성인에서 기관내삽관을 위한 Pipecuronium 일회 정주 적정 용량

Background : Studies in animals suggest that pipecuronium dose not induce hemodynamic chan-ges related to histamine release or to an effect on the autonomic nervous system. Therefore the effects of bolus administration of large doses of pipecuronium, up to 0.20 mg/kg, on the intubation condition, onset and duration of neuromuscular blockade, heart rate and blood pressure were studied during fentanyl- nitrous oxide anesthesia. Method : Forty adults were randomly assigned to receive a bolus injection of either 0.05, 0.10, 0.15, 0.20 mg/kg of pipecuronium. Neuromuscular blockade was measured using mechanomyographic activity of the adductor pollicis muscle after supramaximal stimulation of the ulnar nerve. Four subgroups of 10 patients received pipecuronium doses of 0.05, 0.10, 0.15 and 0.20 mg/kg, respectively, as an intubating dose. Results : The times of onset and clinical duration(mean sem) after each dose were as follows: 0.05 mg/kg, 2.98 0.42 and 41.5 2.42 min; 0.10 mg/kg, 1.54 0.06 and 82.9 7.48 min; 0.15 mg/kg, 1.41 0.14 and 124.8 13.1 min; 0.20 mg/kg, 1.12 0.05 and 187.1 12.8 min. The intubation condition, time of onset and duration after doses of 0.05 mg/kg were significantly different from values after the higer doses. The duration was increased with dose-increments. No dose-related changes in heart rate or blood pressure were observed. Conclusion : The authors conclude that dose of 0.10 mg/kg and over has good intubation condition clinically and large bolus dose of pipecuronium can be safely used with a significantly prolonged duration of action without hemodynamic change. BIPV 건물 외피시스템 개발 : Curtain Wall의 일체화 가능성을 중심으로 Focused on the unification possibility to the Curtain Wall

PV system(Photovoltaic system) as an alternative energy resource for a fossil fuel is judged to have an enormous possibility in a usable point of earth, everywhere. Therefore, this paper intends to advance its practical use by proposing how to get integrated PV system which can be applied to building external curtain wall, and how to apply it. It has materialized the possibility of integrating details which can be applied to curtain wall by making insulating glass PV (the performance of insulation has been improved) through adding the layer of air (12 ㎜) and rear glass (6 ㎜) to integrated sandwich glass PV module. The proposed insulating glass PV module was applied to vision and then, the load was measured. The result shows that because of influence by solar radiation to the southern part in summer, it is advantageous for energy saving and it can make an additional effect of peak cut.

海外事業詩 고려되어야 할 物流要素

우리 기업들의 활발한 해외투자 증대추세에 비해, 물류부문에 관한 關心과 硏究가 상대적으로 미약, 물류단계마다 浪費와 非效率이 적지 않다고 보고, 성공적인 해외사업을 위한 一環策으로, 물류개념의 認識提高 및 적극적인 물류관리 기법의 導入을 촉구하기 위한 논문이다. 이를 위해 국제 물류관리의 特性 및 領域, 국제물류 체계의 구축과 전개방향 등을 개괄하고, 국제기업의 물류관리시 考慮·留意해야 할 諸要所를 생각해 보았다. 그리고 타기업들이 아직 물류에 대한 개념도 갖지 못했던 1949년에, 이미 조직을 물류중심으로 편성, 세계적인 다국적기업으로 성장해 온 코닥사의 물류관리 事例을 분석하여 물류의 중요성을 浮刻하였다. 흔히 물류는 이른바 高次元의 전략 또는 능력을 요하지 않는 단순·반복업무로 간주되고 있으나, 內面으로 파고 들면 高度의 전문지식과 함께 창의적·혁신적 思考를 요하는 부문이다. 따라서 기업의 물류합리화를 위해서는, 우선 全社的 물류마인드의 강화와 함께 유능한 물류지도자를 選任하여, 전반적인 물류체계 및 조직을 정비하고, 종사원들에 대한 직무교육 및 훈련을 강화, 물류과정마다 顯在·內在되어 있는 낭비와 비효율의 방지에 關心을 기울일 것을 제언하였다.

담낭종양에서 CD24의 역할

CD24 is a small heavily glycosylated glycosylphosphatidylinosito-linked cell surface protein, which is expressed in hematologic malignancies as well as in a large variety of solid tumors, It appears to function as a ligand of PUselectin, an adhesion molecule that is present in activated platelets and endothelial cells. The authors aimed to evaluate CD24 protein expression in adenoma and adenocarcinoma of gallbladder to correlate to clinicopathologic data. The staining- was evaluated as stainability (negative, weak-, moderate-, strong-positive) and staining' patterns (membranous vs. intracytoplasmic) for statistical analysis. A highly significant association of cytoplasmic CD24 expression with adenocarcinoma compared to the adenoma of gallbladder was demonstrated. The present study demonstrates that CD24 was abundantly expressed on adenocarcinoma rather than on adenoma, but it was not statistically significant, Interrelation between the stainability or intracytoplasmic staining of CD24 and lymph node metastasis was not statistically significant. The authors conclude that CD24 is involved in tumorigenesis of gallbladder mucosa and intracytoplasmic expression of CD24 is an important molecular marker for malignant transformation.

국소성 분절성 사구체 경화증의 완전 관해 후 발병한 IgA 신병증 1예

We report a rare case of primary glomerular disease with IgA nephropathy in a 43-year old man. 7-years ago, he was diagnosed with minimal change nephrotic syndrome. After treatment with steroid for 1-year, azotemia and proteinuria were improved. But, nephrotic range proteinuria was relapsed within 1 week after cessation of steroid. Renal biopsy revealed focal segmental glomerulosclerosis, and achived a complete remission with the use of steroids and cyclophosphamide. Then, he was followed up with normal renal function for 4-years. On admission, the patient showed generalized edema and marked proteinuria. In 3rd renal biopsy, mesangial proliferative glomerulonephritis with diffuse global sclerosis was observed. Electron microscopy showed electron-dense deposit on the mesangium with slight mesangial proliferation, and immunofluoroscence microscopy showed granular deposits of IgA and IgM. These features were consistent with IgA nephropathy, different from results of previous biopsies. The pathogenesis of IgA nephropathy occurred with primary FSGS is not clear, as well as treatment. Now, proteinuria is improving, steroid and cyclophosphamide are probably useful for the disease.

우리나라 長壽村의 生活習慣과 飮食文化에 關한 考察

In view of the results that we studied about living and eating habit of korean old people who live in longevity village, we can make conclusion as follows. 1. They have regular rising time, bedtime and living routine. Their bedtime is between 9 to 11 and rising time is before 6. It goes to show that they are keeping regular hours. 2. Generally speaking, most of long life peoples are physical labor : agriculture, fishery, housework, etc. 3. They are in balance with their eating habit. The most favorite food of longevity village's old people is made from toenjang which is fermented soybean paste. 4. The geographical features of longevity village are mountain region, coast and island. 5. Most of people who live long are illiteracy and only a few people of them has finished elementary school course. Moreover, in case of illiteracy, there are more unschooled woman compare with both sexes.

한국인에서 혈소판 당단백 Ⅱb/Ⅲa 유전자 다형성과 관동맥 성형술 후 재 협착과의 관계

Platelet aggregation is the final pathway of acute coronary syndrome such as acute myocardial infarction and unstable angina. Platelet glycoprotein IIb/IIIa is a membrane receptor for fibrinogen and yon Willebrand factor and it plays an important role in platelet aggregation and in the pathogenesis of acute coronary syndrome. It is known that polymorphism of the gene that encoding platelet glycoprotein IIb/IIIa(PI^A1/A2) is strongly related to acute coronary syndrome in Caucasian, but not in Koreans. We investigated relationship between platelet glycoprotein llb/Illa gene polymorphism and restenosis of coronary artery after angioplasty in Koreans. Total 371 patients(M=251. F=120) were enrolled. Angioplasty group comprised 143 patients who underwent coronary angioplasty, and in the angioplasty group, restenosis group comprised with the 65 patients who had restenotic lesion over 50% of luminal diameter in follow-up coronary angiography. Normal group comprised 153 patients who had no significant angiographic lesion and variant angina group comprised 75 patients who were positive in ergonovine test. Genomic DNA was extracted from peripheral arterial blood. To determine the frequency of P1^A1/A2 genotype, polymerase chain reaction(PCR) was done and the product was restricted with Mspl. 3%. agarrose gel electrophoresis showed restriction fragment length polymorphism. Clinical profile and risk factor were also reviewed. Among all 371 patients of study group, genotype of only one patients in restenosis group if is proven to be PI^A1/A2 heterozygote. All patients of normal study group, no restenosis group, and the other patients in restenosis group have an PI^A1 homozygote genotype. In our study, platelet glycoprotein IIb/Illa polymorphism has no relationship with restenosis of the coronary artery after angioplasty in Koreans. But the genotypic frequency of platelet glycoprotein IIb/IIIa gene polymorphism in Koreans is concordant with that of previous studies.

경막외 마취시 Clonidine 과 Epinephrine 첨가에 따른 마취연장효과의 비교

Epidural clonidine is reported to produce analgesia in humans. To investigate the analgesic effect and prolongation of epidural and spinal anesthesia, we mixed 0.2mg epinephrine, 150 mcg clonidine, or 1 cc normal saline with 0.5% bupivacaine and compared the hemodynamie and analgesic effects of each drug. Heart rate and blood pressure were checked before, during and after anesthesia. Sensory level was checked by pin-prick method. The results were as follows; 1) The analgesia onset time and the time to highest level of sensory loss was most rapid in the epinephrine group. 2) The two segment regression time was significantly prolonged in the epinephrine group. 3) The analgesia duration was significantly prolonged in the clonidine and epinephrine group. 4) Although the heart rate gradually decreased over 60 min. After injection of each drug, there was no significant change between the groups. 5) Blood pressure decreased over 20-30 min. After injection of each drug, but there was no significant change between the groups.

응집공정에 적합한 In-Line 정적혼화기를 이용한 혼화공정의 개선

The rapid mixing process for coagulation is a critical component in a water treatment plant. However, at the present time a detailed understanding of the rapid mixing by in-line mixer or generally accepted criteria for the design of in-line static mixer are not available. The aim of this work is therefore to improve the understanding of how the in-line static mixer works and how to better utilize it in coagulation process in water treatment. To investigate the performance of static mixer, coagulation experiments were conducted to compare the static mixer to the traditional backmixer under several coagulation conditions. Experiments were also performed to compare the coagulation efficiency of different mixing type of static mixer which were made using different flow rates and the number of elements equipped in a pipe. The static mixer performed better than the backmixer for all coagulation and mixing conditions tested. Especially, the static mixer was very effective in the surface charge neutralization, as showing higher reduction of negative charge with increasing mixing intensity. However, little difference was observed in the performance of the static mixer equipped with 2, 3, and 5 elements. Also, in a series of bench-scale studies of rapid mixing, the in-line static mixer performed better than the traditional backmixer for both adsorption and charge neutralization (A/N) mechanism and sweep coagulation mechanisms. Especially, the static mixer was more efficient at producing A/N mechanism conditions as a result of its fast and uniform dispersion of the coagulant.
