        수평 매복된 상악 중절치의 교정적 견인 : 증례 보고

        본 증례는 수평 변위 매복과 치근 만곡을 가진 상악 중절치를 폐쇄 노출법을 이용한 교정적 견인을 시행하여 영구 전치의 기능과 심미성을 회복하였다. 상악 영구 전치가 수평으로 매복된 경우,가급적 조기에 발견하는 것이 중요하며, 촉진,시진,방사선 사진 등을 이용한 적절한 치료 계획의 수립이 필요하다. 상악 영구 전치의 매복이 조기에 적절히 치료되지 못하면 정중선의 변위,인접치의 이동에 의한 맹출 공간의 감소,치조골 높이의 차이 뿐만 아니라,이로 인한 심미적인 문제를 야기하여 소아 심리 발달에 해로운 영향을 줄 수 있기 때문이다. 매복치의 치료는 매복치의 형태,위치,상태에 따라 다양하나,주기적 관찰 방법,상부 연조직과 경조직을 단순히 제거하여 맹출을 유도하는 방법,외과적 노출 후 매복치를 교정적으로 견인하는 방법,치아 이식술,발치 등을 시행할 수 있다. 이중 이번 증례에서 시행한 매복치의 교정적 견인은 매복치의 치축이 정상 맹출로와 일치되지 않으며,주기적인 관찰 후 더 이상의 맹출이 기대되지 않을 때 시행하는 가장 일반적으로 적용되는 방법으로 매복의 심도,치아의 발육단계,만곡의 부위와 정도,수술방법과 견인방향 등을 고려하여 치료의 예후를 향상 시킬 수 있다. Maxillary central incisor impactions occur infrequently. Their origins include various local causes, such as odontoma, supernumerary teeth, space loss, and disturbances in the eruption path, also trauma and apical follicular cysts. Impacted teeth can cause serious dental and aesthetic difficulties as well as psychological problems especially in anterior regions. Although the impaction of maxillary incisor occurs less frequently than that of the maxillary canine. it is of concern to parents during the early mixed dentition stage because of the uneruption of the tooth. Forced eruption of impacted teeth should be considered in young patients because this technique can lead to suitable results from a periodontal, occlusal, and esthetic perspective at an earlier stage better than with other treatment options. This report presents the surgical and orthodontic treatment of cases with horizontally impacted and dilacerated maxillary central incisors. For each patient. we used the closed eruption method. placed an attachment on the impacted tooth on surgery, and fully closed the flap. Traction was applied immediately. The impacted tooth erupts through the healed tissue in a manner resembling normal eruption.

      • Could cluster of risk factors predict clinical outcome in patients with acute myocardial infarction?

        Purpose: Conventional risk factors are differently contributed to short-term prognosis of acute myocardial infarction (AMI); hypertension and diabetes increase adverse outcome, whereas hyperlipidemia, smoking, and obesity paradoxically decrease adverse outcome of post-MI patients. The aim of this study is to assess whether cluster of conventional risk factors, PARADOCS (Pressure of ARtery Abnormality, Diabetes, Obesity, Cholesterol, Smoking) score, would improve the ability to predict major adverse cardiac events (MACEs) in patients with AMI. Methods: Between November 2005 and July 2011, 24,020 patients (17,232 men; mean age=63.0±12.8) with AMI were analyzed from Korean AMI registry. PARADOCS score was calculated as follows; [number of non-paradoxical risk factors (NRF) -number of paradoxical risk factors (PRF)]+3 in which non-paradoxical risk factors are hypertension and diabetes, and paradoxical risk factors are the rest. The 1-year MACEs were defined as death and non-fatal MI. Results: NRF including hypertension and diabetes were present in 49.6% and 27.0%, whereas PRF including hyperlipidemia, smoking, and obesity were present in 12.3%, 43.0%, and 34.6%, respectively. During the follow-up, 1,409 (5.9%) MACEs occurred. PARADOCS score was significantly higher in patients with 1-year MACEs (3.38±1.03 versus 2.81±1.08, p<0.001). In Cox proportional hazards model, PARADOCS score was an independent predictor of 1-year MACEs (hazards ratio 1.26, 95% confidence interval 1.18-1.34; p<0.001) after adjusting for confounding variables. Patients were categorized into 3 groups according to the PARADOCS score; PARADOCSLOW (0-1, n=2,446), PARADOCSMID (2-3, n=14,594), and PARADOCSHIGH (4-5, n= 6,980). Kaplan-Meier survival curve showed that there were significant differences in the 1-year MACEs among three groups including 3.5% in PARADOCSLOW, 8.5% in PARADOCSMID, and 16.4% in PARADOCSHIGH, respectively (long-rank p<0.001). Conclusions: In post-MI patients, conventional risk factors are differently associated with short-term prognosis, and score of these risk factors, PARADOCS score, can provide useful prognostic information to clinicians.

        다양한 치근단공 크기와 근관단면의 형태를 가지는 치아에서 taper의 정도가 미세누출에 미치는 영향

        The aim of this study was to evaluate the microleakage of teeth according to root canal preparation with & without apical enlargement in various size of apical foramen. 60 extracted one canal roots were cross-cutted at 5 mm from root apex and divided into two groups according to their apical foramen size of large (L) and small (S). Each group was subdivided into two groups accordance with their cross-sectional configuration at 5 mm from apex, round (R) and ovoid (O); SR Group, SO Group LR Group, LO Group. Each group was shaped in .02 taper by Quantec series Nickel-Titanium (NiTi) rotary file, obturated by lateral condensation method. Leakage was measured using a fluid transport model under 40cmH₂O pressure. After the leakage test, blocks which had showed the leakage retreated with .04 taper and ,06 taper and evaluated the degree of fluid filtration in each group. The data was analysed statistically using chi-square test and fisher's exact test. The results obtained were as follows : 1. Significant difference in leakage was found in groups which had different apical foramen size in .02 taper instrumentation (p < 0.05), but not in .04 taper instrumentation (p > 0.05) 2. The difference in microleakage according to the shape of canal was not evident at 5 mm from apex (p > 0.05). 3. There was correlation between .02 taper instrumentation and .04 taper instrumentation in LR group, LO group (p < 0.05). 이 연구의 목적은 근단공의 크기가 다양한 치아에서 근단공 확대의 유무에 따른 근관성형시 치아의 미세누출을 평가하기 위함이다. 1근관 치아 60개를 치근첨 상방 5mm에서 절단후 각 치아의 근단공 크기에 따라 큰 근단공 (L) 작은 근단공 (S)으로 나누었다 이 두 군을 치근첨 상방 5mm의 횡단면상 근관의 형태로 원형 (R)과 난형 (O)으로 나누었다(SR, SO, LR, LO). 각 군은 Quantec series Nickel-Titanium (Ni-Ti) rotary file을 이용하여 .02 taper로 성형 후 측방가압법으로 충전하였다. 누출은 40cmH₂O 압력하에 fluid transport method로 측정하였다. 측정 후 누출이 있는 시편은 .04 taper와 .06 taper로 재근관치료 하여 다세 미세누출을 평가하였다. 통계학적으로 chi-square test와 fisher's exact test를 이용하였다. 결과는 다음과 같다. 1. .02 taper로 성형한 군에서는 근단공의 크기에 따른 미세누출은 통계적으로 유의성이 있었고 (p < 0.05) .04 taper로 성형한 군에서는 통계적으로 유의한 차이를 보이지 않았다 (p > 0.05). 2.근첨 상방 5 mm에서 근관단면의 형태에 따른 미세누출은 통계적으로 유의하지 않았다 (p > 0.05). 3. LR group과 LO group에서는 .02 taper로 성형한 군과 .04 taper로 성형한 군사이에 유의한 차이를 보였다 (p < 0.05).

        영어 광고에 대한 언어학적 연구 및 교육적 활용

        There have been great interests in 'what to teach', i.e., teaching materials, followed by 'how to teach', i.e., teaching methodology in English teaching fields. Along this trend, much attention has been put on authentic materials to compensate for traditional textbooks. The authentic materials should not only develop learners' communicative skills, but also help understand the culture of target language and foster learners' critical thinking (Erkaya 2005). In this respect, advertising English is considered a useful material for authenticity and up-to-date lexicon in English teaching. The purpose of this study is two-folds. First, it examines linguistic aspects of advertising English. About 300 advertisements, listed in four major English magazines in the 2000s, were examined. Various linguistic tactics or strategies were employed in advertising English. Phonological aspects are characterized with alliteration, rhyme, pun, humor, repetition of certain sounds. Morpho-syntactically, simple and colloquial vocabularies are preferred. Neologisms, adjectives and proverbs are commonly used. Imperatives and ellipsis are used to draw people's attention. Semantic ambiguity and paradox are frequently adopted. Advertising English, at the same time, displays a paralinguistic aspect of target language's culture. Secondly, this study seeks how advertising English can be employed in English teaching. Through the real classroom teaching using advertising English as an authentic material, students felt it increased their motivation and interests in class and helped understand English culture. Finally, this study proposes a lesson plan where English learners are encouraged to present or design English abvertisement.

        근관 내 불규칙 확장부에서 sealer 적용방법에 따른 충전 효과 평가

        본 실험은 resin 시편을 이용하여 미리 제작한 근관 내 불규칙 확장부에서 근관 충전용 sealer의 적용 방법에 따른 충전 효과를 평가해 보았다. Endo-training Bloc을 절단하여 사십 개의 규격화된 resin 블록 시편을 획득하였다. 각각의 시편에 $, 08taper GT 파일을 사용하여 근관을 형성하였고 근관에 손상이 가지 않도록 시편을 양분한 후 양분된 시편의 한 쪽 근관 벽에 다양한 근관 내 확장부를 표현하는 구를 형성하였다. 양분된 시편을 결찰하고 절단 부위를 밀봉하였으며 AH26 sealer를 사용하여 단일 cone 충전법으로 근관을 충전하였다. 네 가지의 근관 충전용 sealer의 적용방법을 이용하였다: A군,$ K-file : B군, ultrasonic file : C군, lentulo spiral: D군. EZ-Fill bi-directioanl spiral. 모든 시편은 37˚상대습도에서 1주일간 보관한 후 각 시편을 근단에서 3mm, 4mm, 5mm 부위에서 수평으로 절단하고 각각의 절단면을 위상차현미경과 디지털 카메라를 이용하여 30배율로 관찰하고 촬영하였다. 관찰된 절단면은 scoring system을 이용하여 점수를 산정하였고 각 군 간의 통계적인 유의성 유무는 Fisher's Exact Test를 이용하여 시행하였다. The aim of this study was to evaluate the effectiveness of sealer placement in simulated root canal extensions. Forty resin blocks were attained from the Endo-training Bloc. In each block. The simulated root canal was made with $, 80taper GT file. After each block was longitudinally split into two halves, a standardized groove was prepared on one canal wall of two halves to simulate the canal extensions with various irregularities. The two halves of each block were assembled and all simulated root canals were obturated by single cone method with AH26 sealer. Four different methods of sealer placement were used: group A, $ K-file; group B, ultrasonic file; group C, lentulo spiral; group D, EZ-Fill bi-directional spiral. All obturated blocks were stored in 100% humidity at 37℃ for 1 week, Using a low speed saw, each block was sectioned horizontally. Images of the sections were taken using a stereomicroscope at 30 magnification and a digital camera. The amount of the sealer in the groove was evaluated using a scoring system, a higher score indicated better sealing effectiveness. The data was statistically analysed by Fisher's Exact Test. The sealing score was the lowest, specially at the middle area of canal extensions in group A, and that was statistically significant difference from other groups. In conclusion, the ultrasonic file, lentulo spiral and EZ-Fill bi-directional spiral were effective methods of sealer placement in simulated canal extensions. The K file was the least effective method, specially at the middle area of canal extensions.

      • 흰쥐 중추신경계내 난소로 투사하는 미주신경로에 관한 연구

        본 실험은 부교감신경의 하나인 미주신경이 난소를 지배하는 신경으로 관여하고 있는지를 pseudorabies 바이러스를 이용하여 난소신경로와 난소주사후 미주신경절단을 통하여 조사한 연구 보고이다. 이를 위하여 Sprague Dawley계 암흰쥐를 대상으로 pseudorabies 바이러스를 난소에 주사한 무리와 난소 주사후 미주신경을 절단한 무리에서 뇌를 적출하여 pseudorabies 바이러스에 대한 면역조직화학 염색을 시행하여 비교하였다. 본 실험결과 미주신경 중추신경로내의 상위신경핵들이 pseudorabied 바이러스에 대하여 양성반응이 줄어들었거나 관찰되지 않는 차이를 보였다. 즉 적색핵, 종말판혈관기관, 줄무늬체, 침상핵과 이마엽겉질은 부분적으로 난소의 미주신경로에 관여하고 있으며 미주신경등쪽핵, 고립로핵, 최후영역, 청색반점, 팔옆핵, 코리케퓨즈핵, 흑색질 및 시각교차위핵은 양성반응이 관찰되지 않아 미주신경으로 투사하는 부교감신경핵으로 조사되었다. The mammalian ovary has been known as receiving its innervation by sympathetic and sensory neurons of the peripheral nervous system from the brain. Recently, there were several functional reports that the vagus nerves were also regulating the ovarian function, but the vagus nerve had not been identified by clear morphological evidence. A viral transneuronal tracing technique has been used to demonstrate the morphological evidence for the central vagal involvement in ovarian innervation in brain areas. Bartha strain of pseudorabies virus injection was made into the ovary of Sprague Dawley rats. In experimental group, the vagus nerve of the same injection side was removed right after ovarian injection. At five days after initial injection, all the rats were sacrificed and brains were processed for immunohistochemistry. Several central nuclei including hypothalamic paraventricular nucleus showed strong bilateral positive labelings after unilateral injection in control rats, but the positive labelings were disappeared or decreased in several hypothalamic nuclei and nuclei of the vagus nerve. I n conclusion, these results provide the morphological evidence that vagus nerve has neural connection to ovary and by which the central nervous system may maintains the state of ovulation and reproduction as a possible parasympathetic routes in mammals.

      • 과학문화에 대한 중 · 고등학교 교사들의 인식

        Science culture is a very wide, diverse and complicated notion, In order to define science culture, it is important to scrutinize it from various angles. In this study, we have performed analysis of western notions of science culture which have been discussed and domestic notions which-exist currently. Based on this consideration, we set up the notion of science culture as the combination between science and culture in order to match it to the intention of the study. From the defined notion, we investigated essential areas which compose science culture and constructed the index system of science culture. The system is divided by science history, science philosophy, science literature and art, social activities concerning science, and science mass media. It is used to probe the difference between cognition of secondary school teachers as their major and gender. The result of research is like this. 1. In the teacher's cognition of history of science, there was a statistically significant difference in the questions of 'Understanding of science historic discovery and invention', Frequency of visiting science museums and the time and expenses required', 'Expenses required on visiting science-culture historic sites'(P<-05). And in all questions except 'Expenses required on visiting science museums' and 'Expenses required on visiting science-culture historic sites', science teachers showed higher cognition for science culture than humanities and sociology teachers. In the analysis according to their gender, only the question related to 'understanding of science-historical discovery and invention' showed the meaningful difference between male and female(P<.05); cognition of male teachers is higher than that of female teachers. 2. In the teacher's cognition of philosophy of science, science teachers showed higher cognition for science culture than teachers who majored in humanities and sociology in the questions of 'Whether having interests in scientific issues', 'The extent of interest in recent scientific issues' and 'Whether using scientific methods', and there was a statistically significant difference(P<.05). In the analysis according to their gender, the questions related to 'Concern about scientific issues', 'Concern about new scientific issues' showed the meaningful difference between male and female(P<05); cognition of male teachers is higher than that of female teachers. 3. In the teacher's cognition of science literature and art, science teachers showed the higher cognition for science culture than teachers majored in humanities and sociology in the questions of 'Frequency of seeing science performance', 'Frequency of seeing science movies' and 'The amount of purchasing science novels', and there was a statistically significant difference(P<.05). There wasn't a statistically significant difference in any question between male and fema1e(P>.05). 4. In the teacher's cognition of science-related social activities/ there was a statistically significant difference in the questions of 'frequency of visiting science museum science pavilion experience halls and the time required', 'Frequency of using scientific information at libraries', 'frequency of participating in science-related events', 'frequency of participating scientific organizations and group activities' and 'The interest in science and technology policies'(P<.05). Among the above-mentioned questions, in all questions except 'frequency of science museum science pavilion experience halls and the time required', teachers majored in science showed higher cognition for science culture than teachers majored in humanities and sociology, In the analysis according to their gender, the questions related to Frequency of participating in science-related events', 'frequency of participating scientific organizations and group activities', 'The interest in science and technology policies' showed the meaningful difference between male and fema1e(P<05); cognition of male teachers is higher than that of female teachers. 5. In the teacher's cognition of scientific massmedia, there was a statistically significant difference in the questions of 'Subscription of science papers and magazines', 'Frequency of reading science-related books', 'Frequency of watching science education programs', 'Frequency of watching science channels' and 'Frequency of using science-related internet sites' (P<05). Among the above-mentioned questions, in all questions except 'Frequency of watching science channels', science teachers showed higher cognition for science culture than teachers majored in humanities and sociology. In the analysis according to their gender, the questions related to 'Subscription of science magazines', 'Frequency of reading science-related books', 'Frequency of watching science education programs', 'Frequency of watching science channels' and 'Frequency of using science-related internet sites' showed the meaningful difference between male and female(P<.05); cognition of male teachers is higher than that of female teachers. As above results of the study, there was a significant difference by teacher's major of science, humanities and sociology in many questions. Especially, it was found that science teachers were more sensitive to scientific events or issues, participating in scientific historic sites or occasions, using scientific methods in their daily lives, and using scientific mass media than teachers with any other majors, therefore, in modem times when the importance of science is getting increased, we should raise teachers' cognition about science culture through various research activities.

        In this paper, we invesitigate the chaotic behavior which occurs if we use Euler's method to approach to the solution of nonlinear differential equation. We consider the several sufficient conditions for the existence of the chaos in finite difference scheme. The result can be applied to 2-dimensional difference equations and euler's difference schemes.

        범하수체 기능저하증을 동반한 일차성 중추성 림프종의 1예

        A primary central nervous system lymphoma(PCNSL) is defined as a lymphoma limited to the cranial-spinal axis without systemic involvement. PCNSLs were formerly very rare, only 0.5∼ 1.2% of all intracranial neoplasms, and were usually associated with acquired immuno-deficiency syndrome(AIDS) and other immune-compromised states. Recently, there has been a clear rise in the incidence of this disease among normal individuals. We describe a 77-year-old female with hyponatremia who presented with a confused mental state. Computed tomography(CT) of the brain demonstrated a suprasellar mass with enhancement. We evaluated the basic hormonal studies and the magnetic resonance imaging(MRI). A ventricular biopsy was done, and the result was a diffuse, large, B-cell-type malignant lymphoma. She presented panhypopituitarism and partial central diabetes insipidus. She received chemotheraphy and was discharged in an improved condition. We report a case of panhypopituitarism due to PCNSL and give a review of the literature.
