        사업장의 안전ㆍ보건관리 유형별 산업재해 발생현황 비교

        Objectives: This study is aimed to illustrate the status of the occupational accident rate, fatality rate, and musculoskeletal disorder rate by safety and health management type in workplaces. Methods: In order to analyze the status of the occupational accident rate, fatality rate, and musculoskeletal disorder rate of different safety and health management types in workplaces, selected data from the Occupational Safety and Health Research Institute were used. Results: The occupational accident rate in workplaces where safety and health managers had been appointed was lower than in those where safety and health management was entrusted to other organizations, while the result was the opposite in regards to the fatality rate per 10,000 workers caused by accident. However, the occupational fatality rate per 10,000 workers in workplaces where safety and health managers have other roles was 426% higher than those where safety and health management was entrusted to other organizations. Moreover, the musculoskeletal disorder rate of the workplaces where safety and health managers have other roles was 15% higher than those where health management was entrusted to other organizations. Conclusions: It is necessary to review the effectiveness of systems in which safety and health managers can concurrently engage in other roles.

        선근종에서 aromatase cytochrome P450의 표현 양상

        목적 : 본 연구에서는 선근종과 정상 자궁의 내막 조직에서 여성호르몬의 합성에 관여하는 aromatase의 활성도를 알아보고자 항P450 arom 항체를 이용하여 P450 arom의 면역조직화학적 표현 양상을 관찰하였다. 연구 방법 : 2002년 5월부터 2003년 10월 사이에 자궁 적출술을 시행 받은 환자 중 조직병리학적으로 선근종으로 확진된 23명을 실험군으로, 자궁 경부 질환이나 비기능성 자궁 출혈 등의 소견으로 소파술이나 자궁적출술로 얻은 자궁 내막을 정상 자궁의 내막으로 간주하여 17명의 환자를 대조군으로 관찰하였다. 각각의 동결 절편한 조직들에서 항P450 arom 항체를 이용한 면역조직화학 염색을 시행하였다. 결과 : P450 arom은 내막 조직 (82.6%)와 일부 선근종 환자의 정상 자궁 내막 (23.5%)에서 양성 반응이 나타났다. 정상 자궁 내막 조직의 선상피세포와 기질에서는 P450 arom의 면역반응이 나타나지 않았다. 결론 : 이상의 결과에서 본 연구는 선근종의 자궁 내막 조직에서 P450 arom의 면역조직 화학 염색을 통해 aromatase의 활동이 있다는 것을 확인하고 선근종이 없는 자궁 내막에서는 aromatase 활동이 나타나지 않음을 알 수 있었다. Objective : To determine whether local estrogen production takes place in adenomyosis and in normal endometrium. Methods : The study included 23 cases of adenomyosis and 17 cases of normal uterine endometrium obtained through hysterectomy or curettage at Kangnam Cha Hospital. The frozen tissue specimens were examined by immunohistochemistry using P450 arom. Results : P450 arom was immunolocalized exclusively in the cytoplasm of glandular cells of adenomyotic tissue. However, no apparent staining was observed in stromal cells. Aromatase was expressed in the ectopic glands (82.6%), but also in the eutopic endometrium of patients with adenomyosis (23.5%). In the case of normal endometrium, P450arom was not detected. Conclusion : These results suggest that the aromatase activity is correlated to the pathophysiology of adenomyosis.

        3 차원적 자궁내막 세포배양 체계의 확립 : 성선 호르몬에 의한 분화유도 및 특성 Induction of Differentiation by Sex Steroid Hormone & Characterization

        연구목적 : EEC-ESC-ECM의 3차원적 공배양 체계(three-dimensionally cultured endometrial cell)를 확립하고 성선 호르몬의 첨가에 따른 생체활성 물질(biological marker)의 발현 양상과 조직학적 양상을 관찰하여 착상기 자궁내막 조직과 동일한 분화 양상을 보이는지를 확인하고자 하였다. 연구방법 : 적출된 인간 자궁의 후저부에서 자궁내막세포를 채취하여 상피세포(EEC)와 기질세포(ESC)로 분리한 후에 collagen과 Matrigel을 이용하여 EEC-ESC-ECM 공 배양체계를 만들어 에스트로젠과 프로게스테론이 포함된 DMEM 배양액에 48시간 배양 한 후에 gelatin substrate zymography, electron microscopy, integrins, COX, MMPs에 대한 면역조직화학적 염색(immunohistochemistry)을 시행하여 관찰하였다. 결과 : 배양 48시간째 EEC-ESC-ECM 공 배양체계에서의 EEC의 미세구조는 극성을 갖고 있었으며 세포들 간에는 tight junction과 desmosome이 형성되어 있었다. 또한 일부 세포에서는 microvilli가 감소하면서 pinopode 형태의 돌기가 돌출 되어 있은 것을 관찰할 수 있었다. EEC 혹은 ESC 단층배양 체계 및 ESC-collagen 배양체제에서 면역조직화학적으로 발현되지 않던 MMP-1, -2, -3, -9 과 integrin α1, α4, β3이 강한 발현을 보였다. 결론 : 에스트로젠과 프로게스테론 존재하의 EEC-ESC-Matrigel/collagen의 3차원적 공 배양체계에서 EEC 혹은 ESC 단층배양 체계 및 ESC-collagen의 배양체계와 다르게 EEC와 ESC의 측분비 작용에 의해 in-vivo 에서 착상기 내막세포와 유사한 형태와 생체 활성 물질의 발현을 확인할 수 있었다. 이러한 체외 자궁내막 세포 배양 체계는 내막조직의 분화 및 배아의 착상기전 뿐 아니라 착상관련 질환의 병리학적 연구에 많은 도움이 될 것으로 사료된다. Objective : The aim of this study was to establish three-dimensionally cultured endometrial cell model containing endometrial stromal cell (ESC), endometrial epithelial cell (EEC) and extracellular matrix (ECM) and to compare the morphological and biomolecular expression patterns of this model with mid-luteal endometrium in vivo. Materials and Methods : The EEC and ESC was obtained from hysterectomy specimen and cultured separately. The EEC was overlayered in Matrigel layer on ESC embedded in collagen. The model had been cultured for 48 h in DMEM medium containing estrogen and progesterone. The ultrastructure was evaluated by electron microscopy. The expression of integrins, cyclooxygenases and matrix metalloproteinases were examined by immunohistochemistry and zymography. Results : EEC in three-dimensional culture model grew with polarity and tight junction and desmosome between cells were found. The formation of pinopodes was also detected. In three-dimensionally cultured endometrial cell model, the expression of integrin α1, α4, β3, MMP-1, -2, -3 and 9 was detected which was not expressed in monolayer culture of EEC, ESC or ESC embedded in collagen. Conclusion : The three-dimensionally cultured endometrial cell model possessed the morphological and biomolecular characteristics of in vivo endometrium of implantation period. These characteristics could be achieved by paracrine interactions between ESC and EEC. This model may contribute to the studies of differentiation of endometrium, process of implantation and pathophysiology of implantation-related diseases.

        포상기태 환자에서 악성 임신성 융모종양의로의 전환에 관여하는 임상 및 생물학적 인자들의 역할

        Objective: The purpose of this study is to reevaluate the prognostic factors by investigating the clinical and biological parameters concerned malignant gestational trophoblastic tumor in patients with hydatidiform mole. Methods: From March 1995 to February 2000, 41 patients admitted to department of the Obstetrics and Gynecology, Yonsei University College of Medicine who were diagnosed with pathologically-proven gestational trophoblastic disease were selected. Parameters such as age, gravida, parity, presence of theca lutein cyst, ratio of uterine size to gestational age, hCG level, DNA ploidy, S-phase fraction were compared between malignant gestational trophoblastic tumor group and spontaneous remission group. Results: Considering the clinical prognostic factors, the patients were divided into two age groups; the first group consisted of those older than 40 years of age and the second control group consisted of those under 40. The number of patients older than 40 in the spontaneous remission group and malignant gestational trophoblastic tumor group were 4(15.4%) and 7(46.7%), respectively, showing a significantly higher number in the group over 40years. Other parameters such as gravida, parity, presence of theca lutein cyst, ratio of uterine size to gestational age, hCG level, DNA ploidy, S-phase fraction showed no statistically significant difference between the two groups. Conclusion: The progression rate from hydatidiform mole to malignant gestational trophoblastic tumor was significantly higher in patients over 40 years of age. Therefore, more aggressive therapeutic approach should be considered in such patients.

        정상 임신과 자궁외 임신 사이의 융모막 융모 Telomerase 활성도의 비교

        Objectives : To compare the presence of telomerase activity(TA) in the chorionic villi between eutopic and ectopic first trimester human pregnancies. Methods : Human chorionic villi were obtained from eleven normal eutopic pregnant women who underwent an elective abortion, and from eleven women with ectopic pregnancy who underwent a salpingectomy for tubal pregnancy. TA was analyzed by radioisotope polymerase chain reaction based telomeric repeat amplification protocol(TRAP) assay. Results : TA was expressed in 5 of 11 (45.5%) in eutopic pregnancies, and in 7 of 11(63.6%) in ectopic pregnancies. Chorionic villi in ectopic pregnancies were expressed more frequently than those of eutopic pregnancies, this difference was not significant.(p$gt;0.05) Conclusion : Our results suggest that the expression of TA in human chorionic villi was not affected according to the implantation site during early placental development.
