Amino-Functionalized Mesoporous Silica Particles for Ocular Delivery of Brimonidine

<P>To treat glaucoma, conventional eye drops are often prescribed. However, the eye drops have limited effectiveness as a result of low drug bioavailability due to their rapid clearance from the preocular space. To resolve this, we proposed amino-functionalized mesoporous silica (AMS) particles as delivery carriers of the glaucoma drug, brimonidine. Because of the presence of mesopores, brimonidine (BMD) could be encapsulated in the AMS with a loading amount of 41.73 μg/mg (i.e., drug loading capacity of about 4.17%) to give the BMD-AMS, which could release the drug in a sustained manner over 8 h. BMD-AMS was also shown to be mucoadhesive due to the presence of both hydroxyl and amino groups in the surface, allowing for formation of hydrogen bonds and an ionic complex with the mucin, respectively. Therefore, when topically administered to rabbit eyes in vivo, BMD-AMS could reside in the preocular space for up to 12 h because of its adherence to the mucous layer. To assess in vivo efficacy, we examined the variance in intraocular pressure (IOP) and brimonidine concentration in the aqueous humor (AH) after applying BMD-AMS to the eye, which was compared with that induced by Alphagan P, the marketed brimonidine eye drops. For BMD-AMS, the duration in the decrease in IOP and the area under the drug concentration in the AH-time curve (AUC) were 12 h and 2.68 μg·h/mL, respectively, which were about twice as large as those obtained with Alphagan P; this finding indicated enhanced ocular bioavailability of brimonidine with BMD-AMS.</P> [FIG OMISSION]</BR>

피아노 음의 파형을 분석함으로써 우리는 피아노 연주에 중대한 영향을 주는 터치와 페달링에 의한 음향학적 변화에 관한 결론을 얻을 수 있다. 피아노 연주자가 할 수 있는 것은 단지 건반을 특정 속도로 치는 것이고 따라서 음의 셈여림을 조절하는 것이다. 건반을 누르고 있는 동안 연주자는 현에 접촉할 수 없고 이것은 피아노 액션의 특성에 의한 것이다. 피아노의 음색에 변화를 주는 가장 중요한 요소는 배2음의 구조이다. 각각의 현의 특성에 따라 일정한 배음 구조를 갖지만 여러 가지 현을 동시에 연주하거나 페달을 사용함으로써 음 사이의 상호관계와 현들 간의 공명현상을 조절함으로써 보다 풍부한 연구가 가능하게 된다. 피아노의 연주는 암시적인 힘을 가진 것으로 평가되고 있으며, 음향 자체만을 본다면 결렬하고 심지어 야만적이라고도 한다. 그러나 곧 상상력이 활동을 재개하고 의미 있는 음들의 세계를 재창조함으로써 가장 풍부한 악기가 되는 것이다.

The purpose of this study was to investigate the effect of various electroacupuncture duration induced by acupuncture point-Zusanli(S36) electrical stimulation on inhibition of amplitude of digastric electromyogram (dEMG)evoked by noxious electrical stimuli around the mental foramen. <Experiments were conducted on adult rats (SD) were anesthetizde with> intraperitoneal sodium pentobarbital in an initial dose of 50mg/kg and maintenance doses of 4.5mg/kg/h were given through a cannula in the femoral vein using a constant infusion pump. A pair of stimulating electrodes were inserted for noxious stimuli around the mental foramen. An irritant electronic stimuli pulse (0.2 Hz,0.1 ms duration) was produced with an intensity of about 1.5-2 times threshold for evoking the dEMG. The anterior belly of the digastric muscle was exposed and a pair of 0.1mm wire electrodes were inserted for dEMG recording. Acupuncture point stimulation on Zusanli (2 Hz, 250㎲, biphasic pulse. 2 V) was delivered by Dental Electronic Anesthesia (3M,U.S.A). For periods of electronic stimulation of 10, 20, and 30min, the amplitudes of dEMG were measured on the oscilloscope and on the monitor connected to the amplifier. The following results were obtained: The dEMG was decreased to 73.4% of that in the control set after 10 min electroacupunture stimulation (Group Ⅰ): The dEMG was decreased to 77.1%(10min), 54.0% (20min)of that in the control set after 20minutes of electroacurunture stimulation (Group Ⅱ). The dEMG was decreased to 73.7% (10min), 61.9%(20min), 76.2%(30min) of that in the control set after 30 min of electroacupunture stimulation (Group Ⅲ). From these results, it may be that in the electroacupuncture stimulation on the Zusnali resulted in a reduction of amplitude of daEMG and that the most effective electroacupuncutre stimulation period was 20min.

This study was investigated to the Korean restaurant menus from eight of the first class and five of the second class hotels in Seoul, and attempted to evaluate the restaurant evaluation and cooks job satisfaction with questionnaire surveys. The results were as follows : Korean restaurants of the first class hotel was on sale classified with a large table of food (so called kjo-ja-sang), Korean tabled hotel dinner(chong-sik), a Ia carte (il-pum yori), dessert, and wine with various prices and dishes. The first class hotel sold a lot of a large tables, second class hotel merchandised many Korean tabled hate dinner, both of them served from soup and stew of low price to high class dishes, served rice drink(sik-hei) and ginger tea with dried persimmons (su choung-kwa) with dessert, and served so-ju and bop-ju with Wine. Cooks whose sex were composed of 20% of female and 80% of male, ages were 52% of 20th, most of them graduated from high school or more. They had Korean, Western. Japanese or two or more of cooking license, had engaged in cooking for 5-10 years, worked 8 hours a day, made 1,000,000-1,500,000 won in a month. Most of them were satisfied with their job, possibilities, education training, and salary, whereas they dissatisfied with a cuisine environment such as room temperature, noise, illuminations. Therefore Korean restaurants need improve their working environment. Korean restaurants of the first class hotel had job description. standard cooking recipe, and job planing, hence we know that they were managed with scientific systems. We must attempt to survey menu design of food service system, western and Chinese restaurant as well.

한국구강보건사업이 초등학교 아동들의 유치 및 영구치 우식실태에 미치는 영향

Purpose : The aim of this study was to evaluate the effects of oral health care programs in a school-based oral health care center among primary schoolchildren. Methods : School-based oral health care programs included fluoride mouth rinsing, pit and fissure sealing for permanent premolars and molars, fluoride gel application and chewing of xylitol candy. All of the programs were carried out by one dental hygienist among 'D' primary schoolchildren in Daegu city under the supervision of a dentist. Baseline dental examinations were completed and preventive care was implemented for 544 children during one year. All of the children visited a school-based oral health care center every three months for a regular check-up. The final oral examination was conducted from March 15 to April 1, 2004. The data analysis data was made on the basis of SAS 8.01. Mean differences between 2003 and 2004 data were compared by paired t-test. Corresponding p-values were considered significant at values less than 0.05. Results : The DMF rate and DFT index were reduced to 8.0% and 8.4% during one year respectively, but there were no statistically significant differences. The DMF rate was significantly reduced (16.3%) after a one year program of school-based oral health care practice. The DMFT(Decay Missing Filling Tooth) index was also reduced compared to 2003 throughout the entire grade. Conclusion : School-based oral health care programs can reduce the prevalence of dental caries prevalence among schoolchildren during one year. This program also improved the oral health capacity of schoolchildren. It is recommend that the school-based oral health care program should be extended to every primary school in Korea

인천 인현동 호프집 화재 피해자 분석

Purpose : This study was conducted to develop field triage, transportation, distribution, and prehospital care at a fire disaster by analyzing the victims of the fore that broke out at a bar in Incheon. Method : We analyzed the cases of the victims of a fire in Incheon in Oct. 1999. We determined the primary care hospital, the arrival time, the burn size, the outcome, and the injury type from the medical records, the concerned organ records, and interviews with concerned persons. Result : The total number of victims was 137: 56 prehospital deaths, 1 hospital death, and 80 survivals. The Pearson correlation coefficient between the burn size and the severity was -0.175. There were 121(89.6%) cases of inhalation injury, 59 (43.7%) cases of flame burns, 66 (48.9%) cases of hypoxic brain damage, and 16 (11.9%) cases involving other types of injury. Conclusion : The causes of death of the fire victims were inhalation injury and hypoxic brain damage due to CO poisoning and other toxic inhalants. We propose the use of a simple triage and rapid treatment(START) system and a reassessment the delayed category in fire disasters.

Familial adenomatous polyposis (FAP) is an autosomal dominant syndrome, typically characterized by multiple colorectal adenomas and increased incidence of colorectal carcinomas if it is not treated. Moreover, a variety of extracolonic manifestation are seen. The prevalence of thyroid tumors developing in patients with FAP is about 1∼2%. Generally, papillary thyroid cancer indicates low mortality but ,however, as people get older, the cancer becomes more aggressive. So, the operation is required. Recently we experienced a case of FAP, presenting with papillary thyroid carcinoma, and reported with a brief review of literatures.

Malic Acid를 사용한 탈지 대두박 기반 항균 가식성 필름 개발

Formation of edible films from food-process by-products, defatted soy bean meal (DSM), soybean meal-extracted soy protein, and persimmon peel and whey protein, has been investigated. The DSM-based film had a higher tensile strength (TS) (4-12 MPa) and percentage elongation at break (%E) (2.5-16.0%) than the films made of soybean meal-extracted soy protein or persimmon peel and whey protein, and the DSM-based film was selected as the base film material for developing an antimicrobial edible film incorporating malic acid. The malic acid-incorporating DSM-based film (MA-DSM film) inhibited Salmonella Typhimurium (2.2, 3.2, and 4.2 log CFU/cm2). The inhibition increased with the increase of the concentration of malic acid in the film from 10 to 30% (w/w, dry basis). The addition of malic acid increased lightness and %E (stretchability) and decreased TS (strongness) and Young’s modulus (stiffness) of the MA-DSM film. However, the water vapor permeability did not change after the addition of malic acid, indicating the moisture barrier property of the DSM-based film was not affected by the incorporation of malic acid in the film matrix. The results demonstrated the potential of applying MA-DSM films to food products and control surface contamination of the products with respect to Salmonella.

우리나라 지역 직업성질환 감시체계의 현황과 전망

기존의 특수건강진단과 작업환경측정을 통한 직업병 관리가 진폐증, 소음성난청 등의 소수 특정질환에 국한되고 실제 직업병 발생 규모 파악이나 신종 직업병의 발견에 한계를 보인다는 사실은 산업의학전문가들 사이에서도 공감을 이루고 있다. 미국과 영국 등에서의 직업성질환 감시체계에 대한 경험은 우리 나라의 직업성질환 감시체계 구축에도 새로운 자극제가 되면서, 1998년이후 인천, 대전, 여천, 구미, 부울경 지역에서 지역 직업성질환감시체계를 산업보건관리의 중요한 시스템으로 구축하려는 노력이 확산되고 있다. 새로이 구축되어지는 이들 지역 직업성질환 감시체계는 감시하고자 하는 대상질환, 활용 가능한 인적자원 및 자료원, 지역 의료체계의 특수성 등에 따라서 목적과 방법을 달리하면서 독특한 형태로 발전을 하고 있다. 각 지역단위 감시체계들이 그 상황에 맞게 독특한 목적과 전략들을 발전 시키면서도, 향후 발전할 국가적인 차원의 직업성질환 감시체계 구축을 위하여 직업성질환 감시의 기본 전략 등을 공유하는 등의 노력이 필요하다. 환례 정의 및 기본적인 등록 서식의 공유, 직업성질환 감시 자료원의 발굴, 공동의 정보 네트워크 및 직업성질환 감시 데이터베이스 구축 등 직업성질환 감시활동을 지원하기 위한 여러 기초 인프라 구축에 힘을 모아야 할 것이다. 우리 나라에서 직업성질환 감시체계를 성공적으로 구축하기 위해서는 수집된 자료의 질 관리를 위한 직업성질환 감시의 원칙 제정과 감시 전략의 공유 등이 필요하며, 전국적인 직업성질환 감시체계의 하부구조라고 할 수 있는 지역감시체계의 기초 토대 마련과 강화작업이 절실하게 필요하다.

EGFR 돌연변이와 ROS1 전위를 동시에 가진 폐선암 환자의 Erlotinib 치료 1예

The rearrangement of c-ros oncogene 1 (ROS1) has been recently identified as an important molecular target in non small cell lung cancer (NSCLC). ROS1 rearrangement and epidermal growth factor receptor (EGFR) mutation were mutually exclusive each other in previous studies, and the clinical implication of co-existence of the two genetic alterations has not been determined. We report a case of 46-year-old female never-smoker NSCLC patient whose tumor harbored ROS1 rearrangement and EGFR mutation concomitantly. She had undergone curative surgery for stage IIIA NSCLC, and the recurrence in left pleura and brain occurred at 2 years after the surgery. She received several lines of chemotherapy including docetaxel plus carboplatin, erlotinib, pemetrexed, and gemcitabine. Erlotinib therapy showed a favorable treatment response with progression-free survival of 9.5 months and partial response of tumor on radiologic evaluations. This case represents a successful erlotinib treatment in a NSCLC patient with concurrent ROS1 rearrangement and EGFR mutation.
