        수술시 PTH 측정으로 진단한 부갑상샘 낭종성 선종에 의한 원발성 부갑상샘 기능 항진증 1예

        Primary hyperparathyroidism is the most frequent cause of hypercalcemia, and its prevalence is increasing due to the routine examination of serum calcium levels. Primary hyperparathyroidims is most commonly caused by an adenoma or hyperplasia of the parathyroid gland. A cystic parathyroid adenoma is an extremely rare cause of primary hyperparathyroidism.In our case, a-79-year old female presented with lower back pain and constipation. Her serum calcium, phosphate and immunoreactive parathyroid homone levels were 15.6, 1.8mg/dL and 371.8pg/mL, respectively. Neck CT revealed a cystic mass and a contour bulging heterogeneous mass in the left inferior right thyroid gland, respectively. These mass lesions were removed, and the intra-operative parathyroid hormone levels monitored, to confirm the complete resection. After removing the left cystic mass to the inferior thyroid, the serum calcium and immunoreactive parathyroid hormone levels quickly returned to normal.We report a case of primary hyperparathyroidism, caused by a cystic parathyroid adenoma, with a brief review of the literature 저자들은 심한 고칼슘혈증을 동반한 부갑상샘 낭종에 의한 원발성 부갑상샘 기능 항진증을 경험하였고, 수술중 부갑상샘 호르몬 값을 측정함으로써 원인이 된 부갑상샘내의 병변이 완전 절제되었음을 확인한 후, 이를 통해 최소 침습적 수술을 시행한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

        Clicker를 활용한 한국어 교실 상호 작용 증진 방안 연구

This study aims to find the method to promote interaction in L2 classrooms. Active interaction between learner-to-learner and learner-to-teacher in L2 classroom plays an important role in language acquisition. In light of this, L2 classroom would benefit with the help of learning tools such as Clickers which helps learners to express their level of understanding during the process of learning itself. This is because the anonymity of Clickers allows learners to express their needs without the social risks associated with speaking up in the class. It allows for an evaluative feedback loop where both learners and teachers understand the level of progress of the learners, better enabling classrooms to adapt to the learners`` needs. Eventually this tool promotes participation from learners, This is in turn, believed to be effective in fostering classroom interaction, allowing learning to take place in a more comfortable yet vibrant way. This study is finalized by presenting the result of an experiment conducted to verify the effectiveness of this approach when teaching pragmatic aspect of the Korean expressions with similar semantic functions. As a result of the research, the learning achievement of learners in the experimental group was found higher than the learners`` in a control group. Analyzing the data collected from a questionnaire given to the learners, the study presented data suggesting that this approach increased the scope of interactivity in the classroom, thus enhancing more active participation among learners. This active participation in turn led to a marked improvement in their communicative abilities.

      • 씀바귀 잎의 전처리 방법에 따른 항산화 활성 변화

        씀바귀는 우리나라에서 자생하는 여러해살이풀로 그 뿌리와 잎은 나물이나 민간요법으로 사용되는 식품원료이다. 본 연구에서는 씀바귀 잎을 다양한 방법(데치기, 찜, 과열증기)으로 전처리하였을 때 기능성 성분의 함량 및 항산화 활성의 변화를 알아보았다. 총 플라보노이드 함량은 생것(13.962 mg/g)과 과열증기(13.705 mg/g)를 한 시료에서 가장 높게 나타났다. 총 폴리페놀화합물의 함량은 생것(34.425 mg/g)에서 가장 높았으며, 전처리를 한 시료 중에서는 과열증기 시료(31.215 mg/g)에서 가장 높았다. 또한 데치기를 한 시료에서 가장 낮은 플라보노이드 및 총 폴리페놀화합물 함량을 보여주었다. 비타민 C의 함량을 분석한 결과, 생것은 980 mg/g이었으며, 전처리를 한 경우에는 모두 비타민 C의 함량이 현저히 감소하였다. 특히 씀바귀 잎을 찐 경우 비타민 C의 함량이 약 90% 감소(97.407 mg/g)하였으나, 과열증기를 한 경우 전처리한 시료 중에서 가장 높은 함량(246.747 mg/g)을 보여주었다. ABTS 및 DPPH 라디칼 소거능 측정 결과, 생것의 소거능이 가장 우수하였으며, 전처리한 시료 중에서는 과열증기한 시료의 소거능이 가장 높았다. 본 연구결과 과열증기 전처리는 데치기에 비하여 기능성 성분의 함량 및 항산화 활성이 전반적으로 뛰어났으며, 이는 건조하여 저장하는 많은 엽채류 및 나물류의 생리활성 보존에 긍정적인 방법이 될 수 있을 것으로 기대된다.

        L자형 평면 건축물의 비정형성을 고려한 풍하중 할증계수의 제안

        비정형 형태의 건축물에서 발생하는 풍하중은 KBC-2016의 풍하중 산정식으로 산정할 수 없기 때문에 풍동실험을 통해 풍하 중을 평가할 수밖에 없다. KBC-2016으로 비정형건축물을 정형적인 건축물로 가정하여 풍하중을 평가한다면 과소평가될 우려가 있 다. 그러므로 보다 합리적인 평가를 위해 풍하중을 할증시켜줄 필요가 있다. 본 연구에서는 평면형태가 L자인 건축물을 대상으로 풍력 실험을 실시하여 풍하중을 산정하였으며, 이를 KBC-2016으로 산정한 풍하중과 비교하였다. 풍동실험을 통해 구한 L자형 건축물의 풍 하중과 KBC-2016으로 L자형 건축물과 동일한 폭과 깊이를 가진 사각형평면 건축물을 대상으로 구한 풍하중의 비로 풍하중 할증계수 를 도출하였다. 풍하중 할증계수는 1.6~2.2로 나타났다. KBC-2016에 의해 평가한 사각형 건축물의 풍하중에 풍하중 할증계수를 곱하 면 L자형 건축물의 풍하중이 된다. Wind loads generated from irregular buildings can't be evaluated by KBC-2016, so wind loads should be evaluated by wind tunnel test. If the wind load is evaluated using KBC-2016, which assumes irregular buildings as regular buildings, it may be underestimated. Therefore the wind loads need to be increased for more reasonable evaluation. In this study, wind loads was evaluated by wind tunnel test on L-shaped buildings and compared with wind loads evaluated by KBC-2016. The amplification factor on wind load was derived from the ratio of wind loads by wind tunnel tests on L-shaped building to wind loads by KBC-2016 on rectangular building. The amplification factor ranged from 1.6 to 2.2. The wind load of a rectangular building evaluated by KBC-2016 is multiplied by the amplification factor to become the wind load of an L-shaped building.

        Hepatokines as a Link between Obesity and Cardiovascular Diseases

        Non-alcoholic fatty liver disease, which is considered a hepatic manifestation of metabolic syndrome, independently increases the risks of developing cardiovascular disease (CVD) and type 2 diabetes mellitus. Recent emerging evidence suggests that a group of predominantly liver-derived proteins called hepatokines directly affect the progression of atherosclerosis by modulating endothelial dysfunction and infiltration of inflammatory cells into vessel walls. Here, we summarize the role of the representative hepatokines fibroblast growth factor 21, fetuin-A, and selenoprotein P in the progression of CVD.

      • A LDPC Decoder for DVB-S2 Standard Supporting Multiple Code Rates

        For forward error correction, DVB-S2, which is the digital video broadcasting forward error coding and modulation standard for satellite television, uses a system based the concatenation of BCH with LDPC inner coding. In DVB-S2 the LDPC codes are defined for 11 different code rates, which means that a DVB-S2 LDPC decoder should support multiple code rates. Seven of the 11 code rates, 3/5, 2/3, 3/4, 4/5, 5/6, 8/9, and 9/10, are regular and the rest four code rates, 1/4, 1/3, 2/5, and 1/2, are irregular. In this paper we propose a flexible decoder for the regular LDPC codes. We combined the partially parallel decoding architecture that has the advantages in the chip size, the memory efficiency, and the processing rate with Benes network to implement a DVB-S2 LDPC decoder that can support multiple code rates with a block size of 64,800 and can configure the interconnection between the variable nodes and the check nodes according to the parity-check matrix. The proposed decoder runs correctly at the frequency of 200MHz enabling 193.2Mbps decoding throughput. The area of the proposed decoder is $16.261m^2$ and the power dissipation is 198mW at a power supply voltage of 1.5V. 디지털 비디오 방송표준(DVB-S2)은 순방향 에러 코딩방법으로 BCH와 LDPC을 연결한 시스템을 내부코딩으로 사용한다. DVB-S2에서 LDPC 코드는 11개의 서로 다른 부호화 율을 정의하고 있기 때문에, DVB-S2 LDPC 복호기는 다양한 부호화 율을 지원해야 한다. 11개의 부호화 율 중에서 7가지(3/5, 2/3, 3/4, 4/5, 5/6, 8/9, 9/10)는 균일한 부호화 율이고, 나머지 4가지(1/4, 1/3, 2/5, 1/2)는 비균일 부호화 율이다. 본 논문에서는 균일한 LDPC 코드를 위한 유연한 복호기를 제시한다. 제안된 복호기는 칩의 면적, 메모리의 효율, 처리속도 등에서 많은 장점을 갖는 반 병렬 복호 구조와 변수노드와 체크노드의 내부 연결선을 줄이고 다양한 부호화 율을 지원할 수 있도록 Benes 네트워크를 결합하여 블록크기가 64,800까지 사용가능하도록 설계하였다. 제안하는 복호기는 200MHz에서 193.2MbPs의 처리속도를 갖으며, 면적은 $16.261m^2$이고, 전력은 공급전압이 1.5V에서 198mW의 소모를 보인다.

        Integrated Whole Body MR/PET: Where Are We?

        Whole body integrated magnetic resonance imaging (MR)/positron emission tomography (PET) imaging systems have recently become available for clinical use and are currently being used to explore whether the combined anatomic and functional capabilities of MR imaging and the metabolic information of PET provide new insight into disease phenotypes and biology, and provide a better assessment of oncologic diseases at a lower radiation dose than a CT. This review provides an overview of the technical background of combined MR/PET systems, a discussion of the potential advantages and technical challenges of hybrid MR/PET instrumentation, as well as collection of possible solutions. Various early clinical applications of integrated MR/PET are also addressed. Finally, the workflow issues of integrated MR/PET, including maximizing diagnostic information while minimizing acquisition time are discussed.

        X-Linked Spondyloepiphyseal Dysplasia Tarda: Identification of a TRAPPC2 Mutation in a Korean Pedigree

        Spondyloepiphyseal dysplasia (SED) comprises a heterogeneous group of skeletal dysplasias that primarily affect the epiphyses and vertebral bodies. Patients affected by SED usually exhibit short stature and experience early development of degenerative osteoarthritis. SED is subdivided into congenita and tarda forms according to the age at onset and clinical severity, and further subdivided into genetically different forms according to the mode of inheritance and the gene involved. We report a 14-yr-old Korean male who presented with a disproportionately short stature and a short trunk. A pedigree analysis of 3 generations with 6 affected persons revealed an X-linked recessive mode of inheritance. Mutation analysis of the TRAPPC2 (previously called SEDL) gene, the only gene associated with X-linked spondyloepiphyseal dysplasia tarda (X-linked SEDT; MIM 313400), was performed, and a splice-donor site mutation in intron 3 of the TRAPPC2 gene (c.93+5G>A) was identified in the proband and in his unaffected mother (a heterozygote). This mutation is one of the 2 most frequent mutations reported in the medical literature, and is known to result in exon 3 skipping. This is the first report of a genetically confirmed X-linked SEDT case in Korea and highlights the importance of recognizing the mode of inheritance in the diagnosis of X-linked SEDT.
