        골수염으로 발현된 재발성 콕시디오이데스 진균증 1예

        Coccidioidomycosis is a fungal infection that results from inhaling the airborne arthroconidia of the Coccidioides species. It is an endemic disease in the southwest part of North America and rarely diagnosed in Korea. As tourism to endemic areas and the number of immunocompromised patients have been increasing, the incidence of this infection has increased in non-endemic areas. Treatment is usually successful with antifungal agents; however, recurrence is common. It is difficult to decide when to discontinue the antifungal treatment especially in non-endemic areas where doctors are not familiar with the disease. We report a case of recurrent coccidioidomycosis manifesting as osteomyelitis after the treatment of the patient for disseminated coccidioidal infection. The complement fixation test was a useful tool for the assessment of patient response and to evaluate suspected recurrence.

        디지털 감수성과 대학의 고전문학 교육 : 메타버스를 활용한 <고전문학과 영상>(DT)를 중심으로

        이 글은 변화하는 디지털 환경을 고려하여 고전문학 수업을 재설계하고 진행한, 교육 과정에 대한 보고서이다. 주요 개념으로 ‘디지털 감수성’ 을 설정하고, 일차적으로 그 의미를 ‘디지털 환경에 적응하고 그 변화에 반응하는 정도’라고 파악했다. 구체적으로 수업에서는 제페토와 게더타운을 활용해 영상물을 제작하고 전시회를 구성했다. 그 과정에서 학생들은 고전문학의 ‘담당층’으로 참여하는 ‘당사자적 친밀감’을 경험하고, 고전의 경계를 유연화하여 상호연대가 가능한 ‘월드’ 구축을 상상할 수 있었다. 메타버스를 활용한 고전문학 교육을 통해, 디지털 감수성이 ‘디지털 환경에 적응하는 정도’를 넘어, ‘디지털 환경에서 당사자성을 공유한 친밀감을 체험하고, 그것을 시간적 공간적으로 확장시켜 유연-경계의 연대가 가능한 월드를 창조할 수 있는 상호작용 감각’임을 끌어낼 수 있었다. 그것은 기술을 초월[meta]한 새로운 관계의 우주[universe]를 구성하는 역량으로 서, 미래 세대의 학생들이 과거의 고전과 관계를 맺는 주요 방식이자 고전교육의 정향에 주요한 지남이 될 것이다. This article is a report on the curriculum that redesigned and conducted classical literature classes in consideration of the changing digital environment. “Digital sensibility” was set as the main concept, and the meaning was first understood as “the degree to which one adapts to the digital environment and responds to changes.” Specifically, in the class, using ZEPETO and Gather Town, we made a video and organized an exhibition. In the process, students were able to participate as a “responsible person” of classical literature, experience “inter-personal intimacy”, soften the boundaries of classics, and imagine building a “world” in which mutual solidarity is possible. Through classical literature education using metaverse, digital sensibility goes beyond “adaptation to the digital environment,” to “experiencing the intimacy of shared identity in the digital environment,” and expands it temporally and spatially. It was possible to derive a sense of interaction that can create “a world where solidarity is possible.” It is the capacity to construct a new universe of relationships that transcends technology [meta], the main way future generations of students relate to the classics of the past, and will become a major criterion for setting the direction of education of the classic.

      • PECVD로 제작한 수소화된 비정질 탄소(a-C:H) 박막의 화학결합 구조에 관한 연구

        Hydrogenated amorphous carbon(a-C:H) films were prepared by PECVD(Plasma enhanced-chemical vapor deposition) method. The films were characterized using various spectroscopic measurements such as Raman scattering, FTIR(Fourier transform infrared) absorption spectroscopy. FRIR absorption reflects only the optically active hydrogen in the a-C:H and, using it together with Raman spectroscopy. The spectral analysis indicated that changes in Raman spectroscopy intensity occurring with increasing substrate temperature 50℃~300℃ were determined to be caused by changes in optically resonant components in the films. Furthermore, a significant correlation was the two crystalline forms of carbon-graphite and -diamond consist of sp² and sp³ bonds, respectively. Significant changes in chemical bonding(sp²/sp³) and micro-structure of hydrogenated amorphous carbon thin films occur as the substrate temperature varies from 50 to 300℃ during.

      • 원격 와이핑 작업을 위한 제어 기법

        This study proposes a technique utilizing virtual information instead of real information in a teleoperated wiping operation. A teleoperation system composed of a master manipulator, a slave manipulator, and a real-time control system is used for this study. Both the master and slave manipulator have a kinematic structure effectively conveying and following the operator’s command respectively. The master manipulator uses DC servo motors to do a major role for counter balancing. A force sensor attached at the end of the slave manipulator’s end effector is used to measure a contact force. MATLAB/xPC Target is used for real-time control of the teleoperation system in Windows OS. This study proposes algorithms for generating the virtual force and the virtual velocity. They are generated as the master manipulator’s grip moves up and down, and back and forth. A force-force control technique proposing in this paper uses the virtual force as a reference input for the contact force control, and uses the virtual velocity as a reference input for control the slave manipulator’s horizontal movement. A PID controller is used as a local controller for the contact force control. In order to show the effectiveness of the proposing force-force control technique, experiments are performed on a plastic plate with two bumps.

      • 로드 시뮬레이터를 이용한 대형트럭의 승차성능 평가에 관한 연구

        This paper proposes a scheme to assess a heavy truck's ride performance in an objective way using road simulator test results. The road simulator has six posters. They are excited in in-phase mode or out-of-phase mode to produce bounce, pitch or roll motion independently. In the road simulator test with a real vehicle, 8-accelerometer, 4-displacement sensor, and one gyro system are used for measuring the vehicle's vertical acceleration, vertical displacement, pitch angle and roll angle. The accelerometers are attached at each axle, left and right frame, and driver seat floor of the cab. They are used to monitor the vehicle's bouncing motion. The displacement sensors are attached at the center of the first and last cross member of the frame, and left and right of the deck. They are used to monitor pitch and roll motion of the frame. The gyro system is attached at the mass center of the cab. It is used to monitor the cab's pitch and roll motion. The real vehicle's road simulator test results are processed to identify the natural frequencies of bounce, pitch and roll mode with respect to a driver. The identified natural frequencies are compared with ride performance criteria defined by ISO2361 to assess the vehicle's ride performance.

        재식된 상악 중절치의 장기간에 걸친 추적례 : A 8-YEAR FOLLOW UP

        완전 탈구된 치아의 치료 목적은 치주인대 세포의 손상과 치수 조직의 감염에 의한 염증 반응을 예방하거나 최소화하는 것이다. 치주인대 세포의 생활력은 완전 탈구된 치아의 예후에 있어 중요한 요소인데 치주인대 세포가 생활력을 유지하는 경우 재식시 재부착을 하게 되어 최소한의 염증반응 후 신생 백악질로 회복되지만, 생활력을 상실하게 되면 심한 염증반응을 야기하게 되어 광범위한 치근 흡수와 유착이 나타나게 된다. 치근이 유착되면 특히 성장하는 어린이일 경우 치조돌기의 성장을 방해하여 저위교합을 유발하게 된다. 또한 광범위한 치근 흡수나 유착으로 인한 치아의 조기 상실은 치조골의 흡수와 인접치의 지속적인 이동을 야기하여 기능적, 심미적인 문제를 발생케 한다. 본 증례는 상악 우측 중절치의 완전 탈구 후 재식한 후 항생제 투여 및 장기간의 수산화 칼슘 적용을 통해 9년에 걸쳐 기능적, 심미적으로 양호한 결과를 얻었기에 이에 보고하는 바이다. Tooth avulsion usually causes inflammatory root resorption and ankylosis, and ankylosis cause severe functional and esthetic problems, especially in childhood. A 7-year-old female visited the Dept. of Pediatric Dentistry, Yonsei University with the chief complaint of avulsive trauma to the upper right incisor which was left dry for 40 minutes. Tooth was irrigated with saline and replanted immediately and splinted. Anti bacterial agent and anti inflammatory agent were prescribed. After 4 months of replantation slight external root resorption and apical radiolucency was seen at radiographic examination, therfore pulp extirpation and calcium hydroxide(Vitapex^(TM)) canal filling were carried out. After 16 months, root canal was filled with gutta-percha, and bleaching treatment was done. Treatment results were satisfactory both esthetically and functionally for 8 years and 5 months.

      • 원격조작에 의한 작업대상물 강성과 마찰 특성의 동정

        This study performs a workpiece stiffness and frictional characteristic identification experiments by teleoperation. Experiments are performed using a bilateral force feedback teleoperation system. It is composed of a master manipulator, the slave manipulator equipped with a force/torque sensor, and a real-time control system. To control the teleoperation system, a position-force controller is developed. The slave manipulator is controlled by the master manipulator's position command and the master manipulator is controlled by the force information from the slave manipulator. This study presents a method to find the slave manipulator's stiffness. And, this study presents a method to compute the workpiece stiffness based on the measured pressing force and deflection at the slave manipulator. Also, this study performs a workpiece frictional characteristic identification experiments. In the experiments, the slave manipulator rubs a workpiece with an arbitrary pressing force. While the slave manipulator rubs a workpiece, a vertical force and a frictional force are measured and the ratio of a frictional force to a vertical force is considered as a frictional characteristic.

        복수과 진료 환자의 중복 처방에 대한 분석 및 평가

        In an aging society, there is an increasing possibility of the duplication of the drugs given to patients because they take many kinds of drugs. Among the prescriptions given to the patients who was treated at multiple ambulatory clinics in St. Mary's hospital for one month of September, 2002, in which drugs that has identical or similar effects are prescribed, we analysed and evaluated them by patients' characteristics, kinds of medication, severity of side effects on a case by case basis. More, we assessed the potential additional costs. As a result of this study, the duplication rate turned out to be 6.69% and it was shown that the proportion of the elderly patients over 60 was high. The gastro-intestinal medications took up a large part and most of the cases showed that the danger caused by double taking of medicine was slight, but there were some examples in which the serious side effects were predicted. Expected additional costs were the average 11.349 won and it ranges from the minimum of 56 won to the maximum of 135,720 won. In conclusion, the management of drug histories of the patients who need the plural treatments is very important and the necessity of the individual and professional guidance of taking medicines for the elderly patients is emerged, too.

      • 소규모사업장 보건관리기술지원사업 지원여부에 따른 사업장내 의사소통 비교

        목적: 소규모사업장 보건관리기술지원사업 지원여부에 따라 사업장내 위험의사소통채널이 마련되었는지와 이 채널이 기능을 하는지, 그리고 의사소통채널마련과 사업장내 위험의사소통 활성화의 기본적이고 중요한 배경이 되는 사업주의 산업보건에 대한 관심이 차이가 있는지에 대해 살펴보고자 하였다. 방법: 서울, 안산, 대전, 광주, 포항 지역에서 보건관리기술지원사업을 받은 사업장과 받지 않은 사업장을 대상으로 비례층화무작위표본추출법을 사용하여 추출한 후 1991년 12월 6일부터 10일간 우편설문조사를 실시하였다. 설문대상은 각 사업장의 남녀 각 1명, 보건업무담당자, 사업주였으며, 설문내용은 사업장 및 응답자의 일반적 특성 및 사업장내 위험의사소통이었다. 설문에 응답이 온 463개 사업장의 자료를 자료처리하여 총 228개 사업장(단해년도지원 사업장 189개, 연속2년지원 사업장 13개, 비지원사업장 26개)을 대상으로 분석하였다. 결과: 사업장내 의사소통에 대한 단변량분석에서는 보건업무 담당자의 선정여부만이 통계적으로 유의한 차이가 있었다. 소규모사업장 보건관리기술지원사업을 지원받지 않은 사업장과 지원받은 사업장의 사업장내 위험의사소통을 비교하기 위한 로지스틱회귀분석 결과, 모든 항목에서 통계적으로는 유의한 차이를 보여주지 않았다. 그러나 사업주의 관심(OR ; 1.63, 95% CI ; 0.38~7.01)과 보건업무담당자의 선정(OR ; 3.67, 95% CI ; 1.00~13.44) 및 결정권한(OR ; 1.48, 95% CI ; 0.25~8.91), 사업주의 산업보건문제 소통(OR ; 3.07, 95% CI ; 0.81~11.71), 보건업무담당자의 정보요청경험(OR; 4.48, 95% CI ; 0.97~20.57)에서는 지원받지 않은 사업장에 비해 지원받은 사업장에서 교차비가 증가하는 양상을 보여주었다. 반면 노동자의 산업보건문제 소통가능성여부는 오히려 교차비가 0.53(95% CI ; 0.11~2.44)로 감소하는 경향을 보여주었다. 결론: 소규모사업장 보건관리기술지원사업을 의사소통의 측면에서 평가해보면, 사업주나 보건업무담당자에게는 다소 효과가 있었지만 노동자가 지속적으로 참여할 수 있도록 의사소통문화를 바꾸어내는 데에는 한계가 있었다. 개선을 위해서는 노동자 중심의 접근 프로그램의 개발 및 시행이 시급하다 할 수 있다. Objectives :This study was to evaluate the effectiveness of the government-funded subsidized occupational health program for small-scale enterprises(GSOHP_SSE) by occupational health com-munication in workplace. Methods : We sampled the 1,835 enterprises of study by proportional stratified random sampling among industries of supported and not supported by GSOHP_SSE in Seoul, Ansan, Daejeon, Kwang-joo and Pohang. Information on the general characteristics of enterprises and respondants and occupational health communication in the workplace was collected on self-reported questionnaires by post-survey between December 6 and 15 in 2001. We received answers from 463 enterprises and analyzed 228 enterprises through data-cleaning by logistic regression to evaluate effectiveness of GSOHP-SSE about occupational health communication in the workplace. Results : By multiple logistic regression analysis, GSOHP_SSE turned out not to be statistically significant factor in all dependent variables about occupational health communication in the work-place. The concern of owner(odds ratio (below OR) ; 1.63, 95% confidence interval(below 95% CI) ; 0.38-7.01), formation(OR ; 3.67, 95% CI ; 1.00-13.44) and empowerment(OR ; 1.48, 95% CI ; 0.25-8.91) of health manager, request about occupational health problem of health manager(OR ;4.48, 95% CI ; 0.97-20.57) and occupational health communication of owner (OR ; 3.07, 95% CI ; 0.81-11.71) had the trend increasing OR in the industries supported GS-OHP_SSE relative to the industries not supported. But in recognition of possibility on occupational health communication of laborers, OR of GSOHP_SSE was decreased in 0.53(95% CI ; 0.11-2.44). Conclusions: This study found that the GSOHP_SSE was some effective to the side of employer and health manager, but had a limitation about change of communication culture useful to the laborers. We suggested the laborer-centered health training programs or the empowerment-based health training programs for effective occupational health management in the workplace.

        구내 표준 방사선사진을 이용한 구내방사선촬영기의 선질 평가

        Purpose : This study was to evaluate the beam quality of intraoral X-ray equipments used at Yonsei University Dental Hospital (YUDH) using the half value layer (HVL) and the characteristic curve of intraoral standard X-ray film. Materials and Methods : The study was done using the intraoral X-ray equipments used at each clinical department at YUDH. Aluminum filter was used to determine the HVL. Intraoral standard film was used to get the characteristic curve of each intraoral X-ray equipment. Results : Most of the HVLs of intraoral X-ray equipments were higher than the least recommended thickness, but the REX 601 model used at the operative dentistry department and the X-707 model used at the pediatric dentistry department had HVLs lower than the recommended thickness. The slopes of the characteristic curves of films taken using the PANPAS 601 model and REX 601 model at operative dentistry department, the X-705 model of prosthodontic dentistry department, and the REX601 model at the student clinic were relatively low. Conclusion : HVL and the characteristic curve of X-ray film can be used to evaluate the beam quality of intraoral X-ray equipment. In order to get the best X-ray films with the least radiation exposure to patients and best diagnostic information in clinical dentistry, X-ray equipment should be managed in the planned and organized fashion.
