        토씨 생략과 관련 양상 -중세 국어를 중심으로-

        생략은 일반적으로 정보전달 구조에서 보이는 월성분의 생략을 가리키 지만, 이와 달리 제 훌로 정보 단위 요소가 되지 못하는 의폰 형식으로서 문법 요소인 토씨가 생략되는 경우가 있다. 월성분 생략이 일차척으로 담화척인 요인에 기인한다변, 토씨의 생략은 다분히 구조 의존적이다. 이러한 판점에서 앞선 연구들은 통사 구조 및 의미 구조와 관련하여 토씨의 쓰임파 생략에 있어서의 의미 차이를 여러 각도에서 고찰하여 왔다. 본 연구에 앞서 이소령(1995)은 토씨 생략에 대한 다각적이고 구체적인 연구의 일환으로 중세 국어를 대상으로 그 생략 양상을 통어적인 요인에 초점올 두고 고찰한 바 었다. 이러한 토씨 생략 의 주된 요인을 통어적 조건이라 한다면, 이 밖에 음성적 또는 형태 · 음 소적으로 생략올 톱거나 제한하논 환경이 생략의 양상에 영향을 미친다 따라서 본 연구는 토씨 생략과 관련하여 이러한 생략올 강화하는 보조 적 요인과 이를 제한하는 요인올 살피고, 통어적 요인과 보조적 요인에 따른 토씨 생략의 결과적인 양상으로서 문법화와 관용화를 정리하고자 한다.

        매인 이름씨의 문법적 특성 - 「 동안」「 만에 」를 중심으로 -

        이 글에서는 매인이름씨의 특징으로서, 앞선연구에서 확인된 바 있는 이 들 매인이름씨와 앞선요소인 매김씨와의 결합양상과 뒤선요소인 토씨와의 결합적 제약을 먼저 정리하고, 이에 더하여 매인이릅씨가 월에 쓰임에 있어 서의 풀이씨와의 공기양상을 매인이름씨 「동안」과 「 만에 」를 중심으로 살피 기로 한다. 선택제한에 따른 풀이씨와의 공기제약은 이름씨의 고유한 자질이 므로 매인이름씨를 여타의 문법요소와 구분짓는 근거가 될 것이다.

        고혈압 환자에서 대사증후군 평가의 유용성

        Background: The value of metabolic syndrome (MetS) evaluation in predicting cardiovascular disease is recently criticized. We investigated, in hypertensive patients without diabetes mellitus, the influence of MetS on the target organ damage. Methods: Data from the fourth Korean National Health and Nutrition Examination Survey performed in 2008 were analyzed. Metabolic syndrome is defined by the 2001 National Cholesterol Education Program-Third Adult Treatment Panel guideline. The category of hypertension is defined following the seventh report of the Joint National Commitee-7 guideline. Results:The prevalence of target organ damage (TOD), defined as history of myocardial infarction/angina/stroke/chronic renal disease as well as the presence of macroalbuminuria, was increased according to blood pressure; 8.5% in the population of normal blood pressure, 12.5% in those of prehypertensive range, and 20.5% in hypertensive population. Hypertensive population associated with MetS showed greater prevalence of TOD than those without MetS even excluding diabetic population. The presence of MetS in hypertensive population showed 2.2 fold increased risk for TOD. Any single parameter of MetS diagnostic criteria as well as obesity did not show the comparable range of risk prediction as MetS. Conclusions:These results indicate a strong relationship of Mets with TOD in hypertensive population. Evaluating the metabolic components in hypertensive population is necessary in establishing management strategies for overall risk.

        Atrial Innervation Patterns of Intrinsic Cardiac Autonomic Nerves

        Background: Although ganglionated plexi (GPs) are important in the pathogenesis of arrhythmia, their patterns of atrial innervation have remained unclear. We investigated patterns of GP innervation to cardiac atria and the neuroanatomical interconnections among GPs in an animal model. Methods: Atrial innervation by GPs was evaluated in 10 mongrel dogs using a retrograde neuronal tracer (cholera toxin subunit B [CTB] conjugated with fluorescent dyes). In Experiment 1, CTB was injected into the atria. In Experiment 2, CTB was injected into the major GP, including the anterior right GP (ARGP), inferior right GP (IRGP), superior left GP (SLGP), and ligament of Marshall (LOM). After 7 days, the GPs were examined for the presence of tracer-positive neurons. Results: GPs in either right or left-side were innervating to both the same and opposite sides of the atrium. In quantitative analysis, right-sided GPs, especially ARGP, showed numerical predominance in atrial innervation. Based on the proportion of CTB-labeled ganglion in each GP, atrial innervation by GPs showed a tendency of laterality. In Experiment 2, CTB that was injected to a particular GP widely distributed in different GP. ARGP projected the largest number of innervating neurons to the IRGP, SLGP and LOM. Conclusion: This study demonstrated that GPs project axons widely to both the same and opposite sides of atria. ARGP played a dominant role in atrial innervation. Furthermore, there were numerous neuroanatomical interconnections among GPs. These findings about neuronal innervation and interconnections of GPs could offer useful information for understanding intrinsic cardiac nervous system neuroanatomy.

        Label Adherence for Non-Vitamin K Antagonist Oral Anticoagulants in a Prospective Cohort of Asian Patients with Atrial Fibrillation

        Purpose: Label adherence for non-vitamin K antagonist oral anticoagulants (NOACs) has not been well evaluated in Asian patientswith non-valvular atrial fibrillation (AF). The present study aimed to assess label adherence for NOACs in a Korean AF populationand to determine risk factors of off-label prescriptions of NOACs. Materials and Methods: In this COmparison study of Drugs for symptom control and complication prEvention of AF (CODE-AF)registry, patients with AF who were prescribed NOACs between June 2016 and May 2017 were included. Four NOAC doses werecategorized as on- or off-label use according to Korea Food and Drug Regulations. Results: We evaluated 3080 AF patients treated with NOACs (dabigatran 27.2%, rivaroxaban 23.9%, apixaban 36.9%, and edoxaban12.0%). The mean age was 70.5±9.2 years; 56.0% were men; and the mean CHA2DS2-VASc score was 3.3±1.4. Only one-thirdof the patients (32.7%) was prescribed a standard dose of NOAC. More than one-third of the study population (n=1122, 36.4%)was prescribed an off-label reduced dose of NOAC. Compared to those with an on-label standard dosing, patients with an off-labelreduced dose of NOAC were older (≥75 years), women, and had a lower body weight (≤60 kg), renal dysfunction (creatinineclearance ≤50 mL/min), previous stroke, previous bleeding, hypertension, concomitant dronedarone use, and anti-platelet use. Conclusion: In real-world practice, more than one-third of patients with NOAC prescriptions received an off-label reduced dose,which could result in an increased risk of stroke. Considering the high risk of stroke in these patients, on-label use of NOAC is recommended.

        Prevalence of Atrial Fibrillation in Korean Population

        Atrial fibrillation (AF) is the most common cardiac arrhythmia in clinical practice and has been a major public health problem. The prevalence of AF globally has been reported to range from 0.4% to 2% in the adult population; however, the prevalence widely varies depending on the study population. Although AF prevalence in Asian population is about 1% lower than that in European and North American population, the number of AF patients in Asia is rapidly increasing concomitant with the increased aging of the population. However, previous studies that reported AF epidemiology in the Korean population are limited. According to a recent report based on the national health claims database, prevalence of AF in 2015 was 0.7% in the entire Korean adult population and showed a continuous increase with population aging. Additionally, among patients with AF, mean CHA2DS2-VASc score was significantly increased and the number of high thromboembolic risk patients has increased over time. Although oral anticoagulation (OAC) prescription in AF patients increased from 35% to 51%, especially after introduction of non-vitamin K antagonist anticoagulants, a substantial proportion of Korean patients with AF still remain undertreated. In this review, we aimed to summarize temporal changes in the prevalence and incidence of AF and to evaluate the thromboembolic risk in patients with AF. In addition, we also discussed the utilization of OAC therapy in patients with AF in Korean population.

        Prognostic Implication of QRS Variability during Hospitalization in Patients with Acute Decompensated Heart Failure

        Background and Objectives: Heart failure (HF) patients display more varied QRS duration. We investigated whether QRS variability during hospitalization for acute decompensated HF is associated with poor clinical outcomes after discharge. Subjects and Methods: One hundred seventy three patients (64% males; age 60±13 years) admitted for acute decompensated HF with severe left ventricular (LV) dysfunction (LV ejection fraction ≤35%) were consecutively enrolled. QRS variability was calculated by the difference between maximum and minimum QRS duration acquired during hospitalization. The prognostic implications on composite endpoints of death or urgent heart transplantation were analyzed. Results: Forty-two patients (24.3%) died and three patients (1.7%) underwent urgent heart transplantation during the follow-up of 51±18 months. Patients who reached composite endpoints (n=45) showed greater QRS variability than those who did not (n=128) (20±23 ms vs. 14± 14 ms, p=0.046). Patients who had high QRS variability (more than 22 ms; n=36) tended to have a higher event rate than those with QRS variability <22 ms {39% vs. 23%, hazard ratio (HR), 1.88; 95% confidence interval (CI) 1.001-3.539, p=0.05}. Adjusting with other variables, high QRS variability was an independent predictor for composite outcome (HR 1.94; 95% CI 1.023-3.683, p=0.042). Conclusion: QRS variability measured during hospitalization for acute decompensated HF has a prognostic impact in HF patients with severe LV dysfunction.

        문언, 백화 그리고 백화소설 -전통시기 중국의 문학 언어에 관한 탐색 시론

        There are two kinds of language in Chinese written language. Those are Literary language(文言) and Vernacular language(白話). Vernacular language originated from the language of 先秦 兩漢 and was independent from natural language. In contrast, vernacular language depended on and reflected natural language. These two written languages existed together, at least from the late Tang era, but functioned in different, social and cultural area. The coexistence, contradiction and change of two languages can be seen cleary in the literature, especially in the novel. Vernacular language grew and developed as a language of the novel genre. The vernacular story was a kind of reflection of the structure of vernacular language and also the best instument which the novelists made use of with naturalistic spirit. Therefore the unique grammar of the vernacular story was formed, which is different from that of classical tale(文言小說). The vernacular story and vernacular language was inseparable and vernacular language affected the development of the vernacular novel and vice versa. After the nineteenth century, when the ideological conversion occurred in China, vernacular language and the vernacular novel became one of the essential concepts which helped to discern between the traditionality and the modernity.
