        This study aims at finding a effect of various position and length of laminar tear on vibrational characteristics on beam including shear deformation. General solution for nonlaminar tear and laminar tear regions are first established. Recurrence equations relating integration constants for adjacent interior regions are established by satisfying continuity conditions at junctions of interior region. The frequency determinant is then obtained by satisfying contiuity conditions at junction between end region and interior regions immediately next to the end region. Numerical results are compared for experimental and references results.

      • ZnSe 단결정에 대한 열자극 발광과 열자극 전류의 동시측정

        두 종류의 ZnSe 분말을 사용하여 sublimation 방법으로 성장한 ZnSe 단결정(as grown)에 대하여 TSC와 TSL을 동시 측정하였으며, PL과 DLTS를 측정하였다. PL 측정으로 I_1, I_2와 DAP 발광을 관측하였으며 DLTS에 의한 깊은준위를 관측하였다. TSL과 TSC의 동시 측정으로부터 얻은 그로우 곡선을 점근 해법으로 분해하여 세개의 준위를 얻었으며 그들의 활성화에너지값은 0.22 eV, 0.30 eV, 0.39 eV이었다. PL spectrum과 열자극 완화과정의 동시 측정 결과로부터 0.22 eV 근방에 impurity에 의한 주게준위와 native defect에 의한 받게준위가 각각 전도대의 아래와 가전자대의 위에 존재하는 것으로 판명되었다. DLTS 측정과 동시측정 결과로부터 0.30 eV와 0.39 eV의 준위는 V_se vacancy와 관련되는 주게 준위임을 알았다. Simultaneous measurements of TSL were carried out on ZnSe single crystals grown by high pressure Bridgman technique and the PL and DLTS signals were observed. Photoluminescence spectrum at 10 K on the ZnSe crystal reveals I_1 and I_2 lines, as well as DAP emission line. DLTS spectrum on the ZnSe crystal show electron trap at 0.33 eV. Two TSC and TSL peaks were observed near 215 K and 230 K, which are identified as having originated from two donor trap levels at 0.30 eV and 0.39 eV, respectively, below the bottom of the conduction band. We also observed single TSL and TSC peaks at 150 K which were identified as having originated from a donor and acceptor trap levels at about 0.22 eV below the conduction band and over the top of the valence band, respectively.

      • 포괄수가지불제도 적용에 따른 산부인과 진료수입의 변화

        Objectives: Under the fee-for-service system, Korean hospitals have been suffering from the worsening profitability caused by law medical care fees. To overcome the difficulties, they have maximized the quantity of medical services and made relatively large investment in developing new medical services using high-priced medical equipment, medicines and medical supplies which were not covered by medical insurance system rather than reduced running costs. Materials and Methods: This study was made to analyze and forecast the revenue change of the department of Obstetrics and gynecology of an university hospital in Seoul, expecting the implementation of DRG system in 2000. Results: The results were as follows : 1.During the first two days of hospitalization. 41% of total medical service fee was occurred, which was thought that most of the check-ups and operations were made during this period. After two days of hospitalization, the new occurrence of medical service fee tended to reduce. 2.Out of total medical service fee, 67% of admission fee was occurred after 5 days of hospitalization. This was because medical service fees in obstetrics and gynecology department occurred during the first 5 days of hospitalization. 3.Out of fees for operation, treatment, medical supplies and blood transfusion. 71% was occurred during the first two days. 4. In case of fees for examination, 50% was occurred during the first two days. 5.A total of 53% of fees for medication and injection was occurred during the first 5 days. Conclusions: By the implementation of the DRG system, the income is forecast to increase by 800 won to 310,000 won by the disease group of obstetrics and gynecology department. To increase hospital income with the implementation of DRG system, the results of this study suggest that examinations should be done at out-patient departments before the hospitalization of patient, the discharge of patient should be noticed in advance to reduce the period of hospitalization, and admission appointment system should be implemented for the immediate operation and delivery.

        Gastrointestinal stromal tumor (GIST) is the most common non-epithelial neoplasm arising in the gastrointestinal tract, but this tumor is rarely seen in association with type l neurofibromatosis (NFl) Generally, the majority of GISTs are isolated neoplasm and they are sporadically found in the stomach, but the GISTs in NFl patients are usually multiple and usually found in the small intestine. We report a case of multiple GISTs in the jejunum of a 63-year-old woman diagnosed as NFl accompanied by complication of gastrointestinal bleeding, In this case, Patient had multiple cafe-au lait spots and neurofibroma on skin and had freckling on axilla and groin, and then, we made a diagnosis of NFl. Gastrointestinal bleeding is controlled by resection of multiple GISTs. Generally, only 3-5% of all gastrointestinal bleeding comes from the small bowel. Causes of small intestinal bleeding are angiodysplasia, neoplasm, NSAIDs induced ulcer etc. but, If patients have NFl accompanied by complication of gastrointestinal bleeding, GISTs on small bowel must be considered.

        해외귀국 한국인의 뎅기 바이러스에 대한 항체 양성율

        목적 : 한국인의 해외여행이 지속적으로 증가함에 따라 해외 유행 질병의 유입 및 해외 감염 가능성이 점점 증가하고 있다. 특히 뎅기열은 동남아시아 등의 해외여행 및 거주 한국인이 증가함에 따라 감염기회가 점차 증가할 것으로 예상되고 있다. 따라서 뎅기열 유행지역의 여행 및 체류 한국인 가운데 뎅기열 의심증상을 보인 환자의 검체를 수집하여 혈청학적 및 역학적 상황을 확인함으로서 현재까지 확인되지 않았던 한국인의 뎅기 바이러스 감염 실태를 파악하여 국가의 보건지표를 확립하고자 하였다. 재료 및 방법 : 해외여행 후 뎅기열 의심증세를 보이는 환자의 혈청을 수집하여 상업용 진단 kit인 MAC-ELISA 및 ICA법을 사용하여 뎅기 바이러스에 대한 항체시험을 실시하고 성별, 연령, 방문국 등 뎅기 감염에 영향을 미치는 요인들을 분석하였다. 상업용 진단 kit에 의해 혈청학적 결과가 확인된 99건의 검체 가운데 25건의 검체에 대하여 본 실험실에서 제작한 IFA 슬라이드로 확인시험을 실시하고 그 결과를 상업용 진단 kit와 비교 분석하였다. 결과 : 99건의 뎅기열 유사증상 환자의 검체는 상업용 진단 kit에 의해 333%의 뎅기 항체 양성으로 확인되었으며 이들 환자의 해외 방문국의 조사 결과 타일랜드 등 4개국 여행자에서 절반 이상의 양성자가 확인되었다. 또한 진단법 비교시험을 위해 25개 검체를 상업용 진단 kit와 자체 제작한 IFA법으로 시험한 결과 진단 kit에서 양성 9명, IFA에서 양성 13명으로 나타나 결과의 차이가 있었다. 결론 : 한국인의 뎅기 바이러스 감염자가 국내에서 처음으로 확인되었으며 환자 발생의 원인과 지역, 환자의 발생을 확인할 수 있었다. 이에 따라 뎅기열에 대한 실험실적 감시체계 구축과 진단시험체계의 확립이 필요함을 확인하였다. 또한 상업용 진단 kit를 사용함에 따라 발생하는 고가의 비용을 최소화 할 수 있는 민감도와 특이도 등 효능이 높은 진단법 개발이 요구되었다. Background : Recently, dengue fever has increased throughout tropical regions and emerged as the most important vector borne viral disease in human. 4 serotypes of viruses are circulating concurrently in these regions and thus it may be anticipated to increase risk of dengue hemorrhagic fever. Even though dengue fever is still not endemic in Korea, it is necessary to test antibodies against dengue viruses because the number of Koreans who have visited these regions is continuously increasing. Materials and methods : Serum specimens from persons with suspected dengue fever had been collected. Commercial kit, immunochromatographic test (ICA), and the IgM capture enzyme-linked immunosorbent assay (MAC-ELISA) were employed for dengue fever detection in these studies. For confirmation randomized 25 specimens among total of 99 specimens were selected and compared with those results from commercial kit and IFA. Results : 33 (33.3%) among 99 specimens showed positive antibody against dengue virus by commercial kit. Positive rate of traveller who have visited Indonesia, Philippines, Malaysia, Thiland was 54.5%. To compare the efficiency of test methods, 25 randomly selected specimens were tested by the MAC-ELISA and IFA simultaneously. 9 specimens showed postive results with the MAC-ELISA method whereas 13 speciments were positive with the IFA methods. Conclusion : Confirming the diagnosis of dengue fever with antibody against dengue virus was attempted for the first time in Korea. The results from our study indicate that establishing a national surveillance and/or laboratory diagnostic system in Korea are necessary. In addtion, antibody test strategies for national surveillance system should be carefully considered.

        NaHCO<sub>3</sub>- and NaCl-Type Hot Springs Enhance the Secretion of Inflammatory Cytokine Induced by Polyinosinic-Polycytidylic Acid in HaCaT Cells

        Background: Hot springs have been traditionally used as an alternative treatment for a wide range of diseases, including rheumatoid arthritis, bronchial asthma, diabetes, hypertension, psoriasis and atopic dermatitis. However, the clinical effects and therapeutic mechanisms associated with hot springs remain poorly defined. Objective: The purpose of this study was to demonstrate the different effects of hot springs on cellular viability and secretion of inflammatory cytokines on keratinocyte in two geographically representative types of hot springs: NaHCO<sub>3</sub>-type and NaCl-type, which are the most common types in South Korea. Methods: We performed WST-1, BrdU measurements, human inflammatory cytokine arrays and enzyme-linked immunosorbent assay in HaCaT cells stimulated with toll-like receptor 3 by polyinosinic-polycytidylic acid. Results: The interaction effects of cell viability and cell proliferation were not significantly different regardless of polyinosinic-polycytidylic acid stimulation and cultured hot springs type. Cytokine array and enzyme-linked immunosorbent assay analysis showed increased expression of inflammatory cytokines such as interleukin-6 and granulocyte-macrophage colony-stimulating factor by polyinosinic-polycytidylic acid stimulation, with expression levels differing according to hot springs hydrochemical composition. Cytokine reduction was not significant. Conclusion: The effects and mechanisms of hot springs treatment in keratinocytes were partially elucidated. (Ann Dermatol 33(5) 440∼447, 2021)

      • ZnSe 단결정 성장과 전기-광학적 특성

        수송물질로 요오드를 사용하여 기상 수송방법으로 ZnSe 단결저을 성장 하였다. 성장부분과 시료부분의 온도는 각각 815℃와 1065℃로 하였다. Van-der Pauw방법으로 측정된 전하농도와 Hall 운동도는 실온에서 각각 ?? ??,??/v-s order 정도 이었다. 성장된 sample의 PL방출 스펙트럼은 실온에서 520nm의 peak를 이루었으며 Lambe-Klick모델에 따르는 green 발광으로 분석 되었다. Crytal growth of zinc selenide by vapour phase transport method was carried out using iodine as a transporting agent in a sealed ampoule kept oat temperatures of 815℃ and 1065℃ for the growth part and source part, respectively. The electron carrier concentration and Hall mobility were measured of using Van der Pauw method at room temperature and their values are about ∼?? ?? and about ∼??/v-s, respectively. Photoluminescence emission spectra of the grown sample were peaked at 520nm at room temperature.

      • Comparison of volatile organic compounds in stormwater and groundwater in Seoul metropolitan city, South Korea

        <P>Volatile organic compounds (VOCs) detected in stormwater were compared with VOCs present in emission sources, air, groundwater, and influent to sewage treatment plants in Seoul to understand their fate and transport in the urban hydrological system. Stormwater is a carrier of non-point source pollutants and contains VOCs from land surfaces and air. Samples of stormwater and influent to sewage treatment plants were collected and analyzed for 61 VOCs, while the VOCs in emission sources, air and groundwater were investigated through literature reviews for comparison. The results showed that the most frequently detected VOCs in stormwater were similar to those in air. However, the atmospheric concentrations of benzene, toluene, ethylbenzene, and xylenes (BTEX), and methyl tertiary-butyl ether (MTBE) were too low to explain their frequent detection and high concentrations in stormwater. As a result, land surfaces seem to be a primary source of these VOCs in stormwater. Comparison of the VOCs in stormwater and groundwater showed that toluene and MTBE were frequently detected in both media, but more often and at higher concentrations in stormwater. This finding indicates that stormwater recharge is a source of toluene and MTBE in groundwater. Regarding groundwater, land surfaces seem to be a primary source of toluene, while urban air is the primary source in the case of MTBE. Specifically, the MTBE values in air were sufficiently high to explain its levels in groundwater, which had continually increased and remained low afterward. Furthermore, the high ratios of TEX (toluene, ethylbenzene, and xylenes) to benzene and MTBE in stormwater indicated that TEX had additional sources other than vehicles, most likely hydrocarbon solvents. These solvents seem to be a primary source of TEX and other frequently detected VOCs in stormwater. However, trichloroethylene (TCE), tetrachloroethylene (PCE) and their dechlorination intermediates were far more frequently detected and at higher concentrations in groundwater than in stormwater. Additionally, their concentrations frequently exceeded the water- quality criteria. It seems that halogenated solvents had produced contamination plumes of these chlorinated VOCs in the Seoul aquifer. Based on VOCs detected in Seoul, stormwater was mixed with groundwater in combined sewers and flowed into sewage treatment plants. The results imply that organic solvents should be handled with extreme care to protect groundwater quality.</P>
