean adults with hematologic malignancies who had not responded to prior antifungal therapy. Materials and Methods: We retrospectively reviewed data, collected from January 2007 to October 2008, from patients with proven or probable cases of IA. All were probable IA cases, except for one proven case. All cases were refractory or intolerant to antifungal therapy prior to administration of VCZ. Efficacy and safety were assessed in patients treated with VCZ for more than 3 days and for more than one dose, respectively. A favorable response [complete (CR) or partial (PR)] was defined by significant improvement of all clinical symptoms, signs, and radiologic abnormalities. Results: Fifty patients who met the inclusion criteria were enrolled. There were 27 male and 23 female patients with mean age of 44.4 years (range, 15-65 years). Underlying diseases were acute leukemia (35 cases), chronic myelogenous leukemia (4 cases), myelodysplastic syndrome (3 cases), lymphoma (3 cases) and other hematologic diseases (5 cases). Twenty-two patients had received chemotherapy and 13 patients had undergone hematopoietic stem cell transplantation. The lung was the main infection site (94%) followed by the sinus (6%). Amphotericin B deoxycholate alone was the most frequent previous antifungal therapy. The mean duration of antifungal therapy prior to VCZ therapy was 13.9±8.8 days (2-44 days). The median duration of VCZ therapy was 19 days (interquartile range, 49 days). Sixteen patients (32.0%) showed favorable responses (CR:PR=8:8) at the end of VCZ therapy. The numbers of patients with stable disease, progression and death were, 6 (12%), 6 (12%) and 22 (44%) respectively. Most of those with unfavorable responses had relapsed underlying malignancies or refractory graft versus host diseases. Twelve patients developed drug-related adverse events but only one patient stopped VCZ treatment prematurely. Conclusions: VCZ demonstrated an acceptable level of toxicity in patients with hematologic malignancies but further studies are required to prove its efficacy as salvage therapy.

      • 렙토스피라증의 진단에 있어 수동혈구응집반응법과 육안응집반응법의 정확도에 관한 연구

        Before 1984, Leptospirosis had been reported sporadically in middle inland of Korea as hemorrhagic pneumonia like disease, epidemic pulmonary hemorrhagic fever or epidemic pneumonitis since 1975. The epidemiologic characteristics of this disease were found out and the agent was isolated microbiologically after a large epidemic in 1984. Thereafter many consecutive studies for leptospirosis had been performed in the realm of epidemiology, microbiology and clinical field. But in order to appropriate control of Leptospirosis there are many problems such as organizing surveillance system for early diagnosis and treatment, identifying sero-group and serovar in each epidemic area and assessing vaccine efficacy. The objectives of this study was to find out easily applicable diagnostic methods to be used in a rural community surveillance system. Two diagnostic methods, passive hemagglutination(PHA) test and macroscopic agglutination test(maAT) were measured against their accuracies, sensitivity, specificity and predictive value of positive and negative. 301 sample sera from 270 human cases was collected through Leptospirossis surveillance system in a rural community during 1989-1992. Microscopic agglutination test(MAT) were used for confirmed diagnosis according to WHO guideline with 4 strains; Icterohemorrhagiae lai and Canicola canicola Hond Utrecht IV as standard strians and 6P-014-1 and YP-35-1 as strains isolated in Korea in 1986. The results are as follows. 1. 75(24.9%) among 301 sample sera and 49(18.1%) among 270 persons was serologically positive with MAT. 2. Agreement between PHA and MaAT was very low(k-coefficient≤0.252). 3. In PHA, specificity was very high(≥0.96) but sensitivity was very low(≤0.28). 4. In MaAT, specificity(≥0.73) was relatively lower than PHA but sensitivity (≥0.79) was significantly higher than PHA, especially false-positive rate case was absent in MAT's high antibody titer(above 1:640). It was concluded that MaAT method was a very useful diagnostic tool for early diagnosis of Leptospirosis in a rural community surveillance system, provided that confirmed diagnosis with MAT method be carried out in laboratory of medical school or research institute such as National Institute of Health or Provicial government Institute of Health and Environment.

        인천 인현동 호프집 화재 피해자 분석

        Purpose : This study was conducted to develop field triage, transportation, distribution, and prehospital care at a fire disaster by analyzing the victims of the fore that broke out at a bar in Incheon. Method : We analyzed the cases of the victims of a fire in Incheon in Oct. 1999. We determined the primary care hospital, the arrival time, the burn size, the outcome, and the injury type from the medical records, the concerned organ records, and interviews with concerned persons. Result : The total number of victims was 137: 56 prehospital deaths, 1 hospital death, and 80 survivals. The Pearson correlation coefficient between the burn size and the severity was -0.175. There were 121(89.6%) cases of inhalation injury, 59 (43.7%) cases of flame burns, 66 (48.9%) cases of hypoxic brain damage, and 16 (11.9%) cases involving other types of injury. Conclusion : The causes of death of the fire victims were inhalation injury and hypoxic brain damage due to CO poisoning and other toxic inhalants. We propose the use of a simple triage and rapid treatment(START) system and a reassessment the delayed category in fire disasters.

      • 중추에서 Prostaglandin계가 Renin-angiotensin System에 미치는 영향

        Role of prostaglandins on the renin-angiotensin system in the central nervous system was examined in normotensive and 2-kidney, 1 clip (2K1C) hypertensive rats. The experiment was done under thiopental (50 ㎎/㎏, IP) anesthesia. Captopril and indomethacin were injected into the right lateral cerebral ventricle. Arterial blood pressure and heart rate were recorded from the femoral artery. Intracerebroventricular (ICV) captopril (1 ㎎) caused a decrease in mean arterial pressure in both normotensive and 2K1C hypertensive rats. The depressor response to captopril was more sensitive in hyper-tensive rats than in normotensive rats. Indomethacin treatment (ICV, 200 ㎎) altered the depressor response to captopril neither in normotensive nor in hypertensive rats. These results suggest that the cardiovascular effect of renin-angiotensin system in the central nervous system may not be mediated via prostaglandin systems in normotensive and 2KlC hypertensive rats.

      • 오렌지 껍질로부터 분리된 프라보노이드의 화학요법 감작효과

        Background and Objectives : The overexpression of P-glycoprotein (Pgp) or multidrug resistanceassociated protein (MRP) confers multidrug resistance (MDR) to cancer cells. MDR cells could be sensitized to anticancer drugs when treated concomitantly with a chemosensitizer. In this study, orange peel have been screened for the development of chemosensitizers reversing MDR. Materials and methods : Expression of Pgp in AML-2/WT and AML-2/D100 were determined using western blot analysis and RT-PCR. Candidates for chemosensitizers were isolated, purified from orange peel using sephadex LH-20 column chromatography and silica-gel column chromatography, whose structure was determined using ^1H-NMR, ^13C-NMR, ^1H-^1H-COSY, 13C-NMR, EI-Mass, IR. Chemosensitizing effect of NP-28 on AML-2/DX100 over expressing Pgp and MRP was determined by the MTT assay. Membrane toxicity was evaluated by the RBC hemolysis test. Antioxidant effects were examined by using a fluorescence probe (2’-7’-dichlorofluorescein diacetate). Results : A potent candidate, NP-28, has a flavonoid nucleus with some methoxy moiety. NP-28 is believed to have high therapeutic index, being non-transportable inhibitor and possibly, of inhibiting other efflux pumps such as MRP and antioxidant effects. NP-28 is also unlikely to be a substrate of Pgp. NP-28 did not increase Pgp levels. Conclusion : NP-28 has a Chemosensitizing effect to reverse Pgp-mediated MDR by increasing the intracellular accumulation of drugs. Furthermore, they do not increase expression of Pgp, which can allow a long term use without the concern about Pgp activation. NP-28 is anticipated as an ideal second-generation flavonoid chemosensitizer and/or possibly other therapeutic agents by further investigation.

      • 열화콘크리트 폐기물의 재생시멘트 활용방안에 관한 기초적 연구

        The purpose of this study is development of technique to use cementitious powder as recycle cement produced from deteriorated Concrete waste which has a large quantity of calcium carbonate. Therefore, after having theoretical consideration based on the properties of high-heated concrete and concerning about neutralization of Concrete, we analysis chemical properties of ingredients of cementitious powder. After making origin cement paste, then processing the accelarated carbonation, we consider the properties of hydration and chemical properties of cementitious powder under various temperature conditions. As a result of the thermal analysis, the CaCO₃content of cementitious powder would affect decision of heat temperature to recover its hydrated ability because CaCO₃content is increased when neutralization is progressed. And as a result of XRD analysis, in case of origin powder of non-neutralized paste, CaO peak is found at 700℃. but, heat temperature to generate CaO would increase when the content of neutralized ingredients is increased. Finally, recycle cement heated at 700℃ shows the best compressive strength when the content of neutralized ingredients in recycle cement is less then 50%. However, it would be quite difficult to manage quality of recycle cement according to recycling points of various concrete waste.

      • 다중 특징의 반복 분석에 의한 퍼지 분류기

        A fuzzy classifier which needs various analyses of features using genetic algorithms is proposed. The fuzzy classifier has a simple structure, which contains a classification part based on fuzzy logic theory and a rule generation part using genetic algorithms. The rule generation part determines optimal fuzzy membership functions and inclusion or exclusion of each feature in fuzzy classification rules. We analyzed recognition rate of a specific object, then added finer features repetitively if necessary to the object which has large misclassification rate. And we introduce repetitive analyses method for the minimum size of strings and population, and for the improvement of recognition rates. This classifier is applied to three examples of the classification of iris data, the discrimination of thyroid gland cancer cells and the recognition of confusing handwritten and printed numerals, each sample numeral is classified into one of the groups which are diveded according to the sample structure. The fuzzy classifier proposed in this paper has recognition rates of 98.67% for iris data, 98.25% for thyroid gland cancer cells and 96.3% for confusing handwritten and printed numerals.

        Bipolar release와 Z-Plasty를 이용한 선천적 사경증의 치험례

        Congenital muscular torticollis(CMT) is a disorder characterized by shortening of at least one of the cervical muscles and tilting of the head to opposite side. The most commonly affected muscle is the sternocleidomastoid muscle. Pathogenesis and etiology of congenital muscular torticollis were not clearly identified, but considered as fetal malposition, birth trauma, vascular accident, heredity, infection and CNS pathology. Untreated congenital muscular torticollis often causes facial asymmetry and This is the rasult of tensional rotation of the face toward affected side. So early treatment may prevent facial and neck asymmetry and limitation of neck movement. There are many treatment methods in CMT, including conservative and operative method, but presently Bipolar release and Z-Plasty of SCM muscle has been introduced when the conservative treatment had failed. The benefits of this method are to preservation of the normal Neck V-contour and improvement of the neck motion. We treated CMT using Bipolar release and Z-plasty in two patients. After that the patients improved on the range of neck motion and maintained the normal V-conture of the neck, so we report two cases of CMT with literatures.
